    • In two experiments, 15 and 13 subjects estimated the loudness of 12 sound-pressure levels (38–104 dB; 6-dB intervals) of a 1000-Hz tone by the method of magnitude estimation with a modulus assigned to the first stimulus presented. The tone duration was 1 sec. and the interstimulus interval was 6 sec. The presentation order was systematically ascending-descending in one experiment and balanced-irregular in the other. The results indicate that (1) loudness is a power function of sound pressure with an exponent of 0.60 for the systematic order and 0.29 for the irregular order. (2) For both the irregular and systematic orders, a large step-size (12 or 18 dB) between the stimulus on Trial n and on Trial n-1 (or n-3) results in a slight assimilation effect. This also occurs for the small step-size (6 dB) in the irregular order. (3) The size of momentary exponents (based on two points, Trials n and n-l or n-3) depends on the sound pressures of successive stimuli, whether the steps are positive or negative, and whether the stimuli have been presented in systematic or irregular order. For positive steps, the momentary exponent is lower for a soft tone (Trial n) than for a loud tone, whereas for negative steps the momentary exponent is lower for a loud tone than for a soft tone. These effects are more pronounced when these stimuli are presented in an irregular order. A relative judgment model is offered for magnitude estimation. It assumes that subjects judge the loudness of a stimulus in terms of three reference markers: the minimum and maximum sound pressures as well as the sound pressure of the previous stimulus.

    • The two major complications of atherosclerosis are acute myocardial infarction (AMI) and acute ischemic stroke. Both are life-threatening conditions characterised by the abrupt cessation of blood flow to respective organs, resulting in an infarction. Depending on the extent of the infarction, loss of organ function varies considerably. In both conditions, it is possible to limit the extent of infarction with early intervention. In both conditions, minutes count. This article aims to describe differences and similarities with regard to the way patients, bystanders and health care providers act in the acute phase of the two diseases with the emphasis on the pre-hospital phase.

  • Depressive symptoms postpartum among parents are associated with marital separation : A Swedish cohort study

    • Aims: To study whether there is an association between dyadic consensus, depressive symptoms, and parental stress during early parenthood and marital separation 6–8 years after childbirth, among couples in Sweden.Methods: At baseline, 393 couples were included. The couples answered three questionnaires, including: Dyadic consensus at 1 week post-partum, depressive symptoms at 3 months post-partum and parental stress at 18 months post-partum. The parents’ addresses were followed up after 6–8 years, to study the marital separation rate.Results: We found, 6–8 years after childbirth, that 20% of study couples were separated. Separation was associated with less dyadic consensus (mothers p < 0.001; fathers p < 0.001), depressive symptoms (mothers p = 0.022; fathers p = 0.041) and parental stress (mothers p = 0.002; fathers p = 0.040). The hazard ratio (HR) for marital separation was related to dyadic consensus for fathers (HR 0.51; 95% CI 0.28–0.92), depressive symptoms for mothers (HR 1.69; 95% CI 1.01–2.84) and fathers (HR 1.92; 95% CI 1.12–3.28), and the mother’s parental stress (HR 2.16; 95% CI 1.14–4.07).Conclusions: Understanding how dyadic consensus, depressive symptoms and parental stress are associated with marital separation is important for health professionals. It could be useful in developing interventions to provide parents with adequate support during pregnancy and early parenthood. This knowledge is also important for the public. Parents should get support in pregnancy and while bringing up children, which may help prevent marital separation and optimize conditions for the children.

  • Rätten till platsen : Tillhörighet och samhörighet i två lokala industrisamhällen under omvandling

    • This thesis is based on the demise of industry that has led to a search for new solutions for the survival of smaller communities traditionally dependent on industrial production. In two local industrial communities in central and northern Sweden, Surahammar and Timrå, industrial dependency has gradually been replaced by a dependency on immigration, largely from the neighboring cities of Västerås and Sundsvall. In the changed order of these local communities, old and new inhabitants have had to negotiate positions in order to co-exist. The objective of the thesis is to present how old and new inhabitants relate to and are attached to the local community as a place in which they reside and live their lives. In focus is an interest in the local identity construction of the inhabitants, i.e. how they identify with the place in which they dwell and live their everyday lives, with and in reference to each other and the outside world. Another concern relates to just how different local options for identification can be for individuals and for collective groups of people.Adopting a phenomenological and constructivist perspective, I regard local place as something that both old and new inhabitants are shaped by and shape in a collective process based on contrasts. The analysis is based on empirical data gathered during several years of fieldwork in the studied communities. The empirical data mainly consists of interviews and participant observations with old and new inhabitants in Surahammar and Timrå. Moreover, biographical literature, newspaper articles and data retrieved from the Internet have contributed important insights into how the local place is experienced and valued among the different groups of inhabitants. The thesis recognises the values and symbols that old and new inhabitants agree upon and that often result in spatial attachments. In addition, ideological, social and cultural differences are depicted as factors that shape a place as old and new inhabitants articulate local attachment and identification based on different grounds, needs and dreams.

    • Rationale and aims To evaluate the extensive use of peripheral venous catheters (PVCs), including catheter-related complications, a reliable tool for PVC assessment is needed. The aim of this study was to develop such a tool to evaluate PVCs in relation to management, documentation and signs and symptoms of thrombophlebitis (TH), as well as to determine its inter-rater and test-retest reliability. Method The tool development included confirmation of content and face validity. Two groups of registered nurses used the new tool (PVC assess) to assess PVC management and signs of TH independently. Group A (n = 3) assessed 26 items in 67 PVCs bedside (inter-rater reliability). Group B (n = 3) assessed photographs (67 PVCs, 21 items) of the same PVCs as those in Group A with a 4-week interval (test-retest reliability). Proportion of agreement P(A) and Cohen's kappa were calculated to evaluate inter-rater and test-retest reliability. Results Among nurses assessing PVCs at bedside, the P(A) was good to excellent (0.80-1) in 96% of the items in PVC assess. In 80% of the items kappa was substantial to almost perfect (0.61-1). TH sign erythema fell into the fair range (kappa = 0.40). In test-retest reliability analysis the P(A) was within the good and excellent range (0.80-1.0) and kappa varied from moderate to almost perfect (0.41-1.0) in 95% of the items. One item 'outer dressing is clean' was in fair range (0.21-0.40). Conclusions The PVC assess instrument shows satisfactory inter-rater and test-retest reliability. Reliability tests on reviewing documentation remain to be performed.

    • Purpose: Nursing students should be given opportunities to participate in clinical audits during their education. However, audit tools are seldom tested for reliability among nursing students. The aim of this study was to present reliability among nursing students using the instrument PVC assess to assess management of peripheral venous catheters (PVCs) and PVC-related signs of thrombophlebitis. Methods: PVC assess was used to assess 67 inserted PVCs in 60 patients at ten wards at a university hospital. One group of nursing students (n= 4) assessed PVCs at the bedside (inter-rater reliability) and photographs of these PVCs were taken. Another group of students (n= 3) assessed the PVCs in the photographs after 4 weeks (test-retest reliability). To determine reliability, proportion of agreement [P(A)] and Cohen's kappa coefficient (k) were calculated. Results: For bedside assessment of PVCs, P(A) ranged from good to excellent (0.80-1.0) in 55% of the 26 PVC assess items that were tested. P(A) was poor (< 0.70) for two items: "adherence of inner dressing to the skin" and " PVC location." In 81% of the items, k was between moderate and almost perfect: moderate (n= 5), substantial (n= 3), almost perfect (n= 5). For edema at insertion site and two items on PVC dressing, k was fair (0.21-0.40). Regarding test-retest reliability, P(A) varied between good and excellent (0.81-1) in 85%-95% of the items, and the k ranged between moderate and almost perfect (0.41-1) in 90%-95%. Conclusions: PVC assess demonstrated satisfactory reliability among nursing students. However, students need training in how to use the instrument before assessing PVCs.

  • Getting a Feel for Tactile Space : Exploring Haptic Perception of Microtexture

    • The present thesis is based on three studies that research different aspects of fine texture perception. The goal is to better understand the mechanisms involved in haptic perception of textures below 200 µm, also known as microtextures. Study I was conducted to establish a friction measurement model and relating the friction measurements to perceived coarseness of fine textures. A set of printing papers was used as stimulus material. In Study II an expanded set, including the set of Study I, was used as stimuli in a multidimensional scaling (MDS) experiment of haptic fine texture perception. Through scaling of perceptual attributes and similarities, a three dimensional space was found to best describe the data and the dimensions were interpreted as rough-smooth, thick-thin and distinct-indistinct. In Study III a series of model surfaces were manufactured with a systematically varied sinusoidal pattern, spanning from 300 nm to 80 µm. As in Study II, a similarity experiment was conducted and a two dimensional space was chosen, the dimensions of which were explained well through friction and the wavelength. Together these three studies form a better picture of fine texture perception. The dimensionality found with paper stimuli was very similar to the corresponding spaces for marcrotextures of everyday materials, even though a different perceptual system is used for fine texture perception. Regardless if the information is coded through the spatial or the vibratory sense, the perception does not seem to differ in dimensionality. Further, the largest among the microtextures seem to have been perceived as carrying spatial information. On the systematically varied, rigid, textures, the MDS space did not come out in a similar fashion to those of everyday materials but instead similar to the physical properties that characterizes the change in the textures. It was further found that the participants in Study III successfully discriminated textures with an amplitude of 13 nm from the unwrinkled surfaces. From these studies the main conclusions are (a) haptically measured friction and surface roughness are important contributors to fine texture perception, (b) even at microscales, spatial information is retrieved haptically, probably through vibrations, and (c) persons can haptically discriminate textures at a nanoscale.
