  • A new oxygen carrier for improved long-term storage of human pancreata before islet isolation

    • BACKGROUND: Pancreas oxygenation during cold storage has been established in islet isolation and transplantation to prevent ischemic tissue damage using perfluorodecalin (PFD) as hyperoxygen carrier. However, studies in humans and pigs provided conflicting results about the efficiency of PFD for pancreas oxygenation. The aim of this study was to compare PFD with a newly developed oxygen carrier composed of perfluorohexyloctane and polydimethylsiloxane 5 (F6H8S5) for long-term storage of human pancreata.METHODS: After 24-hr storage in preoxygenated PFD or F6H8S5, pancreata were processed using Liberase HI for pancreas dissociation and a Ficoll gradient for islet purification. Islet quality assessment was performed measuring glucose-stimulated insulin release, viability, islet ATP content, and posttransplant function in diabetic nude mice.RESULTS: Compared with PFD, F6H8S5 significantly increased the intrapancreatic partial oxygen pressure and islet ATP content. This corresponded to an increase of islet yield, recovery after culture, glucose stimulation index, viability, and improved graft function in diabetic nude mice.CONCLUSIONS: The present findings indicate clearly that F6H8S5 improves isolation outcome after prolonged ischemia compared with PFD. This observation seems to be related to the significant lipophilicity and almost pancreas-specific density of F6H8S5. Moreover, these characteristics facilitate pancreas shipment without using custom-made transport vessels as required for PFD.

Vi diskuterar även hur familjepolitiken påverkar barnafödandet och hur den kan modifiera eventuella samband mellan ekonomisk tillväxt och benägenheten att skaffa barn. Bakgrunden till bidraget är den senaste tidens ekonomiska kris, en utveckling som i Sverige inträffat under en period av relativt höga och stigande fruktsamhetstal.

  • Cohort fertility patterns in the Nordic countries

    • Previous analyses of period fertility suggest that the trends of the Nordic countries are sufficiently similar to speak of a common "Nordic fertility regime". We investigate whether this assumption can be corroborated by comparing cohort fertility patterns in the Nordic countries. We study cumulated and completed fertility of Nordic birth cohorts based on the childbearing histories of women born in 1935 and later derived from the population registers of Denmark, Finland, Norway, and Sweden. We further explore childbearing behaviour by women’s educational attainment. The results show remarkable similarities in postponement and recuperation between the countries. Median childbearing age is about 2−3 years higher in the 1960−64 cohort than in the 1950−54 cohort, but the younger cohort recuperates the fertility level of the older cohort at ages 30 and above. A similar pattern of recuperation can be observed for highly educated women as compared to women with less education, resulting in small differences in completed fertility across educational groups. Another interesting finding is that of a positive relationship between educational level and the final number of children when women who become mothers at similar ages are compared. Despite some differences in the levels of childlessness, country differences in fertility outcome are generally small. The cohort analyses thus support the notion of a common Nordic fertility regime.

    • In a Europe where most countries exhibit low or “lowest-low” fertility, the experience of Sweden and its Nordic neighbors has become of increasing interest to policy makers and social scientists concerned with the causes and consequences of low fertility. Nordic fertility can be labeled as “highest-low”: the fertility of Sweden and its neighbors is below the replacement level of 2.1 children per woman but still high as compared to many other developed countries. Fertility in Sweden has fluctuated during recent decades but, as in the other Nordic countries with a similar welfare-state setup, it has stayed well above the European average. The present chapter provides insights into the recent childbearing developments in Sweden and discusses the role of family policies in shaping childbearing behavior. Evidence is provided that institutional factors appear to be far more decisive than cultural ones in influencing childbearing behavior, and some specific impacts of family policies on childbearing dynamics are demonstrated. In this respect, it is important to note that Swedish family policies have never been aimed directly at encouraging childbirth. Their main goal has rather been to support women’s labor-force participation and to promote gender equality: any fertility-stimulating impact needs to be seen as a side effect of these policies. The reconciliation of family and working life of women in Sweden is supported by the design of the country’s taxation, social-security, and parental-leave systems, and the easy access to high-quality subsidized child-care.

    • Patterns in family formation in Sweden display a number of regularities. After nest-leaving and a period of labor-market adjustments, union formation and union dynamics, couples tend to consider having children. A prerequisite for this normally is that both partners are established properly in the labor market. After becoming parents, both women and men remain active in the labor force. These patterns in behavior are supported by the impact of Swedish family policies whose main goal it is to support women’s and men’s labor-force participation and to promote social and gender equality. A side effect of these policies seems to be a relatively high fertility as the policy setup removes many of the obstacles for women to combine work and family life. The reconciliation of family and working life is facilitated by (i) individual taxation and individual-based social-security systems, which makes it less attractive for couples to pursue gendered segregation of work and care, (ii) an income-replacement based parental-leave system, which gives women incentives to establish themselves in the labor market before considering childbirth, and (iii) subsidized child-care, which allows women to return to work after parental leave. In this review, I discuss the role of Swedish family policy in shaping life-cycle behavior and demonstrate some specific impacts of family policies on childbearing dynamics.
