  • Identification of uniquely expressed transcription factors in highly purified B-cell lymphoma samples.

    • Transcription factors (TFs) are critical for B-cell differentiation, affecting gene expression both by repression and transcriptional activation. Still, this information is not used for classification of B-cell lymphomas (BCLs). Traditionally, BCLs are diagnosed based on a phenotypic resemblance to normal B-cells; assessed by immunohistochemistry or flow cytometry, by using a handful of phenotypic markers. In the last decade, diagnostic and prognostic evaluation has been facilitated by global gene expression profiling (GEP), providing a new powerful means for the classification, prediction of survival, and response to treatment of lymphomas. However, most GEP studies have typically been performed on whole tissue samples, containing varying degrees of tumor cell content, which results in uncertainties in data analysis. In this study, global GEP analyses were performed on highly purified, flow-cytometry sorted tumor-cells from eight subgroups of BCLs. This enabled identification of TFs that can be uniquely associated to the tumor cells of chronic lymphocytic leukemia (CLL), diffuse large B-cell lymphoma (DLBCL), follicular lymphoma (FL), hairy cell leukemia (HCL), and mantle cell lymphoma (MCL). The identified transcription factors influence both the global and specific gene expression of the BCLs and have possible implications for diagnosis and treatment.

  • Prevalence of Subclinical Coronary Artery Atherosclerosis in the General Population

    • Background: Early detection of coronary atherosclerosis using coronary computed tomography angiography (CCTA), in addition to coronary artery calcification (CAC) scoring, may help inform prevention strategies. We used CCTA to determine the prevalence, severity, and characteristics of coronary atherosclerosis and its association with CAC scores in a general population.Methods: We recruited 30 154 randomly invited individuals age 50 to 64 years to SCAPIS (the Swedish Cardiopulmonary Bioimage Study). The study includes individuals without known coronary heart disease (ie, no previous myocardial infarctions or cardiac procedures) and with high-quality results from CCTA and CAC imaging performed using dedicated dual-source CT scanners. Noncontrast images were scored for CAC. CCTA images were visually read and scored for coronary atherosclerosis per segment (defined as no atherosclerosis, 1% to 49% stenosis, or ≥50% stenosis). External validity of prevalence estimates was evaluated using inverse probability for participation weighting and Swedish register data.Results: In total, 25 182 individuals without known coronary heart disease were included (50.6% women). Any CCTA-detected atherosclerosis was found in 42.1%; any significant stenosis (≥50%) in 5.2%; left main, proximal left anterior descending artery, or 3-vessel disease in 1.9%; and any noncalcified plaques in 8.3% of this population. Onset of atherosclerosis was delayed on average by 10 years in women. Atherosclerosis was more prevalent in older individuals and predominantly found in the proximal left anterior descending artery. Prevalence of CCTA-detected atherosclerosis increased with increasing CAC scores. Among those with a CAC score >400, all had atherosclerosis and 45.7% had significant stenosis. In those with 0 CAC, 5.5% had atherosclerosis and 0.4% had significant stenosis. In participants with 0 CAC and intermediate 10-year risk of atherosclerotic cardiovascular disease according to the pooled cohort equation, 9.2% had CCTA-verified atherosclerosis. Prevalence estimates had excellent external validity and changed marginally when adjusted to the age-matched Swedish background population.Conclusions: Using CCTA in a large, random sample of the general population without established disease, we showed that silent coronary atherosclerosis is common in this population. High CAC scores convey a significant probability of substantial stenosis, and 0 CAC does not exclude atherosclerosis, particularly in those at higher baseline risk.

    • Antibody microarrays enable extensive protein expression profiling, and provide a valuable complement to DNA microarray-based gene expression profiling. In this study, we used DotScan (TM) antibody microarrays that contain antibodies against 82 different cell surface antigens, to determine phenotypic protein expression profiles for human B cell sub-populations. We then demonstrated that the B cell protein profile can be used to delineate the relationship between normal B cells and malignant counterparts. Principle component analysis showed that the lymphomas did not cluster with the normal memory B cells or germinal centre B cells, but they did cluster with germinal centre founder cells and naive B cells.

    • Syftet med Vintermodellen är att beräkna och värdera de väsentligaste konsekvenserna för trafikanter, väghållare och samhälle av olika strategier och åtgärder inom vinterväghållningen. Huvudrapporten är i sig en sammanfattning av de rapporter som beskriver Vintermodellens olika delmodeller. Navet i Vintermodellen är Väglagsmodellen, som utgående från väderdata, vidtagna väghållningsåtgärder och trafik beräknar väglaget timme för timme under vintersäsongen. Väglagsmodellen styr beräkningarna i de olika effektmodellerna: Olycksmodellen, Framkomlighetsmodellen, Fordonskostnadsmodellen, Miljömodellen och Modellen för väghållarkostnader. I Olycksmodellen beräknas olyckskvoter, olyckstyper och konsekvenser, allt kopplat till olika väglag och deras varaktigheter.I Framkomlighetsmodellen beräknas olika väglags effekt på medelhastigheter och restider.I Fordonskostnadsmodellen beräknas kostnader för bränsleförbrukning och korrosion på grund av vägsalt.I Miljömodellen beräknas konsekvenserna för vägnära vegetation av vägsalt.I Modellen för väghållarkostnader beräknas dels direkta kostnader för åtgärderna, dels kostnader för skador och slitage på beläggning, vägmarkeringar etc. som följd av vinterväghållningsåtgärder.

    • VTI and the Norwegian Institute of Transport Economics (TØI) have at the request of the Swedish Road Administration carried out a research and development project concerning tourist routes in Sweden. The project was done in two parts, one literature study with examples of how other countries work with tourist routes and one field study about the opinions of visitors and business operators along two Swedish tourist routes (the present publication). The two tourist routes which have been studied are Tourist Route Gränna-Ödeshög-Rök in the Counties of Jönköping and Östergötland, and Tourist Route Kullaberg in the County of Skåne. The study of the experiences of the business operators, the visitors' willingness to pay and a deepened analysis of the landscape was only done along the Gränna route. As for the experiences of the visitors the data for the two Swedish tourist routes have been compared with similar data from a Norwegian tourist route, Tourist Route Sognefjell.

    • A gastrostomy device is removed from the gastrostoma when no longer needed. The aim of the study was to test the hypothesis of whether it is possible for the surgeon to decide which stoma has to be closed with a gastroraphy and which to leave for a spontaneous closure within a reasonable period of time. Out of a cohort of 321 patients, who had been operated with a video-assisted gastrostomy, we included all the 48 patients having had their gastrostomy button removed. These patients were carefully followed and the closure of the gastrostoma was registered. According to the institutional routine we waited at least 3 months after the removal of the gastrostomy device before suggesting to the child's guardians an operative closure of the stoma. In 26 patients the stoma closed within 3 months, whereas in 22 patients a surgical gastroraphy was performed. We found no differences between the two groups regarding the patients' diagnoses, the duration of the gastrostoma use or patient's age at the time of removal of the gastrostomy device. This study rejected the hypothesis of predictability of the gastrostoma closure. Thus, we recommend a routine expectance after the removal of a gastrostomy device for at least 1 month. If no spontaneous closure occurs, then a gastroraphy should be performed.

    • Besides the open sea, ports, fairways, pilot services, and ice-breaking arethe main maritime infrastructure components. Marginal costs of maritimetransport can be divided into the following groups: emissions to water andair, accidents, and costs related to infrastructure usage. The first marginalcost type is the occurrence of negative external effects to third-men outsidethe transport system. The marginal costs from infrastructure use are relatedto the parts that invest, maintain, and provide the services in themselves aswell as to the users. The latter part is affected by restrictions incapacity, which can affect the user costs of the services. This reportconsiders all these marginal costs components, except emissions to air. Thebasic purpose of the report is to narrow down the state of knowledgeregarding: - Marginal cost pricing in theory; - Empirical research regardingmarginal cost pricing; - Review availability of data and how institutionalconditions affect the possibilities for implementation of marginal cost basedpricing. The scope of the report is limited to the state of knowledgeconcerning maritime infrastructure of the types mentioned above and externaleffects from accidents and emissions to water. A secondary purpose of thereport is to give suggestions for further research based on the authors' viewof the state of knowledge.
