  • Assessment of continuous pain in newborns admitted to NICUs in 18 European countries

    • Aim: Continuous pain occurs routinely, even after invasive procedures, or inflammation and surgery, but clinical practices associated with assessments of continuous pain remain unknown.Methods: A prospective cohort study in 243 Neonatal Intensive Care Units (NICUs) from 18 European countries recorded frequency of pain assessments, use of mechanical ventilation, sedation, analgesia, or neuromuscular blockade for each neonate upto 28 days after NICU admission.Results: Only 2113/6648 (31·8%) of neonates received assessments of continuous pain, occurring variably among tracheal ventilation (TrV, 46·0%), noninvasive ventilation (NiV, 35·0%), and no ventilation (NoV, 20·1%) groups (p<0·001). Daily assessments for continuous pain occurred in only 10·4% of all neonates (TrV: 14·0%, NiV: 10·7%, NoV: 7·6%; p<0·001). More frequent assessments of continuous pain occurred in NICUs with pain guidelines, nursing champions, and surgical admissions prompted (all p<0·01), and for newborns <32 weeks gestational age, those requiring ventilation, or opioids, sedatives-hypnotics, general anesthetics (O-SH-GA) (all p<0·001), or surgery (p=0·028). Use of O-SH-GA drugs increased the odds for pain assessment in the TrV (OR:1·60, p<0·001) and NiV groups (OR:1·40, p<0·001).Conclusion: Assessments of continuous pain occurred in less than one-third of NICU admissions, and daily in only 10% of neonates. NICU clinical practices should consider including routine assessments of continuous pain in newborns.

  • Effects of morphine analgesia in ventilated preterm neonates : primary outcomes from the NEOPAIN randomised trial

    • BACKGROUND: Opioid analgesia is commonly used during neonatal intensive care. We undertook the Neurologic Outcomes and Pre-emptive Analgesia in Neonates (NEOPAIN) trial to investigate whether pre-emptive morphine analgesia decreases the rate of a composite primary outcome of neonatal death, severe intraventricular haemorrhage (IVH), and periventricular leucomalacia (PVL) in preterm neonates.METHODS: Ventilated preterm neonates (n=898) from 16 centres were randomly assigned masked placebo (n=449) or morphine (n=449) infusions. After a loading dose (100 microg/kg), morphine infusions (23-26 weeks of gestation 10 microg kg(-1) h(-1); 27-29 weeks 20 microg kg(-1) h(-1); 30-32 weeks 30 microg kg(-1) h(-1)) were continued as long as clinically justified (maximum 14 days). Open-label morphine could be given on clinical judgment (placebo group 242/443 [54.6%], morphine group 202/446 [45.3%]). Analyses were by intention to treat.FINDINGS: Baseline variables were similar in the randomised groups. The placebo and morphine groups had similar rates of the composite outcome (105/408 [26%] vs 115/419 [27%]), neonatal death (47/449 [11%] vs 58/449 [13%]), severe IVH (46/429 [11%] vs 55/411 [13%]), and PVL (34/367 [9%] vs 27/367 [7%]). For neonates who were not given open-label morphine, rates of the composite outcome (53/225 [24%] vs 27/179 [15%], p=0.0338) and severe IVH (19/219 [9%] vs 6/189 [3%], p=0.0209) were higher in the morphine group than the placebo group. Placebo-group neonates receiving open-label morphine had worse rates of the composite outcome than those not receiving open-label morphine (78/228 [34%] vs 27/179 [15%], p<0.0001). Morphine-group neonates receiving open-label morphine were more likely to develop severe IVH (36/190 [19%] vs 19/219 [9%], p=0.0024).INTERPRETATION: Pre-emptive morphine infusions did not reduce the frequency of severe IVH, PVL, or death in ventilated preterm neonates, but intermittent boluses of open-label morphine were associated with an increased rate of the composite outcome. The morphine doses used in this study decrease clinical signs of pain but can cause significant adverse effects in ventilated preterm neonates.

  • Mathematics in the Swedish Upper Secondary School Electricity Program : A study of teacher knowledge

    • Mathematical knowledge is often a prerequisite for students at Swedish upper secondary vocational programs to be able to study vocational courses, for example electricity courses in the Electricity Program. Electricity Program students study mathematics in their electricity courses as well as in their mathematics course. The mathematics in those two settings has a different character. A goal of this thesis is to investigate what constitutes that character. In this study three mathematics and five electricity teachers have been interviewed about how they would explain three mathematical electricity tasks to students on the Electricity Program. Teacher knowledge in both electricity and mathematics has been used in the analyses and has been compared between the different teacher groups. In addition to providing an overview analysis of all the teachers’ explanations, detailed analyses have been carried out, comparing pairs of teachers’ explanations. The teachers’ choices of explanations and their use of specific and general mathematical knowledge have been studied. Mathematics contains a wide range of subject areas but also a wide range of representations and methods that highlight different aspects of mathematics. This study shows that different teachers emphasize different aspects of mathematics in their explanations of the same tasks, even though intended to the same students, both in the their choices of explanation and in their use of mathematics. The electricity teachers drew upon their practical electrical knowledge when they connected their explanations of mathematics to vocational work. The electrical knowledge they used not only grounded the tasks in a, for them, well-known real-world environment. The electrical knowledge actually helped them to solve the tasks, albeit in a more concrete/specific way than the mathematics teachers. The electricity teachers drew upon more specific mathematical knowledge in their explanations of the interview tasks, whereas the mathematics teachers drew upon more general mathematical knowledge in their explanations.  The different explanations of mathematics from the two kinds of teachers are markedly different, depending on whether they have a more practical/vocational or a more general/algebraic approach. The solutions to the interview tasks turned out to be the same but the character of the solutions paths are substantially different. This raises questions regarding the students’ abilities to reconcile the different approaches.

  • Arsenic accumulation in plants for food and phytoremediation : Influence by external factors

    • Arsenic (As) appears in the environment as various As species, which may vary in plant uptake and toxicity. Moreover, As exposure may vary between habitat due to availability and speciation, both of which are influenced by redox potential. To decrease As uptake, addition of silicate may be a tool.The aim of the study was to investigate how the external factors As availability, plant habitats, silicon and oxygen level, influenced the accumulation and speciation of As in plants for food and phytoremediation in a temperate region. The external factors were chosen due to their previously showed influence on As in plants.The risks with dietary As was investigated by plant As accumulation and speciation in carrot, lettuce and spinach grown in alum shale and glassworks soils, and by the influence of silicon on As accumulation in lettuce in hydroponics. Suitable plants for As phytoremediation was investigated by analysing plants from various habitats, and by the O2 influence on phytofiltration.The results showed that vegetables accumulated more As in soils with higher As extractability, and the As extractability in the rhizosphere was higher than in bulk soil. Also, the As concentration in lettuce was higher in hydroponics than in soil, but silicon reduced the accumulation of As in lettuce in hydroponics. Also, the more toxic inorganic As were the main As species detected in vegetables, compared with the less toxic organic As. For phytoremediation, the results showed a low As accumulation in emergent and terrestrial plants. Submerged plants had had a higher shoot As concentration. In general, the habitat had a major influence on the As accumulation in plants. The results also showed that the As accumulation properties in Elodea canadensis was reduced at higher O2.In conclusion, consumption of vegetables cultivated in As polluted soils can result in an elevated intake of inorganic As, and E. canadensis is a promising candidate for As phytofiltration in a temperate region.

  • Monitoring of chromatin organization in live cells by FRIC. Effects of the inner nuclear membrane protein Samp1

    • In most cells, transcriptionally inactive heterochromatin is preferentially localized in the nuclear periphery and transcriptionally active euchromatin is localized in the nuclear interior. Different cell types display characteristic chromatin distribution patterns, which change dramatically during cell differentiation, proliferation, senescence and different pathological conditions. Chromatin organization has been extensively studied on a cell population level, but there is a need to understand dynamic reorganization of chromatin at the single cell level, especially in live cells. We have developed a novel image analysis tool that we term Fluorescence Ratiometric Imaging of Chromatin (FRIC) to quantitatively monitor dynamic spatiotemporal distribution of euchromatin and total chromatin in live cells. A vector (pTandemH) assures stoichiometrically constant expression of the histone variants Histone 3.3 and Histone 2B, fused to EGFP and mCherry, respectively. Quantitative ratiometric (H3.3/H2B) imaging displayed a concentrated distribution of heterochromatin in the periphery of U2OS cell nuclei. As proof of concept, peripheral heterochromatin responded to experimental manipulation of histone acetylation. We also found that peripheral heterochromatin depended on the levels of the inner nuclear membrane protein Samp1, suggesting an important role in promoting peripheral heterochromatin. Taken together, FRIC is a powerful and robust new tool to study dynamic chromatin redistribution in live cells.

  • Evidence of Construct Validity for the Modified Mental Fatigue Scale When Used in Persons with Cerebral Palsy

    • Introduction: Fatigue impacts negatively on everyday activities in individuals with cerebral palsy (CP). More knowledge is needed about how mental fatigue is manifested in this target group. The purpose of this study was to gather evidence about the validity of the modified Mental Fatigue Scale (m-MFS) in adults with CP. Methods: Mixed sequential exploratory design. The respondents were ten persons aged 22-56 with CP (MACS I-II). Results: The respondents perceived the m-MFS as easy to read and understand. Its structure was characterised as straightforward and the text of the rating options was deemed to assist identification with life situations. Very good agreement was seen between the respondents' and the instrument designers' intended meaning for the items in the m-MFS; the weighted kappa was 0.92. Conclusion: This study showed evidence of construct validity, based on response processes and content, for use of the modified MFS in adults with CP.
