  • In search of a caring relationship : nursing students' notions of interactions in the nurse-patient relationship

    • The aim of the study is to explore how nursing students talks about their notions on interactions in the relationship between nurse and patient. Empirical data has been obtained from 22 students in their first semester of the Swedish undergraduate nursing programme, applying thematic, individual interviews and using qualitative content analyses. The most significant finding is that although the students had none or limited pre-understanding of caring and the interaction between nurse and patient they were aware of the pre-supposes for establishing such an interplay, articulated in a desire to find out how to build an authentic and trustful caring relationship. Empirical data also show that the students, in the stage of beginners, were trying to identify and relate to basic concepts within caring science. The restricted understanding could then be understood as a matter of the students not being able to express more than they had words for. The results provide new insights into the interactions in the nurse-patient relationship, seen from the perspective of beginner students. These insights could be useful, for lecturers and clinical supervisors, who play a paramount role in the development of each student's acquisition of theoretical and practical knowledge.

    • The primary aim of the present thesis is to increase understandig of the relationship between organisational structure, gender and power. Understanding of this relationship requires a closeness to the individual being studied as well to the content and consequences of that individuals´actions. The empirical object of study is a group of professional women at a somatic clinic. The problem investigated is based on a theoretical perspective on organisations; feminist theories and power theories. Our starting point was that these women should be seen as subjects who actively shape their own daily living situation, experience and actions. We thus felt it appropriate to choose a qualitative method. Data was collected by means of observation and qualitative interviews. The results show that the first foundation of gender-related domination at the clinic was hierarchial and bureaucratic and was characterised by a clear power symmetry and division of labour. The second foundation of gender-related domination resulted in the organisational structure of the clinic allowing a few men at the top of the hierarchy to exercise power over the women in the middle and lower levels. An important conclusion is that the women studied were clearly at a disadvantage in a hierarchial organisation. The difference in influence between men and women in the study was further reinforced by the fact that the various professional categories belonged to different sub-cultures. The unisexual sub-cultures when combined with the doctor´s normative sub-culture further increased the opportunities for the men studied to dominate the women, thereby reinforcing the impression that the organisation was based om male norms and the logic of segregation. Male domination can also be understood from an individual perspective. The men studied adopted different strategies as a means of accentuating their position in relation to the women. The study comprised a few women who used specific domination tactics. They possessed knowledge of the different power techniques open to them, and they used these in their relations with men. Knowledge of the two foundations mentioned stimulates the desire to initiate changes. One element of significance in such changes is the re-forming of structural conditions. It is important to bring about changes at individual level. Men and women´s consciousness of, and knowledge about, gender-related domination must be the focus of attention.

  • Validation of the verbal and social interaction questionnaire for nursing students : The focus of nursing students in their relationship with patients

    • The Verbal and Social Interaction Nursing Students questionnaire (VSI-NS) has been created to measure the development ofverbal, social and interactional skills of nursing students with patients, from their perspective in nursing care. The aim of thepresent study was to determine the construct validity and internal consistency reliability of the questionnaire. The study had amethodological and developmental design and was carried out in four steps: adjustment of the items, face validity, data collectionand data analysis. The number of items was reduced from 48 to 31. The factor analysis of the final 31 items resulted in four quitedistinct factors: “Inviting to talk about feelings and thoughts”, “Building a caring relationship”, “Encouraging social and practicalaspects in daily life” and “Caring towards health and wellbeing”. The results showed satisfactory psychometric properties interms of content validity, construct validity and the internal consistency reliability of the questionnaire. It could be concluded thatthe original conceptual model could serve as a theoretical foundation to explain and understand nurses’ caring interactions withtheir patients

  • Development-oriented learning in a project team : applying an interactive research approach

    • The overall purpose of the article is to describe a joint learning process where both practicable and theoretically anchored knowledge are in the foreground. The empirical data derives from an EU project. In focus is a group of course leaders and their experiences of carrying out a training programme targeted for a group of individuals with a weak position on the labour market. The author brings out what happens when individuals try to understand perspectives from one another. The results demonstrate the knowledge that is developed when members of a project team are included in the entire research process, from the definition of problems to the analysis, presentation of results and suggestions of change. Further the outcome illustrates how an interactive research approach can be conducted in close co-operation with those concerned. Active participation, a structured learning process, critical reflections and common understanding then became essential prerequisites. Taken together the study reveals that it is possible to learn how to approach complex problems and situations. By communicating experiences and thoughts that could be attributed to interaction patterns in social relations, communication structures, emotions, influence and power, the course leaders and the researcher jointly created a learning environment where reflection, understanding and development-oriented learning were of vital importance.

  • Dialogue in a learning process : problematization of gender equality in higher education

    • The overall aim with the study was to describe the knowledge building that was developed in a group of students who were trained nurses enrolled in a Swedish higher education programme. With a point of departure in an interactive research approach, the authors applied the dialogue seminar as method and pedagogical model. When the students in close to practice talks, shared their thoughts about how gender and gender equality could be experienced and understood, the dialogue seminar became useable. At the analysis of the stories which the students shared at the dialogue seminars, it became clear that most of the students had an ambivalent approach to gender equality. When they reflected on gender relations, they brought out the hierarchical and separate gender relations which they experienced as existing within healthcare. Furthermore, their professional lives seemed to be embedded in gender-related practices. An important conclusion is that the students’ reflections oscillated between critical reflection on conditions and critical reflection on processes within their own practice in healthcare. Another finding was that the students’ reflections in dialogue form became important in a learning process, not least in the light of that gender relations and gender equality only to a limited degree had been included in their nurse’s education at the basic level.

  • Effekter och kostnader av total höftledsplastik i Karlskrona och Karlshamn

    • Syftet med projektet är att mäta hälsoeffekter av total höftledsplastik med hjälp av hälsoindex och höftindex, att göra en beräkning av kostnaderna för total höftledsplastik inom hälso- och sjukvårdssystemets olika delar, samt att analysera skillnader i resursförbrukning vid Karlskrona och Karlshamns lasarett. Hälsoindexen Rosser, Sickness Impact Profile (SIP) och Euroqol har använts och visar förbättrat hälsoindex 6 månader efter operationen. Höftindexen Charnley, Andersson-Möller/Nielsen och Harris har också testats. Avgörande skillnader mellan höft- och hälsoindex påvisas ej. Kostnaden för ett ingrepp är ca 60 000 kr. Variationer i kostnader mellan olika kliniker kan förklaras med olika behandlingsrutiner.
