    • The purposes of this descriptive study were threefold: first, to describe how patients perceived their participation in the planning of their rehabilitation; second, to describe nurses' and occupational therapists' view of the strategies they used to encourage patients' participation; and third, to compare the perceptions of the two groups. Data was collected through semi-structured interviews with 57 hospitalized patients, 39 nurses and 11 occupational therapists. On the basis of the data patients were categorized as (a) 'relinquishers', (b) 'participants', and (c) 'occasional participants'. Professionals were categorized as 'information providers' and 'rehabilitation practitioners'. The strategies used by the professionals to encourage patient participation were approximately the same irrespective of patient category. It is suggested that professionals need to be sensitive to the patient's desire to participate in the planning of a rehabilitation programme. Further research is recommended to investigate circumstances affecting patients' participation and which strategies professionals can use to encourage their participation

    • PURPOSE: The main aim of this study was to compare the effects of Body Awareness Therapy (BAT), the Feldenkrais (FK) method and conventional physiotherapy on changes of health-related quality of life (HRQL), self-efficacy and sense of coherence (SOC) in patients with non-specific musculoskeletal disorders. A second aim was to explore the relationships between SOC, HRQL and self-efficacy and to examine whether SOC could be a predictor of the treatment outcome.METHOD: A total of 78 patients, 64 women and 14 men, were recruited consecutively to the three treatment groups. The instrument used were the Swedish version of SF-36, the 20 items Arthritis Self-efficacy Scale and the 29-item questionnaire by Antonovsky.RESULTS: The results showed that there were significant improvements on all subscales of SF-36 except for one. By using effect-size values it was found that the BAT and FK groups reached larger effect-size than did the conventional therapy group. These two groups also improved in self-efficacy of pain and stayed stable while the third group deteriorated at the one-year follow-up. There were significant correlations between the mental dimensions of SF-36 and SOC indicating that the instruments may measure aspects of the same global construct.CONCLUSIONS: Although few significant differences between the three treatment groups the BAT and FK seemed to improve health-related quality of life and self-efficacy of pain to a somewhat higher degree than the conventional physiotherapy. SOC seemed to be a stable trait measure over time.

  • Significant resources needed for return to work after sick leave

    • There are increasing numbers of people on long-term sick leave, especially with stress-related ill health and musculoskeletal disorders. The main purpose of this study is to, early on in the sick leave, find predictors of individual resources for a return to work. The study group consists of women and men on sick leave compared with reference groups of healthy persons. A questionnaire was used regarding diagnosis given on the doctor's certificate, self-reported symptoms, residential area, education, and kind of work. It also contained open questions about consequences in daily life and beliefs about future. Checklists concerning life satisfaction, sense of coherence, locus of control, coping resources and sick leave information over a four-year period have been used. Significant differences between the study group and the reference groups were found, relating to sense of coherence, locus of control, life satisfaction and coping resources. More than half of the study group were still on sick leave at a two-year follow-up. Important predictive factors turned out to be: previous sick leave, own belief about future and self-reported symptoms.

    • The purpose of this study was to find out whether an occupational therapy model based on activity preferences, occupational roles and life satisfaction could serve as a frame of reference in the rehabilitation of clients with neurological disorders. Fifty-three patients, 34 women and 19 men, participated in treatment periods of an average length of 4 weeks. Checklists about activity preferences, occupational roles and life satisfaction combined with interviews formed the basis for goal planning. Goals were formulated in terms of the individual's fulfilment of valued occupational roles. Role-focused groups were established as treatment methods. Home maintenance, leisure and social activities were frequently reported as highly valued. The majority of women wanted to improve their abilities and skills for fulfilling the role as home maintainer and outdoor hobbyist. Among the men, the latter role was identified as being the most important to retain or resume. Less than one-third reported satisfaction with life as a whole, ADL, vocational situation and sexual life, and only a minority were satisfied with leisure. The inability to perform valued activities may cause loss of internalized roles and thereby decreased life satisfaction. Such an approach could be useful as a means of helping rehabilitation professionals offer a more holistic view in preparing the individual for everyday life.

  • Life satisfaction in subjects with chronic obstructive pulmonary disease

    • OBJECTIVES: To examine the levels of life satisfaction for patients with chronic obstructive disease (COPD) and to explore the relationships between life satisfaction, lung function, walking distance test/exercise capacity and quality of life. STUDY DESIGN: The population comprised 91 patients with COPD, 28 patients with COPD using long term oxygen therapy (LTOT) and a reference group (R) of 150 healthy individuals. Before the study, a number of the COPD patients had been tested with regard to spirometry, walking distance and quality of life using the Chronic Respiratory Disease Questionnaire (CRQ). All subjects filled in an eight-item checklist on levels of life satisfaction. RESULTS: Significantly lower levels of satisfaction were reported by both patient groups than the R group for satisfaction with life as a whole and satisfaction derived from vocational/occupational situation, sexual life and ADL, and by the LTOT group, furthermore, also with family life. Only 10% of the patients were satisfied with their health. The LTOT group reported significantly lower levels of satisfaction than the COPD group for life as a whole, satisfaction with their vocational/occupational situation, leisure, ADL situation and their satisfaction with family life and partnership relation. Factor analyses demonstrated different patterns between the R group and the patient group. No correlation was found between satisfaction with life as a whole and lung function parameters whereas three of the CRQ dimensions; emotional function, fatigue and mastery correlateded significicantly with satisfaction with life as a whole and several of the domains. No relationships was found between satisfaction with life as a whole or any of the domains and the CRQ dimension dyspnea. CONCLUSIONS: The low levels of satisfaction with life as a whole, in addition to low levels of satisfaction with several domains, indicate that the majority of the patients had not managed to cope successfully with the consequences of their impairment. More attention must, thus, be given to these patients in the rehabilitation work.
