  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

    • We report the observation of the reversible adsorption of core-shell gold-silver nanoparticles at the polarized water/1,2-dichloroethane interface using the nonlinear optical technique of surface second-harmonic generation. This study unambiguously demonstrates the excellent stability against aggregation of these core-shell nanoparticles, namely, gold core nanoparticles coated with silver layers of variable thickness, in the presence of an electrolyte salt like lithium chloride. Furthermore, it is also demonstrated that the adsorption of the nanoparticles is reversible by modulating the applied potential at water/1,2-dichloroethane interface. The analysis of these results is performed within the Debye-Huckel approximation of the electrostatic interactions between the nanoparticles. This approach shows that the stability of core-shell nanoparticles can be attributed to the formation of a silver oxide layer at the Surface of the particles.

  • Large positive in-plane magnetoresistance induced by localized states at nanodomain boundaries in graphene

    • Graphene supports long spin lifetimes and long diffusion lengths at room temperature, making it highly promising for spintronics. However, making graphene magnetic remains a principal challenge despite the many proposed solutions. Among these, graphene with zig-zag edges and ripples are the most promising candidates, as zig-zag edges are predicted to host spin-polarized electronic states, and spin-orbit coupling can be induced by ripples. Here we investigate the magnetoresistance of graphene grown on technologically relevant SiC/Si(001) wafers, where inherent nanodomain boundaries sandwich zig-zag structures between adjacent ripples of large curvature. Localized states at the nanodomain boundaries result in an unprecedented positive in-plane magnetoresistance with a strong temperature dependence. Our work may offer a tantalizing way to add the spin degree of freedom to graphene.

    • Trilayer graphene exhibits exceptional electronic properties that are of interest both for fundamental science and for technological applications. The ability to achieve a high on-off current ratio is the central question in this field. Here, we propose a simple method to achieve a current on-off ratio of 10(4) by opening a transport gap in Bernal-stacked trilayer graphene. We synthesized Bernal-stacked trilayer graphene with self-aligned periodic nanodomain boundaries (NBs) on the technologically relevant vicinal cubic-SiC(001) substrate and performed electrical measurements. Our low-temperature transport measurements clearly demonstrate that the self-aligned periodic NBs can induce a charge transport gap greater than 1.3 eV. More remarkably, the transport gap of ∼0.4 eV persists even at 100 K. Our results show the feasibility of creating new electronic nanostructures with high on-off current ratios using graphene on cubic-SiC.

    • Microtubule affinity-regulating kinase 4 (MARK4) is a potential drug target as the same is found to be over expressed in several types of cancers. In search of effective MARK4 inhibitors, we have synthesized and characterized Isatin-triazole hydrazones (9a-i) and evaluated their inhibitory potential. Of all the compounds, 9g showed better binding affinity and enzyme inhibition potential in sub micromolar range. Human serum albumin (HSA) binding assay suggested an easy transportation of 9g in blood stream due to its binding affinity. In vitro anticancer studies performed on MCF-7, MDA-MB-435s and HepG2 cells using 9g showed inhibition of cell proliferation and cell migration. Further, 9g induces apoptosis in these cancerous cells, with IC50 values of 6.22, 9.94 and 8.14 mu M, respectively. Putatively, 9g seems to cause oxidative stress resulting in apoptosis. Functional assay of 9g with a panel of 26 kinases showed MARK4 specific profile. In conclusion, 9g seems to possess an effective inhibitory potential towards MARK4 adding an additional repertoire to anticancer therapeutics.

    • The red flour beetle, Tribolium castaneum, and the mold Aspergillus flavus are well known threats of stored grain commodities, causing nutritional loss and poisoning of stored products, respectively. T. castaneum has developed resistance against most insecticides, leading to the use of extensive amounts of synthetic insecticides to protect stored grains. Synthetic pesticides not only toxify the environment but also cause serious health issues in humans using pesticide treated grains. This study aimed to identify plant-based natural pesticides to control T. castaneum and A. flavus. Essential oils were extracted from fresh aerial parts of Chenopodium ambrosioides, Conyza sumatrensis, Erigeron canadensis, and Tagetes minuta through steam distillation and investigated for insecticidal and anti-fungal activities against adult T. castaneum and A. flavus, respectively. GC-MS analysis of C. sumatrensis revealed the presence of 37.7% cis-lachnophyllum ester, 13.4% germa-crene D, and 21.6% limonene, whereas in E. canadensis the major compounds were limonene, ger-macrene D, and cis-lachnophyllum ester (43.4%, 12.9% and 5.9%, respectively). In bioassays with treated grain, C. sumatrensis and E. canadensis essential oils exhibited excellent toxicity against adult T. castaneum with LD50 of 3.7 and 5.6 mg per 10 g grains whereas in a fumigation bioassay they showed LD50 of 6.6 and 10.6 mg/L, respectively. The essential oils extracted from C. ambrosioides and E. canadensis exhibited good anti-fungal activity against A. flavus. Our findings suggest that essential oils of C. sumatrensis and E. canadensis can play an important role in protecting stored grains from T. castaneum and A. flavus contamination.

    • A polycarbonate (PC) microchannel network supporting gold nanowires was developed to be a SERS-active microchip. Observations of large increases in a Raman cross-section, allowed us to collect vibrational signatures which are not easily detectable by Raman techniques due to the high fluorescence level of bare PC. Compared to other SERS experiments, this study relies on the use of dielectric polymer/metal surfaces which are well defined at microscale and nanoscale levels. This device seems a promising tool for sensing the adsorption of biomolecules.

    • Cytogenetic and molecular abnormalities are known to influence post-transplant outcomes in acute myeloid leukemia (AML) but data assessing the prognostic value of combined genetic models in the HCT setting are limited. We developed an adapted European LeukemiaNet (aELN) risk classification based on available genetic data reported to the Center for International Blood and Marrow Transplant Research, to predict post-transplant outcomes in 2289 adult AML patients transplanted in first remission, between 2013 and 2017. Patients were stratified according to aELN into three groups: favorable (Fav, N = 181), intermediate (IM, N = 1185), and adverse (Adv, N = 923). Univariate analysis demonstrated significant differences in 2-year overall survival (OS) (Fav: 67.7%, IM: 64.9% and Adv: 53.9%; p < 0.001); disease-free survival (DFS) (Fav: 57.8%, IM: 55.5% and Adv: 45.3; p < 0.001) and relapse (Fav: 28%, IM: 27.5% and Adv: 37.5%; p < 0.001). Multivariate analysis (MVA) revealed no differences in outcomes between the Fav and IM groups, thus they were combined. On MVA, patients in the Adv risk group had the highest risk of relapse (HR 1.47 p <= 0.001) and inferior DFS (HR 1.35 p < 0.001) and OS (HR 1.39 p < 0.001), even using myeloablative conditioning or in those without the pre-HCT measurable-residual disease. Novel approaches to mitigate relapse in this high-risk group are urgently needed.
