    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

  • Surgical site infection after gastrointestinal surgery in children: an international, multicentre, prospective cohort study

    • Surgical site infection (SSI) is one of the most common healthcare-associated infections (HAIs). However, there is a lack of data available about SSI in children worldwide, especially from low-income and middle-income countries. This study aimed to estimate the incidence of SSI in children and associations between SSI and morbidity across human development settings.MethodsA multicentre, international, prospective, validated cohort study of children aged under 16 years undergoing clean-contaminated, contaminated or dirty gastrointestinal surgery. Any hospital in the world providing paediatric surgery was eligible to contribute data between January and July 2016. The primary outcome was the incidence of SSI by 30 days. Relationships between explanatory variables and SSI were examined using multilevel logistic regression. Countries were stratified into high development, middle development and low development groups using the United Nations Human Development Index (HDI).ResultsOf 1159 children across 181 hospitals in 51 countries, 523 (45·1%) children were from high HDI, 397 (34·2%) from middle HDI and 239 (20·6%) from low HDI countries. The 30-day SSI rate was 6.3% (33/523) in high HDI, 12·8% (51/397) in middle HDI and 24·7% (59/239) in low HDI countries. SSI was associated with higher incidence of 30-day mortality, intervention, organ-space infection and other HAIs, with the highest rates seen in low HDI countries. Median length of stay in patients who had an SSI was longer (7.0 days), compared with 3.0 days in patients who did not have an SSI. Use of laparoscopy was associated with significantly lower SSI rates, even after accounting for HDI.ConclusionThe odds of SSI in children is nearly four times greater in low HDI compared with high HDI countries. Policies to reduce SSI should be prioritised as part of the wider global agenda.

    • Le Havre Port Authority is putting into service a multimodal hub terminal with massified hinterland links (trains and barges) in order to restrict the intensive use of roads, to achieve a more attractive massification share of hinterland transportation and to provide a river connection to its maritime terminals that do not currently have one. This paper focuses on the rail-rail transshipment yard of this new terminal. In the current organizational policy, this yard is divided into two equal operating areas, and, in each one, a crane is placed, and it is equipped with reach stackers to enable container moves across both operating areas. However, this policy causes poor scheduling of crane moves, because it gives rise to many crane interference situations. For the sake of minimizing the occurrence of these undesirable situations, this paper proposes a multi-agent simulation model including an improved strategy for crane scheduling. This strategy is inspired by the ant colony approach and it is governed by a new configuration for the rail yard's working area that eliminates the use of reach stackers. The proposed simulation model is based on two planner agents, to each of which a time-horizon planning is assigned. The simulation results show that the model developed here is very successful in significantly reducing unproductive times and moves (undesirable situations), and it outperforms other existing simulation models based on the current organizational policy.

    • Nowadays, seaports seek to achieve a better massification (massive transportation of containers) share of their hinterland transport by promoting rail and river connections in order to more rapidly evacuate increasing container traffic shipped by sea and to avoid landside congestion. The attractiveness of a seaport to shipping enterprises depends not only on its reliability and nautical qualities but also on its massified hinterland connection capacity. Contrary to what has been observed in Europe, the massification share of Le Havre seaport has stagnated in recent years. To overcome this situation, Le Havre Port Authority is putting into service a multimodal hub terminal linked only with massified modes. In this study, we focus on rail–rail transshipment of this new terminal, specifically on minimizing unproductive situations of cranes to improve crane productivity and to speed up freight train processing. To this end, an improving agent-based engineering strategy called the “crane anti-collision strategy” is proposed and tested using multi-method simulation software (Anylogic). In a numerical study, the simulation results reveal that our developed model is very satisfactory and outperforms other existing simulation models.

    • Risk evaluation of the containers remains a difficult task, often due to incomplete or ambiguous information on containers. In addition, the evaluation process needs to be adapted on an ongoing basis to cope with emerging risk factors. Furthermore, high-risk container inspection is commonly hindered by a low inspection capacity, which leads to a major issue: how can we prioritize the container inspection if the number of suspect containers exceeds the inspection capacity? Container inspection prioritizing may be the answer. In this paper, we propose a novel approach for adaptively prioritizing container inspection (APRICOIN). First, we enhance the container information flow to alleviate the problem of incomplete information by proposing an enhanced container descriptive. Second, we introduce the APRICOIN algorithm, which combines frequent pattern mining and a fuzzy logic system, to assess the container’s risk score. The frequent pattern growth algorithm is proposed to retrieve the key criteria for evaluating container risk. This is done through mining frequent criteria sets within the historic data set of container inspections by customs. The mined frequent criteria sets are used to assess fuzzy inference rules which are periodically readjusted to integrate new key criteria. Thereafter, the fuzzy logic system uses the obtained fuzzy inference rules to calculate a container’s risk score. Our major contribution consists of providing a new adaptive approach for assessing a container’s risk through combining frequent criteria mining and fuzzy logic. An illustrative study and a comparison with alternative approaches are performed to validate the proposed algorithm.

    • The Precambrian inliers of the Anti Atlas belt located at the southern part of Morocco contain numerous mafic dyke and sill swarms thought to represent the erosional remnants of Proterozoic Large Igneous Provinces (LIPS). As part of a continuing research campaign to date and characterize these LIPs, four dykes have been dated by the U-Pb TIMS (Thermal Ionization Mass Spectrometry) method on baddeleyite and zircon: ca. 1758 Ma in the Tagragra of Akka inlier, 1741 +/- 10 Ma in the Tafeltast-Kerdous inlier, 1746.8 +/- 3.7 Ma in the Iguerda-Tanst and 1734 +/- 5 Ma in the Zenaga inlier. The age matches (and chemical features akin to continental flood basalts) confirm a widespread intraplate magmatic event at 1750 Ma, herein termed the Tagragra of Akka LIP, that represents an important new magmatic tarcode' line for the West African Craton (WAC). Contemporaneous 1750 Ma LIP magmatism is reported in other crustal blocks including northwestern Laurentia and Siberia; together with the newly defined Tagragra of Akka event of the Anti Atlas region of the WAC this widespread 1750 Ma magmatism is postulated to collectively constitute a single huge LIP associated with a breakup attempt of Nuns (Columbia) shortly after its final assembly at ca. 1.8 Ga.     • This research investigates the effects of styrene-butadiene-styrene (SBS) copolymer and other plastomers on the behavior of Saudi bitumen. Three bitumen samples, extracted from three Saudi oil refineries, namely Ras Tanura, Riyadh, and Yanbu, were used in this study. The bitumen samples were mixed with different modifiers in various compositions, forming polymer modified bitumens (PMBs). Elasticity parameters, such as ductility and elastic recovery were measured and evaluated. Characteristics of raw bitumen were analyzed to observe the compatibility of bitumen with SBS and plastomers. Bitumen samples were suitable for SBS modification and SBS PMBs showed superior elasticity behaviors compared to plastomer blends.

  • Assessment of Knowledge, Perception, Experience and Phobia toward Corticosteroids Use among the General Public in the Era of COVID-19 : A Multinational Study

    • Background: Corticosteroids play a significant role in managing the vast majority of inflammatory and immunologic conditions. To date, population-based studies on knowledge and attitudes concerning corticosteroids are scarce. This study aims to comprehensively assess knowledge, perception, experience and phobia toward corticosteroid use among the general population in the era of COVID-19. Methods: A cross-sectional self-administrated questionnaire was used to collect the data from 6 countries. Knowledge and corticophobia scores, descriptive statistics and logistic regression were computed. Results: A total of 2354 participants were enrolled in this study; the majority were females (61.6%) with an average age of 30. Around 61.9% had been infected previously with COVID-19, and about one-third of the participants had experience with corticosteroid use. The mean knowledge score was relatively satisfactory (8.7 ± 4.5 out of 14), and Corticophobia ranked a high score in all countries. Age, female gender, and history of COVID-19 were positively correlated with developing corticophobia. Conclusion: Our study highlights that the general knowledge about steroids was satisfactory. However, the phobia toward its use upon indication is high. Therefore, enhancing awareness and providing essential counseling regarding the rational use of corticosteroids may reduce corticophobia.
