  • PCASTt/SPCG-17-A randomised trial of active surveillance in prostate cancer: Rationale and design

    • Introduction Overtreatment of localised prostate cancer is substantial despite increased use of active surveillance. No randomised trials help define how to monitor patients or when to initiate treatment with curative intent. Methods and analysis A randomised, multicentre, intervention trial designed to evaluate the safety of an MRI-based active surveillance protocol, with standardised triggers for repeated biopsies and radical treatment. The aim is to reduce overtreatment of prostate cancer. 2000 men will be randomly allocated to either surveillance according to current practice or to standardised triggers at centres in Sweden, Norway, Finland and the UK. Men diagnosed in the past 12 months with prostate cancer, ≤T2a, prostate-specific antigen (PSA) <15 ng/mL, PSA density ≤0.2 ng/mL/cc, any International Society of Urological Pathology (ISUP) grade 1 are eligible. Men with ISUP grade 2 in <30% of cores on systematic biopsy and <10 mm cancer in one core on systematic or targeted biopsy are also eligible. Men diagnosed on systematic biopsy should have an MRI and targeted biopsies against Prostate Imaging and Reporting Data System V.2 3-5 lesions before inclusion. Identical follow-up in the two study arms: biannual PSA testing, yearly clinical examination and MRI every second year. In the experimental arm, standardised triggers based on MRI and PSA density elicit repeated biopsies. MRI and histopathological progression trigger radical treatment. Primary outcome measure is progression-free survival. Secondary outcome measures are cumulative incidence of metastatic disease, treatments with curative intent, pT3-4 at radical prostatectomy, switch to watchful waiting, prostate cancer mortality and quality of life. Inclusion started in October 2016 and in October 2018; 275 patients have been enrolled. Ethics and dissemination Ethical approval was obtained in each participating country. Results for the primary and secondary outcome measures will be submitted for publication in peer-reviewed journals. Trial registration number NCT02914873.

  • Anti-TNF therapy in RA and risk of malignant lymphomas Relative risks and time-trends in the Swedish Biologics Register

    • Background: Tumour necrosis factor (TNF) antagonists have proved effective as treatment against rheumatoid arthritis (RA), but the unresolved issue of whether the use of anti-TNF therapy increases the already elevated risk of lymphoma in RA remains a concern.Methods: Using the Swedish Biologics Register (ARTIS), the Swedish Cancer Register, pre-existing RA cohorts and cross-linkage with other national health and census registers, a national RA cohort (n  =  67 743) was assembled and patients who started anti-TNF therapy between 1998 and July 2006 (n  =  6604) were identified. A general population comparator (n  =  471 024) was also assembled and the incidence of lymphomas from 1999 to 31 December 2006 was assessed and compared in these individuals.Results: Among the 6604 anti-TNF-treated RA patients, 26 malignant lymphomas were observed during 26 981 person-years of follow-up, which corresponded to a relative risk (RR) of 1.35 (95% CI 0.82 to 2.11) versus anti-TNF-naive RA patients (336 lymphomas during 365 026 person-years) and 2.72 (95% CI 1.82 to 4.08) versus the general population comparator (1568 lymphomas during 3 355 849 person-years). RA patients starting anti-TNF therapy in 1998–2001 accounted for the entire increase in lymphoma risk versus the two comparators. By contrast, RR did not vary significantly by time since start of first treatment or with the accumulated duration of treatment, nor with the type of anti-TNF agent.Conclusion: Overall and as used in routine care against RA, TNF antagonists are not associated with any major further increase in the already elevated lymphoma occurrence in RA. Changes in the selection of patients for treatment may influence the observed risk.

Informationspåverkan avser kort sagtanvändningen av (vilseledande) information i syfte att påverka den allmännaopinionen, demokratiska processer och det politiska beslutsfattandet. Målet med kapitlet är att bidra till en bättre förståelse av denbetydelse som humanistisk kunskap har haft i det svenska försvaret ochdärmed den svenska staten under slutet av 1900-talet. Jag studerar kunskapsom vanligen knyts till utbildningen och forskningen inom humanioraoch humanistiska fakulteter vid universitet och högskolor. Det handlarsärskilt om filosofisk kunskap om bedömningen eller värderingen av argumentoch utsagor i text samt källkritik. Källkritiken utvecklades ursprungligenav historiker under 1800-talet och har som vetenskaplig metod haftett stort inflytande på historievetenskapen i de nordiska länderna, kanskesärskilt i Sverige.

    • In several parts of Sweden, difficulties arise regarding access to water. In this article, I explore the biopoliticsthat take shape when low groundwater levels are turned into an object of government. Firstly, I examinethe formation of a so-called anatomical politics which refers to a disciplining form of power in which theresponsibilities and actions of the population are at the center. Secondly, I explore the priorities that are madein the political work where human life, health, and well-being are to be protected and other life forms that arenot vital for human living are instead deprioritized or allowed to die. The problems have clear consequencesbut do not pose a threat to human survival. Political efforts aim to bring about changes in people’s everydaylives and affect the use and decoration of households and public spaces. Empirically, I attach particularsignificance to the irrigation bans that Swedish municipalities can introduce to reduce water consumption.

    • During the post-war period, major research investments were made to improve military and defense capabilities both in Sweden and abroad. Much remains undone as to exploring the impact of the Swedish National Defense Research Establishment (FOA) on Swedish state and society. Historians have primarily explored the development and impact of technological and natural science research and its connections with e.g. the military industry. This article provides new insights into the societal and political significance of Swedish defense research by studying the human sciences at FOA. At the same time, new insights are gained on the knowledge provision in Swedish preparedness efforts. The aim of the article is to provide new knowledge about the influence of Swedish defense research on the Swedish civil defense and crisis preparedness in the late 20th century. The purpose of the article is to explore the significance of behavioral science research at the Department of Human Sciences (FOA 5) in relation to a process at the end of the Cold War in which crisis and preparedness were framed as problems pertaining to population behavior and risk awareness. The analysis is inspired by the theory and research on biopolitics, originating in the works of Michel Foucault. I examine the influence of FOA's behavioral science research in relation to what I call “biopolitical preparedness, ” which refers to an assemblage of research or knowledge about human life, health and behavior, and political governance. Empirically, the article explores Swedish governmental commissions (statens offentliga utredningar) and FOA reports.

    • This chapter explores knowledge associated with the humanities that hasbeen developed in practice-oriented research domains of the Swedishgovernment to help solve societal challenges. I study the Swedish NationalDefense Research Establishment [Försvarets forskningsanstalt] (FOA). Theconcept of “borderline humanities” refers to research activities that didnot abide by academic distinctions between the humanities, the socialsciences, and the natural sciences. The study shows how knowledge onhuman culture, history, language, and beliefs developed in a researchenvironment that drew on diverse fields of both research and practice.The chapter brings to the fore shared themes and concepts betweendifferent research fields and draws attention to how this affects the viewof research impact.

    • Throughout the Cold War, there was an extensive production of ideas and knowledge around radioactivity as a social hazard. Previous research shows that there is a lack of knowledge about the relationship between the preparedness against nuclear weapons that was built up in Sweden from the 1950s onwards and the environmental commit-ment that took shape a few decades later. The aim of this article is to contribute new knowledge about these historical conditions by examining the radioactivity research conducted at the Swedish National Defence Research Establishment [Försvarets for-skningsanstalt] (FOA). The exploration is focusing on a period during the 1960s and 1970s when questions about radioactivity in relation to nuclear weapons, nuclear power and the environment had a clear impact in Sweden. Swedish preparedness is usually associated with efforts to protect the human population. I show how animals, plants and other non-human life also became objects in defence research and the preparedness against radioactivity that the scientific work at FOA enabled. FOA was a Swedish governmental agency that provided scientific advice to the Swedish Total Defence. FOA operated at the intersection of military and civil science. The article is concerned with FOA’s ambitions and expressed interests. The primary empirical material consists of FOA publications. I focus on how radioactivity research was presented in Foatidningen that was published by FOA between 1963 and 2000.

  • Frihetstida policyskapande : Uppfostringskommissionen och de akademiska konstitutionerna 1738–1766

    • Regulating education is a vital part of government. This thesis is inspired by recent changes on the political landscape of higher education. It is guided by an interest in how political objectives and concepts of ideal social relationships are transformed and expressed through government university policies and their consequences. An early stage of what is now commonly referred to as the modern state and the modern research university, rather than present or relatively recent developments, will be explored. Instead of studying trends on the European continent, the thesis inquiries into an attempt made by the Swedish government to revise the constitutions of Swedish schools and universities through the so-called Educational Commission appointed in 1745.The purpose of the thesis is to apply a modern policy perspective to the Educational Commission’s attempt at reforming the constitution of the Swedish universities. The aim is to illuminate the construction of university regulations and to place this within a larger framework of policy making during the Age of Liberty (Frihetstiden) in Sweden.The Commission was an attempt by the Swedish government to implement educational changes based on a holistic view of the realm. It was one of several contemporary initiatives with nationwide ambitions. The Commission did not, however, succeed in reaching its formal objectives, but by placing too much emphasis on what the Commission did not achieve one risks overlooking other results and consequences. It initiated new communication structures, operating procedures and accountability schemes. It changed the regulations for assessing higher education making the university transparent and accountable to the government in new ways. New administrative routines for producing university reform were introduced.The Commission also provided university actors with a legitimate channel for voicing their opinions in relation to the government. They were given a legitimate position to formulate problems, questions and solutions regarding the university. The demands of the professors for increased autonomy in seeking knowledge and providing education stood against the claims made by the government for added control and insight into academic affairs. Through the Commission, the views of the professors were put into circulation in an official political context.
