  • Exploring the Medical Caregivers' Perceptions of Technology Acceptance for an Online Speech and Language Assessment Application Among Stroke Patients

    • Stroke is a globally increasing disease and speech and language deficiencies are common in stroke survivors. To facilitate medical caregivers in their professional work and to improve patients’ quality of life, technology can play an important role. However, the use and acceptance of technology are uncertain and more research is needed in this direction. This study evaluates the technology acceptance and adoption of an online speech and language assessment application. The evaluation-focused Design Science Research strategy was adopted for that purpose. Two physiotherapists, one occupational therapist and three speech therapists participated in the study. The Unified Theory of Acceptance and Use of Technology (UTAUT) was used as the theoretical base for interview questions formation and data analysis. The study findings show that the suggested application is useful and easy to use; however, it should be better synchronised with speech therapists’ daily work routines. The speech therapists stressed that the functionalities of the application should be designed in close collaboration with them, and it should be compatible with the already existing systems and services in place. Due to impairments after stroke, the patients have some specific preferences for software and hardware; such as a tablet with a touch pen is the preferred hardware. Additionally, the interface should have bigger text fonts and pictures, and highly contrastive colours in the graphics should be used for patients’ convenience. The user’s privacy and security, the patient’s current health, and their previous knowledge and experience with technology were also found important determinants for the intention to use the given technology.

  • Population Generation for Agent-based Simulations of Stroke Logistics Policies : A Case Study of Stroke Patient Mobility

    • For acute medical conditions, for instance strokes, the time until the start of the treatment is a crucial factor to prevent a fatal outcome and to facilitate the recovery of the patient’s health. Hence, the planning and optimization of patient logistics is of high importance to ensure prompt access to healthcare facilities in case of medical emergencies. Computer simulation can be used to investigate the effects of different stroke logistics policies under realistic conditions without jeopardizing the health of the patients. The success of such policies greatly depends on the behavior of the individuals. Hence, agent-based simulation is particularly well-suited as it imitates human behavior and decision-making by means of artificial intelligence, which allows for investigating the effects of policies under different conditions. Agent-based simulation requires the generation of a realistic synthetic population, that adequately represents the population that shall be investigated such that reliable conclusions can be drawn from the simulation results. In this article, we propose a process for generating an artificial population of potential stroke patients that can be used to investigate the effects of stroke logistics policies using agent-based simulation. To illustrate how this process can be applied, we present the results from a case study in the region of Skåne in southern Sweden, where a synthetic population of stroke patients with realistic mobility behavior is simulated.

    • This paper discusses supporting collaborative care of elderly through a reward system based on distributed ledger technologies. The design and implementation of such a reward system that connect elderly and volunteers by mutual agreements involve technologies such as smart contracts and blockchains. The work is motivated by the demographic change, where an aging population consequently increases the need for care. This causes a great tension in our society, as care resources become increasingly constrained, both regarding costs and availability of care staff. Much of the daily care of the elderly is today done by family members (spouses, children) and friends, often on a voluntarily basis, which adds to the tension. The core idea of this work is to help broaden the involvement of people in caring for our elderly, enabled by a system for collaborative care. The proposed system benefits from recent advances in distributed ledger technologies, which similarly to digital currencies, are build on the ability for mutual agreements between people who do not know each other. The system also benefits from recent gamification techniques to motivate people to collaborate on a larger scale through performing simple daily tasks. The proposed system benefits from inherent distributed ledger technologies advantages, such as a high level of decentralization, thus a high availability, and strong data consistency. These advantages make it interesting to develop the possible links between blockchains and the outside world to allow for a higher level of automation and distribution of services such as collaborative care. New models for distributed ledger technologies, such as Iota tangles or the Swirld platform, may however scale and perform better than blockchains. These should thus be considered for a full implementation and test of the system. In summary, this paper presents a novel framework and prototype implementation of a reward system supporting collaborative care of elderly, that is based on distributed ledger technologies.

  • Novel Weight Estimation Analyses and the Development of the Wearable IngVaL System for Monitoring of Health Related Walk Parameters

    • The total amount of lifted weights and lift frequency are moderate to strong risk factors for lower back pain. Measurement of carried weight is thereby of interest. The aim of this paper is to (1) present three novel analyses methods for estimation of weight during walk and (2) to describe the design process of the cost-effective research system IngVaL based on pedobarography. The paper will also (3) present the durability of the sensors. Motivations for choices in the system design are given for hardware, selection of sensor type, sensor implementation and calibration of sensors. To measure weight during walk with IngVaL, fifteen test persons made five walks each with a pseudo-random added extra weight. Three analyses methods were tested, for estimation of weight while walking, resulting in Root Mean Square Errors of 11.3 kg, 7.1 kg and 6.1 kg respectively. The durability of the sensors were tested in an outdoors walking condition. It can be concluded that the IngVaL system shows good durability and that weight during walk is possible to measure with simple analyses methods.

    • —The increase of urban traffic confronts individualsand transport authorities with new challenges regarding trafficmanagement, personal mobility, sustainability, or economicefficiency. Existing resources cannot be arbitrarily extendedwithout negative effects on pollution, costs, or quality of livingand have to be used more efficiently. The SUNSET(Sustainable Social Network Services for Transport) projectaims to improve this situation on a city-wide level bymotivating users on a personal level to change their mobilitybehavior. To make personal mobility more sustainable,flexible, and rewarding for users, SUNSET combines mobilitydata and patterns from mobile sensing, a dynamic incentivesystem, and feedback from social networks. This paperdescribes how the tripzoom application implements thisconceptual approach, discusses critical issues and outlines theforthcoming living lab evaluation in several European cities.

    • E-health has undergone many changes during theyears. One is the development of web-based self-help services forpatients and citizens with health concerns. This article presentsdesign ideas of a web-based stress intervention system that takesthe development of self-help services further. The aim of thesystem is to help people with mild to intermediate levels of stresschange patterns of behavior and take control of their stressful lifesituations. The system includes not only self-help throughindividual work with stress-related issues, but it alsocomplements the individual efforts with help from peers andmedical professionals. The system then becomes a platform forcombining multiple help online. The system design ideas haveresulted from research studies and practical experiences of stressmanagement and web-based self-help systems. The article alsopresents a map of the system structure and a couple of scenariosto illustrate in what ways the system usage can be characterizedby multiple help online and a holistic approach to stressintervention. The article ends with reflections on how to evaluatethis type of intervention system.

  • Making Medication Prognoses for Prostate Cancer Patients by the Application of Linguistic Approaches

    • Apart from the probabilistic model and the model of 2-tuple linguistic representations, a new extension of the fuzzy set, known as the hesitant fuzzy linguistic term set can be seen as the third representative of linguistic approaches. In this paper, we focus on multi-expert decision-making problems, in which a group of physicians are independently asked for assessing the effectiveness of a set of treatment therapies. Our goal is to rank the effectiveness of treatment modalities from the most recommended to the contraindicated. Two individual prostate cancer patients have been taken into account in the practical studies. For the first patient, the probabilistic model and the model of 2-tuple linguistic representations have been adopted to accomplish the medical application. Whereas, for the second patient, the approach of hesitant fuzzy linguistic term set has been used to make the medication prognoses. Moreover, the continuous fuzzy numbers in the Left-Right representations are used to mathematically express the experts’ judgments and s-parametric membership functions are designed to represent the fuzzy linguistic terms.

    • Digitization exceeds the limits of healthcare meetings, which gives renewed relevance to examine the collaboration between engineers and nurses. Caring for people is no longer just something going on in the hospital but at new arenas at home and in the middle of people's everyday lives. In caring situations nurse's responsibility is, unlike the physician, to make observations and to follow in detail the patient's caring needs, and where engineers provide technological devices to support and monitor the course of the disease. When digitizing the caring situation person-centered care gets a new meaning. For engineers the understanding of how technology is contextualized and domesticated becomes even more important to make applications and systems work outside laboratories. This paper presents two cases of interaction between engineers and nurses aimed at improving the implementation of robots and sensorsin elderly people ́s homes; and learning how to improve patient safety in hospitals.The result shows that conflicting epistemologies, differences in professional languages and lack of joint learning opportunities are factors that create obstacles for interactions. The conclusionsreject the idea of linear innovation processes and showthat successful ccollaborationtake more than just adding two and two together. Especially digitization is breaking up traditional barriers and hierarchies. For nurses to be proactive requires knowledge about technological developments and the ability toparticipate in design and innovation processes. For engineers a more thorough understanding of caring situations and users will contribute to a more reliable provision of digitalsolutions and point at new ideas leading up to innovations. The main output of the paper is that it is deepening the understanding of what factors leading to successfulcollaborations between nursing and engineering and what are the missing links.
