    • Structural studies of soft material while exposed in situ to stretch or shear provide a new experimental technique which gives important insight into polymer dynamics. Such devices can be used for making single-domain block-copolymer and colloidal mesophases, and to study various growth mechanisms. In addition, combined structure and shear instruments can give detailed insight into the influence of fluctuations. Two types of shear device, the Couette cell and a parallel plate device, are presented, and some applications are shown.

    • Utilising simple acid-base titration chemistry, a new family of Linear-b-Amphiphilic Comb (L-b-AC) ionic supramolecules [Soft Matter 2013, 9, 1540-1555] featuring multi-tail side-chains have been synthesized and examined by synchrotron SAXS. To three different parent diblock copolymers of poly(styrene)-b-poly( methacrylic acid) PS-b-PMAA multi-tail ammonium side-chains of (C8)(2)-, (C8)(4) -or (C12)(2)-type were attached at various side-chain grafting densities (X), making it possible to separate effects of the details of the AC-block architecture from effects of the overall volume fraction of the AC block. The micro-segregated morphologies of these systems include AC-block filled spherical microdomains in a liquid-like state (SPHLL), AC-block filled hexagonally-packed cylinders (CYL), alternating lamellar microlayers of AC- and L-blocks (LAM) and L-block filled hexagonally-packed cylinders (CYL*). For systems with the same parent diblock copolymer that also carry a constant volume fraction of AC-blocks, an increase in the side-chain length (l(sc)) and/or in the number of alkyl tails at a side-chain branch point (m) result in a microdomain morphology that is either unchanged or changed into one with a less curved interface. For lamellae-forming L-b-AC ionic supramolecules made up of the same parent diblock copolymer, a body of experimental data showing small variations in the repeat distance of the LAM micro-segregated structure (d(LAM)) could be obtained by making appropriate combinations of X, lsc and m. For such lamellae-forming L-b-AC ionic supramolecules, the average of d(LAM) values decreases when l(sc) and/or m increase. For systems with (C12)(2)-type side-chains, the effective Flory-Huggins interaction parameter between L-and AC-blocks chi'(C12)(2)/X was determined at various side-chain grafting densities and it was observed that chi'(C12)(2)/X increases when X is increased.

  • Glycaemic control of Type1 diabetes in clinical practice early in the 21st century: an international comparison

    • AimsImproving glycaemic control in people with Type1 diabetes is known to reduce complications. Our aim was to compare glycaemic control among people with Type1 diabetes using data gathered in regional or national registries. MethodsData were obtained for children and/or adults with Type1 diabetes from the following countries (or regions): Western Australia, Austria, Denmark, England, Champagne-Ardenne (France), Germany, Epirus, Thessaly and Thessaloniki (Greece), Galway (Ireland), several Italian regions, Latvia, Rotterdam (The Netherlands), Otago (New Zealand), Norway, Northern Ireland, Scotland, Sweden, Volyn (Ukraine), USA and Wales) from population or clinic-based registries. The sample size with available data varied from 355 to 173880. Proportions with HbA(1c) <58mmol/mol (<7.5%) and 75mmol/mol (9.0%) were compared by age and sex. ResultsData were available for 324501 people. The proportions with HbA(1c) 58mmol/mol (<7.5%) varied from 15.7% to 46.4% among 44058 people aged <15years, from 8.9% to 49.5% among 50766 people aged 15-24years and from 20.5% to 53.6% among 229677 people aged 25years. Sex differences in glycaemic control were small. Proportions of people using insulin pumps varied between the 12 sources with data available. ConclusionThese results suggest that there are substantial variations in glycaemic control among people with Type1 diabetes between the data sources and that there is room for improvement in all populations, especially in young adults. We present HbA(1c) data from registries in 19 different countries describing control in 324501 people with Type1 diabetes, across all age groups. These data are the best representation of diabetes care available and therefore describe the state of the art'. We show clearly that Type1 diabetes control is not as good as suggested in guidelines, but that some healthcare systems appear to result in better control than others. These data present a challenge to diabetes services. Leaders in diabetes units/service can compare their local data to our data and encourage improvement.

  • Hemoglobin A1c and Fructosamine Evaluated in Patients with Type 2 Diabetes Receiving Peritoneal Dialysis Using Long-Term Continuous Glucose Monitoring

    • Introduction: Shortened erythrocyte life span and erythropoietin-stimulating agents may affect hemoglobin A1c (HbA1c) levels in patients receiving peritoneal dialysis (PD). We compared HbA1c with interstitial glucose measured by continuous glucose monitoring (CGM) in patients with type 2 diabetes receiving PD.Methods: Fourteen days of CGM (Ipro2, Medtronic) were performed in 23 patients with type 2 diabetes receiving PD and in 23 controls with type 2 diabetes and an estimated glomerular filtration rate over 60 mL/min/1.73 m2. Patients were matched on gender and age (±5 years). HbA1c (mmol/mol), its derived estimate of mean plasma glucose (eMPGA1c) (mmol/L), and fructosamine (µmol/L) were measured at the end of the CGM period and compared with the mean sensor glucose (mmol/L) from CGM.Results: In the PD group, mean sensor glucose was 0.98 (95% con-fidence interval (CI): 0.43-1.54) mmol/L higher than the eMPGA1c compared with the control group (p = 0.002) where glucose levels were nearly identical (-0.05 (95% CI: -0.35-0.25) mmol/L). A significant association was found between fructosamine and mean sensor glucose using linear regression with no difference between slopes (p = 0.89) or y-intercepts (p = 0.28).Discussion/Conclusion: HbA1c underestimates mean plasma glucose levels in patients with type 2 diabetes receiving PD. However, the clinical significance of this finding is undetermined. Fructosamine seems to more accurately reflect glycemic status. CGM or fructosamine could complement HbA1c to increase the accuracy of glycemic monitoring in the PD population.

    • INTRODUCTION: The accuracy of hemoglobin A1c (HbA1c) as a glycemic marker in patients with type 2 diabetes (T2D) receiving hemodialysis (HD) remains unknown. To assess accuracy, we compared HbA1c and fructosamine levels with interstitial glucose measured by continuous glucose monitoring (CGM) in patients with T2D receiving HD.METHODS: Thirty patients in the HD group and 36 patients in the control group (T2D and an estimated glomerular filtration rate >60 mL/min/1.73 m2) completed the study period of 17 weeks. CGM (Ipro2®, Medtronic) was performed 5 times for periods of up to 7 days (with 4-week intervals) during a 16-week period. HbA1c (mmol/mol), the estimated mean plasma glucose from HbA1c (eMPGA1c [mmol/L]) and fructosamine (μmol/L) was measured at week 17 and compared with mean sensor glucose levels from CGM.FINDINGS: In the HD group, mean sensor glucose was 1.4 mmol/L (95% confidence interval [CI]: 1.0-1.8) higher than the eMPGA1c, whereas the difference for controls was 0.1 mmol/L (95% CI: -0.1-[0.4]; p < 0.001). Adjusted for mean sensor glucose, HbA1c was lower in the HD group (-7.3 mmol/mol, 95% CI: -10.0-[-4.7]) than in the control group (p < 0.001), with no difference detected for fructosamine (p = 0.64).DISCUSSION: HbA1c evaluated by CGM underestimates plasma glucose levels in patients receiving HD. The underestimation represents a clinical challenge in optimizing glycemic control in the HD population. Fructosamine is unaffected by the factors affecting HbA1c and appears to be more accurate for glycemic monitoring. CGM or fructosamine could thus complement HbA1c in obtaining more accurate glycemic control in this patient group.

  • Nanoporous zirconia microspheres prepared by salt-assisted spray drying

    • Nanoporous zirconia with high surface area and crystallinity has a wide range of industrial applications, such as in inorganic exchangers for ion exchange columns, catalyst substrates, and packing material for HPLC. Spherical particles of crystalline nanoporous zirconia are highly desired in various industries due to easy handling of the materials in a fluidized bed. Here, spray drying was adopted to produce spherical nanoporous zirconia powders in both laboratory scale and pilot plant scale. Effect of salts on spray-dried ZrO2 powders and their crystallization behavior was studied. It was found that addition of salts to the zirconia precursors has a huge effect on the crystallization of nanoporous zirconia powders. These results have a great impact on the development of microspheres of nanocrystalline ZrO2 and potentially open up a new opportunity to the low-cost production of porous ceramic microspheres with the salt templating method, in general.
