Relocation of persons with dementia might be considered when quality of care cannot be sustained due to increasing needs. Relocation has been studied from the aspects of mortality and morbidity. Aim. To explore the thoughts and experiences of care workers when relocating a person with dementia within municipal sheltered housing. Method. This qualitative study was conducted in Sweden and based on interviews with six care workers involved in the relocation of two persons with dementia. Latent content analysis was applied. Results. The findings of one narrative are summarised in the theme 'Walking the tightrope: trying to balance conflicting needs'. In the other narrative the theme 'Balancing closeness and distance' emerged. Eight sub-themes were identified illustrating the relocation time-line. Conclusion. Relocation from the care workers' perspective is a complex process with conflicting values and practical issues that hinder the provision of good care and security. Implications for practice. Supporting next of kin in the relocation decision-making process presupposes time for reflection in dialogue with staff. The risk of moral distress should be observed and support for care workers could be provided in the form of supervision, ethical consultations and local guidelines.

    • Health status and functional profile at admission of nursing home residents in Iceland over 11-year period. The knowledge over time of the health status and changes in care needs of newly admitted nursing home residents is limited. Objectives. To investigate trends in residents' health status and functional profile at admission to nursing homes and compare rural and capital areas in Iceland over an 11-year period. Design. Retrospective analysis of nursing home data over 11 years (1996-2006). Participants. Residents, who had been assessed with the Minimum Data Set assessment within 90 days from admittance (n = 2206). Method. Non-parametric tests for descriptive statistics and linear regressions were used to analyse time trends. Results. The mean age ranged from 80.1 to 82.8, and women accounted for 52.7% to 67.1%. The level of independency indicated intact cognitive performance in 28.6-61.4% and in 42.5-68% in activities of daily living performance. A weak, but significant, linear trend was seen in residents' health becoming less stable, their cognitive performance improving, more pain being reported and greater participation in social activities over the 11 years. Conclusion. Some residents might have stayed at home longer had they been given appropriate home care and the opportunity of rehabilitation. Pain management and social activities are areas where more staff knowledge seems to be needed. Implications for practice. Resources to enable old people to remain at home need to be explored before their entry into nursing homes. Whereas providing services at the appropriate level is important for society as well as older people.

    • The use of drama to support reflection and understanding of the residents' situation in dementia care: a pilot study. International Journal of Older People Nursing doi: 10.1111/j.1748-3743.2012.00333.x Background.  One key aspect of person-centredness is striving to understand both the patients' experiences and behaviours from their perspective. These aspects are precisely those that staff in dementia care highlight as causing them most difficulty because the people in their care have major problems expressing themselves. There is thus a need to develop a method to help the staff to achieve interpretation through reflection. Aim.  The aim of this study was to explore the use of drama as a tool to support reflection among staff working in the residential care of people with dementia. Design.  A qualitative evaluation of a programme consisting of three drama sessions with staff working in residential care (n = 10 nurse assistants). Methods.  Data comprised observations and tape recordings of the sessions, the researchers' reflections after each session and a focus-group interview with the participants. The texts were analysed using qualitative content analysis. Results.  The analysis showed that: (i) the exercises stimulate reflection about daily caring practice; (ii) the participants must receive extensive information about the purpose of the sessions; (iii) the research team must secure the defined frames and conditions and have practical knowledge about caring for people with dementia and (iv) the management needs to be stable, committed and supportive. Conclusion.  Drama seems to be a valid tool to aid reflection, but several adjustments are needed concerning both the content of the sessions and the methodology. When designing a larger intervention study, it would be preferable to the sessions to be combined with staff support to effect changes in care provision resulting from their increased awareness of the residents' situation and experience. Implications for practice.  Our results showed that drama can be a means to enhance reflection among staff in residential care for people with dementia. Further research is however needed concerning the effects for the staff's situation and nursing care quality.

    • Background.  Nurse assistants working in residential care facilities need support to ensure that they provide high-quality care for the residents and support for relatives, from admission to bereavement.Aim.  The aim was to describe the nurse assistants’ experience of how an intervention with a palliative care approach, had influenced them in their work in residential care for older people.Participants.  Fourteen nurse assistants working in three different municipal residential care facilities.Methods.  Data were collected by means of semi-structured individual interviews following an intervention consisting of study circles combined with workshops. The data were analysed using content analysis.Result.  The nurse assistants felt that, through the intervention, they had gained insight into their understanding of the importance of quality of care. This included an increased awareness of, and respect for, residents’ and relatives’ needs, and an increased understanding of the importance of the outcome of encounters with residents and their relatives. After the intervention, they also felt there was increased openness and understanding between colleagues. However, the nurse assistants also expressed frustration over obstacles to implementing a palliative care approach, such as lack of resources and supportive leadership.Conclusion.  The nurse assistants felt that the intervention was positive and encouraged them to provide more person-centred care within the framework of a palliative care approach. Although the intervention was intended to involve and support the management, it was not sufficient. Nurse assistants described lack of resources and supportive leadership. There is, therefore, a need to place greater emphasis on leadership and their support of nurse assistants so that they can provide high-quality care.

    • Aim. The aim of this study was to describe the interaction between cage-birds and older people as it arises in spontaneous situations in a Swedish hospital ward setting. Background. Older people are at risk of being affected by mental fatigue because of various age-related reductions and disease. Mental fatigue manifests itself through a reduced ability to concentrate. The method of alleviating mental fatigue with the use of stimulating natural settings or animal-assisted activities is so far unexplored in Sweden and subsequently it is important to study the method in a Swedish context to implement it as a recognized nursing intervention in the care of older people. Design. The study had an explorative observational design, and was conducted at a geriatric ward with a consecutive selection of participants. Method. The observations were analysed using a constant comparative method influenced by Grounded Theory. 'Attention restorative theory' was used as the theoretical framework to guide the study. Result. Our findings indicate that animal-assisted therapy has a positive effect on involuntary attention and mental restoration, as well as on enhanced social behaviour among older people. Relevance to clinical practice. The result strengthens the assumption that animal companionship should be considered a beneficial nursing intervention for older people to prevent mental fatigue.

    • Background.  Comparisons of different stakeholders’ ratings of the quality of older people care can help to drive quality improvement. Aim.  The aim was to compare staff, older care recipients’ and their relatives’ quality of care ratings. Design.  Cross-sectional questionnaire surveys in 2003 and 2004, using a repeated measures design on an organizational level. Methods.  Nursing staff, care recipients and relatives in two older people care organizations were included. The ratings of an overall quality grade, information, activities, general care and staff skills were compared between the respondent groups. Results.  Care recipients in both organizations rated the overall quality grade significantly higher than nursing staff and relatives. Staff ratings of the information given to care recipients were significantly more positive than care recipients’ and relatives’ ratings. All three groups gave lowest ratings to the quality of activities offered to care recipients, with lowest ratings from nursing staff. Conclusions.  Concurrent measurements of staff, care recipients and relatives’ care quality perceptions can provide a broad evaluation of an organization’s strength and limitations. Relevance to clinical practice.  Staff, care recipients’ and relatives’ perceptions can be useful for older people care organizations and decision makers in developing care processes and outcomes of care.

    • Objectives. The aim of this study was to illuminate older immigrants' experience of their life situation in the context of receiving public care in Sweden. Background. To meet the varying needs from the growing number of older immigrants, more knowledge about the experience of their life situation, health, illness and family ties is needed, especially in the context of receiving public health care and service. Methods. Data were collected through interviews with sixteen immigrants aged 65 years and older, originally from nine different countries, living in Southern Sweden and receiving continuous care and service. The texts were analysed using qualitative content analysis. Results. The overall impression from the texts was that the informants expressed a debt of gratitude to Swedish society. Their experience could be understood as – being burdened with health complaints; experiencing painful losses; wanting to manage by oneself and feeling exposed and deserted. Conclusions. The results show that older immigrants are vulnerable when receiving public care. This was related to previous life events, cultural differences as well as communication difficulties. When planning for and providing care for older immigrants, it is important to be aware of their exposed situation and to acknowledge their life history and family context.

  • A literature review on fatigue among older people

    • Objective. The aim of the study was to survey existing research about fatigue among older people with chronic non-malignant pain. Method. A structured literature review was carried out using the Medline database. The terms ‘fatigue’ and ‘tiredness’ were used synonymously. These subject headings were combined with pain. A literature search was made in 2004 and included articles back to 1995. Results. The review of studies from the last decade resulted in 11 articles of relevance. The result showed that fatigue was common among older people and even more common among those in pain. However, there was discordance regarding the prevalence, and the figures ranged from 30% to 98%. Fatigue was found to be associated above all with pain, functional limitations/functioning, and low quality of life. No articles were found focusing on interventions against fatigue. Conclusions. Fatigue is common in old age and even more common when people are in pain. However, knowledge seems to be sparse especially regarding interventions in daily care. Thus, more studies are needed that focus on fatigue to get firm evidence about the prevalence among older people in pain. In addition, studies about interventions against fatigue are urgently needed so that the care of older people can include actions against fatigue and to improve quality of life. Relevance to clinical practice. It is of importance that fatigue is not ignored in the care of older people. By intervening against symptoms such as fatigue it will most likely ease the pain and improve quality of life.

    • This focused ethnographic study aimed to illuminate a group of South Africans’ experiences of being old and of care and caring in a transitional period. With a growing number of older people in Africa, studies on the individual experiences may help to develop care which is more sensitively based on the needs for older people in a changing Southern Africa context. Data were collected through group and individual in-depth interviews and participant observations which involved 16 individuals, aged 52–76. Data were analysed using a qualitative content analysis. The study showed two interrelated themes reflections on life and ubuntu – an orientation towards others. Findings were discussed from the viewpoint of the theory of gerotranscendence, showing similarities as well as differences, possibly due to societal and cultural differences. Shortage of formal care for older people living in poor conditions in Southern Africa, gave rise to the discussion for the need of a contextualized development of gerontological care. To enhance knowledge on the theory of gerotranscendence and develop guidelines for nursing in home-based care/community-based care in a South African context may be a first step to support older people in their process towards gerotranscendence.
