  • A pilot study of an organised population-based testing programme for prostate cancer

    • ObjectiveTo determine the feasibility of a digitally automated population-based programme for organised prostate cancer testing (OPT) in Southern Sweden.Patients and MethodsA pilot project for a regional OPT was conducted between September 2020 and February 2021, inviting 999 randomly selected men aged 50, 56, or 62 years. Risk stratification was based on prostate-specific antigen (PSA) level, PSA density (PSAD), and bi-parametric prostate magnetic resonance imaging (MRI). Men with a PSA level of 3-99 ng/mL had an MRI, and men with elevated PSA level (& GE;3 ng/mL) had a urological check-up, including a digital rectal examination and transrectal ultrasonography (TRUS). Indications for targeted and/or systematic transrectal prostate biopsies were suspicious lesions on MRI (Prostate Imaging-Reporting and Data System [PI-RADS] 4-5) and/or PSAD > 0.15 ng/mL/mL. Additional indications for prostate biopsies were palpable tumours, PSA ratio < 0.1, or cancer suspicion on TRUS. Patient selection, mail correspondence, data collection, and algorithm processing were performed by an automated digital management system. Feasibility is reported descriptively.ResultsA total of 418 men had a PSA test (42%), with increasing participation rates by age (50 years, 38%; 56 years, 44%; and 62 years, 45%). Among these, 35 men (8%) had elevated PSA levels (& GE;3 ng/mL: one of 139, aged 50 years; 10/143, aged 56 years; and 24/146, aged 62 years). On MRI, 16 men (48%) had a negative scan (PI-RADS < 3), seven men (21%) had PI-RADS 3, nine men (27%) had PI-RADS 4, and one man (3%) had PI-RADS 5. All men with PI-RADS 4 or 5 underwent prostate biopsies, as well as two men with PI-RADS 3 due to PSAD > 0.15 ng/mL/mL or a suspicious finding on TRUS. Prostate cancer was diagnosed in 10 men. Six men underwent active treatment, whereas four men were assigned to active surveillance.ConclusionOur OPT model is feasible from an operational point of view, but due to the limited scale of this study no conclusions can be made regarding the efficacy of the diagnostic model or outcome.

  • Correlation between ADC, ADC ratio, and Gleason Grade group in prostate cancer patients undergoing radical prostatectomy : Retrospective multicenter study with different MRI scanners

    • Background: MRI is an important tool in the prostate cancer work-up, with special emphasis on the ADC sequence. This study aimed to investigate the correlation between ADC and ADC ratio compared to tumor aggressiveness determined by a histopathological examination after radical prostatectomy.Methods: Ninety-eight patients with prostate cancer underwent MRI at five different hospitals prior to radical prostatectomy. Images were retrospectively analyzed individually by two radiologists. The ADC of the index lesion and reference tissues (contralateral normal prostatic, normal peripheral zone, and urine) was recorded. Absolute ADC and different ADC ratios were compared to tumor aggressivity according to the ISUP Gleason Grade Groups extracted from the pathology report using Spearman’s rank correlation coefficient (ρ). ROC curves were used to evaluate the ability to discriminate between ISUP 1-2 and ISUP 3-5 and intra class correlation and Bland-Altman plots for interrater reliability.Results: All patients had prostate cancer classified as ISUP grade ≥ 2. No correlation was found between ADC and ISUP grade. We found no benefit of using the ADC ratio over absolute ADC. The AUC for all metrics was close to 0.5, and no threshold could be extracted for prediction of tumor aggressivity. The interrater reliability was substantial to almost perfect for all variables analyzed.Conclusions: ADC and ADC ratio did not correlate with tumor aggressiveness defined by ISUP grade in this multicenter MRI study. The result of this study is opposite to previous research in the field.

  • Deep learning performance on MRI prostate gland segmentation : evaluation of two commercially available algorithms compared with an expert radiologist

    • PURPOSE: Accurate whole-gland prostate segmentation is crucial for successful ultrasound-MRI fusion biopsy, focal cancer treatment, and radiation therapy techniques. Commercially available artificial intelligence (AI) models, using deep learning algorithms (DLAs) for prostate gland segmentation, are rapidly increasing in numbers. Typically, their performance in a true clinical context is scarcely examined or published. We used a heterogenous clinical MRI dataset in this study aiming to contribute to validation of AI-models.APPROACH: We included 123 patients in this retrospective multicenter (7 hospitals), multiscanner (8 scanners, 2 vendors, 1.5T and 3T) study comparing prostate contour assessment by 2 commercially available Food and Drug Association (FDA)-cleared and CE-marked algorithms (DLA1 and DLA2) using an expert radiologist's manual contours as a reference standard (RSexp) in this clinical heterogeneous MRI dataset. No in-house training of the DLAs was performed before testing. Several methods for comparing segmentation overlap were used, the Dice similarity coefficient (DSC) being the most important.RESULTS: The DSC mean and standard deviation for DLA1 versus the radiologist reference standard (RSexp) was 0.90±0.05 and for DLA2 versus RSexp it was 0.89±0.04. A paired t-test to compare the DSC for DLA1 and DLA2 showed no statistically significant difference (p=0.8).CONCLUSIONS: Two commercially available DL algorithms (FDA-cleared and CE-marked) can perform accurate whole-gland prostate segmentation on a par with expert radiologist manual planimetry on a real-world clinical dataset. Implementing AI models in the clinical routine may free up time that can be better invested in complex work tasks, adding more patient value.

  • No gadolinium K‐edge detected on the first clinical photon‐counting computed tomography scanner

    • PurposeThis study aimed to elucidate whether gadolinium contrast in clinically relevant doses can be used with photon-counting computed tomography (PCCT) as an alternative contrast agent in clinical applications.Material/methodsA CTDI phantom with 3D printed rods filled with different concentrations of gadolinium and iodine contrast was scanned in a PCCT and an energy-integrated computed tomography (EICT). Attenuation values at different monoenergetic steps were extracted for each contrast concentration.ResultsFor PCCT, gadolinium reached an attenuation >100 HU (103 HU) at 40 keV with a concentration 5 mmol/L whereas the same level was reached at 50 keV (118 HU) for 10 mmol/L and 90 keV (114 HU) for 25 mmol/L. For iodine, the same level of attenuation was reached at 100 keV (106 HU) with a concentration 8.75 mg I/mL.For EICT the lowest gadolinium contrast concentration needed to reach >100 HU (108 HU) was 10 mmol/L at 50 keV. For 25 mmol/L 100 HU was reached at 100 keV. For iodine contrast 108 HU was reached at 110 keV for 8.75 mg I/mL.ConclusionNo K-edge potential or difference in attenuation curves between iodine and gadolinium contrast is detected on the first clinical available PCCT. Clinically relevant attenuation levels were barely achieved in this setting with gadolinium concentrations approved for human use. The results of this study suggest that, given current scanning technology, gadolinium is not a clinically useful contrast agent for computed tomography because no K-edge was detected.

  • Amputation Rates, Mortality, and Pre-operative Comorbidities in Patients Revascularised for Intermittent Claudication or Critical Limb Ischaemia : A Population Based Study

    • Objectives: The aims of this population based study were to describe mid-to long-term amputation risk, cumulative incidence of death or amputation, and differences in pre-operative comorbidities in patients revascularised for lower limb peripheral artery disease (PAD).Methods: This was an observational cohort study. Data from the Swedish National Quality Registry for Vascular Surgery (Swedvasc) were combined with mandatory national health care registries and patient medical records. All patients who underwent revascularisation in Sweden between May 2008 and May 2013 for intermittent claudication (IC) or critical limb ischaemia (CLI), aged 50 years and older, were identified through the Swedvasc database. The mandatory national health care registries and medical records provided data on comorbidities, mortality, and major amputations.Results: A total of 16,889 patients with PAD (IC, n = 6272; CLI, n = 10,617) were studied. The incidence of amputations in IC patients was 0.4% (range 0.3%-0.5%) per year. Among CLI patients, the amputation rate during the first 6 months following revascularisation was 12.0% (95% CI 11.3-12.6). Thereafter, the incidence declined to approximately 2% per year. The cumulative combined incidence of death or amputation 3 years after revascularisation was 12.9% (95% CI 12.0-13.9) in IC patients and 48.8% (95% CI 47.7-49.8) in CLI patients. Among CLI patients, compared with IC patients, the prevalence of diabetes, ischaemic stroke, heart failure, and atrial fibrillation was approximately doubled and renal failure was nearly tripled, even after age standardisation.Conclusion: The risk of amputation is particularly high during the first 6 months following revascularisation for CLI. IC patients have a benign course in terms of limb loss. Mortality in both IC and CLI patients is substantial. Revascularised CLI patients have different comorbidities from IC patients.

  • Chronic limb-threatening ischaemia - Prognosis after intervention

    • Introduction: Lower-extremity arterial disease (LEAD) is a major health problem worldwide. Chronic limb-threatening ischaemia (CLTI) is the most severe manifestation of LEAD. Open or endovascular recanalization is recommended for most patients with CLTI. The aim of this thesis has been to investigate the impact of comorbidities, medications, gender, preoperative symptoms, and revascularized arterial segment on outcome after revascularization in terms of amputation or death and to investigate whether it is possible to make a correct preoperative assessment of infrapopliteal lesions in CLTI patients using the TransAtlantic Inter-society Consensus (TASC) II classification on magnetic resonance angiography (MRA). Methods: All patients who were revascularized for intermittent claudication (IC, n = 6,272) and CLTI (n = 10,617) from May 2008 to May 2013 in Sweden were assembled in a database. Patients were identified using the Swedvasc register and the data was complemented with mandatory national healthcare registers. In addition, medical records of 1,366 patients were reviewed to safeguard accuracy on ipsilateral amputation. Descriptive statistics, Cox regression models with hazard ratios (HRs), and Kaplan-Meier curves were used in Papers I‒III. In Paper IV, 68 preoperative MRAs and perioperative digital subtraction angiographies (DSAs) were evaluated using the infrapopliteal TASC II criteria. Visual grading characteristics (VGC) analysis and Krippendorff’s α were used for analysis of differences and agreement between modalities and observers. Results: CLTI patients had different preoperative comorbidities than IC patients. The risk of amputation following revascularization for CLTI was particularly high during the first 6 months. Mortality was high in all revascularized LEAD patients. Renal insufficiency, diabetes, heart failure, atrial fibrillation, male gender and presence of tissue loss, were found to be independently associated with increased risk of amputation after revascularization for CLTI. Conversely, medication with low-dose acetylsalicylic acid and statins were associated with improved limb salvage and survival. No systematic difference was detected between MRA and DSA in grading of infrapopliteal lesions. Agreement between observers in preoperative assessment of infrapopliteal TASC II class was poor, mainly due to a variable choice of intended target vessel between observers. A suggested novel infrapopliteal aggregated lesion severity score (IALSS), based on evaluation of all four infrapopliteal arteries, had better inter-observer agreement. Conclusions: CLTI and IC affects different patient populations. Mortality is substantial in both. Amputation rate is particularly high in the first 6 months following revascularization for CLTI and associated with well-defined risk factors, most markedly renal insufficiency, diabetes, and tissue loss. Statins and platelet inhibitors should be considered for all patients with LEAD. Infrapopliteal TASC II grading can be done using MRA, but the required choice of a target vessel is a concern. An alternative score, independent of intended target vessel, may provide a more reproducible tool for assessment of infrapopliteal disease severity.

  • Comparison of Magnetic Resonance Angiography and Digital Subtraction Angiography for the Assessment of Infrapopliteal Arterial Occlusive Lesions, Based on the TASC II Classification Criteria

    • This paper aimed to study the agreement and repeatability, both intra- and interobserver, of infrapopliteal lesion assessment with magnetic resonance angiography (MRA), using the TransAtlantic Inter-Society Consensus (TASC) II criteria, with perioperative digital subtraction angiography (DSA) as a reference. Sixty-eight patients with an MRA preceding an endovascular infrapopliteal revascularization were included. Preoperative MRAs and perioperative DSAs were evaluated in random order by three independent observers using the TASC II classification. The results were analyzed using visual grading characteristics (VGC) analysis and Krippendorffs alpha. No systematic difference was found between modalities: area under the VGC curve (AUC(VGC)) = 0.48 (p = 0.58) or intraobserver; AUC(VGC) for Observer 1 and 2 respectively, 0.49 (p = 0.85) and 0.53 (p = 0.52) for MRA compared with 0.54 (p = 0.30) and 0.49 (p = 0.81) for DSA. Interobserver differences were seen: AUC(VGC) of 0.63 (p < 0.01) for DSA and 0.80 (p < 0.01) for MRA. These results were confirmed using Krippendorffs alpha for the three observers showing 0.13 (95% confidence interval (CI) -0.07-0.31) for MRA and 0.39 (95% CI 0.23-0.53) for DSA. Poor interobserver agreement was also found in the choice of a target vessel on preoperative MRA: Krippendorffs alpha = 0.19 (95% CI 0.01-0.36). In conclusion, infrapopliteal lesions can be reliably determined on preoperative MRA, but interobserver variability regarding the choice of a target vessel is a major concern that appears to affect the overall TASC II grade.

  • Editor's Choice - Impact of Comorbidity, Medication, and Gender on Amputation Rate Following Revascularisation for Chronic Limb Threatening Ischaemia

    • Objective/background: Chronic limb threatening ischaemia (CLTI) has a high risk of amputation and mortality. Increased knowledge on how sex, comorbidities, and medication influence these outcomes after revascularisation may help optimise results and patient selection. Methods: This population based observational cohort study included all individuals revascularised for CLTI in Sweden during a five year period (10,617 patients in total). Data were retrieved and merged from mandatory national healthcare registries, and specifics on amputations were validated with individual medical records. Results: Mean age at revascularisation was 76.8 years. Median follow up was 2.7 years (range 0-6.6 years). Male sex (hazard ratio [HR] 1.20, 95% confidence interval [CI] 1.09-1.33), renal insufficiency (HR 1.57, 95% CI 1.32-1.87), diabetes (HR 1.45, 95% CI 1.32-1.60), and heart failure (HR 1.17, 95% CI 1.05-1.31) were independently associated with an increased amputation rate, whereas the use of statins (HR 0.71, 95% CI 0.64-0.78) and low dose acetylsalicylic acid (HR 0.77, 95% CI 0.70-0.86) were associated with a reduced amputation rate. For the combined end point of amputation or death, an association with increased rates was found for male sex (HR 1.25, 95% CI 1.18-1.32), renal insufficiency (HR 1.94, 95% CI 1.75-2.14), heart failure (HR 1.50, 95% CI 1.40-1.60), and diabetes (HR 1.31, 95% CI 1.23-1.38). The use of statins (HR 0.74, 95% CI 0.67-0.82) and low dose acetylsalicylic acid (HR 0.82, 95% CI 0.77-0.881) were related to a reduced risk of amputation or death. Conclusions: Renal insufficiency is the strongest independent risk factor for both amputation and amputation/ death in revascularised CLTI patients, followed by diabetes and heart failure. Men with CLTI have worse outcomes than women. These results may help govern patient selection for revascularisation procedures. Statin and low dose acetylsalicylic acid are associated with an improved limb outcome. This underlines the importance of preventive medication to reduce general cardiovascular risk and increase limb salvage. (C) 2018 European Society for Vascular Surgery. Published by Elsevier B.V.   • Impact of Preoperative Symptoms and Revascularized Arterial Segment in Patients With Chronic Limb-Threatening Ischemia

    • Background: Little is known about the relative impact of the preoperative symptoms rest pain and tissue loss, and of the arterial segment revascularized, on amputation rate and mortality in patients with chronic limb-threatening ischemia (CLTI). We wanted to investigate this topic further. Method: This population-based observational cohort study involved 10 419 patients revascularized for CLTI in Sweden, 2008 to 2013. Data were collected from health-care registries and medical records. The effect of preoperative symptoms and revascularized arteries was determined using Cox regression models. A competing risk analysis was used to determine the effect of symptoms on the combined endpoint "amputation or death". Results: The amputation rate during a mean follow-up of 2 years was 7.5% in patients with rest pain, 15.6% in patients with tissue loss only, and 20.1% when both symptoms were present. Mortality was 39% lower in patients with rest pain only than in those with both symptoms. Revascularizations targeted the aortoiliac, femoropopliteal, and infrapopliteal segments in 19.4%, 76.8%, and 30.6%, respectively. Distal revascularizations were associated with a higher amputation rate, but this difference disappeared after adjustment for comorbidities. Aortoiliac revascularizations were associated with high mortality. Competing risk analysis showed that mortality became the major determinant of amputation-free survival outcomes from 1 year after revascularization. Conclusions: Tissue loss implies a clearly worse prognosis compared to rest pain for patients with CLTI. Most revascularizations for CLTI are done in the femoropopliteal segment. Infrapopliteal procedures are associated with a higher amputation rate, whereas aortoiliac revascularizations are associated with higher mortality.
