  • Prevalence of severe-to-Profound hearing loss in the adult Swedish population and comparison with cochlear implantation rate

    • Background: The prevalence of disabling hearing loss is increasing worldwide. However, previous studies on hearing loss prevalence have enrolled small populations or only provided estimates.Aim: To establish the prevalence of severe-to-profound hearing loss (STPHL) in the adult Swedish population and compare it with the cochlear implantation rate in Sweden.Material and methods: We established a database containing over 15 million audiograms obtained from regions covering > 99% of the Swedish population by extracting audiogram data from the computer software application, Auditbase. We used this database to calculate the percentage of adult patients with bilateral hearing levels >= 70 dB. We collected data regarding cochlear implantations in Sweden from the National Board of Welfare and Health.Results: The prevalence of STPHL in the adult Swedish population was 0.28%. There were regional variations in the prevalence and rate of cochlear implantation; however, there was no association between both parameters.Conclusions: This study presents an updated and reliable prevalence figure for STPHL in Sweden.Significance: Patients with STPHL have extensive rehabilitation requirements; accordingly, it is important to determine the accurate prevalence of STPHL to inform the allocation of adequate resources.

    • The dysgenetic lens (dyl) mouse mutant has mutations in Foxe3, which inactivate DNA binding by the encoded forkhead transcription factor. Here we confirm, by targeted inactivation, that Foxe3 mutations are responsible for the dyl phenotype, which include loss of lens epithelium; a small, cataractic lens; and failure of the lens to detach from the surface ectoderm. In contrast to a recent report of targeted Foxe3, we found no phenotypic difference between dyl and Foxe3(-/-) mutants when congenic strains were compared, and thus nothing that argues against Foxe3(dyl) being a null allele. In addition to the lens, most tissues of the anterior segment-iris, cornea, ciliary body and trabecular meshwork-are malformed or show differentiation defects. Many of these abnormalities, such as irido-corneal and irido-lenticular adherences, are present in a less severe form in mice heterozygous for the Foxe3 mutation, in spite of these having an intact lens epithelium. Early Foxe3 expression is highly sensitive to a halved Pax6 gene dosage and there is a striking phenotypic similarity between Pax6 and Foxe3 mutants. We therefore propose that many of the ocular malformations associated with Pax6 haploinsufficiency are consequences of a reduced expression of Foxe3.

  • Leading by example? EU citizens’ preferences for climate leadership

    • For global problems like climate change, strong international agreements are difficult to achieve. Alternative solutions might therefore be necessary. In this paper, we study the support for climate leadership in seven European countries. Climate leadership means that an individual country takes the lead by decreasing its carbon emissions above its level of commitment in the current EU agreement and with the intention of inspiring other countries to do likewise. Overall, we find that at realistic cost levels, a majority of people oppose their country taking the lead, and most do not expect that taking the lead will result in other countries following suit. The lack of support is caused by expectations that such leadership will result in other countries behaving as free riders. We do, however, find evidence of preferences for conditional leadership: People are more positive about their country taking the lead if assured that other countries will follow. These preferences are stronger among those who identify as leftwing. Moreover, citizens in smaller countries are more pessimistic that other countries would follow their country’s lead and more sensitive to the response of other EU countries.

    • We examine how attitudes and willingness to pay (WTP) for climate policies have changed over the past decade in the United States, China, and Sweden. All three countries exhibit an increased willingness to pay for climate mitigation. Ten years ago, Sweden had a larger fraction of believers in anthropogenic climate change and a higher WTP for mitigation, but today the national averages are more similar. Although we find convergence in public support for climate policy across countries, there is considerable divergence in climate attitudes and preferences within countries, particularly the United States. Political polarization explains part of this divergence.

    • Susceptibility differences are common causes for artifacts in magnetic resonance (MR); therefore, it is important to choose phantom materials in a way that these artifacts are kept at a minimum. In this study, a previously proposed MR imaging (MRI) method [Beuf O, Briguet A, Lissac M, Davis R. Magnetic resonance imaging for the determination of magnetic susceptibility of materials. J Magn Reson 1996; Series B(112):111-118] was improved to facilitate sensitive in-house measurements of different phantom materials so that such artifacts can more easily be minimized. Using standard MRI protocols and distilled water as reference, we measured magnetic volume susceptibility differences with a clinical MR system. Two imaging techniques, echo planar imaging (EPI) and spin echo, were compared using liquid samples whose susceptibilities were verified by MR spectroscopy. The EPI sequence has a very narrow bandwidth in the phase-encoding direction, which gives an increased sensitivity to magnetic field inhomogeneities. All MRI measurements were evaluated in two ways: (1) manual image analysis and (2) model fitting. The narrow bandwidth of the EPI made it possible to detect very small susceptibility differences (equivalent susceptibility difference, Deltachi(e)> or =0.02 ppm), and even plastics could be measured. Model fitting yielded high accuracy and high sensitivity and was less sensitive to other image artifacts as compared with manual image analysis.

    • High quality, artifact free, ¹H MRS require a homogeneous magnetic field over the volume of interest (VOI) and an effective global water suppression (WS). However, the shim coils act globally and, hence, affect the resonance frequency over the entire object, making the frequency selective WS spatially selective. Unsuppressed water signal may cause spurious echo artifacts. Aim: Study and visualise the effect localised shimming has on WS.
