  • Using quality improvement methods to implement guidelines to decrease the proportion of urinary retention in orthopaedic care

    • In patients treated with indwelling urethral catheter (IUC), complications such as catheter associated urinary tract infections are common, while underuse of IUC may cause harmful urinary retention (UR). A quality improvement (QI) program called ‘Indwelling urethral catheter (IUC) - only when needed’ was developed in Jönköping County Council, Sweden, aiming at creating a new approach: hospital staff should be able to identify and manage patients with risk of UR, prevent UR or treat UR without delay, and only use urinary IUCs on appropriate indications. The aim of this study was to describe the process of application of the quality improvement program. The Model for Improvement was used, and process coaches were appointed in the participating units. Their training was based on clinical issues and facts about UR, IUCs, guidelines, QI methods and measurements. Data were collected through prospective and retrospective patient record reviews, and differences were analyzed by inferential statistics.Before the intervention, only two patients out of 296 were cared for following the guidelines perfectly. During the intervention, adherence to guidelines showed a rising trend, and reached a new stable level, with an average of 67% adherence to guidelines. A systematic improvement program supported by coaches and improvement tools can increase the adherence to new guidelines and incorporate them into local practice. This study also shows that adherence to guidelines can improve patient safety, in this case a decreased risk for and incidence of UR in an orthopaedic patient population.

  • Working Through Others - A Qualitative Content Analysis of Nursing Students' Experience with Malnutrition at One Hospital in Vietnam

    • Abstract Background Malnutrition causes problems in Vietnam, including illnesses with physiological and psychological complications. The aim of this study was to describe nursing students' experiences with malnutrition at one hospital in Vietnam. Methods Interviews of eight nursing students were performed, and the results were analysed by qualitative content analysis. Findings The results are presented in one category (working through others) and three subcategories (awareness of malnutrition, developing family-centered care and developing health promotion). Conclusion This study highlights the use of guidelines in collaboration (between staff and patients' relatives) to facilitate early identification and/or prevent malnutrition by improving self-care, as supported through nursing activities using family-centred care as well as teamwork.

    • Aim The aim of the study is to describe nurses' experiences of working with health promotion and prevention of cardiovascular diseases related to smoking in Indonesia. Methods A phenomenographic approach was used. Semi-structured interviews were conducted with nurses in health care centers and hospitals in Yogyakarta. The interviews were analysed according to Alexandersson's model. Results Three themes emerged from the analysis: Conceptions in relation to the patient and their family, conceptions in relation to the hospital and health care centers and conceptions in relation to the culture and policies. All nurses are working with health promotion and prevention of cardiovascular diseases related to smoking in one way or another. However, distinct guidelines and resources are missing. The nurses also mentioned the need of training in smoking cessation to better work with health promotion and prevention of cardiovascular diseases with a focus on smoking. Conclusion The difficulties rise from lack of resources in the health care facilities, the patients' lack of motivation and weak policies from the government. For the situation to be improved the nurses need guidelines, training and more time to work with health promotion and prevention. What this study adds: Nurses stated that guidelines are not available to work with health promotion and smoking cessation in Indonesia. Nurses in Indonesia state they need training or retraining regarding the subject to be able to give good and proper education in tobacco cessation for cardiovascular ill patients. Smoking is considered a tradition in Indonesia and the environment is not supportive for the ones who want to quit. Even though, involving the family shows to be a successful method for smoking cessation. The implications of this paper: Provide training in smoking cessation and in communication for Indonesian nurses to feel more secure when motivating the patients to quit smoking. Establish specific guidelines to help and support the nurses in their work with cardiovascular diseases related to smoking. Involving family is a successful approach that nurses can use in their work with smoking cessation which other studies also indicated.

  • Managing Uncertainty in Nursing – Newly Graduated Nurses’ Experiences of Introduction to Nursing Profession

    • Background The introduction to nursing, which is the conversion of theoretical knowledge into practice skills in complex healthcare settings, is emphasized as difficult due to patient safety. Therefore, the aim of this study was to describe newly graduated nurses' experiences of introduction to nursing in a medical department (medical, emergency) at a university hospital in western Sweden. Method A manifest qualitative content analysis with an inductive approach was used based on five semistructured interviews with newly graduated nurses who work in a medical department at Sahlgrenska University Hospital in Gothenburg, Sweden. Findings One main category was found, managing uncertainty in the nursing profession, which includes three subcategories, namely, being humble as a newly graduated nurse, being adaptable as a newly graduated nurse and being a staff member instead of a nursing student. This category and these subcategories are described in the results. Conclusion The transition from education to registered nurse is a difficult step due to the anxiety that stems from making mistakes regarding limited experience related to the demands and requirements of the job. Therefore, standardized (routines, guidelines) and individualized (experiences) introductions facilitate patient safety. Moreover, person-centeredness ensures that every nurse is a capable person with resources to use in health care, i.e. a partnership within a healthy working environment that transforms novices to experts in nursing.

  • The Impact of Self-Efficacy and Depression on Self-Care in Patients with Heart Failure: An Integrative Review

    • Background: Although the importance of self-efficacy and depression are mapped out in previous research; there is a lack of clarity about associations between self-efficacy and depression to self-care, and potential implications for nursing care. The purpose of this review was to identify the association between self-efficacy and depression and how it affects self-care in the HF population. Methods: The study was carried out by conducting a library search in the CINAHL, PubMed, PsycINFO and Cochrane databases from January 2000 - May 2017 yielded 24 studies. Results: The analysis of these articles showed that several studies documented an inverse association between self-efficacy and depression in the context of self-care. Experimental studies also showed that interventions aimed at increasing self-efficacy and treating depression could encourage self-care practices. Conclusions: The results suggest that a more holistic view of self-care recommendations that incorporates self-efficacy and depression is of potential value in the future treatment of HF patients.

  • Managing a stressful work environment through improved teamwork - a qualitative content analysis of nurses working environment within emergency care

    • Aim: The aim of this study was to describe nurses’ experiences of their working environment in emergency departments at a general hospital in Manila, Philippines. Background: The working environment within emergency care is complex and unpredictable and may influence patient safety. Nurses are challenged by increased patient flow, staff shortages and heavy workload. Methods: This study used a qualitative content analysis with an inductive approach based on semi-structured interviews with nine nurses at emergency departments. Results: One category, improved teamwork toward a healthy working environment, which included two subcategories, barriers to providing a high quality of care and managing a stressful work environment, describe nurses’ experiences with their work environment at emergency departments in the Philippines. Conclusion: Workload manifests through a high patient ratio and patient safety-affected prioritizations of patients, which is why basic nursing is limited. Therefore, improved teamwork built upon person-centered care is needed to increase the high quality of care as well as a healthy working environment.

  • Drama, Leadership and Conflict Management to Support Nurse’s Professional Development - A Qualitative Study in Indonesia

    • Abstract Background A need exists for developing training interventions for nurses regarding conflict management and leadership to facilitate professional development. Therefore, we aimed to explore and describe the use of applied drama to support professional development with focus on nursing leadership and conflict management of nurses (practitioner, lecturer) in Indonesia. Method A descriptive qualitative method was performed using four scenarios (forum play) with 19 participants (nurses) and was analysed by qualitative content analysis. Findings The data were divided into two parts: 1) A description of four scenarios and 2) Four themes - sharing different perspectives, developing self-reflection, understanding the role of emotions in the communication and managing sensitivity and developing empathy. Conclusion Nursing leadership and conflict management skills play a key role in developing professional development in nursing. Drama, using forum play as experiential learning, improve nurse's knowledge and skills in holding a leading role to ensure a high quality of care within a satisfied working environment.
