  • Aerosolized particulate matter from fragmentation of carbon nanotube-enhanced concrete

    • Construction and demolition workers are exposed to high levels of particulate matter (PM) from building materials throughout their working life. Although nano-enabled building materials (NEBMs) may improve the performance and functionality of buildings, concerns are being raised regarding health risks from occupational exposure to PM from NEBMs. In this work, an experimental set-up for integrated resuspension and characterization of PM from NEBMs was developed and tested using three types of concrete (low density, normal, high strength), each enhanced with Carbon Nanotubes (CNTs) at different concentrations (0, low, high). The performance of portable devices used in occupational exposure assessments (DustTrak and NanoTracer) was compared with stationary instruments and gravimetric filter techniques. 40-70% of the mass and 90-98% of the number of particles were within the respirable fraction, with primary modes at 150 nm and 2-3 µm. Addition of CNTs significantly decreased mean particle number concentrations (PNCs) across the entire characterized size range (7 nm - 20 µm) for low density concrete, whereas the opposite was the case for normal strength and high strength concrete. It was hypothesised that the concrete matrix primarily governs the PM formation, which is in turn modulated by CNT-matrix interactions either suppressing or supporting fragmentation during crushing. SEM imaging could display partially submerged CNTs protruding from concrete fragments. Fundamental interactions at the interface of the nanomaterial and the surrounding matrix needs to be investigated to determine how the PM generated from NEBMs differ from their non-nano counterparts and how to prevent future exposure during demolition.

  • Characterization of airborne dust emissions from three types of crushed multi-walled carbon nanotube-enhanced concretes

    • Dispersing Multi-Walled Carbon Nanotubes (MWCNTs) into concrete at low (<1 wt% in cement) concentrations may improve concrete performance and properties and provide enhanced functionalities. When MWCNT-enhanced concrete is fragmented during remodelling or demolition, the stiff, fibrous and carcinogenic MWCNTs will, however, also be part of the respirable particulate matter released in the process. Consequently, systematic aerosolizing of crushed MWCNT-enhanced concretes in a controlled environment and measuring the properties of this aerosol can give valuable insights into the characteristics of the emissions such as concentrations, size range and morphology. These properties impact to which extent the emissions can be inhaled as well as where they are expected to deposit in the lung, which is critical to assess whether these materials might constitute a future health risk for construction and demolition workers. In this work, the impact from MWCNTs on aerosol characteristics was assessed for samples of three concrete types with various amounts of MWCNT, using a novel methodology based on the continuous drop method. MWCNT-enhanced concretes were crushed, aerosolized and the emitted particles were characterized with online and offline techniques. For light-weight porous concrete, the addition of MWCNT significantly reduced the respirable mass fraction (RESP) and particle number concentrations (PNC) across all size ranges (7 nm – 20 μm), indicating that MWCNTs dampened the fragmentation process by possibly reinforcing the microstructure of brittle concrete. For normal concrete, the opposite could be seen, where MWCNTs resulted in drastic increases in RESP and PNC, suggesting that the MWCNTs may be acting as defects in the concrete matrix, thus enhancing the fragmentation process. For the high strength concrete, the fragmentation decreased at the lowest MWCNT concentration, but increased again for the highest MWCNT concentration. All tested concrete types emitted <100 nm particles, regardless of CNT content. SEM imaging displayed CNTs protruding from concrete fragments, but no free fibres were detected.

  • DeltaT50 - a new method to assess temporal ventricular repolarization variability

    • Background Increased beat-to-beat variability in cardiac repolarization time is a tentative risk marker of drug-induced torsades de pointes. We developed a new, automatic method based on the temporal variability of the T-wave down slope to assess this variability. Method and Results Leads V1 to V6 of resting electrocardiograms were recorded in 42 healthy subjects (18-68 years, 22 men). The temporal variability at 50% of the T-wave down slope, deltaT50 (1.5 ± 0.41 milliseconds; range, 0.86-2.66 milliseconds), was measured with an accuracy of 1 millisecond on at least 9 pairs of electrocardiogram complexes with a signal-to-noise ratio more than 10 and changes in the R-R interval less than 150 milliseconds. The correlation between repeated measurements of deltaT50 was high. DeltaT50 was measured without corrections for age, sex, heart rate, T-wave amplitude, signal-to-noise ratio, R-R variability, and QTcF because none of these factors explained more than 4% of the within-subject deltaT50 variability. Conclusion The beat-to-beat repolarization variability was measured with high fidelity with the deltaT50 method and was a robust measure in healthy volunteers.

  • Effects of Voice Therapy: A Comparison Between Individual and Group Therapy

    • Objective. The primary aim of this clinical evaluation project is to study the effect of voice therapy given in the speech-language pathology clinic, as individual and group therapy, as well as finding out some of the reasons for not attending the recommended therapy. Method. All patients visiting the speech-language department during the study period were asked to participate in a clinical evaluation project. The project included filling out the questionnaire Swedish Voice Handicap Index (VHI-11) twice: at their first visit at the clinic, and approximately 1 year later. Depending on the degree of voice problems, the patients were offered either individual or group therapy. Result. The study included 187 patients at their first visit to the clinic and 109 patients at follow-up. All participants completed self-evaluation of voice function with the VHI-11 and separate questions regarding overall voice problems, hoarseness, and vocal fatigue. For the patients who responded to the follow-up survey, statistically significant improvements of self-perceived voice function were demonstrated in individual and group therapy. The improvement between the first visit and the follow-up was found to be of moderate to large effect size, with statistically significant improvements for both patients who attended individual therapy and those who attended group therapy. The most common reason for not attending the recommended voice therapy was lack of time. Conclusion. Individual and group therapy is effective, resulting in improved VHI-11 scores. The magnitude of improvement is similar when comparing individual and group therapy. Patients with higher scores of the VHI-11 were generally recommended individual voice therapy.

  • Mervärde och begränsningar i Nationell patientöversikt (NPÖ) : en implementeringsstudie

    • Nationell Patientöversikt (NPÖ) är ett informationssystem som gör det möjligt för behörig vårdpersonal att med patientens samtycke ta del av journalinformation som har registrerats hos andra vårdgivare. Denna rapport behandlar mervärden och begränsningar i NPÖ och är ett resultat av en studie som på uppdrag av SKL genomförts i Jönköpings Län. Studien pågick från den 1 september 2012 till den 28 februari 2013 och var en av tre delar i projektet ”Gemensam informationsåtkomst mellan landsting och kommun genom NPÖ – en kunskapsöversikt i tre län”.Studien genomfördes genom semistrukturerade intervjuer som analyserades kvalitativt. Studiens syfte var att besvara följande fyra frågor: Vilken journalinformation kommer att visas i NPÖ?Ger informationen i NPÖ en tillräckligt fullständig bild av patientens tillstånd?Vilket förtroende har personalen för NPÖ som stöd?Hur kan journalinformationen i NPÖ kvalitetskontrolleras?Svaret på de tre första är beroende av den producerande organisationens rutiner och arbetssätt. Om tekniken är stabil, om journalinformation snabbt kan speglas upp i NPÖ, om alla producenter lägger upp de informationsmängder som verkligen behövs och om de dokumenterar på ett standardiserat sätt i dessa informationsmängder samt om det finns möjlighet att inhämta samtycke från alla patientkategorier så tidigt i processen som det behövs är NPÖ en bra källa till information för kommunernas personal. Alla dessa förutsättningar är i nuläget inte uppfyllda. NPÖ löser inte heller behovet av tillförlitlig information om patienternas läkemedel.I de olika verksamheterna finns en mångfald av redan etablerade informationssystem, såväl manuella som digitala, i vilken NPÖ ska hitta sin roll. När det gäller svaret på den sista frågan kan konstateras att ingen kvalitetsgranskning av den sammanlagda informationen i NPÖ kan ske. Patientdatalagen hindrar i nuläget detta.Rapporten avslutas med följande frågor som behöver besvaras inför en fortsatt utveckling av NPÖ:Vem har det övergripande ansvaret för informationen i NPÖ och hur kan kvaliteten säkerställas?Ska NPÖ vara en primär källa till information för kommunernas legitimerade hälso- och sjukvårdspersonal behöver informationsöverföringen standardiseras. Vilka informationsmängder ska produceras av samtliga landsting? Vilka reservrutiner ska finnas?Vilka konsekvenser för kommunernas personal får sena diktat och sen signering av information i NPÖ?Hur kan integrering av olika informationssystem ske för att minska dubbeldokumentation och av/omskrivningar av information?Läkemedelsinformationen i NPÖ kan för närvarande utgöra en risk för patientsäkerheten. Hur kan läkemedelsinformationen hanteras på ett patientsäkert sätt?Hur skapas en stabil utbildningsmiljö och hur sprids goda exempel på användande av NPÖ?Hur sprids kunskaper om att loggning främst ska ses som trygghet för personal, snarare än som kontroll?Vilka riktlinjer för samtycke ska gälla så att de uppfattas som relevanta för patienter (mängden samtycken och samtycke innan möte med patient)?Hur skapas säkra kanaler för interaktion mellan olika vårdgivare (exempelvis för frågor och svar mellan sjuksköterska på kommunalt boende och läkare på sjukhusklinik)?Kan patienters planerade besök i landstingsvården på ett säkert sätt kommuniceras via NPÖ?Hur kan ansvarig personal i kommunen få en signal om att ny information om en patient producerats i NPÖ?Hur kan rapportering av fel uppmuntras så att en bevakning att NPÖ fungerar tekniskt kan ske? Hur kan support erbjudas även på icke kontorstid?

  • A Study Protocol for Validation and Implementation of Whole-Genome and -Transcriptome Sequencing as a Comprehensive Precision Diagnostic Test in Acute Leukemias

    • Background: Whole-genome sequencing (WGS) and whole-transcriptome sequencing (WTS), with the ability to provide comprehensive genomic information, have become the focal point of research interest as novel techniques that can support precision diagnostics in routine clinical care of patients with various cancer types, including hematological malignancies. This national multi-center study, led by Genomic Medicine Sweden, aims to evaluate whether combined application of WGS and WTS (WGTS) is technically feasible and can be implemented as an efficient diagnostic tool in patients with acute lymphoblastic leukemia (ALL) and acute myeloid leukemia (AML). In addition to clinical impact assessment, a health-economic evaluation of such strategy will be performed. Methods and Analysis: The study comprises four phases (i.e., retrospective, prospective, real-time validation, and follow-up) including approximately 700 adult and pediatric Swedish AML and ALL patients. Results of WGS for tumor (90×) and normal/germline (30×) samples as well as WTS for tumors only will be compared to current standard of care diagnostics. Primary study endpoints are diagnostic efficiency and improved diagnostic yield. Secondary endpoints are technical and clinical feasibility for routine implementation, clinical utility, and health-economic impact. Discussion: Data from this national multi-center study will be used to evaluate clinical performance of the integrated WGTS diagnostic workflow compared with standard of care. The study will also elucidate clinical and health-economic impacts of a combined WGTS strategy when implemented in routine clinical care.
