  • Cost-effectiveness of internet-based cognitive behavior therapy for irritable bowel syndrome: results from a randomized controlled trial

    • Background: Irritable Bowel Syndrome (IBS) is highly prevalent and is associated with a substantial economic burden. Cognitive behavior therapy (CBT) has been shown to be effective in treating IBS. The aim of this study was to evaluate the cost-effectiveness of a new treatment alternative, internet-delivered CBT based on exposure and mindfulness exercises. Methods: Participants (N = 85) with IBS were recruited through self-referral and were assessed via a telephone interview and self-report measures on the internet. Participants were randomized to internet-delivered CBT or to a discussion forum. Economic data was assessed at pre-, post- and at 3-month and 1 year follow-up. Results: Significant cost reductions were found for the treatment group at $16,806 per successfully treated case. The cost reductions were mainly driven by reduced work loss in the treatment group. Results were sustained at 3-month and 1 year follow-up. Conclusions: Internet-delivered CBT appears to generate health gains in IBS treatment and is associated with cost-savings from a societal perspective.

  • Plug-in Hybrid Electric Vehicles as Regulating Power Providers - Case Studies of Sweden and Germany

    • This study investigates plug-in hybrid electric vehicles (PHEVs) as providers of regulating power in the form of primary, secondary and tertiary frequency control. Previous studies have shown that PHEVs could generate substantial profits while providing ancillary services. This study investigates under what conditions PHEVs can generate revenues using actual market data, i.e. prices and activations of regulating power, from Sweden and Germany from four months in 2008. PHEV market participation is modelled for individual vehicles in a fleet subject to a simulated movement pattern. Costs for infrastructure and vehicle-to-grid equipment are not included in the analysis. The simulation results indicate that maximum average profits generated on the German markets are in the range 30–80 h per vehicle and month whereas the Swedish regulating power markets give no profit.In addition, an analysis is performed to identify strengths, weaknesses, opportunities, and threats (SWOT) of PHEVs as regulating power providers. Based on the simulation results and the SWOT analysis, characteristics for an ideal regulating power market for PHEVs are presented.

    • Initial viral decay rate may be useful when comparing the relative potency of antiretroviral regimens. Two hundred twenty-seven ART-naïve patients were randomized to receive efavirenz (EFV) (n = 74), lopinavir/ritonavir (LPV/r) (n = 77), or atazanavir/ritonavir (ATV/r) (n = 79) in combination with two NRTIs. The most frequently used NRTI combinations in the EFV and ATV/r groups were the nonthymidine analogues tenofovir and emtricitabine or lamivudine (70% and 68%, respectively) and, in the LPV/r group, lamivudine and the thymidine analogue zidovudine (89%). HIV-1 RNA was monitored during the first 28 days after treatment initiation. Phase 1 and 2 decay rate was estimated in a subset of 157 patients by RNA decrease from days 0 to 7, and days 14 to 28. One-way ANOVA and subsequent Tukey's post hoc tests were used for groupwise comparisons. Mean (95% CI) HIV-1 RNA reductions from days 0 to 28 were 2.59 (2.45-2.73), 2.42 (2.27-2.57), and 2.13 (2.01-2.25) log(10) copies/ml for the EFV-, LPV/r-, and ATV/r-based treatment groups, respectively, with a significantly larger decrease in the EFV-based group at all time points compared with ATV/r (p < 0.0001), and with LPV/r at days 7-21 (p < 0.0001-0.03). LPV/r gave a greater RNA decrease compared with ATV/r from day 14 (p = 0.02). Phase 1 decay rate was significantly higher in the EFV group compared with LPV/r (p = 0.003) or ATV/r (p < 0.0001). No difference was found in phase 2 decrease. EFV-based treatment gave a more rapid decline in HIV-1 RNA than did either of the boosted protease inhibitor-based regimens. The observed differences may reflect different inherent regimen potencies.

  • Double photoionization of alcohol molecules

    • The double valence photoionization spectra of methanol, ethanol, and n-propyl alcohol have been recorded using a time-of-flight photoelectron-photoelectron coincidence technique. The spectra show a well-defined onset followed by broad rounded bands. The lowest vertical double ionization energies have been determined for all molecules and are found to be 32.1, 29.6, and 28.2 eV, respectively. These energies have been applied along with single ionization energies from conventional photoelectron spectra to investigate a recently derived rule of thumb for determination of the lowest double ionization energy in molecules. Many-electron ab initio calculations have been performed on the dicationic ground states in good agreement with the experimental values. For methanol, also excited dicationic states have been calculated up to about 40 eV and used for a detailed interpretation of the experimental spectrum.

  • Conservation of Affinity Rather Than Sequence Underlies a Dynamic Evolution of the Motif-Mediated p53/MDM2 Interaction in Ray-Finned Fishes

    • The transcription factor and cell cycle regulator p53 is marked for degradation by the ubiquitin ligase MDM2. The interaction between these 2 proteins is mediated by a conserved binding motif in the disordered p53 transactivation domain (p53TAD) and the folded SWIB domain in MDM2. The conserved motif in p53TAD from zebrafish displays a 20-fold weaker interaction with MDM2, compared to the interaction in human and chicken. To investigate this apparent difference, we tracked the molecular evolution of the p53TAD/MDM2 interaction among ray-finned fishes (Actinopterygii), the largest vertebrate clade. Intriguingly, phylogenetic analyses, ancestral sequence reconstructions, and binding experiments showed that different loss-of-affinity changes in the canonical binding motif within p53TAD have occurred repeatedly and convergently in different fish lineages, resulting in relatively low extant affinities (KD = 0.5 to 5 mu M). However, for 11 different fish p53TAD/MDM2 interactions, nonconserved regions flanking the canonical motif increased the affinity 4- to 73-fold to be on par with the human interaction. Our findings suggest that compensating changes at conserved and nonconserved positions within the motif, as well as in flanking regions of low conservation, underlie a stabilizing selection of "functional affinity" in the p53TAD/MDM2 interaction. Such interplay complicates bioinformatic prediction of binding and calls for experimental validation. Motif-mediated protein-protein interactions involving short binding motifs and folded interaction domains are very common across multicellular life. It is likely that the evolution of affinity in motif-mediated interactions often involves an interplay between specific interactions made by conserved motif residues and nonspecific interactions by nonconserved disordered regions.

We previously found that variations in p53TAD from zebrafish result in a 20-fold weaker interaction with MDM2 as compared to other vertebrates. Here we investigate the evolution of the p53TAD/MDM2 among ray-finned fishes (Actinopterygii), the largest vertebrate clade. Intriguingly, phylogenetic analyses and binding experiments showed that different loss-of-affinity changes in the canonical binding motif within p53TAD have occurred on several occasions in different lineages of fishes. Moreover, in D. rerio p53TAD we found that the C-terminal flanking region of the canonical motif rescued affinity through a new, overlapping binding motif that is not present in other fish p53TADs. In a wider perspective, protein-protein interactions involving short, conserved binding motifs and folded interaction domains are very common across multicellular life and it is probable that flanking regions in general play a prominent role in fine tuning the affinity of motif-domain interactions.

    • A rate-based model is developed to elucidate the chemistry behind the simultaneous absorption of NOx and SOx under pressurized conditions (pressures up to 30 bar) that are applicable to the flue gases obtained from CO2 capture systems. The studied flue gas conditions are relevant to oxy-fuel and chemical-looping combustion systems. The kinetics of the reactions implemented in the model is based on a thorough review of the literature. The chemistry of nitrogen, sulfur, and N-S interactions are evaluated in detail, and the most important reaction pathways are discussed. The effects of pH, pressure, and flue-gas composition on the liquid-phase chemistry are also examined and discussed. Simulations that use existing kinetic data reveal that the pH level has a strong influence on the reaction pathway that is followed and the types of products that are formed in the liquid phase. In addition, the pressure level and the presence of NOx significantly affect the removal of SO2 from the flue gas.
