    • Bose-Einstein correlations between identified charged pions are measured for p+Pb collisions at root s(NN) = 5.02 TeV using data recorded by the ATLAS detector at the CERN Large Hadron Collider corresponding to a total integrated luminosity of 28 nb(-1). Pions are identified using ionization energy loss measured in the pixel detector. Two-particle correlation functions and the extracted source radii are presented as a function of collision centrality as well as the average transverse momentum (k(T)) and rapidity (y*(pi pi)) of the pair. Pairs are selected with a rapidity -2 < y*(pi pi) < 1 and with an average transverse momentum 0.1 < k(T) < 0.8 GeV. The effect of jet fragmentation on the two-particle correlation function is studied, and a method using opposite-charge pair data to constrain its contributions to the measured correlations is described. The measured source sizes are substantially larger in more central collisions and are observed to decrease with increasing pair k(T). A correlation of the radii with the local charged-particle density is demonstrated. The scaling of the extracted radii with the mean number of participating nucleons is also used to compare a selection of initial-geometry models. The cross term R-ol is measured as a function of rapidity, and a nonzero value is observed with 5.1 sigma combined significance for -1 < y*pi pi < 1 in the most central events.

    • Ratios of top-quark pair to Z-boson cross sections measured from proton-proton-collisions at the LHC centre-of-mass energies of root S = 13 TeV, 8 TeV, and 7 TeV are presented by the ATLAS Collaboration. Single ratios, at a given root S for the two processes and at different root S , for each process, as well as double ratios of the two processes at different root S , are evaluated. The ratios are constructed using previously published ATLAS measurements of the t (t) over bar and Z-boson production cross sections, corrected to a common phase space where required, and a new analysis of Z -> l(+)l(-) where l = e, mu at root S = 13 TeV performed with data collected in 2015 with an integrated luminosity of 3.2 fb(-1). Correlations of systematic uncertainties are taken into account when evaluating the uncertainties in the ratios. The correlation model is also used to evaluate the combined cross section of the Z -> e (+) e (-) and the Z -> mu (+) mu (-) channels for each value. The results are compared to calculations performed at next-to-next-to-leading-order accuracy using recent sets of parton distribution functions. The data demonstrate significant power to constrain the gluon distribution function for the Bjorken-x values near 0.1 and the light-quark sea for x < 0.02.

  • Perimetric probability maps to separate change caused by glaucoma from that caused by cataract

    • We describe a new method for analysis of change in glaucomatous visual fields with the object to differentiate between changes caused by glaucoma from those caused by cataract, New pattern deviation change probability maps were developed from a prospectively collected glaucoma material and designed to be sensitive to changes in localized field loss, but to be unaffected by media-induced perimetric change. We compared the new change probability maps with the commercially available total deviation change probability maps in series of Humphrey perimetric tests in a glaucoma material of 43 eyes of 35 patients, who had undergone cataract surgery, When using the total deviation maps, considerable differences were seen between fields obtained before and after cataract surgery. Much smaller differences were seen when using the new change probability maps, that almost eliminated the common and disturbing effect of increasing cataract, This new tool could be of considerable help in differentiation between progressive glaucomatous visual field loss and deterioration caused by increasing media opacities.

    • Graves' orbitopathy is a common complication of Graves' disease. The mild form dominates, whereas moderate to severe and sight threatening forms are rarer. They require quick, adequate care, involving cooperation between county hospitals and the regional hospital, and adequate diagnostics and choice of treatments of treatments. A new national guideline for hyperthyroidism has been published in Sweden in January 2023, where these aspects are highlighted. The present guideline requires implementation regionally, as all components are not in place, but reflects the needs of patients. This article highlights important aspects for patients such as information, to be listened to, accessibility and adequate care, but also defines actions necessary in primary care where patients most often seek help at first. Lastly, important medical and practical aspects are reviewed by the endocrinologist and the ophthalmologist.

  • Creating a system for Quality and Progress – The Purposeful Need for Technology?

    • The use of a learning management/assessment system has become common in higher education during the past 20 years. However, such systems are frequently not grounded in approaches to support longitudinal educational needs, such as promoting professional development. One reason may be that digital support is designed to assist with isolated tasks e.g. running MCQ tests, and therefore is not developed so cannot support integrated educational needs such as longitudinal learner development. However, for assessment and feedback to be utilized both synergistically and longitudinally, efficient holistic support systems are required.Our aim was to use self-determination theory to define and implement a shared purposeful digitalization plan to support longitudinal learner development in the medical and health science educational programs at our university.Summary of workIn an action driven process during 2016-2017, teachers from thirteen educational programs and student representatives were invited to participate in workshops, discussions and surveys.Concepts of competence (developing mastery as a student and teacher), relatedness (developing a sense of connectedness to other students and faculty members), and autonomy (developing a sense of control of your own learning behavior based on systematic feedback systems, both as a student and as a teacher) underpinned our design philosophy and subsequent activities.Following these activities, a procurement process followed, during 2018, to identify a digital supplier able to meet our needs.Summary of resultsThere was an agreement on the need for a new digital system that needed to deliver the following key aspects from our needs-assessment:•the ability to visualize and analyze complex information for both quality and progress purposes•learning activities and assessment results must be augmented by a system where common entities would be searchable, traceable, and triangulated to add longitudinal meaning.•the ability to deliver personalized learning through enabling individuals to see the outcomes of various triangulated assessment sources, creating opportunities for longitudinal follow up, feedback, and feed forward.•enable shared entities across multiple health educational programs for the benefit of both students and teachers.Discussion & ConclusionSeveral frameworks in contemporary medical education such as student-centered personalized learning, programmatic assessment, entrustable professional activities, interprofessional education, and development of autonomy, relatedness and competencies all call for purposeful infrastructural support, to be fully utilized. A challenge is the lack of digital systems meeting those needs.We believe that teams of educational developers together with staff and students have an important role to play in future digital learning space development. The link to medical education theories as well as the link to the health care needs should be considered. If systems are designed for collaboration and collective development, the transition to collaboration in health care and life-long learning may be better facilitated.Take-home messageThere is a need for development of digital systems based on educational research and contemporary health care strategies to augment future education. This need can only be fully realized by cooperative working between educators and developers.

    • PURPOSE: To report clinical findings after accidental instillation into the eye of sap from Angel's trumpet (Datura suaveolens). METHODS: We report findings on seven patients who developed sudden onset of unilateral mydriasis. At least three of them also had ipsilateral cycloplegia and one developed transient tachycardia. RESULTS: The symptoms evolved after ocular exposure to sap from Angel's trumpet, a plant containing natural alkaloids with parasympatholytic properties. Six patients were initially unaware of the cause of their symptoms. In these cases, patient history revealed recent contact with Angel's trumpet. CONCLUSION: Accidental ocular instillation of sap from Angel's trumpet should be noted as a cause of sudden onset of mydriasis in otherwise unaffected patients and also of general symptoms like tachycardia.

  • Adjuvant Treatment of Graves' Disease with Diclofenac : Safety, Effects on Ophthalmopathy and Antibody Concentrations

    • BACKGROUND: Orbital morphological changes are often present in patients with Graves' disease (GD) already at diagnosis, and cyclooxygenase type 2 (COX-2) is overexpressed in active Graves' ophthalmopathy (GO).OBJECTIVE: To investigate if adjuvant treatment of GD with the COX inhibitor and peroxisome proliferator-activated receptor-γ (PPAR-γ) antagonist diclofenac decreases the development of ophthalmopathy and if laboratory parameters are affected.METHODS: This is a multicenter trial where 61 subjects were randomized to methimazole (block and replace with l-thyroxine) either with or without diclofenac 50 mg 1 × 2 for 12 months. The primary end point development of GO after 24 months was evaluated. Smoking habits were registered and the thyroid parameters TSH, free T4, free T3, TSH receptor antibodies (TRAb) and anti-TPO were followed. Safety parameters (kidney, liver and blood) and adverse events were regularly registered.RESULTS: GO developed in 11% (n = 3) of the patients treated with diclofenac and in 21% (n = 6) of the controls (p = 0.273). The adverse event profile was acceptable without any severe events related to diclofenac. Both TRAb and anti-TPO concentrations decreased during treatment with methimazole, but the anti-TPO concentrations were lower in patients treated with diclofenac after 15 months (p = 0.031). The TRAb concentrations were not significantly changed between groups. Smokers had higher concentrations of TRAb than nonsmokers both at diagnosis of GD (p = 0.048) and after 15 months (p = 0.042).CONCLUSIONS: Treatment with diclofenac had no significant influence on development of GO. Diclofenac reduces anti-TPO concentrations and seems to be safe to use in GD patients.
