Increase of estrogen dose deteriorates mood during progestin phase in sequential hormonal therapy

- Previous studies have indicated that the addition of progestinsduring sequential hormonal replacement therapy (HRT)causes negative mood and physical symptoms. History of premenstrualsyndrome, type of progestin, and dose of progestinhave thus far been shown to influence the progestin-inducedadverse mood symptoms during HRT.The aim of this study was to compare adverse mood effectsof two different doses of estradiol, in combination with a progestin,during postmenopausal HRT. Twenty-eight perimenopausalwomen were included in this randomized, doubleblind,crossover study comparing 2- or 3-mg continuousestradiol, with an addition of 10 mg medroxyprogesteroneacetate on d 17–28 during each treatment cycle. The mainoutcome measures were mood and physical symptoms kept ona daily rating scale. Together with the progestin, the higherdose of estrogen caused significantly more negative moodsymptoms than the lower dose. Tension, irritability, and depressedmood were all significantly augmented during theprogestin phase of cycles with 3mg estradiol (P<0.001). Physicalsymptoms also increased during the progestin phase of3-mg estradiol cycles (P<0.001), whereas positive mood symptomswere less affected. The only positive mood that changedwith estrogen dose was friendliness, which decreased duringthe progestin phase of high estradiol cycles compared withcycles with lower estradiol (P < 0.05).Our conclusion is that an increase of the estrogen doseaccentuates negativemoodand physical symptoms during theprogestin phase of sequential hormonal therapy.

Characterisation of fines from unbleached kraft pulps and their impact on sheet properties

- Fines are an essential component in the papermaking process because they have a profound influence on the behaviour of the wet web and on the mechanical properties of the final sheet. Primary fines are present in the pulp prior to refining, and secondary fines are produced during refining. In the present investigation, two commercially manufactured unbleached pulps with kappa numbers of 45 and 90 were studied in terms of how they responded to refining with respect to the quality of fibre and fines. Primary and secondary fines were collected and characterised and their impact on sheet strength was evaluated by addition of known amounts to a refined and decrilled pulp. All the measured paper strength properties improved when primary and secondary fines were added. The strength improvement was generally somewhat higher in the second case. The effect was more pronounced at a higher level of addition. We attribute the main strength improvements associated with fines to improved consolidation by the creation of capillary forces between the surfaces.

- BACKGROUND: Worldwide, injuries represent one of the leading causes of mortality, and nearly one-quarter of all injuries are road traffic related. In many high-income countries, the burden of road traffic injuries (RTIs) has shifted from premature death to injury and disability with long-term consequences; therefore, it is important to assess the full burden of an RTI on individual lives.OBJECTIVE: To describe how men and women with minor and moderate injuries reported the consequences of an RTI on their health and lives.METHODS: The study was designed as an explorative qualitative study, in which the answers to an open-ended question concerning the life and health consequences following injury were analysed using systematic text condensation.PARTICIPANTS: A total of 692 respondents with a minor or a moderate injury were included.RESULTS: The respondents reported the consequences of the crash on their health and lives according to four categories: physical consequences, psychological consequences, everyday life consequences and financial consequences. The results show that medically classified minor and moderate injuries have detrimental long-term health and life consequences. Although men and women report some similar consequences, there are substantial differences in their reported psychological and everyday life consequences following an injury. Women report travel anxiety and PTSD-like symptoms, being life altering for them compared with men, for whom these types of reports were missing.CONCLUSION: These differences emphasise the importance of considering gender-specific physical and psychological consequences following an RTI.

- Measuring the presence of a number of quality values and aspects can help to understand how to approach improving transitional care in a healthcare setting. The purpose of this paper is to present the results from a questionnaire measuring the quality culture in both ICU and two different general wards. The purpose is also to present an analyze of the result in order to create an understanding of how differences and/or similarities in the quality culture could affect the application of Quality Management (QM) in order to improve ICU transitional care.

- Severe dissociative states involving the experience of being in parts, typically associated with diagnosis such as dissociative identity disorder and other specified dissociative disorders, continue to be a controversial and rarely studied area of research. However, because persons with severe dissociative states are at risk of being harmed instead of helped within psychiatric care, their experiences of living with such states warrant further examination, while innovative ways to include them in research remain necessary. Against that background, this study aimed to illuminate the meanings of living with severe dissociative states involving the experience of being in parts. This is a phenomenological hermeneutic study with data collected from three social media sources, one personal blog and two Instagram accounts, in February and March 2023. The results were illuminated in light of four themes; Striving to remain in the world, Balancing exposure and trust, Balancing belonging and loneliness and Owning oneselves. The interpretation of the themes suggests that living with severe dissociative states means being a human under inhuman conditions, striving for coherence and meaning in a world that is often unsupportive. This calls for a trauma-informed care to better support recovery for persons with severe dissociative states.

- Introduction Psychiatric inpatient care (PIC) is often characterised by high pressure and thresholds for admission, brief periods of care and limited time for caring activities. Dissociative identity disorder (DID) is a contested diagnosis, and persons with DID are at risk of not receiving adequate support when cared for in PIC. Because the limited literature addressing the topic includes no overview on how persons with DID are cared for in psychiatric inpatient settings, the aim of this scoping review is to map the area of knowledge on PIC for persons experiencing DID. This scoping review will provide an overview with the possibility to elucidate gaps in the evidence base and needs for future research on PIC for persons experiencing DID.Methods and analysis This scoping review will follow Preferred Reporting Items for Systematic Review and Meta-Analysis for Scoping Reviews and steps 1–5 described in the established method for scoping reviews: identifying research question, identifying relevant studies, study selection, charting the data and collating, summarising and reporting results.Ethics approval Not applicable.Dissemination This scoping review will be submitted for publication in an international, peer-reviewed journal.

- Innventia AB's LignoBoost process enables the extraction of high purity lignin efficiently from the black liquor in kraft mills. A stream of black liquor is taken from the evaporation plant and the lignin is precipitated by acidification and filtered. The filter cake is redispersed and acidified and the resulting slurry is filtered and washed. High purity lignin can be produced at several scales, namely 10g, 1kg, 10kg and over 1,000kg. Innventia has invested significantly to demonstrate the potential of lignin as a viable feedstock for carbon fibre manufacture. Initially, the fibre melt spinning performance of the lignin is assessed using single filament melt extrusion and then melt spinning is performed at the multifilament scale, where fine fibres can be produced for conversion to carbon fibre. Oxidative thermostabilisation of the lignin fibres is carried out so that carbonisation can proceed. The effects of thermal treatment programmes and tensioning have been studied by using either thermomechanical analysis or by using test equipment specially designed to monitor carbonisation profiles with either stress or strain control. In addition, continuous processes for the conversion of lignin fibre to carbon fibre are being developed.
