  • How Meetings affect the accomplishment of Broad Responsibility in a municipal Company

    • Municipal companies are important actors in the pursuit of the Agenda 2030 goals and are often formally obliged by their owners to work in this direction. This has however shown to be quite challenging, and managers lack knowledge about how to develop new ways of organizing to meet such responsibilities. The aim of this article is therefore to understand how the work of a top management team in meetings affects the accomplishment of broad responsibility. The analysis, which is underpinned by a communicative constitution of organizing (CCO) perspective, shows how the way specific communicative practices (agendas, minutes, timeslots, turn-taking, and stakeholder voicing) are enacted leads to the re-production of parts of the organization at the expense of the whole, the present at the expense of the future, and profit at the expense of the other dimensions of sustainability. This study contributes to the literature on public management by showing how communicative practices enacted in meetings make certain concerns present and others absent, thereby creating the conditions for the accomplishment of broad responsibility.

  • Municipalities working for Agenda 2030: Review and Agenda for future Research

    • Considering the population density of urban areas, their levels of consumption and emissions, their influence on social equality and inclusion, and the resources and power allocated to urban actors, it is important to examine how municipalities organize work as they proceed to implement Agenda 2030 and to what effect. A systematic literature review is used to analyse 77 articles, published between 2017 and 2022, that examine the sustainability activities of municipal actors in Europe. Two themes are identified and explained: a) a paradigm shift in local government; b) the mobilization of socio-technical approaches (e.g., smart cities and regenerative land use). Also, the limitations of the literature and some areas for future research are discussed: research comparing practices across regions and countries; research analysing the interaction of different initiatives within a municipality; and research examining the role of municipal companies.

  • Negotiating shared Responsibility for sustainable urban Development: Pronouns and In-here-ness as rhetorical Resources

    • This article explores how issues of shared responsibility are negotiated in the realm of managing collaborative efforts between organizational actors for sustainable urban development abiding to the Agenda 2023 – with a particular focus on the rhetorical resources used in such negotiations. Through empirical material from recorded meetings in a one-year project at a municipal company, the article contributes to previous literature on collaborative ways of organizing and managing complex public challenges. With a focus on the discursive construction of shared responsibility, the concept of in-here-ness is introduced to denote accepted and assumed responsibility, which may shift through the use of pronouns: from a narrow ‘I’ or ‘we’ of stakeholders to a wider ‘we’ of collaborating parties. The article further contributes to the empirical field of sustainable development at the municipal level.

  • Validity and reliability of physical activity measures in multiple sclerosis

    • PURPOSE: The purpose was to evaluate the psychometric properties of physical activity measures in persons with multiple sclerosis (PwMS).METHODS: Adults with multiple sclerosis were recruited, n = 30 (validation) and n = 57 (test-retest). Steps measured with PiezoRX, Yamax SW200 and ActiGraph GT9X Link (AGlink) and time in different positions measured with AGlink were validated against data from video analysis. Psychometric properties of the Physical Activity and Disability Survey - Revised Swedish version (PADS-R(Sw)) was evaluated.RESULTS: The most valid measures were AGlink using the low-frequency extension filter, and PiezoRX with median absolute percentage errors (MeAPEs) of 0.9-3.1% and 1.3-3.3%. The MeAPEs were higher for Yamax SW200 (2.9-21.0%), AGlink display (3.6-44.8%) and AGlink normal filter (8.9-48.9%), indicating low validity. AGlink was not valid in measurements of sitting (MeAPE 12.0-12.5%) or lying (MeAPE 31.0-41.7%). The correlation between PADS-R(Sw) and AGlink steps was r = 0.492 (p = .009). The relative reliability of PADS-R(Sw) was ICC2,1 0.85 (CI 0.76-0.91), and absolute reliability was SEM 0.54.CONCLUSION: AGlink and PiezoRX were valid measures of steps in PwMS. The questionnaire PADS-R(Sw) was valid, with high relative reliability, but its absolute reliability was unsatisfactory.

  • The extent of gynaecological cancer : Evaluation, outcome and quality of life

    • The overall aim of this thesis was to enhance treatment planning for gynaecological cancer patients and identify women that are more likely to have impaired quality of life (QoL) after treatment. In a retrospective cohort study on ovarian cancer, the peritoneal cancer index (PCI) was examined in relation to incomplete cytoreductive surgeries (CRS) and surgical complications (n=167). The PCI was found to be an excellent predictor of incomplete CRS (AUC 0.94). Complete CRS was obtained for only 67.2% of the patients with a PCI score higher than 24, who also experienced an increased rate of complications (p = 0.008). In a prospective study, radiologic PCI assessed with integrated PET/MRI and DW-MRI was compared with the surgical PCI as the gold standard (n=34). The median total PCI for PET/MRI (21.5) was closer to the surgical PCI (24.5) (p = 0.6) than to DW-MRI (20.0, p = 0.007). PET/MRI was more accurate (p = 0.3) for evaluating patients at primary diagnosis and for evaluating high tumour burden in inoperable patients.In a nationwide study, endometrial cancer patients included in the Swedish Quality Registry for Gynaecologic Cancer 2017-2019 (n=1401) were analysed with the aim of describing methods of evaluating myometrial invasion (MI). The main methods for the MI assessment were transvaginal sonography (59%) and MRI (28%). The sensitivity of transvaginal sonography (65.6%) was lower than for the other methods.In a longitudinal questionnaire-study, QoL in women with advanced gynaecological cancer was compared to women with local disease (n=372). No difference in QoL was found at the one-year follow-up. With multiple regression analyses, previous psychiatric illness, high BMI and comorbidities were identified as risk factors for impaired QoL.In conclusion, the PCI is an excellent predictor of incomplete CRS, and PCI ≥24 is a possible cut-off. PET/MRI is superior to DW-MRI for estimating total PCI. The assessment of MI in endometrial cancer in Sweden is usually performed with transvaginal sonography, but the sensitivity is lower than for other methods. Women with advanced gynaecological cancer have equally good QoL one year after diagnosis as women with limited disease, and psychiatric illness, high BMI, and comorbidities are risk factors for impaired mental health.

  • Validation of F-18-FDG PET/MRI and diffusion-weighted MRI for estimating the extent of peritoneal carcinomatosis in ovarian and endometrial cancer : a pilot study

    • Background: The extent of peritoneal carcinomatosis is difficult to estimate preoperatively, but a valid measure would be important in identifying operable patients. The present study set out to validate the usefulness of integrated F-18-FDG PET/MRI, in comparison with diffusion-weighted MRI (DW-MRI), for estimation of the extent of peritoneal carcinomatosis in patients with gynaecological cancer.Methods: Whole-body PET/MRI was performed on 34 patients with presumed carcinomatosis of gynaecological origin, all scheduled for surgery. Two radiologists evaluated the peritoneal cancer index (PCI) on PET/MRI and DW-MRI scans in consensus. The surgeon estimated PCI intraoperatively, which was used as the gold standard.Results: Median total PCI for PET/MRI (21.5) was closer to surgical PCI (24.5) (p = 0.6), than DW-MRI (median PCI 20.0, p = 0.007). However, both methods were highly correlated with the surgical PCI (PET/MRI: beta = 0.94 p < 0.01, DW-MRI: beta = 0.86, p < 0.01). PET/MRI was more accurate (p = 0.3) than DW-MRI (p = 0.001) when evaluating patients at primary diagnosis but no difference was noted in patients treated with chemotherapy. PET/MRI was superior in evaluating high tumour burden in inoperable patients. In the small bowel regions, there was a tendency of higher sensitivity but lower specificity in PET/MRI compared to DW-MRI.Conclusions: Our results suggest that FDG PET/MRI is superior to DW-MRI in estimating total spread of carcinomatosis in gynaecological cancer. Further, the greatest advantage of PET/MRI seems to be in patients at primary diagnosis and with high tumour burden, which suggest that it could be a useful tool when deciding about operability in gynaecological cancer.
