  • Anorectal Function After Ileo-Rectal Anastomosis Is Better than Pelvic Pouch in Selected Ulcerative Colitis Patients

    • BACKGROUND: With a lifelong perspective, 12% of ulcerative colitis patients will need a colectomy. Further reconstruction via ileo-rectal anastomosis or pouch can be affected by patients' perspective of their quality of life after surgery.AIM: To assess the function and quality of life after restorative procedures with either ileo-rectal anastomosis or ileal pouch-anal anastomosis in relation to the inflammatory activity on endoscopy and in biopsies.METHOD: A total of 143 UC patients operated with subtotal colectomy and ileo-rectal anastomosis or pouches between 1992 and 2006 at Linköping University Hospital were invited to participate. Those who completed the validated questionnaires (Öresland score, SF-36, Short Health Scale) were offered an endoscopic evaluation including multiple biopsies. Associations between anorectal function and quality of life with type of restorative procedure and severity of endoscopic and histopathologic grading of inflammation were evaluated.RESULTS: Some 77 (53.9%) eligible patients completed questionnaires, of these 68 (88.3%) underwent endoscopic evaluation after a median follow-up of 12.5 (range 3.5-19.4) years after restorative procedure. Patients with ileo-rectal anastomosis reported better overall Öresland score: median = 3 (IQR 2-5) for ileo-rectal anastomosis (n = 38) and 10 (IQR 5-15) for pouch patients (n = 39) (p < 0.001). Anorectal function (Öresland score) and endoscopic findings (Baron-Ginsberg score) were positively correlated in pouch patients (tau: 0.28, p = 0.006).CONCLUSION: Patients operated with ileo-rectal anastomosis reported better continence compared to pouches. Minor differences were noted regarding the quality of life. Ileo-rectal anastomosis is a valid option for properly selected ulcerative colitis patients if strict postoperative endoscopic surveillance is carried out.

  • Risk of Rectal Cancer After Colectomy for Patients With Ulcerative Colitis: A National Cohort Study

    • BACKGROUND amp; AIMS: Patients with ulcerative colitis (UC) have an increased risk of rectal cancer, therefore reconstruction with an ileal pouch-anal anastomosis (IPAA) generally is preferred to an ileorectal anastomosis (IRA) after subtotal colectomy. Similarly, completion proctectomy is recommended for patients with ileostomy and a diverted rectum, although this approach has been questioned because anti-inflammatory agents might reduce cancer risk. We performed a national cohort study in Sweden to assess the risk of rectal cancer in patients with UC who have an IRA, IPAA, or diverted rectum after subtotal colectomy.METHODS: We collected data from the Swedish National Patient Register for a cohort of 5886 patients with UC who underwent subtotal colectomy with an IRA, IPAA, or diverted rectum from 1964 through 2010. Patients who developed rectal cancer were identified from the Swedish National Cancer Register. The risk of rectal cancer was compared between this cohort and the general population by standardized incidence ratio analysis.RESULTS: Rectal cancer occurred in 20 of 1112 patients (1.8%) who received IRA, 1 of 1796 patients (0.06%) who received an IPAA, and 25 of 4358 patients (0.6%) with a diverted rectum. Standardized incidence ratios for rectal cancer were 8.7 in patients with an IRA, 0.4 in patients with an IPAA, and 3.8 in patients with a diverted rectum. Risk factors for rectal cancer were primary sclerosing cholangitis in patients with an IRA (hazard ratio, 6.12), and colonic severe dysplasia or cancer before subtotal colectomy in patients with a diverted rectum (hazard ratio, 3.67).CONCLUSIONS: In an analysis of the Swedish National Patient Register, we found that the risk for rectal cancer after colectomy in patients with UC is low, in relative and absolute terms, after reconstruction with an IPAA. An IRA and diverted rectum are associated with an increased risk of rectal cancer, compared with the general population, but the absolute risk is low. Patients and their health care providers should consider these findings in making decisions to leave the rectum intact, perform completion proctectomy, or reconstruct the colon with an IRA or IPAA.

  • A study of some elemental distributions between slag and hot metal during tapping of the blast furnace

    • This paper investigates the distribution of elements between slag and hot metal from a blast furnace through calculation of distribution coefficients from actual production data. First, samples of slag and hot metal tapped from a commercial blast furnace were taken continually at 10-minute intervals for a production period of 68 hours. Distribution coefficients of manganese, silicon, sulphur and vanadium were then calculated from the results of the sample analyses. A major conclusion drawn from examination of the results was that the behaviour of the studied elements was as could be expected when approaching the equilibrium reactions from thermodynamic theory. The distributions of the elements in the slag-metal system showed clear tendencies which did not appear to be influenced by the operational conditions of the furnace. For example, for manganese, vanadium and sulphur, it was found that a higher basicity led to a decreased distribution coefficient L-Mn and L-V, but an increased L-S, which is according to theory. Another observed relationship was that slag basicity increased with an increased carbon content in the hot metal, which indicated that SiO2 was reduced to [Si] when the oxygen potential decreased. Furthermore, it was found that sulphur and silica behaviour likened that of acidic slag components, while the manganese oxide and vanadium oxide behaviour was similar to that of basic slag components.

    • To determine the quality of the hot metal and the thermal conditions inside the blast furnace, the composition of the hot metal and slag must be known. Obtaining representative metal and slag samples during tapping is thus highly important to blast furnace operation. The study covered in the present report focused on hot metal and slag composition variation during tapping from a commercial blast furnace. From the results, optimal sampling time points for obtaining elemental concentrations that can be taken as representative for the whole tapping sequence were identified. It was furthermore concluded, that the reliability of hot metal composition data is significantly improved by averaging elemental concentrations determined from two samples, each taken at a particular time point. One sampling, however, was found to be adequate for slag. Results from the study also showed a fairly strong correlation between amounts of silicon and carbon, sulphur and carbon, and silicon and sulphur in the hot metal, while a weaker correlation between hot metal temperature and each of these elements was observed.

    • The effects of temperature, air humidity, air exchange rate, artificial light and fatty acid constitution on the emissions of aldehydes from linseed oil paint were investigated. Experiments were designed using a multivariate approach and data were analysed using different projection methods (principal component analysis, partial least-squares). Paints used were produced out of two different types of oils, where the fatty acid chains were either mainly linoleic or linolenic acid. The paints were applied onto glass plates and dried in chambers with a controlled climate. Aldehydes were sampled using silica gel impregnated with 2,4-dinitrophenylhydrazine and analysed by HPLC with UV and MS detection. Consecutive sampling of aldehydes every eighth hour as well as total aldehyde sampling was performed during the first 5 days after application of the paints. Hexanal and propanal were the dominating aldehydes emitted for linoleic and linolenic acid rich paint, respectively. Temperature and humidity sped up the emission process, giving a higher total amount of aldehydes. A higher amount of linoleic acid slowed down the emission process and gave higher total amounts of aldehydes. Artificial light and air exchange rate showed no significant effects on the emission process.

  • Effects of continuous feedback on households’ electricity consumption: Potentials and barriers

    • Two field experiments were carried out to study (a) the effects on energy savings of continuous visual feedback via in-home displays, and (b) the motives for responding or not. In study 1, 40 participants living in separate or semi-detached houses in two different towns participated. All participants received a questionnaire and a list of possible energy saving measures. Households were then randomly assigned to an experimental condition (display) or a control condition (no display). In study 2, 32 households in rented apartments participated. No significant differences between the conditions were found for either of the studies. In study 2, semi-structured interviews were conducted among nine of the households. Through an analysis of interview transcripts barriers were identified explaining why the feedback intervention was not sufficient to change behaviour and reduce consumption. The barriers experienced indicate that there is a risk of overconfidence in IHDs. For the development of energy policies and more wide-scale implementation, it is important to be aware of the potential obstacles to success.

  • The Difference Approach to Narrative Fiction : A Recurring Critique of Narratology and Its Implications for the Study of Novels and Short Stories

    • The aim of this thesis is to advance the critical examination of narratology, or the study of storytelling. I analyze four versions of a critique of the dominant theory of narrative fiction in narratology and discuss this critique’s methodological implications. The critics, Sylvie Patron, Lars-Åke Skalin, Richard Walsh, and the proponents of unnatural narratology have, I suggest, similar understandings of narratology’s handling of works like novels and short stories as well as similar alternative approaches. I situate the critique among relevant theories of fiction and salient aspects of narratology, and conclude that the most radical critics have a difference approach to narrative fiction. This means treating this literary practice as following another rule system for creating meaning than other kinds of storytelling. These critics seem to base their reasoning on their readerly intuitions about how novels and short stories function; yet their approach also lends itself to, for instance, discussions on how such works afford life visions or worldviews. In contrast to this approach, I describe narratology, in the critics’ view, as having a sameness approach that treats narrative fiction as a subtype of “narrative” in the sense of the communication of events by a narrator.The three opening articles of the thesis comprise a metadiscussion of the critique. I here describe, in part with Greger Andersson, the critics’ ideas, characterize the critique as a whole, and speculate about why it has had no apparent effect on narratology. The two latter articles utilize the difference approach in analyses of Angela Carter’s “The Loves of Lady Purple” and Sara Stridsberg’s Drömfakulteten (The Faculty of Dreams) while discussing narratological concepts and issues. Future studies might continue this discussion or inquire further about, for example, the relations between different narrative practices or what role different intuitions about narrative fiction play in descriptions and analyses.
