  • Methodologic aspects of computed microtomography to monitor the development of osteoporosis in gastrectomized rats

    • Rationale and Objectives We investigated the methodologic development of computed microtomography (CMT) for monitoring the development of osteoporosis in male Sprague-Dawley rats. Methods Eight rats were gastrectomized and eight rats were sham operated. Femurs, tibias, and tails were prepared, and CMT scans with spatial resolutions of 5–500 μm were made. Bone diameters, bone areas, and moments of inertia were determined from the CMT scans. Optimal slice position and the need for spatial resolution and energy optimization for future in vivo applications were investigated. Results Gastrectomy caused dramatic changes in the bone architecture of the tibia and the femur. The main features were vacuolization of the bone and reduced amounts of compact bone. Although the outer diameters of tubular bones (femur and tibia) were largely unaffected, their inner diameters were greatly increased following gastrectomy. Relative bone area and moment of inertia were greatly reduced. The optimal photon energy was 12 keV. Conclusion It is possible to monitor gastrectomy-evoked changes in bone morphology at various sites in rats using CMT scanning. The changes are suggestive of osteoporosis. By optimizing the energy spectrum and spatial resolution, as well as choosing the proper slice position, it should be possible to keep absorbed doses low enough to avoid acute radiation injury in repeated in vivo measurements.

  • A Risk for Hepatocellular Carcinoma Persists Long-term After Sustained Virologic Response in Patients With Hepatitis C-Associated Liver Cirrhosis

    • Background. The long-term effect of sustained virologic response (SVR) to antiviral therapy on the risk of developing hepatocellular carcinoma (HCC), liver complications, liver-related death, and overall death in hepatitis C virus (HCV)-infected patients with liver cirrhosis is not fully known. Methods. These risks were evaluated during long-term follow-up in 351 patients with HCV-related cirrhosis. One hundred ten patients with SVR, 193 with non-SVR, and 48 who were untreated were included in a multicenter cohort that was initiated in 2001 and prospectively followed up for a mean of 5.3 (SD, 2.8) years. Complementary follow-up data from national registries were used to minimize the loss of patients during follow-up. Results. Six patients with SVR developed HCC at 0.04, 0.64, 2.4, 7.4, 7.4, and 7.6 years, respectively, after achieving SVR. The incidences of HCC, any liver complication, liver-related death, and overall death per 100 person-years were significantly lower in SVR time with 1.0, 0.9, 0.7, and 1.9, compared to 2.3, 3.2, 3.0, and 4.1 in non-SVR and 4.0, 4.9, 4.5, and 5.1 in untreated time. The long-term consequences did not decline significantly after >3 years versus during the first 3 years of follow-up. Conclusions. The risk for HCC, liver decompensation, and death in patients with liver cirrhosis related to HCV was markedly reduced after SVR, but a long-term risk of developing HCC remains for up to 8 years. Cirrhotic patients with HCV who achieve SVR should therefore maintain long-term surveillance for HCC. Future studies aimed to better identify those with remaining long-term risk for HCC are needed.

  • Inventering av violgubbe (Gomphus clavatus) och rödlistade fjälltaggsvampar (Sarcodon spp.) i Västmanlands län 2008.

    • Denna inventering av violgubbe (Gomphus clavatus) och rödlistade fjälltaggsvampar (Sarcodon spp.) har utförts på uppdrag av Länsstyrelsen i Västmanlands län. Syftet har varit att öka kunskapen om arternas förekomst i länet samt behovet av skydd och andra åtgärder på de olika lokalerna. Inventeringen har genomförts inom ramen för åtgärdsprogrammen (ÅGP) för bevarande av violgubbe och rödlistade fjälltaggsvampar. Inventeringen har genomförts under september 2008 av Leif Andersson, Kurt- Anders Johansson, Rolf-Göran Carlsson, Stig Jacobsson och Tomas Fasth. Alla kända lokaler för violgubbe och rödlistade fjälltaggsvampar besöktes. Även lokaler för arter med likartade miljökrav har besökts och områden kända för kalkrik berggrund. Under inventeringen har mycel för de eftersökta arterna noterats och koordinater för dessa tagits. Även andra svampar har noterats om än i olika omfattning. Totalt har ett hundratal lokaler besökts varav svampar har noterats på 80 lokaler. Säsongen var mycket god för fjälltaggsvampar, möjligen med undantag för brödtaggsvamp (Sarcodon versipellis). För violgubbe var säsongen något sämre. Totalt noterades violgubbe och/eller rödlistade fjälltaggsvampar på 17 lokaler. Av de 19 kända lokalerna för violgubbe noterades den på sex och en ny lokal upptäcktes. Tidigare var 20 lokaler kända för de olika rödlistade fjälltaggsvamparna varav återfynd gjordes på sex av dessa. Dessutom gjordes 14 nya fynd av rödlistade fjälltaggvampar. Nya för Västmanlands län är blåfotad taggsvamp (Sarcodon glaucopus), sammetstaggsvamp (Sarcodon martioflavus) och den ännu obeskrivna Sarcodon ”modestum”. Den goda svamptillgången 2008 gjorde att särskilt många rödlistade spindelskivlingar noterades. Flera arter är nya för Västmanlands län. Även andra nyfynd av rödlistade svampar för Västmanlands län gjordes t. ex. narrvaxskivlingen (Hygrophorus subviscifer) som noterades på två platser. Totalt gjordes 146 fynd av 42 rödlistade svampar inklusive violgubbe och rödlistade fjälltaggsvampar. Ett antal lokaler och områden har kristalliserats ut som särskilt intressanta för de eftersökta svamparna. I Sala kommun finns många värdefulla områden, särskilt nära tätorten, ofta i närheten av där gruvdrift bedrivits. Särskilt Snarpsätraskogen norr om staden samt områden söder och sydväst om staden hyste många hotade arter. Områdena söder om staden ingår i Salakalkens Natura 2000-område, men skogsnaturtyperna är i dagsläget inte anmälda som naturtyp. I Fagersta och Norberg finns också värdefulla stråk för violgubbe och rödlistade fjälltaggsvampar. Särskilt ska här framhållas Jättåsarna i Fagersta kommun. Flera områdena är inte utpekade som naturvärden eller nyckelbiotoper och saknar också formellt områdesskydd. Ett antal lokaler för de eftersökta svamparna finns i naturreservat och biotopskydd.

    • BACKGROUND: Treatment with local glucocorticoids (GCs) decreases symptoms and the size of nasal polyps. This might depend on the downregulation of proinflammatory genes, as well as the upregulation of anti-inflammatory genes. OBJECTIVE: We sought to identify GC-regulated anti-inflammatory genes in nasal polyps. METHODS: Affymetrix DNA microarrays were used to analyze the expression of 22,283 genes in 4 nasal polyps before and after local treatment with fluticasone (400 microg/d). Expression of uteroglobin and mammaglobin B was analyzed with real-time PCR in 6 nasal polyps and in nasal biopsy specimens from 6 healthy control subjects. RESULTS: Two hundred three genes had changed in expression in treated polyps, and 139 had known functions: 54 genes were downregulated, and 85 were upregulated. Genes associated with inflammation constituted the largest single functional group. These genes affected key steps in inflammation (eg, immunoglobulin production; antigen processing and presentation; and the chemoattraction and activation of granulocytes, T cells, and B cells). Several proinflammatory genes were downregulated. In contrast, some anti-inflammatory genes were upregulated. The gene that increased most in terms of expression was uteroglobin. This was confirmed with real-time PCR. By contrast, expression of uteroglobin was lower in untreated polyps than in healthy nasal mucosa. Immunohistochemical investigation showed staining of uteroglobin in the epithelium and in seromucous glands in control subjects and in nasal polyps. CONCLUSION: Upregulation of anti-inflammatory genes, such as uteroglobin, might contribute to the effects of local treatment with GCs in nasal polyps.

  • Periodontal disease in patients from the original Kostmann family with severe congenital neutropenia

    • Patients with Kostmann syndrome (severe congenital neutropenia [SCN]) typically normalize their absolute neutrophil count (ANC) upon granulocyte colony-stimulating factor (G-CSF) therapy. However, although they no longer experience life-threatening bacterial infections, they frequently still have recurrent gingivitis and even severe periodontitis, often starting in early childhood. METHODS: We studied the periodontal disease in the four surviving patients belonging to the family originally described by Kostmann. Their odontological records, x-rays, color photos, bacterial cultures, serum antibodies to oral bacteria, and histopathological examinations were reviewed. The data were also correlated to previous investigations on their antibacterial peptides and molecular biology. RESULTS: Three patients had periodontal disease, despite normal ANC and professional dental care, and had neutrophils deficient in antibacterial peptides. One of these patients also had a heterozygous mutation in the neutrophil elastase gene, had severe periodontal disease and overgrowth of the periodontal pathogen Actinobacillus actinomycetemcomitans in the dental flora, and 15 permanent teeth had been extracted by the age of 27. One bone marrow-transplanted patient had no periodontal disease. CONCLUSIONS: Normalized ANC levels are not sufficient to maintain normal oral health in SCN patients, and because neutrophils are important for first-line defense and innate immunity, the deficiency of the antibacterial peptide LL-37 probably explains their chronic periodontal disease. Professional dental care is still important for SCN patients, despite treatment with G-CSF and normal ANC levels. Whether antibacterial peptides play a role in the pathogenesis of periodontitis in other patients remains to be elucidated.

    • The lethal and mutagenic effects of hydrogen peroxide were studied in exponentially growing cultures of Salmonella typhimurium strain TA102. Exposure of the cultures to non-lethal levels of sodium sulfide significantly increased the lethality and mutagenicity of hydrogen peroxide. The catalase activity was decreased in cells exposed to sodium sulfide, but there were no changes in the cellular levels of superoxide dismutase, glutathione reductase, or NADPH-dependent alkyl hydroperoxide reductase. Hydrogen peroxide-induced mutagenesis and killing of S. typhimurium strain TA102 in the presence of sulfide may in part be explained by an inactivation of catalase by sulfide.
