  • When a parent dies : a systematic review of the effects of support programs for parentally bereaved children and their caregivers

    • Background: The death of a parent is a highly stressful life event for bereaved children. Several studies have shown an increased risk of mental ill-health and psychosocial problems among affected children. The aims of this study were to systematically review studies about effective support interventions for parentally bereaved children and to identify gaps in the research.Methods: The review’s inclusion criteria were comparative studies with samples of parentally bereaved children. The focus of these studies were assessments of the effects on children of a bereavement support intervention. The intervention was directed towards children 0–18 years;but it could also target the children’s remaining parent/caregiver. The study included an outcome measure that dealt with effects of the intervention on children. The following electronic databases were searched up to and including November 2015: PubMed, PsycINFO, Cinahl, PILOTS, ProQuest Sociology (Sociological Abstracts and Social Services Abstracts). The included studies were analysed and summarized based on the following categories: type of intervention, reference and grade of evidence, study population, evaluation design, measure, outcome variable and findings as effect size within and between groups.Results: One thousand, seven hundred and six abstracts were examined. Following the selection process, 17 studies were included. The included studies consisted of 15 randomized controlled studies, while one study employed a quasi-experimental and one study a pre-post-test design. Thirteen studies provided strong evidence with regards to the quality of the studies due to the grade criteria; three studies provided fairly strong evidence and one study provided weaker evidence. The included studies were published between 1985 and 2015, with the majority published 2000 onwards. The studies were published within several disciplines such as psychology, social work, medicine and psychiatry, which illustrates that support for bereaved children is relevant for different professions. The interventions were based on various forms of support: group interventions for the children, family interventions, guidance for parents and camp activities for children. In fourteen studies, the interventions were directed at both children and their remaining parents. These studies revealed that when parents are supported, they can demonstrate an enhanced capacity to support their children. In three studies, the interventions were primarily directed at the bereaved children. The results showed positive between group effects both for children and caregivers in several areas, namely large effects for children’s traumatic grief and parent’s feelings of being supported; medium effects for parental warmth, positive parenting, parent’s mental health, grief discussions in the family, and children’s health. There were small effects on several outcomes, for example children’s post-traumatic stress disorder (PTSD) symptoms, anxiety, depression, self-esteem and behaviour problems. There were studies that did not show effects on some measures, namely depression, present grief, and for the subgroup boys on anxiety, depression, internalizing and externalizing.Conclusions: The results indicate that relatively brief interventions can prevent children from developing more severe problems after the loss of a parent, such as traumatic grief and mental health problems. Studies have shown positive effects for both children’s and remaining caregiver’s health. Further research is required including how best to support younger bereaved children. There is also a need for more empirically rigorous effect studies in this area.

  • Parenting programs during adolescence: Outcomes from universal and targeted interventions offered in real-world settings

    • The aim of this naturalistic study was to explore short and long-term outcomes of five different group-based parenting programs offered to parents of 10 to 17-year-olds. Three hundred and fifteen parents (277 mothers and 38 fathers) who had enrolled in a parenting program (universal: Active Parenting, COPE; Connect; targeted: COMET; Leadership training for parents of teenagers [LFT]) answered questionnaires at three measurement waves (baseline, post-measurement, and one-year follow-up). The questions concerned parenting style, parental mental health, family climate and adolescent mental health. Results revealed small to moderate changes in almost all outcome variables and in all parenting programs. Overall, parents in COMET reported the largest short and long-term changes. No substantial differences in change were seen between the other programs. The results support the general effectiveness of parenting programs for parents of adolescents.

  • Assessing and treating three to twelve-year-olds displaying disruptive behaviour problems

    • The aim of this thesis was (a) to examine a Swedish version of a measure for early identification and treatment evaluation of children with disruptive behaviour problems and (b) to explore the effectiveness, in terms of reduction of children’s disruptive behavioural problems, of interventions not specifically directed towards the children but to their caregivers and other adults in their immediate environment. The focus of this thesis has been on clinical applicability. However, their have been considerable advances in the knowledge of the origins, development and maintenance of disruptive behaviour in children. This has led to corresponding changes in treatment. In line with this, research on phenomenology, prevalence, etiology, treatment and assessment of children that display disruptive behaviour are presented as an introduction to the empirical studies. The aim of study I was to develop and examine a systemic school-based model for detection and early intervention among 4 to 12 year old children who displayed externalizing behaviour problems. The intervention was a combination of the Marte Meo model and Coordination meetings. Treatment effects in the group who had received the intervention (N= 33) were compared with a group (N=16) who had received treatment-as-usual in their ordinary school setting. Assessments were carried out before and two years after the intervention. There was a significant decrease in children’s symptoms for the intervention group, but not for the comparison group. The aim of study II was to evaluate the effectiveness of the structured parent training programme Incredible Years Series in diverse clinical settings in Sweden. Parents of 113 children aged 3 to 9 participated in the study. Pre-data were collected prior to commencement of the parent training groups and post-data immediately after the training group sessions had finished. Significant reduction was found on all symptom-related measures in parents’ ratings. A significant increase in the self-rated well-being of the parents was also found. The aim of study III was to examine the psychometric properties and obtain Swedish norms for the Eyberg Child Behaviour Inventory (ECBI). Parents of 841 children aged 3 to 10 participated. The ECBI showed sound psychometric properties and seems to be a very useful measure in a Swedish context. Normative data from the Swedish sample was also presented. The aim of Study IV was to examine a three factor solution of the ECBI in a confirmatory factor analysis based on 22 items in a Swedish sample. The same sample as in study III was used. The results were consistent with the results from an American sample indicating that the 22-item version of the ECBI is a robust and useful alternative that can be used for evaluating and measuring treatment outcome. Preliminary normative data was also presented.

  • Barn till föräldrar som har kontakt med vuxenpsykiatrin – hur har de det? Rapport från en nationell studie

    • Psykisk ohälsa hos vuxna som depression, ångest eller bipolär sjukdom är idag ett be-tydande folkhälsoproblem. Många av de drabbade är föräldrar till minderåriga barn. Psykisk ohälsa hos föräldern påverkar barnens utveckling och välmående, men sam-bandet medieras av föräldraförmågan och relationen mellan barnet och föräldern. Sammantaget finns på gruppnivå en ökad risk att barnen också utvecklar psykiska problem, även om det inte behöver vara så i varje enskilt fall. Barns behov av information, råd och stöd ska enligt Hälso- och sjukvårdslagen beak-tas när en förälder lider av psykisk ohälsa. Det finns idag ett flertal interventioner som kan användas för att tillgodose detta behov. Samtidigt behöver man utvärdera hur dessa interventioner fungerar i Sverige. Denna rapport handlar om en kartläggning av hur barn vars föräldrar är patienter inom den specialiserade vuxenpsykiatrin har det med avseende på psykisk hälsa och generell familjesituation. Kartläggningen ingår i ett större forskningsprojekt vars syfte är att göra en utvärdering av interventioner (Beardslees familjeintervention, Föra barn på tal och reguljär behandling/övriga insatser) för stöd till barn i åldern 8 till 17 år med föräldrar med depression, dystymi, ångeststörning och/eller bipolär sjukdom. Totalt ingår föräldrar, både patienter och deras partners, till 88 barn i studien och 19 barn har gjort egna skattningar. De fick fylla i ett antal frågeformulär, där föräldrarna skattade barnens psykiska hälsa, sitt eget psykiska mående, sin generella livssituation, sin relation till eventuell nuvarande partner, sin upplevelse av att kunna hantera bar-nens beteende och olika aspekter av klimat och funktion i familjen. Barnen skattade sin egen psykiska hälsa samt vissa aspekter av klimat och funktion i familjen. Resultaten visar att föräldrar i vår studie rapporterar signifikant högre nivåer av psy-kisk ohälsa hos barnen än föräldrar i en normgrupp i den vanliga befolkningen. Barn rapporterar större symptombelastning än föräldrarna när det gäller emotionella pro-blem, hyperaktivitet/ouppmärksamhet och total problembelastning. De föräldrar som är patienter rapporterar som väntat lägre psykiskt välmående än sina partners, men även i partnergruppen finns det flera som uppger en egen psykisk problematik på klinisk nivå. Patienter och deras partner ger en samstämmig bedömning av sin re-lation; betydligt fler än i en normalgrupp anger bekymmersamt höga nivåer av stress i relationen. Insatser för barn som anhöriga ses ofta som indikerade preventioner i syfte att minska risken för psykisk ohälsa i vuxenåldern, men det finns utifrån studiens resultat mycket som talar för att det istället kan finnas behov av tidig behandling. Studien pekar på vikten av att man som professionell inom vuxenpsykiatrin i enlighet med lagstiftningen tar reda på om det finns barn i familjen, även ställer frågor om hur patienten upplever att barnet mår psykisk och fysiskt, vilka övriga personer som finns omkring barnet och hur tillgängliga de är som stöd för barnet, hur relationerna mellan olika familjemedlemmar är, samt hur föräldraskapet upplevs.

  • Evaluation of ‘‘The Incredible Years’’ in Sweden: The transferability of an American parent-training program to Sweden

    • Structured parent training has been proven to be effective in reducing disruptive behavior problems (DBP) in children. Most of the programs that are used in Sweden have their origin in North America, and there is an ongoing debate over the transferability to Sweden of manual-based programs developed in other contexts. The goal of the present study was to study effectiveness of the Incredible Years parent-training program (IY), developed in the US, in regular clinical work in Sweden, using a randomized controlled design. Parents of 62 four to eight-year-old children diagnosed with Oppositional Defiant Disorder participated in the study. Parents of 38 children were assigned to parent training (PT) and 24 to a waiting list (WL). The results indicate that the IYS retains the positive effects on children’s disruptive behavior problems when translated and transferred to Swedish. There was a statistically significant difference in reduction of DBP in children between the groups in favor of the PT. The improvement in the PT group was sustained at the one-year follow-up. The improvement also, at least to some extent, generalized over time to the school context. There was also a statistically significant difference in mothers’ report of pre to post change in parenting alliance between the PT and WL groups. The IYS program was appreciated and well received by the participating mothers
