    • Medical professionals are increasingly assuming the role of maker and creator. At the same time, digital innovations, as part of evolving information infrastructures, are becoming increasingly prevalent in healthcare. In this paper, we adopt a Schönian approach to understand how a medical professional, who is not an IS designer by trade, engages in the design of digital practice - turning what may appear as a failed digital innovation effort into a successful design of digital practice. Our inquiry suggests three pragmatic principles that call for further investigation: (a) professionals can make a significant contribution to design work by inventing means for fact-based, reflective engagement with the situation; (b) the reorganization of work practice involves organizational design, information system design, and communication design; and (c) developing design as digital practice entails the development of fact-based design practice and must engage practical theories.

    • This special issue introduction explores the need to study information systems as symbolic action systems, defines broadly the research domain and related assumptions, notes the origins of this perspective, articulates its key lines of study, and discusses the state of the field in light of published research. The essay also positions the three papers of the special issue in the broader Information Systems (IS) discourse and notes their specific contribution in bridging so far unconnected streams of research and expanding research methods amenable to symbolic action research. This introductory essay furthermore observes some unique challenges in pulling together the special issue that invited the editors to combat against the tendency to approach communicative processes associated with information systems as primarily psychological processes. In closing we note several lines of inquiry that can strengthen future studies of symbolic action including better design theories, more flexible and open use of methods, and attentive use of rich traditions that inform symbolic action research in IS.

  • Doing Things in Relation to Machines : Studies on Online Interactivity

    • The Internet is often discussed in conjunction with various notions of interactivity. Recently, conceptualizations of a “Web 2.0″, mainly focusing on harnessing user-generated content, have grown increasingly common in both public discourse and among researchers interested in the continued growth and transformation of the Internet.This thesis approaches the use and non-use of online interactive features by societal institutions. Specifically, the thesis focuses on online newspapers and online political actors, studying the practitioners working within those institutions and on their respective audiences. Consisting of four empirical studies, the thesis is informed theoretically by the application of conceptual tools pertaining to structuration theory. In Anthony Giddens’ original conception, structuration theory posits that social structure is recursively shaped (and possibly altered) as human agents choose to re-enact certain modalities of specific structures. By changing their uses of the rules and resources made available to them by structure, humans are given agency in relation to overarching, macro-level structures. Giddens’ writings have also been contextualized to the study of information technology use by Wanda Orlikowski, who has mostly focused on organizational research.Combining insights from Giddens and Orlikowski, the thesis suggests that most Internet users are enacting a “structure of audiencehood”, entailing somewhat traditional consumer behavior, rather than a “structure of prosumerism”, which would entail extensive uses of the interactive features made available online. Similar traditional use patterns are discerned for practitioners. The thesis suggests that we should not be surprised at relatively low levels of use of interactive features by practitioners and audiences in these contexts. While the chosen areas of study are often surrounded by expectations and “hype” regarding the consequences of online interactivity, institutionalized news and politics can be said to represent stable structures – structures that have functioned in similar ways for extended periods of time, and, thus, are not so easily amended.

    • The European Association for Cardiovascular Imaging (EACVI) has outlined the rationale for setting appropriate use criteria (AUC) in cardiovascular (CV) imaging. Transthoracic echocardiography (TTE) is the most common imaging modality in CV disease and is a central tool in diagnosis, follow-up, management planning and intervention. The purpose of AUC is to inform referrers, both to avoid under-use, which may result in incomplete or incorrect diagnosis and treatment, and also over-use, which may delay correct diagnosis, lead to 'treatment cascade', and wastes resources. The first step in defining AUC for TTE in the adult has been for a panel of experts in echocardiography to review the evidence, guidelines, recommendations, and position papers from the European Society of Cardiology, EACVI and other specialist societies, and current state-of-the-art clinical practice. The attached document summarizes this work, which will be used to under-pin the development of AUC.
