    • Det här är en kokbok för förändringsledare. Du kan använda denna bok om du ska leda  utvecklingsaktiviteter och letar efter bra och beprövade metoder för att leda grupper  och processer. Vi beskriver metoderna som vi har använt inom vårt projekt SMICE, i  vilket sammanhang metoderna använts och när de olika metoderna fungerat bra. Vår förhoppning med att nedteckna dessa metoder är att du ska inspireras och vågar  prova något nytt för att skapa nytta. Vi tror att ett detaljerat sätt att dokumentera  metoderna gör dem mer användbara för dig men också för oss själva.   Metoderna är graderade efter vilket behov av förkunskap och förberedelser du behöver,  från det enklaste, där du kan öppna kokboken och använda metoden på en gång, till  metoder som kräver utbildning eller specialistkompetens innan du sätter igång. Boken är indelad i fyra huvuddelar; Starta, Forma, Utveckla och förankra och Bygga vidare. Till dessa delar har vi identifierat metodstöd som kan användas vid en rad olika tillfällen;  vid idégenerering, vid idéutveckling, vid affärsutveckling, för att inspirera, för att skapa  samsyn och samverkan och för att mobilisera större grupper av människor och  organisationer. Något för alla, alltså. Avslutningsvis finner du tips och stöd för att  driva dessa utvecklingsprocesser på distans med digitala verktyg i digitala möten.  Ska vi sammanfatta någon lärdom av detta arbete så blir det våra nycklar som presenteras på nästa sida. Du kan se dessa nycklar som våra bästa råd för att du ska lyckas med ditt  arbete med att leda processer och att ordna möten. Lycka till! Metoderna har testats och utvecklats inom SMICE - Samskapande Mittnordisk  Innovationsarena för Cirkulär Ekonomi, ett projekt inom Interreg som pågått  2017-2020.

  • Accessibility and factors associated with utilization of mental health services in youth health centers : a qualitative comparative analysis in northern Sweden

    • Background: Youth-friendly health care services can facilitate young people's access to health care services and promote their health, including their mental health. In Sweden, a network of youth health centers exist since the 1970s, incorporated within the public health system. Even if such centers take a holistic approach to youth health, the focus has been in sexual and reproductive health care, and the extent of integrating mental health care services is less developed though it varies notably between different centers. This study aims to analyse the various conditions that are sufficient and/or necessary to make Swedish youth health centers accessible for mental and psychosocial health.Methods: Multiple case study design, using qualitative comparative analysis to assess the various conditions that makes a youth health center accessible for mental and psychosocial issues and mental health. The cases included 18 youth health centers (from a total of 22) in the four northern counties of Sweden.Results: In order to enhance accessibility for mental health services, youth health centers need to be trusted by young people. Trust was necessary but not sufficient, meaning that it had to be combined with other conditions: either having a team with a variety of professions represented in the youth health center, or being a youth health center that is both easy to contact and well-staffed with mental health professionals.Conclusions: Differentiated, first-line services for youth can play an important role in promoting youth mental health if certain conditions are fulfilled. Trust is necessary, but has to be combined with either multidisciplinary teams, or expertise on mental health and easy accessibility.

  • Cyclin A is a proliferative marker with good prognostic value in node-negative breast cancer

    • Background: Proliferative markers are not recommended as prognostic   factors for clinical use in breast cancer due to lack of   standardization in methodology. However, proliferation is driving   several gene expression signatures emphasizing the need for a reliable   proliferative marker IF or clinical use. Studies suggest that cyclin A   is a prognostic marker with satisfying reproducibility. We investigated   cyclin A as a prognostic marker in node-negative breast cancer using   previously defined cutoff values.   Patients and Methods: In a case-control study, we defined 190 women who   died from breast cancer as cases and 190 women alive at the time for   the corresponding case's death as controls. Inclusion criteria were   tumor size <= 50 mm, no lymph node metastases and no adjuvant   chemotherapy. Tumor tissues were immunostained for cyclin A using   commercially available antibodies.   Results: We found a statistically significant association between   expression of cyclin A and breast cancer death in a univariate model:   odds ratio for cyclin A(ave) 2.7 [95% confidence interval (CI),   1.7-4.3] and cyclin A(max) 3.4 (CI, 2.1-5.5). Corresponding odds ratio   for Ki67 were Ki67(ave) 1.9 (CI, 1.2-3.1) and Ki67(max) 1.7 (CI,   1.1-2.7) and for grade 3.1 (CI, 1.8-5.1). Cyclin A was strongly   correlated to Ki67 and grade why a model including all was not   appropriate.   Conclusions: Cyclin A is a prognostic factor for breast cancer death in   node-negative patients using standardized methodology regarding scoring   and cutoff values. Adding cyclin A as a proliferative marker to established clinicopathologic factors will improve the separation of  low and high risk breast cancer.

    • Cyclin D1 has a central role in cell cycle control and is an important component of estrogen regulation of cell cycle progression. We have previously shown that high cyclin D expression is related to aggressive features of ER-positive but not ER-negative breast cancer. The aims of the present study were to validate this differential ER-related effect and furthermore explore the relationship between cyclin D overexpression and CCND1 gene amplification status in a node-negative breast cancer case-control study. Immunohistochemical nuclear expression of cyclin D1 (n = 364) and amplification of the gene CCND1 by fluorescent in situ hybridization (n = 255) was performed on tissue microarray sections from patients with T1-2N0M0 breast cancer. Patients given adjuvant chemotherapy were excluded. The primary event was defined as breast cancer death. Breast cancer-specific survival was analyzed in univariate and multivariable models using conditional logistic regression. Expression of cyclin D1 above the median (61.7%) in ER breast cancer was associated with an increased risk for breast cancer death (OR 3.2 95% CI 1.5-6.8) also when adjusted for tumor size and grade (OR 3.1). No significant prognostic impact of cyclin D1 expression was found among ER-negative cases. Cyclin D1 overexpression was significantly associated to high expression of the proliferation markers cyclins A (rho 0.19, p = 0.006) and B (rho 0.18, p = 0.003) in ER-positive tumors, but not in ER-negative cases. There was a significant association between CCND1 amplification and cyclin D1 expression (p = 0.003), but CCND1 amplification was not statistically significantly prognostic (HR 1.4, 95% CI 0.4-4.4). We confirmed our previous observation that high cyclin D1 expression is associated to high proliferation and a threefold higher risk of death from breast cancer in ER-positive breast cancer.

    • The objectives of this study were to evaluate registered nurses' (RN) compliance with a local clinical central venous access device (CVAD) protocol after completing an educational program and to determine RNs' perception of the program. Seventy-five RNs working in hematology participated in the educational part of the program. Sixty-eight RNs were examined while changing CVAD dressings or placing a Huber needle into a port on actual patients. Sixty percent of the RNs passed the examination and reported that the program increased their knowledge. The results indicated that the educational program could be recommended for use when implementing a new clinical protocol.

    • The communication of the intangible benefits to different stakeholders is important at the development for organizational resource and could be described as a boundary-spanning activity. In this study we build on Ahlin’s model (2014) and illustrates categorization of intangible benefits of digital information by using Carlile’s (2002; 2004) efficient boundary objects, the syntactic, semantic and pragmatic. Qualitative empirical pictures from three cases are illustrated by questions, derived from the efficient boundary objects.     • Seeking out and understanding the benefits of information are essential to an organization. Internal in an organization it is important to understand created benefit in order to communicate, prioritize and increase or decrease prioritization in strategically important business areas. In modern society digitalized information is an area that is on the increase in enterprises, as is the knowledge that information is of importance. Therefore there should be a demand for understanding and measuring the beneficial factors that information gives an organization. In this study, empirical material was collected in a literature study from journals highly ranked in the Information Systems Area and with special focus on the benefit of information. The perspective of the study has been intra organizational. The purpose is to derive found benefit factors and use them as a basis to discuss and problematize on the nature of benefit of information and hereby give organizations help to understand this area.

  • A semiotic perspective on semantics interoperability

    • While information technology enables us to access more material than ever before, we need to come to grips with the disagreeable fact that data is not information. Sharing data without safeguarding comprehension may lead to confusion at best and disaster at worst. The issue at hand is one of ensuring semantic interoperability between actors from disparate contexts. Research into this problem is plentiful, but typically focused around specific subject matters, limiting its appeal to a limited range of scholars and practitioners. Based upon a literary study, we identify two extreme approaches to managing semantic interoperability. These are denoted top-down and bottom-up. We illustrate real world instances of these approaches using the TFI-model based on a case study encompassing two organizations. Our theoretical model is found to be a viable lens through which to generalize and interpret issues pertaining to semantic interoperability between human actors. We therefore see a need for further research into human-based on semantic interoperability.

  • A Transition Towards Digital Home Visits in Social Care and Home Health Care During the Corona Pandemic

    • During the pandemic, physical meetings were supposed to decrease as much as possible to avoid the virus to spread. Before the pandemic, the physical meeting favoured social care and home health care in Sweden. One solution was to digitize as many of these meetings as possible. Therefore, we investigated this transition in a web survey, including questions with predetermined and open-ended answers. The web-survey was sent to co-workers in home health care and social care in a middle-seized municipality in Sweden. The results showed that not all meetings could be transformed, like meetings with citizens with hearing or cognitive impairments. Challenges related to the transformation were instability in technical equipment, the professionals’ and citizens’ knowledge of handling technical equipment, and access to technical equipment support. Despite this did the co-workers digitize meetings whenever possible, adding operational and problem-solving attitude to the transformation. Due to this study’s limitation, like respondents from one municipality and the pandemic’s length, we intend to investigate further and understand the development of the transformation and how knowledge in the area increases.
