    • Organ chips can recapitulate organ-level (patho)physiology, yet pharmacokinetic and pharmacodynamic analyses require multi-organ systems linked by vascular perfusion. Here, we describe an ‘interrogator’ that employs liquid-handling robotics, custom software and an integrated mobile microscope for the automated culture, perfusion, medium addition, fluidic linking, sample collection and in situ microscopy imaging of up to ten organ chips inside a standard tissue-culture incubator. The robotic interrogator maintained the viability and organ-specific functions of eight vascularized, two-channel organ chips (intestine, liver, kidney, heart, lung, skin, blood–brain barrier and brain) for 3 weeks in culture when intermittently fluidically coupled via a common blood substitute through their reservoirs of medium and endothelium-lined vascular channels. We used the robotic interrogator and a physiological multicompartmental reduced-order model of the experimental system to quantitatively predict the distribution of an inulin tracer perfused through the multi-organ human-body-on-chips. The automated culture system enables the imaging of cells in the organ chips and the repeated sampling of both the vascular and interstitial compartments without compromising fluidic coupling.

  • Quantitative prediction of human pharmacokinetic responses to drugs via fluidically coupled vascularized organ chips

    • Analyses of drug pharmacokinetics (PKs) and pharmacodynamics (PDs) performed in animals are often not predictive of drug PKs and PDs in humans, and in vitro PK and PD modelling does not provide quantitative PK parameters. Here, we show that physiological PK modelling of first-pass drug absorption, metabolism and excretion in humans—using computationally scaled data from multiple fluidically linked two-channel organ chips—predicts PK parameters for orally administered nicotine (using gut, liver and kidney chips) and for intravenously injected cisplatin (using coupled bone marrow, liver and kidney chips). The chips are linked through sequential robotic liquid transfers of a common blood substitute by their endothelium-lined channels (as reported by Novak et al. in an associated Article) and share an arteriovenous fluid-mixing reservoir. We also show that predictions of cisplatin PDs match previously reported patient data. The quantitative in-vitro-to-in-vivo translation of PK and PD parameters and the prediction of drug absorption, distribution, metabolism, excretion and toxicity through fluidically coupled organ chips may improve the design of drug-administration regimens for phase-I clinical trials.

  • A scoping review of person-centred care strategies used in diagnostic Nuclear Medicine

    • IntroductionPerson-centred care (PCC) emphasises the need for the health care professional to prioritise individual patient needs, thereby fostering a collaborative and emphatic environment that empowers patients to actively participate in their own care. This article will explore the purpose of PCC in Nuclear Medicine (NM), while discussing strategies that may be used to implement PCC during diagnostic NM examinations performed on adult patients.MethodsThe scoping review was conducted in accordance with the Joanna Briggs Institute methodology. The search was performed on PubMed, Embase and Cinhal in June 2023 and included studies in English, Spanish, Portuguese and Italian. The research equation combined keywords and Medical Subject Heading terms (MeSH) related to person-centred care (PCC), for all types of nuclear medicine diagnostic examinations performed. Three independent review authors screened all abstracts and titles, and all eligible full-text publications were included in this scoping review.ResultsFifty-three articles, published between 1993 and 2022, met the inclusion criteria for this scoping review. Seven articles were published in 2015 while 56.6 % of all included studies were performed in Europe. Most studies (n = 39/53) focused on the patients only, with the identified patient benefits being: improve patient experience (67.9 %), increase patient comfort (13.2 %), increase patient knowledge (5.7 %), reduction of patient anxiety (9.4 %) and reduction of waiting/scan time (3.8 %).ConclusionThe scoping review identified a lack of research investigating the use of person-centred care strategies in NM. Future research will focus on using an international survey to explore this topic in nuclear medicine departments overseas.Implications for practiceBy applying PCC principles, the NM professional can improve the patient care pathway and increase patient satisfaction, leading to enhanced clinical outcomes.

  • Selected stormwater priority pollutants - a European perspective

    • The chemical characteristics of stormwater are dependent on the nature of surfaces (roads, roofs etc.) with which it comes into contact during the runoff process as well as natural processes and anthropogenic activities in the catchments. The different types of pollutants may cause problems during utilisation, detention or discharge of stormwater to the environment and may pose specific demands to decentralised treatment. This paper proposes a scientifically justifiable list of selected stormwater priority pollutants (SSPP) to be used, e.g., for evaluation of the chemical risks occurring in different handling strategies. The SSPP-list consists of 25 pollutant parameters including eight of the priority pollutants currently identified in the European Water Framework Directive. It contains general water quality parameters (organic and suspended matter, nutrients and pH); metals (Cd, Cr, Cu, Ni, Pb, Pt and Zn); PAH (naphthalene, pyrene and benzo[a]pyrene); herbicides (pendimethalin, phem-nedipham, glyphosate and terbutylazine); and other representative industrially derived compounds (nonylphenol ethoxylates, pentachlorophenol, di(2-ethylhexyl)phthalate, PCB-28 and methyl tert-butyl ether). Tools for flux modelling, enabling calculation of predicted environmental concentrations (PECs), and for ranking the susceptibility of a pollutant to removal within a range of structural stormwater treatment systems or best management practices (BMPs) have been developed, but further work is required to allow all SSPPs to be addressed in the development of future stormwater pollution control measures.     • Man-made nanoparticles (NPs) released into the aquatic environment are likely to accumulate in sediment. Uptake of NPs by benthic invertebrates that serve as food for higher trophic level organisms such as fish may result in their transfer along the food chain. We tested this hypothesis in a proof-of-concept approach consisting of two studies examining the transfer of copper(ii) oxide NPs (CuO NPs) (<50 nm) and dissolved copper (CuCl 2 ). Study 1 focussed on transfer from sediment to worms (Tubifex tubifex) and study 2 on transfer from worms (T. tubifex) to fish (Gasterosteus aculeatus, three-spined stickleback). A key methodological achievement of study 2 was the development of worm homogenate-based food packages, which allowed controlling CuO NPs and CuCl 2 dosing of fish. In T. tubifex, no significant differences in Cu accumulation were detected following 7 days of exposure to sediment-associated CuO NPs or CuCl 2 (100 μg Cu per g dw sediment). Yet, metallothionein-like protein levels were significantly elevated in CuO NP-exposed worms compared to the control, while they were not in CuCl 2 -exposed worms. G. aculeatus fed CuO NP- A nd CuCl 2 -spiked food packages (2 μg Cu per g fish ww per day; 7 days) showed increased Cu concentrations (∼13 and ∼31 μg Cu per g dw compared to control, respectively) and metallothionein (mta) mRNA expression in intestinal tissue. In CuO NP-exposed fish, Cu and mta mRNA levels were noticeably lower than in CuCl 2 -exposed fish and inversely correlated with the amount of Cu in their faeces, suggesting that a large fraction of NPs was egested. Altogether, the results suggest that sediment-associated CuO NPs can enter the aquatic food web, but that transfer of CuO NPs from T. tubifex to stickleback may be limited compared to dissolved Cu. Furthermore, there were no indications of induction of oxidative stress in fish intestine and liver, suggesting that effects from environmentally relevant Cu/CuO NP exposure via the diet may be minimal. © 2019 The Royal Society of Chemistry.

    • Metal nanoparticles (NPs) released into the aquatic environment will likely accumulate in sediment and be available for sediment-dwelling invertebrates that serve as food for other organisms, such as fish. The aim of the present study was to investigate trophic transfer of copper oxide (CuO) NPs and dissolved Cu (CuCl2) from natural sediment to the sediment-dwelling worm Tubifex tubifex, and then to the predatory fish Gasterosteus aculeatus (three-spined stickleback). Cu enriched in the stable isotope Cu-65 was used to increase detection and discriminate newly added/assimilated Cu from background Cu levels. Worms were exposed to sediment spiked with (CuCl2)-Cu-65 or (CuO)-Cu-65 NPs (similar to 20 nm) at environmentally relevant concentrations for 7 days and subsequently fed to fish for 7 days. Worms accumulated Cu-65 during sediment exposure to both (CuCl2)-Cu-65 and (CuO)-Cu-65 NPs (0.7 and 1.1 mu g Cu-65 per g dw tissue, respectively), resulting in Cu-65 body burdens significantly different from control. Furthermore, significantly more Cu-65 was released from the sediment into the overlying water in the 65 CuO NP exposures compared to the (CuCl2)-Cu-65 exposures. Cu-65 accumulation in fish feeding on (CuCl2)-Cu-65 and (CuO)-Cu-65 NP-exposed worms was limited (intestinal tissue: 80 and 65 ng g(-1) dw; fish carcass: 7 and 10 ng g(-1) dw, and liver: 50 and 10 ng g(-1) dw, respectively). Glutathione peroxidase (gpx) mRNA expression was significantly higher in fish feeding on (CuCl2)-Cu-65-exposed worms compared to (CuO)-Cu-65 NP-exposed worms (though Cu-65 tissue concentrations were similar). No significant effects were detected for the other investigated genes (ctr1, mta, gcl, gr, sod, cat, zo-1). Our results show that NP-derived Cu can enter freshwater food webs, but bioaccumulation and trophic transfer under environmentally realistic exposures is low (detectable with a tracer) and not different from that of dissolved Cu.

  • Prospective environmental risk screening of seven advanced materials based on production volumes and aquatic ecotoxicity

    • The number and volume of advanced materials being manufactured is increasing. In order to mitigate future impacts from such materials, assessment methods that can provide early indications of potential environmental risk are required. This paper presents a further development and testing of an environmental risk screening method based on two proxy measures: aquatic ecotoxicity and global annual production volumes. In addition to considering current production volumes, this further developed method considers potential future production volumes, thereby enabling prospective environmental risk screening. The proxy measures are applied to seven advanced materials: graphene, graphene oxide, nanocellulose, nanodiamond, quantum dots, nano-sized molybdenum disulfide, and MXenes. Only MXenes show high aquatic ecotoxicity, though the number of test results is still very limited. While current production volumes are relatively modest for most materials, several of the materials (graphene, graphene oxide, nanocellulose, nano-sized molybdenum disulfide, and MXenes) have the potential to become high-volume materials in the future. For MXenes, with both high aquatic ecotoxicity and high potential future production volumes, more detailed environmental risk assessments should be considered. For the other materials with high potential future production volumes, the recommendation is to continuously monitor their aquatic ecotoxicity data. Based on the application of the proxy measures combined with future scenarios for production volumes, we recommend this environmental risk screening method be used in the early development of advanced materials to prioritize which advanced materials should be subject to more detailed environmental assessments.

    • Microglia are activated following cerebral ischemia and increase their production of the neuro- and immunomodulatory cytokine tumor necrosis factor (TNF). To address the function of TNF from this cellular source in focal cerebral ischemia we used TNF conditional knock out mice (LysMcreTNF fl/fl) in which the TNF gene was deleted in cells of the myeloid lineage, including microglia. The deletion reduced secreted TNF levels in lipopolysaccharide-stimulated cultured primary microglia by ∼93%. Furthermore, phosphorylated-ERK/ERK ratios were significantly decreased in naïve LysMcreTNF fl/fl mice demonstrating altered ERK signal transduction. Micro-PET using 18 [F]-fluorodeoxyglucose immediately after focal cerebral ischemia showed increased glucose uptake in LysMcreTNF fl/fl mice, representing significant metabolic changes, that translated into increased infarct volumes at 24 hours and 5 days compared to littermates (TNFfl/fl). In naïve LysMcreTNF fl/fl mice cytokine levels were low and comparable to littermates. At 6 hours, TNF producing microglia were reduced by 56% in the ischemic cortex in LysMcreTNF fl/fl mice compared to littermate mice, whereas no TNF + leukocytes were detected. At 24 hours, pro-inflammatory cytokine (TNF, IL-1β, IL-6, IL-5 and CXCL1) levels were significantly lower in LysMcreTNF fl/fl mice, despite comparable infiltrating leukocyte populations. Our results identify microglial TNF as beneficial and neuroprotective in the acute phase and as a modulator of neuroinflammation at later time points after experimental ischemia, which may contribute to regenerative recovery.
