The process of identifying, solving and preventing drug related problems in the LIMM-study

- Objective To avoid negative effects of drug treatment and need for additional medical care, drug treatment must be individualised. Our research group has developed a model for clinical pharmacy which improves several aspects of the patient's drug treatment. This study describes the process behind these improvements, i.e. drug related problems identified by pharmacists within a clinical pharmacy service. Setting Three wards at a department of internal medicine. Method Pharmacists performed systematic interventions during the patient's hospital stay, aiming to identify, solve and prevent drug related problems in the elderly. Identified drug related problems were put forward to the health care team and discussed. Information on identified problems, and their outcomes was collected and analysed. A questionnaire was used to evaluate the health care personnel's attitudes towards the process. Main outcome measure The number of drug related problems identified by the clinical pharmacists, the proportion of problems discussed with the physicians, the proportion of problems adjusted by the physicians and whether pharmacists and physicians prioritised any subgroup of drug related problems when choosing which problems to address. Finally, we wanted to evaluate the health care personnel's attitudes towards the model. Results In total, 1,227 problem were identified in 190 patients. The pharmacists discussed 685 (55.8%) of the identified problems with the physicians who accepted 438 (63.9%) of the suggestions. There was no significant difference in which subgroup to put forward and which to adjust. There was a high response rate (84%) to the questionnaire, and the health care personnel estimated the benefits to be very high, both for the patients and for themselves. Conclusion The process for identifying, solving and preventing drug related problems was good and the different types of problems were considered equally important. The addition of a clinical pharmacy service was considered very useful. This suggests that the addition of our clinical pharmacy service to the hospital setting add skills of great importance.

A Conceptual Framework for Studying the Successful Outcome of the IS Outsourcing Process from a Relationship Perspective

- This thesis has its point of departure in the identified knowledge gap, which includes the importance of the information systems (IS) outsourcing relationship for studying the success of IS outsourcing. This research study, therefore, is structured with the belief that the disregard of the client-IS supplier relationship when studying IS outsourcing is paradoxical. Paradoxical since the impact of the relationship on the overall IS outsourcing can make the difference between success, less success and even failure. In contrast to the majority of prior research, which has focused IS outsourcing in its entirety, this research study percieves IS outsourcing as a process. Because prior research shows that the success or failure of IS outsourcing is connected to its implementation, this thesis focuses the post-contract stage of the IS outsourcing. The purpose chosen and addressed is to develop a conceptual framework for describing and explaining the degree of successful outcome of the IS outsourcing process using a relationship perspective. To approach this purpose, a literature review study is conducted. The literature review study includes the integration of three theoretical fields: IS, interorganisational business relationships and IS outsourcing. The knowledge domains of particular interest are 'strategies, approaches and frameworks for the IS life cycle', 'the interaction approach' and 'IS outsourcing success'. The purpose is focused through the identification of key conditions, dimensions and interrelations among dimensions. These identified elements, constituting the core elements of the conceptual framework, are a result of the conducted literature review study. The key conditions are identified according to their influence on the degree of successful outcome of the IS outsourcing process from a relationship perspective. Due to the large amount of key conditions identified, they are categorised into nine dimensions: Atmosphere, Behaviour, Contract, Environment, Interatction, IS, Management, Organisation and Outcome. The identified interrelations among these dimensions are fruitful since they provide insights and a favourable point of departure for studying the degree of successful outcome of IS outsourcing processes. The main contribution of this thesis is the conceptual framework for the IS outsourcing process. The key conditions, dimensions and interrelations constitute the conceptual framework as a result of their potential for describing and explaining the degree of successful outcome of the IS outsourcing process from a relationship perspective. The application of the conceptual framework provides managers with core elements to be considered during IS outsourcing decision-making and subsequent design of IS outsourcing. Its usage, moreover, is believed to support managers during the operationalisation of the post-contract stage of IS outsourcing processes. Finally, the conceptual framework is a fruitful 'tool' for future empirical research. Fruitful in the sense that it can provide useful and important insights into how different key conditions infuence the degree of successful outcome of IS outsourcing processes and how these key conditions are interrelated.

Co-design av smartphoneapplikation i användningsmiljö : en studie inom ramen för projektet DigitalWell Research och innovationscaset Att hitta rätt

- Inom ramen för innovationscaset Att hitta rätt och forskningsprojektet DigitalWell Research genomfördes forskningsstudien Co-design av smartphoneapplikation i användningsmiljö. Det primära syftet var att undersöka hur co-design för mobila applikationer kan bedrivas i användningsmiljöer. Den här rapporten fokuserar på det sekundära syftet med studien vilket var att tydliggöra behovs- och kravbilden över en digital tjänst för att hitta rätt på Centralsjukhuset i Karlstad (CSK). Data samlades in genom att medborgare bjöds in att interagera med och samskapa en prototyp av en navigationsapplikation, medan de tog sig fram till olika platser på CSK. Totalt deltog 36 personer. I den här rapporten presenteras de kravområden för en navigationsapplikation i inomhusmiljöer som identifierats. Rapporten presenterar också de svårigheter och problem med att hitta rätt i sjukhusmiljön som samlades in under studien, liksom viktiga aspekter att beakta vid ett införande av en navigationsapplikation i en sjukhusmiljö.

Co-expression of a disease-associated lysozyme variant with human lysozyme in Drosophila causes accumulation of amyloid deposits and neurodegeneration

- Lysozyme amyloidosis is a dominantly inherited form of amyloid disease. Mutant variants of the protein, with increased tendencies to aggregate compared to the wild type (WT), accumulate in large amyloid deposits in multiple organs, eventually leading to organ failure. Humans affected by lysozyme amyloidosis carry one allele for the wild type protein and one allele encoding for a mutant variant of lysozyme. We have used a Drosophila melanogaster model to investigate the effect of co-expressing WT lysozyme and a mutated variant, F57I, in the central nervous system (CNS) of the fly. In this study, using activity and longevity assays, WT-F57I flies showed a lower activity and a shorter lifespan than flies expressing only WT or the F57I variant of lysozyme (median survival 16 days compared to 34 and 23 respectively). This indicates deteriorating neurological functions in WT-F57I flies; exceeding the decrease in neurological function previously observed for flies only expressing the mutated variant, F57I. In addition, accumulation of insoluble species with amyloid structure was detected for the WT-F57I flies but not for the WT or the F57I flies. Our study show that co-expression of WT lysozyme and the amyloidogenic variant F57I results in neurological damage and is required for accumulation of amyloid deposits, which is characteristic for the disease observed in humans. Our data suggest that insoluble amyloid species or intermediate species, formed on the pathway toward amyloid species, may be cytotoxic and thus contribute to the impaired neurological functions observed for the WT-F57I flies.

IS outsourcing relationships bring challenges that need to be understood and managed. The clientIS supplier relationship as such is one of the less frequently examined topics in IS outsourcing research. This paper contributes to the IS outsourcing re-lationship literature by describing conditions influencing clientIS supplier inter-actions during IS outsourcing. The paper sets out from the interaction approach as it contributes new insights to IS outsourcing relationships. The interaction ap-proach is complemented with elements to increase the description of IS outsourc-ing relationship interactions. - Inom mjukvaruindustrin går det åt mycket tid för att lösa problem som redan är lösta. Förevigandet av den här cykeln att uppfinna hjulet på nytt har delvis uppmuntrats på grund av att det inte finns någon effektiv mekanism för spridning av framgångsrika lösningar på återkommande problem. Design patterns utgör ett verktyg för att återanvända erkända lösningar och kan därmed delvis lösa detta mjukvaruproblem. Detta har väckt vårt intresse och vi har valt att koncentrera vår studie på problemformuleringen; vad, hur och varför design patterns i en utvecklingsverksamhet för mjukvara. För att få en grundläggande kunskap i ämnet har vi samlat in teori genom litteraturstudier och för att besvara vår problemformulering har vi samlat in empiri från en verksamhet. Den verksamhet som vi har valt att basera vår studie på är Ericsson Infotech AB i Karlstad. Empirin har vi samlat in genom att genomföra tre intervjuer. Respondenterna vi har intervjuat har likartad befattning och representerar tre olika avdelningar på företaget. Detta för att erhålla en mer representativ syn på verksamhetens användande av design patterns. Några av de resultat som vi har kommit fram till är att design patterns är ett beprövat sätt för att lösa designproblem samt en strategi att luta sig mot vid designarbete. Den främsta anledningen till varför verksamheten använder design patterns är för att möjliggöra återanvändning. Vad som även framkom vid vår studie var att verksamheten inte har några fasta rutiner för hur användandet av design patterns ska gå till.
