  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

  • Uncovering Barriers for Industrial Symbiosis : Assessing Prospects for Eco-Industrialization through Small and Medium-Sized Enterprises in Developing Regions

    • Industrialization is indispensable for socio-economic development but poses far-reaching implications for resources and the environment. Consequently, industrial symbiosis-based collaborations are gaining recognition as a viable strategy to manage resource consumption for mitigating environmental distress. However, these types of synergistic collaborations are more prevalent in developed regions and are linked to larger-sized industries. Admittedly, such collaborative partnerships are less focused in developing regions and small- to medium-size enterprises (SMEs). This calls for discovering the potentials for synergistic partnerships among the small and medium scaled industrial entities in developing economies. Therefore, the study was designed to identify, assess and explore the prospects of symbiotic collaborations among SMEs in Pakistan. Moreover, the inputs are equally relevant for developing and expanding such productive associations in developing regions. Data regarding inputs and outputs was collected from sixty-one (61) SMEs through field survey during the year 2019. It was processed and analyzed to uncover existing and potential synergies among SMEs. The significant findings transpired that the majority of collaborative linkages are bilateral and are driven to maximize economic gains. However, informal networks of recyclers and absence of a stringent regulatory regime are the ground realities in developing/transforming economies. These factors symptomatically interfere with the process of formal/informal exchanges between and among firms. Furthermore, it was observed that the lack of awareness and impulsiveness of SMEs, as well as the inconsistent supply of by-products, act as barriers for such partnerships in developing countries. It calls for proactive engagements and facilitations from the state institutions through policy instruments. The study addresses a broad audience including industrialists, investors, policy makers and researchers engaged in IS studies. Moreover, the inputs will provide impetus for stimulating eco-industrial progress in developing regions such as Pakistan.

    • Silver nanoparticles (Ag NPs) were synthesized by aqueous chemical growth technique. The above mentioned synthesized materials were characterized by applying scanning electron microscope (SEM) and X-ray diffraction for confirmation of morphological analysis, compositional purity, and crystalline property and emission characteristics as well. In order to fabricate the urea biosensor (potentiometric), a solution of deionized water and chitosan was prepared having Ag NPs. The said solution was dropped on the glass fiber filter having diameter of 2 cm. A wire of copper having thickness of approximately 500 pm was used for the voltage signal to pull out from the said working nanoparticles (NPs). To improve the strength, sensitivity and the quality of the potentiometric urea biosensor, a specific functional surface of Ag NPs was attained by electrostatic restrained of an enzyme (urease) onto the chitosan-Ag (a nanobiocomposite). The potentiometric reaction was measured via electrochemical detection technique. The potentiometric urea biosensor illustrates significant sensibility at room temperature with approximate to 42 mV as per span. Furthermore, the said biosensor showed an appropriate stable response within 7 sec.

  • Application of two-dimensional materials in perovskite solar cells: recent progress, challenges, and prospective solutions

    • Perovskite solar cells (per-SCs) with high performance and cost-effective solution processing have been the center of interest for researchers in the past decade. Power conversion efficiencies (PCEs) have been gradually improved up to 25.2% with relatively improved stability, which is an unparalleled progress in all generations of solar cell (SC) technology. However, there are still some prevailing challenges regarding the stability and upscaling of these promising devices. Recently, 2D layered materials (LMs) have been extensively explored to overcome the prevailing challenges of poor stability (under moisture, light soaking and high temperature), halide segregation, hysteresis, involvement of toxic materials (i.e., lead), and upscaling of devices. A critical review addressing the recent developments in the use of 2D materials, especially transition metal dichalcogenides (TMDCs), is hence necessary. The development of novel synthesis and deposition techniques including liquid-metal synthesis and ultrasonic assisted spray pyrolysis has offered more efficient fabrication of 2D-LMs with controlled thickness and morphology. Effective functionalization approaches to increase the dispersability of 2D-LMs in non-polar solvents has boosted their potential application in solar cell technology as well. Moreover, compositing 2D TMDCs with suitable organic/inorganic compounds has enabled superior charge kinetics in all functional parts of per-SCs. In addition, newly developed materials such as graphyne and graphdyine along with 2D metal organic frameworks (MOFs) and covalent organic frameworks (COFs) have been employed in per-SCs to achieve PCEs up to 20%. This review summarizes the recent progress and challenges in the application of 2D-LMs in per-SCs and outlines the future pathways to further extend the PCE of per-SCs beyond 25%. This review particularly focuses on 2D-LMs as electrode materials and additives, the underlying charge (electron-hole) transport phenomenon in the functional layers, and their chemical and structural stability.

    • In the present study, we demonstrated the use of nanoporous zinc oxide (ZnO NPs) in photodynamic therapy. The ZnO NPs structure possesses a high surface to volume ratio due to its porosity and ZnO NPs can be used as an efficient photosensitizer carrier system. We were able to grow ZnO NPs on the tip of borosilicate glass capillaries (0.5 mu m diameter) and conjugated this with Photofrin for efficient intracellular drug delivery. The ZnO NPs on the capillary tip could be excited intracellularly with 240 nm UV light, and the resultant 625 nm red light emitted in the presence of Photofrin activated a chemical reaction that produced reactive oxygen species (ROS). The procedure was tested in A-549 cells and led to cell death within a few minutes. The morphological changes in necrosed cells were examined by microscopy. The viability of control and treated A-549 cells with the optimum dose of UV/visible light was assessed using the MTT assay, and ROS were detected using a fluorescence microscopy procedure.

    • Energy prices have gone up gradually since last year, but a drastic hike has been observedrecently in the past couple of months, affecting people’s thrift. This, coupled with the load shedding andenergy shortages in some parts of the world, led many to show anger and bitterness on the streets and on socialmedia. Despite subsidies offered by many Governments to their citizens to compensate for high energy bills,the energy price hike is a trending topic on Twitter. However, not much attention is paid to opinion mining onsocial media posts on this topic. Therefore, in this study, we propose a solution that takes advantage of botha transformer-based sentiment analysis method and topic modeling to explore public engagement on Twitterregarding energy prices rising. The former method is employed to annotate the valence of the collected tweetsas positive, neutral and negative, whereas the latter is used to discover hidden topics/themes related to energyprices for which people have expressed positive or negative sentiments. The proposed solution is tested ona dataset composed of 366,031 tweets collected from 01 January 2021 to 18 June 2022. The findings showthat people have discussed a variety of topics which directly or indirectly affect energy prices. Moreover,the findings reveal that the public sentiment towards these topics has changed over time, in particular, in2022 when negative sentiment was dominant.

    • Health and nutritional status of the women of reproductive age (WRA) is expected to be influenced with women literacy status alongside other sociodemographic indicators. However, data are scant to validate if literate young women have lower incidence of anemia prevalence. Nexus to the problem stated, a cross-sectional study was conducted on 1,541 female resident university students (FRUS) aged 17–35 years from a public sector university located in Southern Punjab, Pakistan. Hemoglobin (Hb) led screening for anemia was performed followed by nutritional assessment and structured questionnaire-based sociodemographic and dietary assessment. The data generated were analyzed using independent t-test, Chi-square, and response surface regression models. Response rate for the prevalence of anemia in FRUS was 38 percent with mean Hb levels 10.5 g/dL. With a significant effect (p =.001) of participants’ weight on anemia prevalence, 22.45 percent of the sample population was recorded as underweight. Sociodemographic and dietary parameters analyzed suggested low daily food expenditure (x 2 = 20.59; p =.000) and reduced intake of meat (x 2 = 12.14; p =.01), beans & pulses (x 2 = 18.56; p =.001) to significantly influence rate of anemia prevalence in FRUS. The study concludes high prevalence rate of anemia among FRUS to strongly relate with students’ low monthly stipend, little daily food expenditure, and substandard dietary quality.

  • Tracking development assistance for health and for COVID-19 : a review of development assistance, government, out-of-pocket, and other private spending on health for 204 countries and territories, 1990-2050

    • Background The rapid spread of COVID-19 renewed the focus on how health systems across the globe are financed, especially during public health emergencies. Development assistance is an important source of health financing in many low-income countries, yet little is known about how much of this funding was disbursed for COVID-19. We aimed to put development assistance for health for COVID-19 in the context of broader trends in global health financing, and to estimate total health spending from 1995 to 2050 and development assistance for COVID-19 in 2020. Methods We estimated domestic health spending and development assistance for health to generate total health-sector spending estimates for 204 countries and territories. We leveraged data from the WHO Global Health Expenditure Database to produce estimates of domestic health spending. To generate estimates for development assistance for health, we relied on project-level disbursement data from the major international development agencies' online databases and annual financial statements and reports for information on income sources. To adjust our estimates for 2020 to include disbursements related to COVID-19, we extracted project data on commitments and disbursements from a broader set of databases (because not all of the data sources used to estimate the historical series extend to 2020), including the UN Office of Humanitarian Assistance Financial Tracking Service and the International Aid Transparency Initiative. We reported all the historic and future spending estimates in inflation-adjusted 2020 US$, 2020 US$ per capita, purchasing-power parity-adjusted US$ per capita, and as a proportion of gross domestic product. We used various models to generate future health spending to 2050. Findings In 2019, health spending globally reached $8. 8 trillion (95% uncertainty interval [UI] 8.7-8.8) or $1132 (1119-1143) per person. Spending on health varied within and across income groups and geographical regions. Of this total, $40.4 billion (0.5%, 95% UI 0.5-0.5) was development assistance for health provided to low-income and middle-income countries, which made up 24.6% (UI 24.0-25.1) of total spending in low-income countries. We estimate that $54.8 billion in development assistance for health was disbursed in 2020. Of this, $13.7 billion was targeted toward the COVID-19 health response. $12.3 billion was newly committed and $1.4 billion was repurposed from existing health projects. $3.1 billion (22.4%) of the funds focused on country-level coordination and $2.4 billion (17.9%) was for supply chain and logistics. Only $714.4 million (7.7%) of COVID-19 development assistance for health went to Latin America, despite this region reporting 34.3% of total recorded COVID-19 deaths in low-income or middle-income countries in 2020. Spending on health is expected to rise to $1519 (1448-1591) per person in 2050, although spending across countries is expected to remain varied. Interpretation Global health spending is expected to continue to grow, but remain unequally distributed between countries. We estimate that development organisations substantially increased the amount of development assistance for health provided in 2020. Continued efforts are needed to raise sufficient resources to mitigate the pandemic for the most vulnerable, and to help curtail the pandemic for all. Copyright (C) 2021 The Author(s).     • In this study, a potentiometric urea biosensor has been fabricated on glass filter paper through the immobilization of urease enzyme onto chitosan/cobalt oxide (CS/Co3O4) nanocomposite. A copper wire with diameter of 500 mu m is attached with nanoparticles to extract the voltage output signal. The shape and dimensions of Co3O4 magnetic nanoparticles are investigated by scanning electron microscopy and the average diameter is approximately 80-100 nm. Structural quality of Co3O4 nanoparticles is confirmed from X-ray powder diffraction measurements, while the Raman spectroscopy has been used to understand the chemical bonding between different atoms. The magnetic measurement has confirmed that Co3O4 nanoparticles show ferromagnetic behavior, which could be attributed to the uncompensated surface spins and/or finite size effects. The ferromagnetic order of Co3O4 nanoparticles is raised with increasing the decomposition temperature. A physical adsorption method is adopted to immobilize the surface of CS/Co3O4 nanocomposite. Potentiometric sensitivity curve has been measured over the concentration range between 1 x 10(-4) and 8 x 10(-2) M of urea electrolyte solution revealing that the fabricated biosensor holds good sensing ability with a linear slope curve of similar to 45 mV/decade. In addition, the presented biosensor shows good reusability, selectivity, reproducibility and resistance against interferers along with the stable output response of similar to 12 s.
