  • Effects of feed composition, protein denaturation and storage of milk serum protein/lactose powders on lactosylation

    • During whey powder production, the feed is subjected to several heat treatments which can cause lactosylation of proteins. In this study, lactosylation of whey proteins was evaluated in spray-dried powders before and after storage by varying the native protein fraction as well as the serum protein/lactose ratio in the powders. The lactosylation of native α-lactalbumin and β-lactoglobulin in the powders before storage was not affected to a large extent by the protein denaturation or if the feed had been heat treated in a high or low lactose environment. After storage (relative humidity of 23.5%, 30 °C, 25 days), the kinetic of lactosylation tended to increase with increasing native protein fraction and bulk protein content in the powders. An explanation could be that proteins dissolved in the lactose glassy structure might have a lower reactivity, while proteins present in the protein glassy structure with dissolved lactose may display higher lactosylation reactivity.

    • This chapter takes an interest in the world of platforms for creative and cultural work. Platforms for creative and cultural work started to become popular in the late 20th century, and their popularity has grown rapidly over the years. Taking its point of departure from these platforms, this chapter aims to shed light upon and discuss the work of the professionals who handle and manage these platforms. Through narratives of and about the work of these platform professionals, the chapter illustrates how creativity and creative work are encouraged and facilitated. The main contribution of the chapter is the identification and discussion of two core practices in platform professionals’ narratives of how they enable creativity and cultural work. We reflect upon how creativity is conceived of as a project requiring different kinds of administrative and material support, which the managers of these creative clusters, incubators, and makerspaces provide. Furthermore, we introduce the idea that platform professionals can be perceived as a contemporary guild whose actions, and use of language and terms illustrate the creative imperative of our time.

    • Mixed anxiety and depression is common among older adults. The aim of the study was to compare the effects of an eight-week-long tailored internet-supported cognitive behaviour therapy (ICBT) programme and to compare against the provision of weekly general support. A second aim was to investigate if pre-treatment cognitive flexibility and self-reported cognitive problems would predict outcome. We included 66 older adults (aged over 60 years) with mixed anxiety/depression following media recruitment and randomised them into treatment and control groups. We also included a one-year follow-up. As a measure of executive function, we used the Wisconsin Card Sorting Test (perseverative errors) and the Cognitive Failures Questionnaire during the pre-treatment phase. Results showed a moderate between-group effect on the main outcome measure, the Beck Anxiety Inventory (BAI) (d= .50), favouring the treatment group. Nearly half (45.5%) of that group were classified as responders. One person (3%) in the treatment group deteriorated. There were significant correlations between perseverative errors and outcome (on the BAI r = -.45), but not among self-reported cognitive function. We conclude that guided, tailored ICBT may be effective for some older adults and that the role of cognitive function needs to be investigated further.

  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

  • Functionality of Spray-Dried Whey Protein Powders - Surface Composition, Particle Morphology and Rehydration

    • Whey protein powder functionality is expected to be closely linked to both structure and properties of the proteins. It is essential that whey protein powders are easily dispersed and dissolved in order to fulfil the specified nutrient content and the functionality in the final product. Poor rehydration can cause challenges on an industrial level as well as for the consumers. In this thesis, several studies were carried out, examining the effects of varying the composition in the feed on surface properties, particle morphology and functional properties of spray-dried powders. A membrane filtered product, serum protein concentrate (SPC), with a high fraction of native proteins, was studied with varying lactose content using different techniques. For SPC/lactose (% w/w) systems, the stiffness of the interface of the feed droplet had an impact on the particle morphology. Feed droplets with a high modulus of elasticity and, thus, a stiff interface resulted in particles with thick ridges and deep dents. Systems with a low modulus of elasticity resulted in particles that were either smooth or covered with a high frequency of dents and thin ridges. The time needed to obtain wettability of these powders showed a positive relationship with the protein surface coverage, which was estimated by X-ray photoelectron spectroscopy (XPS). Microstructural investigations of the internal structures of the particles with confocal Raman microscopy revealed that the protein-rich domain in the vicinity of the powder particle tended to become thinner as the bulk protein concentration increased in the powders. This suggests that the protein surface coverage has a more important role for the wettability than the thickness of the protein layer. Scanned electron microscopy images revealed a similar particle morphology as the fraction of native proteins decreased from 100 to 45% as a result of heat treatment of the feed. However, the interior of the particle showed large differences where protein aggregates could be distinguished. The results imply that the surface composition was rather similar. In addition, the rehydration properties of these powders were not affected to a large extent by the protein denaturation. However, in serum protein/lactose 40/60 (% w/w) powders with a large fraction of aggregated proteins (95%), it was observed that an addition of native proteins improved the wettability. Further, the results from the XPS indicated that the powders with <15% native protein had approximately 10-15% of denatured/aggregated proteins at the particle surface which could explain the poorer wettability of these powders. Lactosylation of the native protein fraction had no effect on the rehydration properties of serum protein/lactose (% w/w) 1/99 and 60/40 powders even though the degree of lactosylation increased from 10 to 35% in some of the powders after storage (30°C, aw 0.23 for 25 days). On the other hand, it was observed that lactosylation was more pronounced in powders with a high fraction of proteins and not in the powders with a high fraction of lactose. It is suggested that the rate of lactosylation is higher in protein-rich domains with dissolved lactose than in lactose-rich domains with dissolved proteins. Even though the feed was subjected to severe heat treatment and a large fraction of the protein aggregated, the rehydration properties and the particle morphology of the spray-dried powder was not affected to a large extent. Thus, as long as there are native proteins present in the system, they tend to dominate the particle surface and thereby protect the denatured and aggregated proteins, which are mainly found in the interior of the powder particles. This insight has relevance for formulation of whey powders with improved rehydration properties.

    • This article reviews and compares the use of confinement and other restrictive measures against young people under 18 in child welfare and/or the criminal justice systems in Denmark, Sweden, Finland, and Norway. Young people are confined for a variety of reasons, including protection, care, treatment, and punishment. However, confinement of young people is a contested issue because it can beviewed as necessary but also potentially harmful. Comparison of legislation and practices reveals that while there are some similarities in the service provisions for young people, there are also significant disparities among the four countries regarding the organization, function, and frequency of the use of confinement and restrictive measures. While Denmark and Sweden use secure welfare institutions, Finland and Norway apply other restrictive measures. Despite the differences in approaches to confinement in the Nordic countries, the use of confinement is guided by the principle of the child’s best interest, and the child welfare system is the main frame for confinement and intervention. The article discusses these disparate practices from the perspective of children’s rights and identifies new avenues for research and practice.

    • The primary objective of this multicenter, multinational, epidemiological study is the identification of infectious agents, dietary factors, or other environmental exposures that are associated with increased risk of autoimmunity and type 1 diabetes mellitus (T1DM). Factors affecting specific phenotypic manifestations such as early age of onset or rate of progression or with protection from the development of T1DM will also be identified. The Environmental Determinants of Diabetes in the Young (TEDDY) is an observational cohort study in which newborns who are younger than 4 months and have high-risk human leukocyte antigen alleles in the general population or are first-degree relatives (FDRs) of patients affected with T1DM will be enrolled. Six clinical centers in the USA and Europe will screen 361 588 newborns, of which it is anticipated that 17 804 will be eligible for enrollment with just over 7800 followed. Recruitment will occur over 5 yr, with children being followed to the age of 15 yr. Identification of such factors will lead to a better understanding of disease pathogenesis and result in new strategies to prevent, delay, or reverse T1DM.
