    • Angle-closure glaucoma (ACG) is a subset of glaucoma affecting 16 million people(1-3). Although 4 million people are bilaterally blind from ACG(4,5), the causative molecular mechanisms of ACG remain to be defined. High intraocular pressure induces glaucoma in ACG. High intraocular pressure traditionally was suggested to result from the iris blocking or closing the angle of the eye, thereby limiting aqueous humor drainage. Eyes from individuals with ACG often have a modestly decreased axial length, shallow anterior chamber and relatively large lens, features that predispose to angle closure(6). Here we show that genetic alteration of a previously unidentified serine protease (PRSS56) alters axial length and causes a mouse phenotype resembling ACG. Mutations affecting this protease also cause a severe decrease of axial length in individuals with posterior microphthalmia. Together, these data suggest that alterations of this serine protease may contribute to a spectrum of human ocular conditions including reduced ocular size and ACG.

  • A review of solar thermal cooling technologies in selected Middle East and North African countries

    • Cooling loads are a substantial part of the total electricity demands of countries in the Middle East and North Africa (MENA). Fortunately, because of its warm and sunny climate, the MENA region is naturally suited to solar cooling technologies. This article summarizes the most recent research and developments in solar thermal cooling technologies. The working principles and a general literature survey of solar thermal cooling technologies including absorption, adsorption, and desiccant is presented. This is followed by a summary of the literature specific to the MENA region, along with a survey of the prototypes and commercial installation of solar cooling across the MENA region. Based on this review, pilot solar cooling projects in the region are around 1180 kWc, which are split between space conditioning and industrial refrigeration applications. Most of the pilot projects are of absorption cooling type using an ammonia–water binary cycle and parabolic trough solar collectors. However, a few adsorption cooling systems are employed with a water–silica working pair and flat plate collectors. Finally, desiccant cooling systems are still in their infancy, as research and experimental systems in educational institutes.

    • Introduction: The use of contraceptive methods is very low in Guinea, particularly among adolescents and young people. The purpose of this study is to analyze the experiences and expectations of adolescents and young people regarding the use of contraceptive methods in 2019 in Conakry, Guinea.Methods: We conducted a 6-month qualitative and descriptive study. Data were collected through individual in-depth interviews and focus group discussions with adolescents and young people, health providers and health policy makers. Two approaches of deductive and inductive analysis were used to synthesize the main insights from the data.Findings: Twenty-six participants were included in this study. Adolescents and young people have personal, family and community experiences that positively or negatively influence their contraceptive needs and preferences. Positive experiences include the relative cost of injectable forms, perceived absence of side effects of implants, proven efficacy and duration of action of the modern method used (implants and injectable form). Negative experiences included cost of implants remain high (15 Euros), perceived side effects including weight gain, pill compliance, method indiscretion, and low sensation of sexual pleasure for the condom. The preferences of the young participants were dominated by Implants and injectable forms that better meet their contraceptive needs. In terms of needs, the expectations expressed revolved around needs related to the health system, including sex education, reduction in the cost of some contraceptives (implants), availability of contraceptive methods, and equity in the provision of family planning services to adolescents and young people.Conclusion: Exploring the contraceptive experiences, needs and preferences of adolescents and young people reveals decision-making dilemmas. Adolescents and young people expressed their experiences in terms of the cost of preferred contraceptives (implants), side effects, proven efficacy, and duration of action. However, their decisions are still influenced by availability, equity in service delivery, and the involvement of parents and religious leaders in sex education. Decision-makers should then place particular emphasis on improving health service delivery, adolescent sexual and reproductive health, availability of preferred contraceptive methods at affordable cost, and a program on sexuality education with the involvement of parents and religious leaders and the promotion of condom use.

    • This paper intends to numerically analyse the performance of liquid desiccant systems on the supply air conditions for closed poultry barns. A commercially available evaporative cooler for poultry barns was modified into a desiccant dehumidifier by circulating desiccant solutions of LiCl-H2O and CaCl2-H2O instead of water. Ambient air conditions for one of humid and subtropical climates were simulated based on an ∊-NTU effectiveness model for an externally cooled liquid desiccant dehumidifier. Also, a distinctive temperature-humidity index (THI) was implemented to study the environmental thermal conditions that the poultry were subjected to. Concerning the thermal comfort for broilers, the results show that conventional direct evaporative cooling systems are not feasible when the ambient air is near saturation. The THI values for ambient and direct evaporative cooler were positioned mainly in the severe heat stress region, with THI value in the range of 80 to 82, and most air outlet conditions were near saturation. The numerical results for the liquid desiccant systems show a consistent reduction in humidity ratio and air dry-bulb temperature. The assessment of air outlet temperature and humidity in terms of THI values was shifted out of the emergency and danger zone with a preference for LiCl solution.

    • Stigma remains a significant barrier to HIV testing in South Africa. Despite being a social construct, most HIV-stigma research focuses on individuals; further the intersection of gender, testing and stigma is yet to be fully explored. We examined the relationship between anticipated stigma at individual and community levels and recent testing using a population-based sample (n = 1126) in Mpumalanga, South Africa. We used multi-level regression to estimate the potential effect of reducing community-level stigma on testing uptake using the g-computation algorithm. Men tested less frequently (OR 0.22, 95% CI 0.14-0.33) and reported more anticipated stigma (OR 5.1, 95% CI 2.6-10.1) than women. For men only, testing was higher among those reporting no stigma versus some (OR 1.40, 95% CI 0.97-2.03; p = 0.07). For women only, each percentage point reduction in community-level stigma, the likelihood of testing increased by 3% (p < 0.01). Programming should consider stigma reduction in the context of social norms and gender to tailor activities appropriately.
