  • Women's Perceptions of Their Right to Choose the Place of Childbirth : A Qualitative Study

    • OBJECTIVE: To explore Greek Cypriot women's perceptions of their right to choose the place of childbirth.DESIGN: This study is qualitative and is based on Husserl's phenomenological approach. The field work extended over a 6-month period in 2010–2011.SETTING: In all cities of the Republic of Cyprus.PARTICIPANTS: Purposive sample of 55 women within 1 year after birth. Forty-eight women were recruited for semistructured interviews and six of them took place in first focus group. The second group consisted of seven women that did not participate in interviews.RESULTS: Women's perceptions were categorized into four themes: (a) informed choice for birth place, (b) trusting relationship with health professionals, (c) medicalization of childbirth, and (d) safety of the mother and baby.CONCLUSIONS: There is no equity and accessibility in Cyprus maternity care system because it does not provide correct information and accessibility to all birthplace choices. This study demonstrated the need to explore women's views before formulating policy for maternity care. These views will be helpful for the creation of an innovative evidence-based maternity care policy, taking into account women's needs, and will be helpful to raise awareness among health professionals for maternity care improvement.IMPLICATIONS FOR PRACTICE: Ensuring the right for birthplace choices is a social and political necessity that enhances the existing health care systems and health professionals to provide quality and holistic maternity care. Conducting more studies on maternity care in Cyprus will reinforce the aim for improving the health of the women, neonates, and society.

  • Factors associated with a positive birth experience : An exploration of Swedish women‘s experiences

    • BACKGROUND: The birth of a baby is a powerful life event that has implications for a woman's wellbeing and future health. A positive birth experience promotes a sense of achievement, enhances feeling of self-worth, and facilitates confidence—all of which are important for a healthy adaptation to motherhood and psychological growth. Understanding what constitutes a positive birth experience is critical to providing maternity care that meets childbearing women's individual needs, preferences, and priorities. OBJECTIVE: To explore the prevalence of Swedish women reporting a very positive birth experience 2 months and 1 year after childbirth and identify factors associated with this experience. In addition, the study aimed to identify whether women's assessment of their birth experience changed over time. METHOD: A prospective, longitudinal study where the main outcome variable was perceptions of a very positive birth experience. The study was undertaken in a Northern region of Sweden in 2007. Women were recruited at their ultrasound examination in midpregnancy. Data was collected via questionnaires. There were 928 women who responded to questions about their birth experience at 2 months postpartum. Nearly 83% of these women (n = 763) also completed the questionnaire package 1 year after birth. Descriptive statistics were used together with Friedman's test to detect changes over time. Logistic regression analysis was performed to reveal which factors contributed most to a very positive birth experience. RESULT: More than a third of the women reported a very positive birth experience. Women's assessment of birth changed over time with 22% of the women becoming more positive and 15% more negative. Important associated factors for a very positive birth experience included positive feelings about the approaching birth as well as feeling in control, using no or only cognitive forms of pain management, and achieving a spontaneous vaginal birth. Furthermore, how women rated their midwifery care was also shown to affect their assessment of their birth experience. CONCLUSION: This study found that women's birth experiences changed over time and most becoming more positive after 1 year. Factors associated with a very positive birth experience were related to women's prenatal attitudes, intrapartum procedures, pain relief used, and care received during labor and birth. Respectful individualized midwifery care that remains focused on the woman and keeping birth normal increases positive perceptions of the birth experience.

  • First-Time Parents’ Perception of Antenatal Education in Sweden

    • Antenatal parent education is one part of the parenthood support program and is offered to all firsttime parents in Sweden. The aim was to explore first-time parents' experiences of antenatal education and its helpfulness in preparation for birth and early parenthood. Eight hundred ten first-time parents completed questionnaires. The antenatal education was a positive experience for 56% and 62% of women and men, respectively. Antenatal education was helpful during birth for 68% of women and 63% of men. Forty-seven percent of women and 44% of men found it helpful for early parenthood. High level of education was the only background variable associated with being satisfied with antenatal education. Satisfaction was lower than reported in previous studies and indicates room for change and improvement in antenatal parent education.

    • BACKGROUND: Caregivers need to better understand women's experiences of support during childbirth because research suggests that social support positively influences childbirth. AIM: This study describes women's experiences of support given by caregivers during pregnancy and childbirth. METHOD: The study design was inspired by grounded theory. Seven interviews of women were analyzed with an open coding, and different time-related categories related to the childbirth process emerged. The categories were marked by fear and a negative birth experience, being guided on own terms, feel supported, and transformation into courage to give birth. The analysis continued with a selective coding, reflecting the process of mistrust to trust in caregivers. FINDINGS: The mistrust in caregivers began with feelings of fear of birth and a negative birth experience. Through being guided on own terms and feeling supported by the caregivers, a trusting relationship could be established. If the trusting relationship continued during labor, then a woman could transform the fear of birth into the courage to give birth. CONCLUSIONS: Women's experience of support can be seen as a product of earlier experiences from interactions with caregivers. Therefore, caregivers must be sensitive to the potential power and far-reaching consequences their actions can have.

    • AIM: Perinatal death is often regarded as a critical incident for the healthcare personnel involved. How healthcare personnel respond to traumatic events in their work is a function of their level of awareness or exposure to the incident, as well as their genuine expectations, support, and trust. The aim of this study was to explore coping strategies of Norwegian healthcare professionals including midwifes, obstetricians, and assistant nurses when faced with perinatal death in a clinical setting. METHOD: Midwives, obstetricians, and assistant nurses in two public hospitals in Norway participated in an in-depth and semi-structured interview. The data was analyzed using Kvale's approach, which involves condensing and thematic analysis. FINDINGS: The results are divided into three categories with eight subcategories. Having the support of one's colleagues was described as an important factor for coping with these situations. Both immediate support in the situation and talking about their feelings later with colleagues were found to be important. Discussing and sharing responsibility were also mentioned. CONCLUSION: Informants in this study stated that talking with one another about the challenges they faced in these situations was important. Appropriate education and training programs, together with healthy coping and debriefing strategies must be implemented in maternity units.

    • BACKGROUND: In antenatal care, health checkups, information, and support is provided to women. Midwifery continuity models of care provide women access to evidence-based care. OBJECTIVE: The aim of this study was to evaluate women's experiences of pregnancy and antenatal care in a continuity of midwifery care project, as well as to gauge the impact the project had on the women's emotional well-being and satisfaction. METHOD: This was a cohort study of 226 women enrolled in a continuity of care project in rural Sweden. Profiles of the women were created based on levels of depressive symptoms, worries, fear of birth, and sense of coherence. Data was collected through questionnaires. Odds ratios with 95% confidence intervals were calculated between the clusters for the explanatory variables. RESULT: Women in the two clusters differed in some background characteristics. Women in Cluster 2 had more visits to a doctor. They also received more counseling due to fear of birth and viewed the number of midwives as "too many" They were less satisfied with the medical, emotional, and overall aspects of their antenatal care. Perceived health, preparedness for birth, and parenthood were rated lower by women in Cluster 2. CONCLUSION: This study found that women's assessment of their antenatal care was associated with their emotional health. Negative feelings toward changes in pregnancy were often found in women with poorer emotional health, and these women reported being less prepared for birth and parenthood. Thus, it is important to identify women with emotional distress and to provide them additional support and continuity.

    • Women's experiences of childbirth can be both empowering and traumatic, and negative experiences might lead to fear of forthcoming births. Our aim with this phenomenological study was to describe the meaning of fear of childbirth and of birth drawing on women's long-term perspectives. Six women who had sought help for intense fear of childbirth because of a negative birth experience during their second or third pregnancy 7-11 years ago were interviewed. The essential structure of the phenomenon is "an effort to make all the pieces come together" with the constituents: Every childbirth is a narrative that lingers on, fear of childbirth is connected to the period of childbearing, and the experiences of childbirth are central life experiences. Our findings suggest that fear of childbirth and childbirth experiences have been linked together and integrated as important life experiences. It seems like the meaning of the childbirth experiences can be expressed by women in several nuances and tones, indicating the complexity of the experience. The fear of childbirth has lost its importance and is connected to the previous difficult childbirth experience. It is important to understand women's fear of childbirth and birth experiences with respect to individual needs.
