    • Sports provide physical, psychological, and social well-being to individuals. However, there may be a potentially damaging effect on articular cartilage with mid- and long-term exposure to sports. This chapter is aimed to critically review the existing literature regarding the association between sports participation and ankle osteoarthritis. This review demonstrates that in general, there is not enough evidence to conclude that sports increase the risk of ankle osteoarthritis. The existing literature related to this topic is scarce, and most of the studies have not controlled the risk analysis for the presence of other potentially associated risk factors for ankle osteoarthritis. Further research is needed on this topic with prospective comparative studies controlling for, at least, age, sex, body mass index, occupational workload, and previous ankle injuries. © Springer-Verlag Berlin Heidelberg 2012, 2015, All Rights Reserved.

    • Sports provide physical, psychological, and social well-being to individuals. However, some researchers have suggested a potentially damaging effect on hip articular cartilage with mid- and long-term exposure to sports. The purpose of this chapter is to provide a comprehensive and critical review of the existing studies investigating the relationship between sports and the risk of hip osteoarthritis. This review demonstrates that despite there is some evidence that sports increases the risk of hip osteoarthritis, many studies have not controlled for some of the important risk factors for hip osteoarthritis. In fact, when only considering studies controlling for many associated risk factors in the risk analysis, most studies found that sports did not increase the risk of hip osteoarthritis. Therefore, it cannot be clearly concluded that sports itself increases the risk of degenerative joint disease in the hip when other risk factors are controlled for. Sports can be generally recommended to all patients, but special care must be taken in those with associated risk factors. © Springer-Verlag Berlin Heidelberg 2012, 2015, All Rights Reserved.

    • This chapter presents a nonanatomical double-bundle ACL reconstruction technique. The reconstruction is performed using hamstring tendons that are harvested, maintaining intact the tibial insertion. The anteromedial bundle is restored placing the graft in the “over-the-top position, while the posterolateral bundle is replaced retrieving the graft from a femoral tunnel. Graft fixation is obtained using metal staples. Hundreds of patients have been treated with this technique during the last 10 years, including athletes with high functional requests. Furthermore, in vivo analysis of knee kinematic confirmed the effectiveness of the technique. © Springer-Verlag Berlin Heidelberg 2012, 2015, All Rights Reserved.

    • Acute hamstring muscle injury is the most frequent cause of acute posterior thigh pain, but referred pain to this area is also common. This chapter will focus only on acute hamstring injuries that are detectable on MRI (structural hamstring injuries). Hamstring injuries remain a challenge for clinicians, athletes, and coaches given the high incidence rate and often with persistent symptoms. The primary objective of all rehabilitation protocols is to return the athlete as soon as possible to the prior level of performance with a minimal risk of injury recurrence. The aim of this chapter is to describe the management of acute hamstring injuries including clinical examination, rehabilitation exercises, and return-to-sport criteria.

    • Achilles tendon rupture occurs relatively common (De Jonge, Br J Sports Med 45:1026-1028, 2011). Although it is the thickest and strongest tendon in the human body, it remains susceptible to injury. During the last decades, the incidence of spontaneous ruptures has been rising, which may be due to the increasing keep-fit culture. Ruptures occur most frequently in patients between the age of 30 and 50 years old, with a male predominance. Approximately 75 % of Achilles tendon ruptures occur during sports activities, especially racket games, soccer, and handball. Diagnosis is primarily clinical. However, there is still a lack of consensus on the best management of the Achilles tendon rupture. Generally, open surgical management is advocated, although over the past few years, percutaneous techniques are performed more commonly. Nonsurgical management appears to be a good alternative for those with comorbidity or patients who do not wish to have surgery. Recent systematic reviews have concluded that operative management has a lower re-rupture rate but must be balanced by the risks associated with surgery (Khan and Carey Smith, Cochrane Database Syst Rev CD003674, 2010). © Springer-Verlag Berlin Heidelberg 2012, 2015, All Rights Reserved.

    • Hamstring muscle injuries are one of the most common injuries in sports. Despite a considerable scientific effort to better understand these injuries and offer effective preventive programs, the incidence and reinjury rates have not improved over the last decades. The implementation of adequate prevention programs for muscle injuries depends on a thorough understanding of mechanisms of injury and risk factors for these injuries. The purpose of this chapter is to provide a comprehensive, critical, and up-to-date review of the literature regarding the influence of mechanisms of injury and risk factors for the implementation of prevention programs for hamstring muscle injuries in sports. Research in hamstring muscle injuries has to evolve into studies with high methodological quality and abandon the classical reductionist model in which a linear and unidirectional causal-effect model between a risk factor and its consequent injury is conceived. © Springer-Verlag Berlin Heidelberg 2012, 2015, All Rights Reserved.

    • The goal of the rehabilitation after an anterior cruciate ligament reconstruction is to assure that the athlete can return to sports and avoid reinjury in the short term and that there is minimal risk for other injuries in the long term. The recommendation in the current literature is to implement a well-planned, individualized, and criteria-based rehabilitation program as soon as possible, preferably on the same day as surgery. The rehabilitation has to be based on knowledge about the healing process after reconstruction, about the effect of rehabilitation techniques and exercise on tissue, and about the short- and long-term consequences of the injury. It has also to be individualized to meet each athlete's personal physiological and psychological needs regarding the extent of the injury and sports-related demands on the knee. The structured rehabilitation model that is presented in this chapter consists of four rehab phases, which take the patient and the phases of the tissue healing process into consideration. © Springer-Verlag Berlin Heidelberg 2012, 2015, All Rights Reserved.

    • Pain from the lateral and the medial aspect of the elbow is a common symptom in sports, other recreational activities, or work. In tennis elbow, the pain is elicited from the common extensor origin just distal of the lateral epicondyle, while in golfer's elbow, the pain is elicited from the flexor-pronator mass just distal to the medial epicondyle. The main symptoms are pain from the epicondyles of the elbow and are related to activity level. The pathogenesis is not known, but these conditions are considered to be overuse injuries of degenerative nature. The diagnoses are verified by clinical examination. Further investigation is not necessary with typical history and clinical findings, and the conditions are self-limiting with good prognosis. In the literature, the golden standard for treatment is conservative treatment although the support in the literature is not conclusive. No treatment has convincing evidence for a faster recovery even though there are, of course, methods for reducing pain symptoms. When the pain is under control, it is important that the return to activity is gradual.
