  • Even a single positive blood culture may matter- a case of prosthetic valve infective endocarditis caused by Corynebacterium kroppenstedtii

    • Corynebacterium is a skin commensal bacterium that can contaminate blood cultures. It is however also a rare cause of infective endocarditis (IE). Here we report a case of Corynebacterium kroppenstedtii aortic prosthesis IE in a 76-year-old man where only a single blood culture bottle was positive initially. C. kroppenstedtii is a very rare cause of IE, only reported two times previously. The diagnosis in our case was confirmed by repeated blood culture positivity and eventually by detection of DNA from C. kroppenstedtii on heart valves after valve exchange surgery. At surgery an aortic root abscess was detected and the valve was replaced by a homograft. Recovery was complicated by antibiotic-induced nephrotoxicity and treatment was concluded with moxifloxacin in combination with rifampicin. Recovery was uneventful. This case demonstrates that even a single blood culture bottle may be important in patients with prosthetic heart valves.

  • Perigenital necrotizing soft tissue infection caused by Aerococcus urinae

    • Aerococcus urinae can cause severe invasive infections emanating from the urinary tract especially in older males with comorbidities. Here we describe a case of an 80 year-old man with multiple comorbidities presenting with a peri-penile abscess and signs of severe infection. Upon incision A. urinae was isolated in pure culture and the infection was cured by a combination of drainage and antimicrobials. This case demonstrates the potential of A. urinae to cause severe soft tissue infections in predisposed individuals.

  • Management of dengue hemorrhagic fever in a secondary level hospital in Bangladesh: A case report

    • Dengue is an important tropical infection caused by an arbovirus. As a mosquito borne infection, this disease is widely spread in several tropical endemic countries and this implies the global importance of this infection. In this specific case report, the author discussed the case management of dengue hemorrhagic fever (DHF). A 42-year-old patient came to a secondary level hospital with complaints of diffuse abdominal pain (more in central region) continually for 3 days. Based on his clinical investigations the patient was diagnosed by DHF and managed with intravenously administered fluid resuscitation as he had a history of vomiting, close monitoring of vital status, and gave conservative treatment. Although, the plasma leakage had concerned the doctors about developing DSS. But after seeing his blood report, when the doctors found that the patients platelet count was raised gradually and no other associated signs then they decided to give him discharge from the hospital. Prevention and control of dengue and DHF has become more urgent and the available vaccine is still limited. Hence, effective disease prevention programs, education of the medical community to ensure effective case management, community-based integrated mosquito control are necessary.   • Fatal infective endocarditis caused by Aerococcus sanguinicola : a case report and literature review

    • Aerococcus sanguinicola is a bacterium that can cause urinary tract infections and on rare occasions infective endocarditis (IE). The prognosis of IE caused by aerococci is generally favourable despite that the patients are typically old and have multiple comorbidities. Here we report a case of A. sanguinicola native valve aortic IE in a 68-year-old man with an underlying urinary tract condition. The infection led to severe aortic valve insufficiency and rapid death before the patient could be subjected to surgery. This demonstrates that IE caused by A. sanguinicola can be severe and cause valve destruction. In addition to the case report, we provide a review of the current literature on A. sanguinicola IE.

  • Capnocytophaga canimorsus tricuspid valve endocarditis

    • Capnocytophaga canimorsus is an uncommon cause of infective endocarditis (IE) and mainly affects persons with compromised immune-systems who have been in contact with dogs. We describe a case of C. canimorsus tricuspid valve IE in a 70 year-old dog-owner where diagnosis and treatment were delayed. The reason for the delayed diagnosis in this case was likely due to that initial blood cultures were negative due to preceding antibiotic treatment, discrepancies between echocardiographic investigations, and a thymoma and colonic polyps which were thought to explain the symptoms. A multi-diciplinary approach in cases with suspected IE might help to avoid diagnostic delays.

  • Treatment refractory arthritis and stroke – A case of infective endocarditis caused by Tropheryma whipplei

    • Whipple´s disease is a rare multisystem condition affecting < 1/1.000.000 per year. The condition often presents with polyarthritis, diarrhea, and intestinal malabsorption. Endocarditis is seen in a minority of these patients, and is typically culture negative, as the causative agent Tropheryma whipplei does not grow in ordinary culture media. We present the case of a 78-year-old man with a history of seronegative polyarthritis that was refractory to treatment with several biological agents for a duration of 5 years prior to presentation to the emergency department with stroke. Echocardiography revealed aortic valve endocarditis with a 3.6 cm vegetation and multiple smaller vegetations. The patient underwent surgery with aortic valve replacement followed by prolonged antibiotic treatment. 16 S rDNA PCR analysis of the resected valve revealed T. whipplei as the causative agent. Two years after surgery and treatment with antibiotics, the patient's previously longstanding arthritis had totally disappeared and all rheumatological treatment had been discontinued.

    • Mycoplasma pneumoniae (M. pneumoniae) is a common cause of community acquired pneumonia and although most cases are mild, complications sometimes occur. Cold agglutinin hemolysis is a known complication of M. pneumoniae infection, and usually presents as a mild and transient hemolysis. Here we present a case of infection with M. pneumoniae in a 64-year-old male that caused life threatening hemolysis that required multiple blood transfusions. The patient also presented with acute kidney failure and a marked leukemoid reaction and thrombocytosis. This is a very rare combination of symptoms that could have led the clinicians to suspect a more virulent etiology than M. pneumoniae, thereby delaying adequate antibiotic treatment.
