    • An administrative health register containing health care data for over 2 million patients will be used to search for factors that can affect the treatment of heart failure. In the study, we will measure the effects of employed treatment for various groups of heart failure patients, using different measures of effectiveness. Significant deviations in effectiveness of treatments of the various patient groups will be reported and factors that may help explaining the effect of treatment will be analyzed. Identification of the most important factors that may help explain the observed deviations between the different groups will be derived through generation of predictive models, for which variable importance can be calculated. The findings may affect recommended treatments as well as high-lighting deviations from national guidelines.

  • Salivary resistin reflects local inflammation in Sjögrens syndrome

    • OBJECTIVE: To assess the role of resistin in primary Sjogren's syndrome (pSS) and its relation to local inflammation. METHODS: Blood and saliva were collected from 37 patients with pSS (duration of symptoms 12.6+/-1 yrs) and 32 healthy controls. Expression of resistin in salivary glands was visualized immunohistologically, and levels of resistin were detected by ELISA. Levels of resistin were evaluated at baseline and following oral dehydroepiandrosterone (DHEA) treatment (50 mg/day). The effect of DHEA treatment on the secretion of resistin was assessed in vitro in human leukocytes after challenge with insulin and lipopolysaccharide. RESULTS: Levels of resistin in saliva were significantly higher in patients with pSS than in controls, while circulating levels of resistin were similar in both groups. Resistin was expressed in the epithelial cells of striated ducts and in the lymphocytic foci. Resistin levels in saliva were related to the intensity of inflammation in the minor salivary glands of pSS patients. No changes of the levels of resistin in blood or saliva were observed during DHEA treatment. Exposure of naive leukocytes to DHEA in vitro induced significant expression of resistin compared to nonstimulated peripheral blood mononuclear cells (p=0.031). CONCLUSION: We showed that levels of resistin are upregulated locally in the salivary glands of patients with pSS; and that the levels of resistin correspond to the intensity of lymphocytic inflammation in patients with pSS. We suggest that resistin is expressed in the salivary glands of patients with pSS and may be a driving factor of local inflammation.

  • Role clarity and role conflict among Swedish diabetes specialist nurses

    • AIM: To explore diabetes specialist nurses (DSNs)' perceptions of their role in terms of clarity, conflict and other psychosocial work aspects.METHODS: A cross-sectional study was conducted among DSNs in a county in northern Sweden. The DSNs answered the Nordic Questionnaire of Psychological and Social Factors at Work (QPS Nordic) about psychosocial aspects of their work. Statistical analysis compared DSNs with a reference group of different health professionals. Correlations between role clarity, role conflict, and other variables were analysed.RESULTS: The DSNs perceived more, and higher, job demands, including quantitative, decision-making and learning demands, but also more positive challenges at work compared with the reference group. Role clarity correlated with experiences of health promotion, perception of mastery, co-worker support, and empowering leadership, while role conflict correlated with quantitative and learning demands.CONCLUSIONS: The DSNs perceived high demands but also positive challenges in their work. Their role expectations correlated with several psychosocial work aspects. It is important that DSNs should be presented with positive challenges as meaningful incentives for further role development and enhanced mastery of their work.

  • Functions of PDGF-A and -C. Essential ligands for the PDGF alpha receptor

    • Altered protein kinase activity is a contributing factor in many diseases including most formsof human malignancies, and there is reason to believe that protein kinases will prove to be majordrug targets in the treatment of cancer. Protein kinases mediate most of the signal transduction ineukaryotic cells, regulating cellular events such as transcription, metabolism, proliferation,cytoskeletal rearrangement, migration, differentiation and apoptosis. Protein phosphorylation alsoplays a critical role in intercellular communication during embryonic development, especiallythrough activation of receptor tyrosine kinases (RTK:s).This study focuses on the Platelet-derived growth factor (PDGF) family, secreted moleculeswhose functions are to bind to, and activate, two structurally related RTK:s, PDGF receptoralpha and beta. To gain insight into the developmental role of PDGF-A and -C, three lines ofmice were generated in which these genes were modified by gene targeting.As a common principle, PDGF-A and -C, secreted from epithelial cells, induced proliferation,and possibly migration, of mesenchymal progenitor cells expressing PDGF receptor alpha.Mice deficient for PDGF-A died perinatally and displayed defective lung development due tolack of alveolar formation. This phenotype was coupled to a loss of alveolar smooth muscle cellsand reduced parenchymal elastin, resulting in a picture resembling emphysema.The sixth exon in PDGF-A is normally alternatively spliced, and, when present, it confersbinding to extracellular matrix structures. The second line of mice generated carried a mutationin the sixth exon splice acceptor, so that only the short, freely diffusible, form of PDGF-A wasbeing produced. Analysis of these mice suggested overlapping functions for PDGF-A and -C, andalso revealed that extracellular retention of PDGF-A is important for normal development ofgastrointestinal villi.Depending on genetic background, PDGF-C negative mice died postnatally due to a cleftpalate accompanied by moderate spina bifida. Interestingly, PDGF-A / PDGF-C double deficientmice were phenotypically indistinguishable from mice carrying a null mutation in PDGFreceptor alpha. Taken together, the results of this study imply that PDGF-A and -C are thephysiologically important ligands for PDGF receptor alpha during development.

    • Carl Larsson är berömd och uppskattad för sina idylliska familjebilder från hemmet i Sundborn. Mindre kända är hans monumentalmålningar, trots att flera av dem pryder välkända offentliga byggnader i vårt land. Monumentalmåleriet var den del av hans konstnärskap som han själv satte högst.Ändå har det tilldragit sig begränsat intresse från forskningen. I den här boken sätts hans monumentalmålningar in i sitt europeiska sammanhang och studeras som bärare av samhälleliga, nationella och pedagogiska ideologier. De strategier Carl Larsson utnyttjade för att förmedla sitt budskap undersöks. Också förutsättningar i Carl Larssons egen psykobiografi bidrar till att förklara motivval och gestaltning i bland annat Midvinterblot på National­museum. Hans förhållande till konsthistorien och till sin samtids konst belyses genom egna uttalanden men också genom studium av hans bibliotek och reproduktionssamling på Carl Larsson-gården. Särskilt som monumentalmålare kunde Carl Larsson, utan att bli eklektiker eller imitatör, tillgodogöra sig impulser från äldre eller samtida europeisk konst men också från utomeuropeisk, inte minst japansk konst.Den samtida konsten i de tyskspråkiga länderna spelade en viktig roll för hans senare måleri, något som tidigare knappast uppmärksammats. Det finns all anledning att studera Carl Larssons måleri också utifrån ett genusperspektiv. Hans ofta komplicerade förhållande till mecenater och beställare granskas. Av största intresse är också receptionshistorien, inte minst då det gäller Midvinterblot, den kanske mest omstridda målningen i svensk konsthistoria. Boken handlar om konstverken men tecknar samtidigt en fascinerande bild av konstnärspersonligheten.
