  • Chemical Intolerance Is Associated With Altered Response Bias, not Greater Sensory Sensitivity

    • Chemical intolerance is a surprisingly prevalent condition or affliction characterized by adverse reactions to low levels of chemical, often odorous stimulation. Sufferers often assume that their plight is due to an uncommon sensory acuteness, yet studies repeatedly fail to reveal altered detection thresholds. Here, we investigated whether self-reported chemical intolerance is associated with altered sensory sensitivity or response bias. The sensory acuity (sensitivity; A) and sensory decision rule (criterion; B) to n-butanol was assessed using the method of constant stimuli in 82 participants with different degrees of chemical intolerance (low to high). Higher self-reported chemical intolerance was associated with a lower criterion, but not with sensitivity.

  • Neurocognitive processes underlying heuristic and normative probability judgments

    • Judging two events in combination (A&B) as more probable than one of the events (A) is known as a conjunction fallacy. According to dual-process explanations of human judgment and decision making, the fallacy is due to the application of a heuristic, associative cognitive process. Avoiding the fallacy has been suggested to require the recruitment of a separate process that can apply normative rules. We investigated these assumptions using functional magnetic resonance imaging (fMRI) during conjunction tasks. Judgments, whether correct or not, engaged a network of brain regions identical to that engaged during similarity judgments. Avoidance of the conjunction fallacy additionally, and uniquely, involved a fronto-parietal network previously linked to supervisory, analytic control processes. The results lend credibility to the idea that incorrect probability judgments are the result of a representativeness heuristic that requires additional neurocognitive resources to avoid.

  • The idiopathic environmental intolerance symptom inventory: development, evaluation, and application.

    • OBJECTIVE: To develop, evaluate, and apply a questionnaire-based instrument for investigation of specific symptoms in idiopathic environmental intolerance (IEI), called the Idiopathic Environmental Intolerance Symptom Inventory (IEISI). METHODS: Participants with IEI to chemicals responded to 82 candidate symptoms and to three subscales of the Quick Environmental Exposure and Sensitivity Inventory (QEESI) at a test (n = 207) and retest (n = 193) occasion. RESULTS: The 27 most commonly reported symptoms were selected and grouped into five symptom categories. Internal consistency, test-retest reliability, and concurrent validity were found to be satisfying. Cluster analysis identified two subgroups of IEI to chemicals. CONCLUSIONS: The results provide support for the IEISI being a reliable, valid, and fast tool for the study of specific symptom prevalence in IEI and encourage further study of subgroups.

    • Syftet med denna rapport är att illustrera hur ett praktiskt inriktat ramverk, tekno- logiska innovationssystem (TIS), kan användas av analytiker och beslutsfattare vid departement och myndigheter för att analysera strategiskt viktiga teknikområden ?????????????????????????????????????????????????????????????????????????????I rapporten analyseras fem TIS centrerade kring havsbaserad vindkraft, marin energi, ????????????????????????????????????????????????????????????????????????????????????? systemsvagheter som bromsar områdets vidare utveckling, vilka som kan åtgärdas av systemets aktörer och vilka som motiverar särskilda politiska åtaganden. Rapporten utgör därmed ett underlag för att formulera åtgärder för att åstadkomma ökad innova- tion, teknikspridning och industrialisering inom ovan nämnda teknikområden.Studien har även möjliggjort en jämförande analys av likheter och skillnader ???????????????????????????????????????????????????????????????????????????????? ?????????????????????????????????????????????????????????????????????????????????????? mellan områdena – de är starka respektive svaga av olika orsaker. Detta visar att ???????????????????????????????????????????????????????????????????????Samtidigt har områdena gemensamma drag. TIS-ramverket lyfts här fram som en metod för att skapa ett sådant underlag.   • Comorbidity and multimorbidity of asthma and allergy and intolerance to chemicals and certain buildings

    • Objectives: We tested the hypothesis of high comorbidity between asthma/allergy and chemical intolerance (CI) and between asthma/allergy and building intolerance (BI), and high multimorbidity between asthma/allergy, CI, and BI.Methods: Population-based questionnaire data were used from 530 participants with asthma/allergy (allergic asthma, nonallergic asthma, allergic rhinitis, and/or atopic dermatitis), 414 with self-reported and 112 with physician-diagnosed CI, and 165 with self-reported and 47 with physician-diagnosed BI. Separate reference groups were formed for each of the five case groups.Results: Adjusted odds ratios varied from 4.6 to 13.1 for comorbidity, and from 6.6 to 46.4 for multimorbidity.Conclusion: The large comorbidity and multimorbidity between asthma/allergy, CI, and BI evokes the question as to whether there are similarities in underlying mechanisms between these conditions.

  • Mental fatigue, cognitive performance and autonomic response following sustained mental activity in clinical burnout

    • ObjectiveTo investigate the effects of sustained mental activity on perceptions of mental fatigue, cognitive performance, and autonomic response in patients with clinical burnout as compared to a healthy control group.MethodsPatients with clinical burnout (n = 30) and healthy control participants (n = 30) completed a 3-hour test session, in which they were administered a set of cognitive tests before and after an effortful cognitive task with concurrent sound exposure. Perceptions of mental fatigue and task demands (mental effort and concentration difficulties) were assessed repeatedly over the course of the test session. Heart rate variability was recorded to index autonomic response.ResultsIn comparison with controls, perceived mental fatigue increased earlier in the session for the clinical burnout group and did not recover following a short rest period. Throughout the session, patients rated the tasks as more demanding and showed less improvement on measures of attention and processing speed, inhibition and working memory. While autonomic responses were initially comparable, there was a unique decrease in high-frequency heart rate variability in the clinical burnout group after extended testing and exposure.ConclusionPatients with clinical burnout are affected differently than healthy controls by sustained mental activity, as reflected by ratings of perceived mental fatigue, aspects of cognitive performance and autonomic response. Further investigation into the role of autonomic regulation in relation to cognitive symptoms in clinical burnout is warranted.

    • Being the second leading cause of death and the leading cause of disability-adjusted life years worldwide, infectious diseases remain─contrary to earlier predictions─a major consideration for the public health of the 21st century. Resistance development of microbes to antimicrobial drugs constitutes a large part of this devastating problem. The most widely spread mechanism of bacterial resistance operates through the degradation of existing β-lactam antibiotics. Inhibition of metallo-β-lactamases is expected to allow the continued use of existing antibiotics, whose applicability is becoming ever more limited. Herein, we describe the synthesis, the metallo-β-lactamase inhibition activity, the cytotoxicity studies, and the NMR spectroscopic determination of the protein binding site of phosphonamidate monoesters. The expression of single- and double-labeled NDM-1 and its backbone NMR assignment are also disclosed, providing helpful information for future development of NDM-1 inhibitors. We show phosphonamidates to have the potential to become a new generation of antibiotic therapeutics to combat metallo-β-lactamase-resistant bacteria.

  • Sound intolerance : characteristics, psychosocial work factors and reactions to exposure

    • Sound intolerance refers to an adverse reaction to sounds at sound pressure levels most people do not find bothersome. It is sometimes associated with hearing loss and tinnitus, but neither conditions are a prerequisite for being intolerant to sounds. The processes underlying the heightened reactions to sound, or how sound intolerance is related to other afflictions is not fully known, and research in this area can still be described as being in an early stage. This thesis aims to broaden the knowledge about sound intolerance and the sub-category of hyperacusis by using cross-sectional epidemiological and quasi-experimental methods of inquiry.The thesis consists of three studies, each approaching the subject from a different perspective with the aim to better understand various characteristics of sound intolerance in general, its relation to psychosocial factors in the work environment, and its effects during noise exposure. Studies I & II are cross-sectional studies using two different data sets with similar design. Study I was based on data from a stratified sample in the Västerbotten Environmental Health Study. It examined the characteristics, background descriptions and comorbidities of self-reported and physician diagnosed individuals with hyperacusis, and compared them to a healthy referent group. Analyses revealed that a majority of both hyperacusis groups actively tried to avoid sound sources, and experienced that they for the most part could affect the environment to make it less noisy. There were significantly increased risks for other diagnosed illnesses in the hyperacusis groups compared to the healthy referent group, with large odds ratios (ORs) for the psychiatric illnesses anxiety, depression, post-traumatic stress disorder and exhaustion syndrome. Other common illnesses were tinnitus, hearing impairment and musculoskeletal disorders. Study II used data from the Österbotten Environmental Health Study and focused on the psychosocial work environment for people with hyperacusis. Working participants with hyperacusis who were employed/on leave of absence/paternal leave/long-term sick listed were compared to a working sample without hyperacusis on measures of effort-reward imbalance, work over commitment, emotional and social support and worry at work. The hyperacusis group scored significantly higher on worry, social support, and reward, whereas the groups did not differ significantly with respect to emotional support, over commitment or effort. In the final study, a quasi-experimental design was employed to investigate how individuals with sound intolerance react to noise exposure over time, in terms of perceived sound intensity, unpleasantness, rated distractibility, symptoms, heart rate variability and cognitive performance. Participants were divided into three groups of equal size based on their self-rated sound intolerance, which resulted in low, medium, and high sound intolerance groups. Results revealed large variations across individuals in several outcome variables. Compared to the other two groups, the high sound intolerance group perceived the noise as more unpleasant, stronger, and more distracting. They further rated the symptoms as higher in intensity, showed deviations in heart rate variability, and performed poorer on a cognitive task selected to measure inhibition.In conclusion, the results suggest that persons with sound intolerance have relatively poor general health and hearing as well as high odds of comorbidity with various symptoms, including psychiatric diagnoses and functional somatic syndromes. In a working population, the sound intolerant show high odds for worrying about things at work, perceiving low social support at work and not feeling rewarded at work. Finally, the results suggest that persons with high sound intolerance have increased distractibility, sound unpleasantness, symptoms over time as well as lowered heart rate variability and cognitive performance when exposed to sounds.

    • Background: Hyperacusis is intolerance of certain everyday sounds that causes significant distress and impairment in social, occupational, recreational, and other day-to-day activities. Objective: The aim of this report is to summarize the key findings and conclusions from the Third International Conference on Hyperacusis.Topics covered: The main topics discussed comprise (1) diagnosis of hyperacusis and audiological evaluations, (2) neurobiological aspect of hyperacusis, (3) misophonia, (4) hyperacusis in autism spectrum disorder, (5) noise sensitivity, (6) hyperacusis-related distress and comorbid psychiatric illness, and (7) audiologist-delivered cognitive behavioral therapy for hyperacusis.Conclusions: Implications for research and clinical practice are summarised.
