    • Ectopic thyroid is a rare condition most often found at the base of the tongue, lingual thyroid (LT). The majority of patients are asymptomatic. Recently, transoral robotic surgery (TORS) has emerged as an option for definitive treatment. Here, we present a 20-year-old patient with symptomatic LT, treated with TORS-assisted surgery without adverse events and with depletion of symptoms. We advocate TORS as a very promising means to be considered when encountering the rare condition of symptomatic LT.

    • The anatomical position of the facial nerve is a critical factor in determining surgical candidacy in patients with congenital aural atresia (CAA). All patients with CAA must preoperatively be evaluated using a grading score based on information gained from a high resolution CT scan. In patients not suitable for surgical reconstruction, implantation of novel hearing implants is increasingly used for hearing rehabilitation. We, here, describe a bifurcation of the intratemporal part of the facial nerve in a 5-year old boy with CAA undergoing implantation with a bone conductive hearing device.

    • PANDAS is an acronym for paediatric autoimmune neuropsychiatric disorders associated with streptococcal infections and is defined as a subgroup of PANS, Paediatric acute-onset neuropsychiatric syndrome. PANDAS is considered an autoimmune disease based on a streptococcal infection with a verified immune response. The onset is often dramatic, with symptoms of OCD developing within 24-48 h. Symptoms include both psychological symptoms and physical symptoms. Treatments of PANDAS must address both physical and psychiatric symptoms. Supplemental treatments for PANDAS include tonsillectomy to prevent further streptococcal infections. We present a case of PANDAS with the aim of raising awareness of its diagnosis. PANDAS is a rare condition with an impact on the patients quality of life; thus, awareness is needed. A multidisciplinary team is needed for diagnosis and treatment. Otorhinolaryngologists may play an important role in the well-being of these patients when performing tonsillectomy.

  • Auditory brainstem implant pitch discrimination and auditory outcome

    • We present a pitch discrimination test performed by five experienced adult auditory brainstem implant (ABI) users with neurofibromatosis type 2 (NF2). The ability to discriminate frequency/pitch from different channels on the implant may be an important factor in improving speech performance. The pitch discrimination ability was evaluated by using a triangle test compared to adjacent contacts and the speech perception was measured by the Swedish three-digit test. The test was easy to perform, and all patients were able to answer reliably, even though it cannot be ruled out that patients used attributes other than pitch to differentiate between sounds. Due to the limited number of patients and small variation in results, no conclusive correlations could be made regarding pitch discrimination and auditory outcome. There was a tendency for poorer ability to discriminate pitch (discrimination of tonotopically adjacent electrodes) at testing to result in poorer speech results.

    • We present a case of a male with a history of repeated surgeries for a cerebellar astrocytoma, leading to profound deafness and facial paresis on the left side. A ventriculoperitoneal (VP) shunt was inserted to manage hydrocephalus. At the age of 46 the hearing suddenly disappeared on the right side, where he received a cochlear implant (CI). At implant activation, impedances showed atypical high values. One month later impedance levels had further increased and the patient had no benefit from the CI. A computed tomography (CT) scan suggested air in the cochlea (pneumolabyrinth). The shunt was adjusted to elevate the cerebrospinal fluid (CSF) pressure and the pressure in the perilymph to prevent air from entering the cochlea via the round window. One year after activation the electrode impedances were normal and the hearing outcome from the implant was successful.

    • Diagnostic mediastinoscopy is a procedure with well-known serious complications: Hemorrhage, mediastinitis, pneumothorax and recurrent nerve damage. Esophageal perforation is a less known potentially life-threatening complication. Here the case of a young man with an iatrogenic esophageal perforation following a diagnostic mediastinoscopy is presented with a literary review of previously published cases.

    • Endoscopic balloon dilation of tracheal stenosis is usually a safe procedure. However, there are life-threatening complications that physicians performing the procedure need to be aware of. A 43-year old woman with a multi-level tracheal stenosis following lengthy intubation and a tracheostomy was treated with endoscopic balloon dilation. This resulted in an almost total rupture of the posterior tracheal wall. Here a safe and successful conservative treatment approach is reported.

    • The study aimed at describing a case of auditory brainstem implant (ABI) paediatric re-implantation performed at the Akademiska University Hospital, Sweden. The patient was a boy with Goldenhar syndrome with absent vestibular-cochlear nerves and was first implanted with an ABI in 2009 at the age of two years. A technical device failure in 2015 led to a re-implantation at the age of nine years. The ABI was successfully re-implanted although the implant was closely attached to the surrounding tissue and difficult to remove. The intraoperative electrical auditory brainstem measures (eABRs) gave unclear responses after re-implantation. After 12 months, the patient's hearing thresholds was not as good as it was after the primary implant, but it is still developing. The child is a full-time user. ABI re-implantation is possible even after many years, although there is a risk that the implant might be fixed to the brainstem and difficult to remove.

  • Ectopic tonsil in the floor of the mouth : A case report

    • Tonsil tissue not located at known anatomical locations at the entry of aero-digestive system is called ectopic tonsil. This is rarely presented clinically and may lead to diagnostic confusion when encountered. The diagnosis of ectopic tonsil is challenging as it is usually asymptomatic and requires histopathological examination for definitive diagnosis. A case of patient with sublingual ectopic tonsillar tissue is described here. Otolaryngologists colleagues are encouraged to pay attention to this entity and consider it in the differential diagnosis of an unclear lump in the head and neck region especially, in the oral cavity.

  • Intratonsillar abscess in patients with COVID-19 : Two case reports

    • COVID-19 was first reported of in December of 2019 and has since developed into a global pandemic disease. An intratonsillar abscess is a rarely encountered diagnosis and the clinical presentation can mimic that of a peritonsillar abscess. Here, we report on two cases of intratonsillar abscess and concurrent COVID-19 infection.Case 1: A 19-year old male presented with a sore throat, right-sided neck pain and difficulty swallowing. Needle aspiration of a peritonsillar bulging was negative for pus. Computed tomography showed an intratonsillar abscess. Intratonsillar needle aspiration was again negative for pus. The patient was treated with antibiotics and his condition improved without surgical intervention. Case 2: A 43-year-old female presented with a sore throat, difficulty swallowing, fever and myalgia. Examination showed peritonsillar swelling on the right side. Computed tomography showed an intratonsillar abscess on the right side. Intratonsillar needle aspiration was positive for pus. The patient was treated with antibiotics and her condition improved.To our knowledge, this is the first publication to report the development of intratonsillar abscess in patients with COVID-19.
