  • A Biopsychosocial and Long Term Perspective on Child Behavioral Problems : Impact of Risk and Resilience

    • Mental health has become a prominent issue in society. Yet, much remains unknown about the etiology of psychiatric disorders. The aim of the present thesis was to investigate the association between biological, psychological and social factors of risk and resilience and behavioral problems in a birth cohort of Swedish children. 1723 mothers and their children were followed from birth to the age of 12 as part of the South East Sweden Birth Cohort Study (the SESBiC study). Information was gathered through register data, standardized questionnaires and DNA samples.In study I, stability of maternal symptoms of depression and the impact on child behavior at age 12 were investigated. The prevalence of depressive symptoms was found to be 12.0 % postpartum. Symptoms of postpartum depression significantly increased the risk for subsequent depressive symptoms 12 years later in women. Children whose mothers reported concurrent symptoms of depression and anxiety had an increased risk for both internalizing and externalizing problems at age 12, but no long term effect on child behavior was seen for postpartum depressive symptoms. The greatest risk was seen for children whose mothers reported symptoms of depression on both occasions. In study II, the impact of gene-environment interaction of 5-HTTLPR and BDNF Val66Met and experience of life events together with symptoms of maternal depression and anxiety on child behavior at age 12 was studied. A main effect of 5-HTTLPR was noticed, but no geneenvironment effects were shown. Similarly to study I, concurrent symptoms of maternal depression and anxiety were an important predictor of child behavioral problems. A high degree of psychosocial stress around childbirth was found to have long lasting detrimental effects on child behavior, increasing the risk for internalizing problems at age 12. Study III investigated the impact of geneenvironment interactions of 5-HTTLPR and BDNF Val66Met and life events together with symptoms of maternal depression and birth characteristics on behavioral problems at age 3. Symptoms of postpartum depression were found to predict internalizing as well as externalizing problems in children three years later. Child experience of life events was a stable predictor of behavioral problems across the scales similar to sociodemographic factors such as parental immigration status and unemployment. No gene-environment interaction effects of 5-HTTLPR or BDNF Val66Met were shown. Study IV used the risk factors identified in studies I-III to investigate factors of resilience to behavioral problems at age 12. The l/l genotype of 5-HTTLPR was associated with a lower risk for behavioral problems at age 12, especially for children facing low adversity. Good social functioning was found to be a general resource factor, independent of the level of risk, while an easy temperament was associated with resilience for children with a high degree of adversity. However, effect sizes were small.In summary, the results from the present thesis emphasize the importance of maternal mental health and sociodemographic factors for child mental health at ages 3 and 12, which must be taken into account in clinical settings. Moreover, it adds to the null-findings of the gene-environment effect of 5-HTTLPR and BDNF Val66Met on behavioral problems in children, but indicates a main effect of 5-HTTLPR on internalizing symptoms at age 12.

  • Risk factors for subarachnoid haemorrhage : a nationwide cohort of 950 000 adults

    • BACKGROUND: Subarachnoid haemorrhage (SAH) is a devastating disease, with high mortality rate and substantial disability among survivors. Its causes are poorly understood. We aimed to investigate risk factors for SAH using a novel nationwide cohort consortium.METHODS: We obtained individual participant data of 949 683 persons (330 334 women) between 25 and 90 years old, with no history of SAH at baseline, from 21 population-based cohorts. Outcomes were obtained from the Swedish Patient and Causes of Death Registries.RESULTS: During 13 704 959 person-years of follow-up, 2659 cases of first-ever fatal or non-fatal SAH occurred, with an age-standardized incidence rate of 9.0 [95% confidence interval (CI) (7.4-10.6)/100 000 person-years] in men and 13.8 [(11.4-16.2)/100 000 person-years] in women. The incidence rate increased exponentially with higher age. In multivariable-adjusted Poisson models, marked sex interactions for current smoking and body mass index (BMI) were observed. Current smoking conferred a rate ratio (RR) of 2.24 (95% CI 1.95-2.57) in women and 1.62 (1.47-1.79) in men. One standard deviation higher BMI was associated with an RR of 0.86 (0.81-0.92) in women and 1.02 (0.96-1.08) in men. Higher blood pressure and lower education level were also associated with higher risk of SAH.CONCLUSIONS: The risk of SAH is 45% higher in women than in men, with substantial sex differences in risk factor strengths. In particular, a markedly stronger adverse effect of smoking in women may motivate targeted public health initiatives.

    • Det finns IT-system i verksamheter som inte är verksamhetsstödjande. Dessa ITsystem fungerar mer som hinder än stöd för personalen i utförande av arbetet. Det finns därmed behov av utveckling av IT-system så att de blir verksamhetsstödjande. Denna forsknings förslag är att denna utveckling bör ske med en designteori och metod. Designteorin och metoden har konstruerats genom en eklektisk och kumulativ kunskapsutveckling. Teoretiska komponenter från designteorierna handlingsbarhetsteori och aktivitetsteori har integrerats till en designteori för verksamhetsstödjande IT-system. Metodkomponenter från metoder för verksamhetsutveckling, systemutveckling och utvärdering baserade på handlingsbarhetsteori eller aktivitetsteori har integrerats till en metod för utveckling av verksamhetsstödjande IT-system. Designteorin och metoden har validerats empiriskt genom tillämpning i tre olika utvecklingsprojekt, vilka har studerats. Resultatet och kunskapsbidraget är ett förslag på en designteori och metod för design av verksamhetsstödjande IT-system, DEVIS. Designteorin beskriver och förklarar IT-system, verksamheter och deras relationer och innehåller designprinciper som föreskriver hur verksamhetsstödjande IT-system bör utvecklas. Metoden kan användas för både nyutveckling och vidareutveckling av IT-system och verksamheter och innehåller en arbetsprocess med ett antal olika tekniker och notationer för att studera och beskriva IT-system och verksamheter, analysera problem och förändringar, värdera verksamhetsstödjande och ge förslag på utveckling av IT-system och verksamhet.

    • Child abuse is a major public health problem which is largely hidden in the society.The main aim of the thesis was to study child maltreatment from children’s and young people’s perspectives focusing on prevalence, health, disclosure and support.The thesis is based on four studies. Quantitative and qualitative data from three national surveys of child maltreatment, including children in different age groups and young adults, have been analysed.The results showed that child maltreatment is prevalent and there is a large degree of overlap between maltreatment types. Neglect and witnessing intimate partner violence were the types that overlapped most with other types of maltreatment. Child maltreatment was strongly associated to psychosomatic symptoms and impaired quality of life. Young people who have been victims of child maltreatment described the psychological consequences, such as painful memories, low self-esteem and depression. The psychological abuse was described as particularly detrimental. Victims of sexual abuse expressed feelings of shame and guilt. About half of the severely abused children did not disclose the abuse, mainly because of lack of trust in adults. Many children felt that they did not receive adequate support when they disclosed the maltreatment, particularly because they experienced a lack of child perspective among professionals. Children and young people experienced positive support from the school health services which they considered to have a particularly important role in the detection and support of maltreated children. The results emphasize an increased awareness among professionals to recognise and respond to child maltreatment.

  • Indispensable interaction : parents' perspectives on parent-child interaction and beneficial meetings

    • Parent-child interaction interventions, guided by the aim of promoting child development, have developed in Sweden during the last three decades. The aim of this thesis was to describe families taking part in such interventions and examine short term and long term changes in their problem loads. Praticular interest was directed towards the parents' experiences, both with respect to the treatment they had taken part in at either of four centres for parent-child interaction interventions presented in this thesis and to other persons and/or contexts the parents considered had played an important and beneficial role for the child or the family. The results show that the centres have reached both mothers, fathers and children beset by considerable difficulties in relation to interaction, offering them a treatment which an overwhelming majority of the families have chosen to follow through and which has made a difference to the families. This thesis highlights the significance of beneficial relationships, not only within the intervention but also in other professional contexts, for the enhancing of children's development.

  • 'Stay Awhile and Listen' : Understanding the Dynamics of Mediatization, Authority, and Literacy in Swedish Religious Education

    • The aim of this article-based dissertation is to explore the role media play in Swedish Religious Education (RE). The purpose is to study Swedish RE teachers’ practices and how these can be related to various dynamics of mediatization, authority, and literacy. Conducted within the Teaching Religion in Late Modern Sweden project at Uppsala University, this study draws on a national survey, as well as semi-structured interviews and classroom observations with 22 RE teachers. Four specific research questions are posed, each addressed separately in four articles. (1) What kinds of media materials do Swedish RE teachers use in their teaching? (2) What kinds of media dynamics are present in RE classrooms where media materials are used? (3) How can RE teachers’ and students’ engagements with mediated religion be analysed from a multiple media literacies perspective? And, (4) how do RE teachers construct authority on religion in a mediatized situation? The first question is approached through a quantitative analysis of survey material, while the three remaining questions were analysed by the use of qualitative thematic analysis of the observations and interviews. The study finds that RE teachers in Sweden use and relate to various forms of media to a large extent in their daily teaching practices, and that these engagements with mediated religion illustrate the complex balance between the individual agency of the teacher on the one hand, and the media dynamics of amplification, framing and performative agency, and co-structuring, on the other. The teachers’ practices in relation to this are in the study viewed as a form of institutional work that is both caused by, and a contributing factor to, an increased mediatization of contemporary Swedish RE. The findings of the four articles are discussed in light of previous research on mediatization of religion in a Nordic context, specifically with regard to literacy and authority. The author argues that by studying these processes within an educational setting – a context that until recently has been under-explored within mediatization of religion research – the study provides new empirical as well as theoretical knowledge not only to the field of sociology of religion, but also to research on authority and religious education.

  • Monstret & människan : Paré, Deleuze och teratologiska traditioner i fransk filosofi, från renässanshumanism till posthumanism

    • This dissertation studies the problem of the inhuman in relation to human nature in philosophy from antiquity to the present, highlighting the interrelationship between science and philosophy in the development of concepts of monstrosity in France from mid-sixteenth century to late twentieth century thought. By means of constraint, it focuses on Ambroise Paré (1509/10–90) and Gilles Deleuze (1925–95) as representatives of early humanism and posthumanism, respectively. The study is divided into four chronologically ordered parts. In part I, four teratological traditions of philosophical import are discerned in antiquity: the naturalist, the humanist, the metaphysical, and the hermeneutical (each associated with a set of key names: in particular, Empedocles, Lucretius; Socrates, Protagoras; Plato, Aristotle; and Pliny, Augustine). Part II follows these traditions into the Renaissance where they intersect in the ‘books of wonder’, among which Paré’s Des monstres et prodiges (1573) is viewed to have had a lasting influence on the development of the science of teratology. Criticizing the positivistic conventions of interpretation of the book in question, notions of order, causality, diversity, and novelty are analyzed for the purpose of excavating from Paré’s work a natural philosophy which hinges on man’s capacity for knowledge; in such a humanist conception, monsters are not so much naturalized as nature becomes monstrous, while man is taken to reflect and encompass all the properties of natural things, thereby incorporating monstrosity in his singular variability. Part III provides an overview of the development of a scientific teratology from Cartesian mechanicism and rationalism, through theories of preformation, epigenesis, and transformation, to the materialist and vitalist debates of the early nineteenth century, when Étienne and Isidore Geoffroy Saint-Hilaire create the discipline of teratology, and its aftermath in developmental and evolutionary biology. The general theme is the place of anomalies in the normal scheme of nature (and culture), as man becomes progressively taken as the norm for thought, ultimately rendering the inhuman as such unthought. Finally, part IV looks to Deleuze as an attempt in the late 1900s to construct a posthumanist philosophy of nature where monstrosity is the problem which rather generates thought; it thus chronologically traces formulations of a concept of monstrosity in his body of work, from the 1940s to the 1990s. In Différence et répétition (1968), Deleuze is found to furnish three interconnected theses to define monstrosity, regarding problems of determination, synthesis, and differentiation, where the problematic as such (the nature of difference itself) is conceptualized as the ‘idea’ of monstrosity, not any particular physical shape. After analyzing the concept of the ‘body without organs’ as an issue of identity and materiality, tracing it back to its formulation in Logique du sens (1969), these theses of monstrosity are then applied to a study of Deleuze’s later philosophy, emphasizing Mille plateaux (1980), Logique de la sensation (1981), and Cinéma 1–2 (1983–85), as side-stepping the human norm in order to think its anomaly (the inhuman) as the condition for creativity. This is evidenced in his ideas of technology and the arts as experimental practices of becoming inhuman. The monster is thus regarded as a ‘conceptual persona’ in a Deleuzian philosophy of the virtual Figure—challenging all actual forms—of an inhuman time for the experience of difference in itself.

  • Mindfulness : Relations to attention regulation, decentering, and psychological well-being

    • The current research project consists of three separate studies. The general aim of this project was to contribute to previous mindfulness research by exploring fundamental aspects of mindfulness in an effort to increase the understanding of mindfulness as a construct as well as its mechanisms. The purpose of the study I was to investigate the relation between mindfulness and sustained and executive attention by comparing Buddhist and Western mindfulness meditators (n = 47) and non-meditators (n = 45) in performance on computerized attention. The main purpose of study II was to compare these meditators and non-meditators on self-reported mindfulness, and also to investigate whether facets of mindfulness mediate the relation between meditation experience and psychological well-being. Study III aimed at investigating the unique effects of mindfulness practice as well as the proposed mindfulness mechanism; decentering. A short-term mindfulness-based intervention (MBI) (n = 46) was compared with relaxation training (n = 40) and a waiting-list group (n = 40) on a battery of tests - executive attention, self-reported mindfulness, decentering, psychological well-being, anxiety, depression, and coping styles – in 126 employees with no prior meditation experience. The results showed no significant differences between meditators and non-meditators either in sustained or executive attention. Meditators rated themselves higher than non-meditators on four of the five facets of mindfulness. The multiple mediation analysis showed that the five mindfulness facets mediated the relationship between meditation experience and psychological well-being but no single facet contributed significantly. Simple mediation analyses indicated, however, that Non-React was the primary mediator. No unique mindfulness effects were found since there were no differences between mindfulness and relaxation in any of the variables. However, the mindfulness group scored higher than the waiting-list group on the Five Facet Mindfulness Questionnaire total scale and psychological wellbeing. Meditators may have an increased awareness of internal processes and the ability to quickly attend to them, but this type of refined attentional ability does not seem to be related to performance on attention tests requiring quick responses to external targets. It may be concluded that effects on attention regulation are of less importance compared to other beneficial psychological and physiological health outcomes due to mindfulness meditation. Mediation analyses supported (i) the notion that meditation experience is related to increased mindfulness, which in turn is associated with improved psychological well-being, and (ii) the idea that increases in mindfulness lead to increased decentering abilities which in turn leads to improved psychological well-being. Possible explanations for the absence of unique group differences between mindfulness and relaxation are that the length of the intervention was too short and the sessions too few, similarities between body exercises in MBI and relaxation, and the lack of group differences on decentering. Investigating unique mindfulness effects to distinguish mindfulness effects from relaxation should be prioritized in future studies. The promising theory of mechanisms proposed in the Buddhist Psychological Model (BPM) needs to be empirically evaluated. MBI-related changes in selfperceptions, value systems, and ethical aspects may play a more important role for improved psychological health than what has previously been recognized. Other Buddhist practices such as loving-kindness meditation and compassion meditation also need to be examined. Finally, an in-depth dialogue between Western researchers, expert meditators, and Buddhist theoreticians may be increasingly important for mindfulness research to advance.

  • An Unequal Chance to Parent : Examples on Support Fathers Receive from the Swedish Child Health Field

    • Father involvement benefits children, mothers, and themselves in a number of ways. Swedish legislation and Swedish society have promoted father involvement. At the same time, the Swedish child health field has also unequivocally states that both parents should feel welcomed and supported within that sphere. Despite these statements and policies, fathers feel neglected and invisible within and throughout the Swedish child health field, which includes prenatal clinics, birth and labor wards, postnatal clinics, child health centers, and parent support programs. Less is known however about the factors that influence father involvement in the child health centers, especially from the child health nurses’ perspective and the influence of the built environment. Additionally, parent support programs are another way through which parents receive support regarding their young child’s mental health, but very little research has focused on why fathers participate or the thoughts parents have regarding their participation, especially within a Swedish context.The overall aim of this dissertation was to better understand some of the barriers fathers have when trying to participate in the female-dominated world of the Swedish child health field, especially during the child’s preschool years. In Study I, 17 child health nurses were interviewed regarding their thoughts on fathers, and in Study II, 31 child health centers’ built environments were assessed to see how inclusive they were of fathers. In Study III, a parent support program was assessed to see if mothers and fathers had different background characteristics for participating, and Study IV sought to understand the extent to which parents appreciated and used the information from the program.These studies showed that child health nurses welcomed fathers, but did not actively invite them to participate. In addition, 75% of the child health centers did not have representations of fathers, but most child health centers had representations of mothers and/or children. Paternal behaviors positively changed if they were in an environment with either explicit paternal representations or only child representations. Mothers participated in the parent support program for several reasons, including if their child had perceived behavior problems, while fathers participated if they were stressed and perceived their child as having emotional problems. Parents believed the information they learned in the parent support program was valuable, and they continued using some strategies a year after the intervention.Swedish family policies can affect parental involvement within the child health field, but the child health field is less inclusive of fathers than mothers, and it fails to meet the needs of fathers, which can then, in turn, negatively affect maternal, paternal, and child outcomes. Therefore, the Swedish child health field needs to continue working on improving their practices of treating both parents equally.
