    • City–Region Food Systems (CRFS) is a cutting-edge concept and an emerging field of research. As a new analytical lens, it offers an integrated and multi-dimensional perspective on food’s origins, how it is grown and the path it follows to our plates and beyond. Building on this concept, this presentation reflects a prospective research project which seeks to explore opportunities for innovative and sustainable food systems in the Gothenburg region of Sweden by focusing on how rural and urban regions, food production and market can be integrated to promote regional food security. The project intends to: 1) develop scenarios with stakeholders for local food production in the region; 2) analyze the consequences of the scenarios on landscape change and biodiversity; 3) explore socio-economic consequences for producers and local communities; and 4) evaluate the sustainability and feasibility of scenarios with stakeholders. Five municipalities in Western Sweden (Gothenburg, Kungälv, Lerum, Alingsås and Essunga) will serve as study areas for the project, selected to reflect different kinds of potential for local food production in terms of dissimilar environmental conditions, prerequisites for farming and economic histories. The project responds to expressed interests and knowledge needs in the region and will be developed and implemented in direct cooperation with local and regional actors such as Västarvet, the Västra Götaland Region, the municipalities and various producer organizations. In sum, there are premises suggesting that recent urban food strategies and plans with sustainability ambitions are embracing several Sustainable Development Goals in the environmental, social, economic, and equity dimensions. This, in turn, is a characteristic of the Transition Movements pathway, in which the utility of food strategies in the work with sustainability transitions seems inevitable. The results are therefore likely to be transferable to other regions.

  • Diagnostic Value of Cerebrospinal Fluid Neurofilament Light Protein in Neurology : A Systematic Review and Meta-analysis

    • Importance  Neurofilament light protein (NfL) is elevated in cerebrospinal fluid (CSF) of a number of neurological conditions compared with healthy controls (HC) and is a candidate biomarker for neuroaxonal damage. The influence of age and sex is largely unknown, and levels across neurological disorders have not been compared systematically to date.Objectives  To assess the associations of age, sex, and diagnosis with NfL in CSF (cNfL) and to evaluate its potential in discriminating clinically similar conditions.Data Sources  PubMed was searched for studies published between January 1, 2006, and January 1, 2016, reporting cNfL levels (using the search terms neurofilament light and cerebrospinal fluid) in neurological or psychiatric conditions and/or in HC.Study Selection  Studies reporting NfL levels measured in lumbar CSF using a commercially available immunoassay, as well as age and sex.Data Extraction and Synthesis  Individual-level data were requested from study authors. Generalized linear mixed-effects models were used to estimate the fixed effects of age, sex, and diagnosis on log-transformed NfL levels, with cohort of origin modeled as a random intercept.Main Outcome and Measure  The cNfL levels adjusted for age and sex across diagnoses.Results  Data were collected for 10 059 individuals (mean [SD] age, 59.7 [18.8] years; 54.1% female). Thirty-five diagnoses were identified, including inflammatory diseases of the central nervous system (n = 2795), dementias and predementia stages (n = 4284), parkinsonian disorders (n = 984), and HC (n = 1332). The cNfL was elevated compared with HC in a majority of neurological conditions studied. Highest levels were observed in cognitively impaired HIV-positive individuals (iHIV), amyotrophic lateral sclerosis, frontotemporal dementia (FTD), and Huntington disease. In 33.3% of diagnoses, including HC, multiple sclerosis, Alzheimer disease (AD), and Parkinson disease (PD), cNfL was higher in men than women. The cNfL increased with age in HC and a majority of neurological conditions, although the association was strongest in HC. The cNfL overlapped in most clinically similar diagnoses except for FTD and iHIV, which segregated from other dementias, and PD, which segregated from atypical parkinsonian syndromes.Conclusions and Relevance  These data support the use of cNfL as a biomarker of neuroaxonal damage and indicate that age-specific and sex-specific (and in some cases disease-specific) reference values may be needed. The cNfL has potential to assist the differentiation of FTD from AD and PD from atypical parkinsonian syndromes.

  • Salivary cortisol and cortisone in diagnosis of Cushing's syndrome - a comparison of six different analytical methods

    • Objectives: Salivary cortisol and cortisone at late night and after dexamethasone suppression test (DST) are increasingly used for screening of Cushing's syndrome (CS). We aimed to establish reference intervals for salivary cortisol and cortisone with three liquid chromatography-tandem mass spectrometry (LC-MS/MS) techniques and for salivary cortisol with three immunoassays (IAs), and evaluate their diagnostic accuracy for CS.Methods: Salivary samples at 08:00 h, 23:00 h and 08:00 h after a 1-mg DST were collected from a reference population (n=155) and patients with CS (n=22). Sample aliquots were analyzed by three LC-MS/MS and three IA methods. After establishing reference intervals, the upper reference limit (URL) for each method was used to calculate sensitivity and specificity for CS. Diagnostic accuracy was evaluated by comparing ROC curves.Results: URLs for salivary cortisol at 23:00 h were similar for the LC-MS/MS methods (3.4-3.9 nmol/L), but varied between IAs: Roche (5.8 nmol/L), Salimetrics (4.3 nmol/L), Cisbio (21.6 nmol/L). Corresponding URLs after DST were 0.7-1.0, and 2.4, 4.0 and 5.4 nmol/L, respectively. Salivary cortisone URLs were 13.5-16.6 nmol/L at 23:00 h and 3.0-3.5 nmol/L at 08:00 h after DST. All methods had ROC AUCs =0.96.Conclusions: We present robust reference intervals for salivary cortisol and cortisone at 08:00 h, 23:00 h and 08:00 h after DST for several clinically used methods. The similarities between LC-MS/MS methods allows for direct comparison of absolute values. Diagnostic accuracy for CS was high for all salivary cortisol and cortisone LC-MS/MS methods and salivary cortisol IAs evaluated.

    • BACKGROUND: Laboratory tests to assist in the diagnosis and monitoring of neuroinflammatory diseases are scarce. The soluble form of the CD27 molecule (sCD27) is shed in high concentrations by activated T cells and can be detected in the cerebrospinal fluid. The aim of this study was to investigate whether CSF quantitation of sCD27 could discriminate between inflammatory and non-inflammatory neurological diseases.METHODS: The concentration of sCD27 was measured using a commercially available ELISA in 803 well-defined subjects from a study cohort comprised of 338 patients with neuroinflammatory disease, 338 with non-inflammatory neurological disease and 127 controls without neurological disease.RESULTS: The median value of cerebrospinal fluid sCD27 was 64 pg/mL (IQR 0-200) in controls, 58 pg/mL (IQR 0-130) in patients with non-inflammatory disease and 740 pg/mL (IQR 230-1800) in patients with inflammatory disease. The likelihood ratio of having an inflammatory disease was 10 (sensitivity 74% and specificity 93%) if the sCD27 concentration was >250 pg/mL. In patients with a known inflammatory condition, the likelihood ratio of having an infection was 10 (sensitivity 40% and specificity 96%) if the sCD27 concentration was >2500 pg/mL.CONCLUSIONS: The likelihood of having an inflammatory neurological condition is increased with elevated concentrations of sCD27 in cerebrospinal fluid. Rapid tests of sCD27 should be developed to assist clinicians in diagnosis of neuroinflammatory disease.

    • Since Georg Lukács and Karl Korsch in the 1920s, Hegelian Marxism has played a prominent role as a radical intellectual tradition in modern political theory. This anthology investigates how these Hegelian Marxists, in different historical, political and intellectual contexts during the last century, have employed Hegel’s philosophy with the aim of developing and renewing Marxist theory.Besides Lukács and Korsch the volume includes articles dealing with the thoughts of Herbert Marcuse, Theodor W. Adorno, Walter Benjamin, Evald Ilyenkov, Lucio Colletti and Slavoj Žižek. The overall purpose is to investigate if, and the degree to which, these thinkers could be interpreted as Hegelian Marxists, and how they use the Hegelian philosophy to better understand their own current society as well as situate themselves in relation to orthodox forms of Marxism. Taken together, the articles can hopefully contribute to an intensification of discussions about the critical and self-criticalphilosophy of Marxism today.

    • Som ett av samtidshistoriens mest legendariska år förknippas 1968 med ungdomsuppror, studentrevolter och strejker. Men hur påverkade händelserna kring 68 pedagogiken och utbildningen? Det är den frågan som står i centrum i denna antologi där titelns båda huvudbegrepp förstås i vid mening.1968 står således inte bara för året då studenterna vid Stockholms universitet ockuperade sitt eget kårhus och miljontals människor demonstrerade på gatorna i Paris och många andra storstäder runt om i världen utan också mycket annat som hände vid den här tiden och som kan kopplas till sextio- och sjuttiotalets bredare vänsterradikalisering. Och förutom själva pedagogikämnet handlar det om skolsystemet, universiteten och folkbildningen liksom den vidare diskussionen om hur ett gott samhälle kan skapas med hjälp av skola och utbildning.Samtidigt aktualiseras frågan om vad vi idag, drygt femtio år senare, har att lära av denna tid då man fortfarande var övertygad om att en annan värld – och därmed en annan skola och pedagogik – var möjlig och då det ansågs vara realistiskt att kräva det omöjliga.

  • GH Safety Workshop Position Paper: a critical appraisal of recombinant human growth hormone therapy in children and adults.

    • Recombinant human growth hormone (rhGH) has been in use for 30 years, and over that time its safety and efficacy in children and adults has been subject to considerable scrutiny. In 2001, a statement from the GH Research Society (GRS) concluded that 'for approved indications, GH is safe'; however, the statement highlighted a number of areas for on-going surveillance of long-term safety including; cancer risk, impact on glucose homeostasis and use of high dose pharmacological rhGH treatment. Over the intervening years, there have been a number of publications addressing the safety of rhGH with regard to mortality, cancer and cardiovascular risk and the need for longterm surveillance of the increasing number of adults who were treated with rhGH in childhood. Against this backdrop of interest in safety, the European Society of Paediatric Endocrinology (ESPE), the GRS and the Pediatric Endocrine Society (PES) convened a meeting to reappraise the safety of rhGH. The ouput of the meeting is a concise position statement.
