  • Foreign-born women rated medical and emotional aspects of postnatal care higher than women born in Sweden : a quantitative comparative study

    • INTRODUCTION Although high-quality postnatal care provides information and recognizes women's personal and cultural contexts, foreign-born women are more exposed to poor health and adverse birth outcomes. The aim of this study was to compare the length and model of postnatal care, along with the content of care, between foreign-born and native-born women living in Sweden. Another aim was to explore factors associated with being very satisfied with various aspects of postnatal care.METHODS This was a descriptive cross-sectional study of 483 postnatal women in two Swedish hospitals in 2017. Women completed a questionnaire comprising background data, pregnancy and birth related variables and the Early Postnatal Questionnaire. Data were analyzed using descriptive statistics, analysis of variance and multivariate logistic regression analyses.RESULTS Foreign-born women were more likely to have a shorter (<24 h) or longer (>48 h) length of postnatal stay than women born in Sweden. No differences in birth outcomes emerged between the two groups. Foreign-born women rated the medical (OR=1.77; 95% CI: 1.04–3.03) and emotional (OR=2.0; 95% CI: 1.17–3.40) aspects of postnatal care as being more important than Swedish-born women did. The most important aspect of overall satisfaction was the content of care, and the subscale Caring Relationship (AOR=8.15; 95% CI: 4.87–14.62) outscored all other aspects.CONCLUSIONS Important factors of satisfactory experiences with postnatal care in a Swedish context were receiving information, professional care, and a hospital environment that facilitates recovery after labor and birth. Culturally sensitive and individualized postnatal care with continuity should therefore be prioritized.

    • I den här videon beskrivs hur Region Dalarna valt att organisera ledning och styrning av uppdraget. Uppdrag och organisering i en region styrs främst av Regeringsreformen och Kommunallagen. En region är precis som en kommun, självstyrande inom ramen för lagens ramar. Videon illustrerar skillnaden mellan politisk organisation och tjänstemannaorganisation. Ledning och styrning sätts i sammanhanget av regionens digitalisering. En utvärdering visade att digitaliseringen är organiserad i en parallell beslutsstruktur med en egen uppsättning roller och arbetssätt. Det här leder till svårigheter att utkräva ansvar av ordinarie chefer, att utrymmet för utveckling och innovation är begränsat eftersom majoriteten av digitaliseringsresursen går åt till förfining och förbättring av det befintliga digitala landskapet. Region Dalarna har nu startat ett initiativ för att åtgärda problemen. Målet är att avveckla parallellstrukturen, men initialt skapas funktion i strukturen i syfte att konsolidera det digitala landskapet, samtidigt som en ny organiseringsgrund utforskas parallellt med ny teknik för att öka förmågan att genomföra Regionens kärnverksamhet.

  • Quality improvement in postnatal care : Findings from two cohorts of women in Sweden

    • Introduction: Postnatal care is an important area of midwifery practice. Changes in the length of postnatal stay, models of postnatal care, and the content of care have influenced women’s satisfaction. The aim of this study was to describe women’s assessment of postnatal care in a Swedish hospital in 2017, and to compare this with women who gave birth in 2006 in the same hospital.Methods: A comparative study was conducted of two cohorts of women who gave birth in 2006 and 2017 in a hospital in the middle-north part of Sweden with 1700 annual births. Data were collected by questionnaires, where data from 2017 were compared with data from 2006.Results: In all, 366 women who gave birth in 2006 and 342 in 2017 responded. There was a reduction in time of discharge in 2017 and more women went home directly from the labour ward and fewer women had their postnatal stay in the hotel ward, compared to postnatal women in 2006. A higher percentage of women were ‘Very satisfied’ with the overall aspects of postnatal care in 2017 compared to women in 2006. The content of postnatal care showed statistically significant improvements over time for the majority of variables studied, but some women reported not receiving information/help with specific postnatal aspects. Multiparous women, women older than 35 years, and women who had had a caesarean section received less information and practical help.Conclusions: The study showed an increase in overall satisfaction with postnatal care over time and most areas were improved. Continuous work is therefore needed in order to improve postnatal care and put the women and their families at the centre of care. More research is needed to try new models of care that will increase satisfaction with postnatal care.
