  • Learning through networking in healthcare and welfare : The use of a breakthrough collaborative in the Swedish context

    • Breakthrough Collaborative (BC) aims at learning through networking, mainly at micro level, and is used as a tool to improve care and welfare organizations. The aim of this study was to explore and illuminate the challenges when applying BC model at meso and macro level. In 2010, the Swedish Health and Medical Services Act stated the responsibility of healthcare professionals to consider children’s needs as relatives. This study uses an interactive collaborative research model. To support healthcare organizations in the implementation of the regulation, county councils/regions in Sweden were invited to take part in a BC during 2015. Six teams from different county councils/regions participated. Team members were interviewed several times during the project time. Data were analyzed with an explorative and descriptive qualitative content analysis. The result illuminates the challenges faced when applying BC at meso and macro level. Most challenges concern preparation, support structures and system connections. There are similarities with the challenges met at micro level when BC is used at meso and macro level. But it seems even more important to consider how the team is constituted at meso and macro level to make use of the learnings and achieve long-term impact in the home organization.

    • Child health care has an important role to help parents minimize childrens exposure to tobacco smoke. An instrument has together with other supporting routines for staff, been implemented in different ways. The aim was to compare the effectiveness of two implementation strategies, to analyze passive spreading to colleagues and the sustainability to the methods. All CHC nurses in two counties in South East of Sweden were sent a questionnaire about their tobacco preventive work among parents. Three groups of nurses were compared. One group had taken part in a pilot study comprising extensive support and lectures, one group got a lecture at one occasion and further support via mail, and one group got no support or lecture but heard about the project from colleagues. Included in the study was 63%. Nurses in the pilot study still used most of the strategies included. Few nurses who had participated in the less extensive implementation knew about or used the instrument. More nurses who had heard about the project from colleagues knew about and used the strategies. The results indicate that according to the implementation of new routines in health care, more comprehensive methods are needed to change working routines and get sustainable effect.

  • Compliance with guidelines for postoperative pain management in infants and children

    • Background: Postoperative pain is still often being inadequately assessed and/or recorded in infants and young children despite evidence-based guidelines. Objectives: This prospective, observational study in a paediatric postoperative ward at a Swedish university hospital was designed to evaluate interventional effects on pain management by briefly reminding nursing staff of corresponding local guidelines. Methods: Individual structured postoperative information on the first day and night after mainly otorhinolaryngeal or plastic surgery was obtained in 100 pediatric patients from on-site bedside observation protocols, patient records, and telephone interviews over two 5-week periods before and after a study intervention with brief systematic information on local guideline contents. Results: The intervention was followed by significantly more assessments (P = 0.0012), hourly assessments (P < 0.0001), and use of validated tools for assessment (P < 0.0001) of pain intensity in out-hospital patients, but by no change in guardian satisfaction. There were non-significant corresponding changes in in-hospital patients. Conclusions: Bedside compliance with guidelines for postoperative pain management can be considerably improved in out-hospital (and possibly also in-hospital) paediatric patients by a structured brief reminder of existing guideline contents. Larger prospective studies are required to determine the importance of bedside compliance with clinical guidelines for postoperative comfort and safety in infants and children.

    • Countries’ success in managing the Covid-19 pandemic was independent of their level of education or development and institutional quality. Similarly, different provinces in countries performed differently as compared to their neighboring provinces and the country. This study assesses the efficiency of Sweden’s different municipalities in managing the Covid-19 pandemic. Its aim is determining whether the relative efficiency of Sweden’s primary healthcare system during the Covid-19 pandemic was different in different in regions and whether the regions can learn from each other to improve their efficiency in the provision of healthcare in the future. Performance is measured as efficiency in keeping the number of infected cases and deaths low. We use a data envelopment analysis for measuring efficiency. Variations in the level of efficiency are attributed to the differences in the municipalities’ demography, population concentration, economic and industrial structures, labor markets, geographic locations, and national and local policies during the pandemic. The empirical part is based on a panel of the population of 290 Swedish municipalities observed on a weekly basis during April 2020 to February 2021. The results show large variations in municipalities’ performance that can, to some extent, be attributed to their observable socioeconomic, locational, and demographic characteristics.

    • Introduction : Many situations in health and social care require collaboration to ensure seamless and good care with the person in focus. However, collaboration can be a difficult and confusing process. Hence, organizations often support it by developing and implementing models for collaboration.  Aim : This study aimed to explore health and social care professionals’ perceptions of collaboration models, as well as what difficulties they perceived in developing and implementing them in a municipality.  Method : Data was collected in a Swedish municipality, through mainly interviews with participating professionals and analysed using inductive content analysis. Results: The professionals’ responses illustrated variations in perceptions of what constitutes a collaboration model (i.e. core values, a framework, a process, or a function) and why collaboration models are needed (i.e. for the good of the resident, for the good of the organization, or for the good of society). Furthermore, several perceived difficulties in developing and implementing collaboration models were captured.  Conclusion : The varied perceptions of collaboration models, together with the captured difficulties, help explain why models for collaboration are seen as confusing and difficult to develop and implement. This study can increase healthcare managements’ understanding of the complexity of collaboration models and assist future development and implementation.

    • Vaccination is a critical component of preventing infectious diseases and is one of the most effective public health interventions. While social media is an essential source of health and medical information, it is crucial to understand the link between social media involvement and vaccination engagement. This study examines the relationship between COVID-19 vaccination and social media in Iraq. A web-based questionnaire was administered through Google Forms to elicit responses from a total of 2088 Iraqi social media users aged 18 years and over. The findings suggested significant differences in vaccination engagement due to the level of use of social media and demographic backgrounds, particularly age, educational attainment, and income. Active users and young-aged groups with less educational level and income were more likely to have a positive attitude toward vaccination. Future studies should address demographics and the extent of using social media factors in public health studies. Our findings support the use of social media in raising health awareness.

    • Objective: This paper represents an understanding of the ‘usability’ and ‘user experience’ of the TreCLifeStyle mobile app from an end-user experience in order to consider dietary food habit (e.g. Mediterranean Diet).Methods: A qualitative and quantitative approach was undertaken using the system usability scale questionnaire and semi-structured interviews in 2 phases that occurred over a period of 12 consecutive weeks. Six families (n = 12) of Trentino province, Italy, were included who have one obese child as primary inclusion criteria.Results: Incorporating three different aspects, (1) user preference, (2) usability evaluation and (3) system usability scale result; the study explains diversified end-user experiences. The outcome of the study addresses inadequate concentration of user-centered requirements from design to implementation level of a healthcare app. The users’ prioritise the appB (score >68) in terms of friendly interface usability and elaborative contents. Additionally, they identify necessary technological modifications at some point to operate the app seamlessly.Conclusions: The study concludes with a strong demand for end-users’ engagement to design user-friendly mHealth app interfaces where the socio-cultural needs should be considered significant.

  • Attending physicians believe that hospitalized patients are treated at the appropriate level of care : A qualitative study

    • The purpose of this paper is to investigate the appropriateness of hospitalized patients’ level of care as assessed by their treating physicians. The study was conducted as a qualitative study, based on a questionnaire. The study suggests that physicians generally believe that patients who occupy in-hospital beds are cared for at the appropriate level of care. It is worth to note that a relatively large fraction of patients have had their medical needs attended to, but remain hospitalized while waiting for municipal action. Successful downsizing of in-hospital bed-capacity assumes that either a segment of patients who are inappropriately hospitalized exists and could be identified, or that viable alternatives to in-hospital care exist and are available. The results of the present study argue against the first statement. However, we have identified a segment of patients who experience an unnecessarily long in-patient length of stay due to waiting times for municipal action and other post-discharge follow-up measures. Alternatives to in-hospital care which deserve further attention are supporting services in the outpatient setting.
