  • Perceptions of intimacy and integrity in formal home care : [Föreställningar om intimitet och integritet i hemtjänst och personlig assistans]

    • Sweden has a long tradition of providing social care and support to its citizens in their own homes through formal home care, delivered either by home care services or personal assistance. A majority of people given support by formal home care need assistance with personal and intimate care. The focus of this interview study was on exploring care recipients’, care workers’, personal assistants’ and care unit managers’ perceptions and experiences of intimate and personal care in the context of formal home care in Sweden. In total, 57 interviews were conducted with 42 persons. Three themes emerged in the analysis: Personal Hygiene, Personal Sphere, and The Contextual Variability of Intimacy. Interviewees described intimate care as being inseparable from a person’s service needs as a whole. Highlighted was how caring for and washing intimate body parts, intrusion into recipients’ personal spheres, and the need to preserve integrity vary depending on situational, temporal and relational aspects. To safeguard the care recipient’s influence, integrity and dignity in the reception/provision of care work, home care services and personal assistance, it is important to raise awareness of the variation in perceptions of intimate and personal care in education and inhouse training.

    • Sweden, as a welfare state, has a long tradition of providing formal home care and support to their citizens in their own homes, either through home care services or personal assistance. A large percentage of frail elderly and persons with disability who receive formal home care require personal and intimate care, such as help with eating, showering, getting dressed and personal hygiene. Managing intimacy and safeguarding the care recipient’s integrity pose particular challenges for staff. The aim of this qualitative interview study is to describe and analyse care workers’ (CWs) and personal assistants’ (PAs) strategies for managing situations and challenges related to provision of personal and intimate care in the context of formal home care. Semi-structured interviews with eleven CWs and nine PAs were conducted. Our analysis reveals a complex repertoire of relational and communicative strategies, within an overall approach - which we labelled 'empathetic attuning'—of relating to the current situation and task at hand whilst safeguarding integrity. These strategies were intertwined with dimensions of time. The possibility to accomplish satisfactory personal and intimate care rests on structural and organisational conditions that promote sustainable working conditions, where relations characterised by continuity, integrity and respect can be realised.

  • CO2-system development in young sea ice and CO2 gas exchange at the ice/air interface mediated by brine and frost flowers in Kongsfjorden, Spitsbergen

    • In March and April 2010, we investigated the development of young landfast sea ice in Kongsfjorden, Spitsbergen, Svalbard. We sampled the vertical column, including sea ice, brine, frost flowers and sea water, to determine the CO2 system, nutrients, salinity and bacterial and ice algae production during a 13 day interval of ice growth. Apart from the changes due to salinity and brine rejection, the sea-ice concentrations of total inorganic carbon (C T), total alkalinity (A T), CO2 and carbonate ions (CO3 2–) in melted ice were influenced by dissolution of calcium carbonate (CaCO3) precipitates (25–55 μmol kg–1) and played the largest role in the changes to the CO2 system. The C T values were also influenced by CO2 gas flux, bacterial carbon production and primary production, which had a small impact on the C T. The only exception was the uppermost ice layer. In the top 0.05 m of the ice, there was a CO2 loss of ∼20 μmol kg–1 melted ice (1 mmol m–2) from the ice to the atmosphere. Frost flowers on newly formed sea ice were important in promoting ice–air CO2 gas flux, causing a CO2 loss to the atmosphere of 140–800 μmol kg–1 d–1 melted frost flowers (7–40 mmol m–2 d–1).

  • Biogenic halocarbons in young Arctic sea ice and frost flowers

    • The fate of halocarbons, naturally produced volatile halogenated organic compounds, in young Arctic sea ice was studied to better understand the role of sea ice in halocarbon cycling. In early spring, halocarbons were measured in sea ice frozen in core holes, during 12 days of formation and freezing. In order to understand which factors govern halocarbon concentration and distribution, salinity, temperature and biological parameters were monitored in the growing sea ice. It was found that sea ice participates in the cycling of halocarbons between sea and air. Sea ice concentrations and distributions of these compounds were influenced by production in the ice, where ice-inhabiting microorganisms caused local increases in halocarbon concentrations. Moreover, the halocarbon ice concentration decrease/change with time did not follow ice salinity, suggesting that additional removal processes caused sea ice to be a source of halogens to overlying air. The net production rate of bromoform in the surface of newly frozen ice was estimated to 14 pmol L−1 d−1 and the maximum removal rate was 18 pmol L−1 d−1. In addition frost flowers on newly formed sea ice were identified as contributors of halocarbons to the atmosphere with halocarbon concentrations in the same order of magnitude as in sea ice brine

    • Summary: An interview study was conducted with persons receiving home care services and personal assistance in Sweden (13 individuals, 25 interviews) with the aim to analyze their experiences of managing personal and intimate care. The analytical approach was guided by phenomenologically informed research and Erving Goffman's theoretical work on self-presentation and social life as it differs in frontstage and backstage settings.Findings: A reflected approach to the complex challenges associated with becoming and being a person in need of personal and intimate care was revealed. This involved continuous adaptations and attuning to organizational and relational conditions of formal home care. Being a recipient of personal and intimate care does not mean being passive. It entails relating to and sustaining the care relation, where even choosing to accept suboptimal conditions is an act of agency. The recipients’ private homes were hybridized, transformed both into a waiting room, with the recipient on standby and into a workplace. The homes thus partly lost their character as a backstage realm where one could avoid the gaze of others. This also led to a hybridization of the personal sphere, in the form of marginal scope for true privacy, necessitating strategies for protecting one's own space.Applications: It is important both to acknowledge the intrusive nature of personal and intimate care, which results in extensive hybridization of the home and personal sphere and to recognize care recipients’ agency in the relationship that care establishes.

  • Ett meningsfullt liv med arbete? : Högfungerande individer med autism mellan funktionshinder- och arbetsmarknadspolitik

    • The aim of this thesis is to study notions of employment among high-functioning individuals with autism (HiA) and their experiences of vocational rehabilitation or work-preparatory programs and to analyze how these experiences reflect upon the labor market policy’s and disability policy’s common shared goal of supporting individuals towards a socially included life with meaningful work. This study was conducted through ethnographic fieldwork in a private company (VRP) that operated as a service provider for the Employment Service Agency, and a Swedish residential folk high school (FHS). Both of the organizations provided work-preparative programs specifically for HiA. Three of the four sub-studies are based on qualitative data consisting of individual interviews, focus group interviews, and field notes from participant observations. The fourth sub-study represents an analysis of legal documents targeting employment and health in disability policy.The results in sub-study 1 illustrate that the narratives provided by HiA strongly address the subject position of the citizen-worker. Their suggested ideal jobs were deprioritized in favor of more available jobs on the market. The central incentives to pursue either the ideal or second-option jobs can be identified as the personal perception of attaining a sense of meaningfulness through work, which was depicted as central to the sense of self-fulfillment, identity, and wellbeing. As the second sub-study shows, the fulfillment of personal ambitions and ideas of meaningful work was not met in the case of the VRP participants. Social inclusion was not experienced as a result of the received service provisions. The results rather indicate a manifestation of the normalization principle’s basic attributes with regard to spatial integration in work-like activities (disability-worker contexts) rather than social inclusion on an equal basis with others (citizen-worker contexts). The third sub-study then showcases how social work practices were produced at the intersection of clients’ needs and personas and the organizations’ conditional premises in FHS and VRP. The analysis resulted in the conclusion that a person-centered approach was lacking in the VRP whereas highly present in the FHS. The fourth and last sub-study focuses on how Swedish disability policy is constructed to meet the objectives of the CRPD regarding active citizenship that is associated with health (engagements in private life, work-life, community life, social/family life, etc.) and thereby full participation on an equal basis with others. The results suggest that the policy area of employment, implicitly and explicitly, overshadows the policy area of health and intimately associated life-areas that are important to wellbeing.The conclusions are that FHS and ARP both reflect a strong dedication to provide services that aim towards preparing individuals for work. However, a more holistic approach in professional practices must be practiced in VRPs if a socially included life with a sustainable work situation for HiA is strived for. With regard to a socially included life with meaningful work, the above must be accounted for in addition to individuals’ personal perceptions and notions of meaningfulness.

    • AimThe aim of the study was to explore meaningful everyday life situations as perceived by six-year-old children born preterm. Materials and methodsThe study had a descriptive qualitative design with an inductive approach. Ten, six-year-old children born preterm, not diagnosed with any disabilities, participated. Data was collected by photo-elicitation interviews to stimulate and help the children to describe their meaningful everyday life situations. A qualitative content analysis according to Elo and Kyngas was applied. ResultsThe children's descriptions of meaningful everyday life situations can be understood as being in an active and dynamic process, representing the core category. The analysis resulted in three generic categories, as the children described the significance of having significant circumstances and doing things. The experiences the children gain when they do things create their desire for further development. DiscussionThe results reveal that children born preterm are able to reflect on and give detailed descriptions of situations of importance to them. The study suggests that if six-year-old children born preterm are given the opportunity to share their views they can take an active role e.g. in planning and carrying through of interventions by health care services.
