  • Global Impact of the COVID-19 Pandemic on Stroke Volumes and Cerebrovascular Events: A 1-Year Follow-up.

    • Declines in stroke admission, IV thrombolysis (IVT), and mechanical thrombectomy volumes were reported during the first wave of the COVID-19 pandemic. There is a paucity of data on the longer-term effect of the pandemic on stroke volumes over the course of a year and through the second wave of the pandemic. We sought to measure the effect of the COVID-19 pandemic on the volumes of stroke admissions, intracranial hemorrhage (ICH), IVT, and mechanical thrombectomy over a 1-year period at the onset of the pandemic (March 1, 2020, to February 28, 2021) compared with the immediately preceding year (March 1, 2019, to February 29, 2020).We conducted a longitudinal retrospective study across 6 continents, 56 countries, and 275 stroke centers. We collected volume data for COVID-19 admissions and 4 stroke metrics: ischemic stroke admissions, ICH admissions, IVT treatments, and mechanical thrombectomy procedures. Diagnoses were identified by their ICD-10 codes or classifications in stroke databases.There were 148,895 stroke admissions in the 1 year immediately before compared with 138,453 admissions during the 1-year pandemic, representing a 7% decline (95% CI [95% CI 7.1-6.9]; p < 0.0001). ICH volumes declined from 29,585 to 28,156 (4.8% [5.1-4.6]; p < 0.0001) and IVT volume from 24,584 to 23,077 (6.1% [6.4-5.8]; p < 0.0001). Larger declines were observed at high-volume compared with low-volume centers (all p < 0.0001). There was no significant change in mechanical thrombectomy volumes (0.7% [0.6-0.9]; p = 0.49). Stroke was diagnosed in 1.3% [1.31-1.38] of 406,792 COVID-19 hospitalizations. SARS-CoV-2 infection was present in 2.9% ([2.82-2.97], 5,656/195,539) of all stroke hospitalizations.There was a global decline and shift to lower-volume centers of stroke admission volumes, ICH volumes, and IVT volumes during the 1st year of the COVID-19 pandemic compared with the prior year. Mechanical thrombectomy volumes were preserved. These results suggest preservation in the stroke care of higher severity of disease through the first pandemic year.This study is registered under NCT04934020.

  • Understanding herdsmen's rangeland rent-in behaviour under current rural land tenure system of China

    • Current rural land tenure system in China, characterized by the so-called “separation of three rights”, i.e., the division and parallel of collective ownership, household contract right and transferable management right, provides a solid legal basis for rangeland transfer, which is deemed crucial for addressing a range of sustainability challenges. Drawn on the theory of planned behaviour and applying structural equation modelling to analyse a survey dataset, we investigate the effects of cognitive antecedents, and their possible interactions, on herdsmen's intention and behaviour in regards to rangeland rent-in. Our results show that the most important antecedent is subjective attitude, mainly influenced by herders' economic rationality regarding scale operation and expected benefit. This is followed by perceived behavioural control, in turn largely affected by herders' familiarity with policies and access to transaction information. Finally, subjective norms influenced by the opinions of relatives and friends are weighted stronger than those of authorities. Positive correlations are also found between the three antecedents. The findings are improving our understanding of factors influencing herders' heterogeneous responses in regards to rangeland rental, and can help inform policy interventions to harness individuals' psychology in the development of more prosperous rangeland transfer systems in pastoral regions of China.

    • The new neutron spectrometer time-of-flight enhanced diagnostics (TOFED) for the EAST tokamak is presented and its characteristics are described in terms of simulation results, as well as the interface in the torus hall along with new neutral beam (NB) injectors. The use of TOFED for studies of the slowing down of NB-injected deuterons is illustrated. The implications of measuring the neutron emission on a long pulse machine are discussed together with the experimental challenges and diagnostic possibilities approaching those to be encountered in continuous operation.

  • Practical Attacks on Relational Databases Protected via Searchable Encryption

    • Searchable symmetric encryption (SSE) schemes are commonly proposed to enable search in a protected unstructured documents such as email archives or any set of sensitive text files. However, some SSE schemes have been recently proposed in order to protect relational databases. Most of the previous attacks on SSE schemes have only targeted its common use case, protecting unstructured data. In this work, we propose a new inference attack on relational databases protected via SSE schemes. Our inference attack enables a passive adversary with only basic knowledge about the meta-data information of the target relational database to recover the attribute names of some observed queries. This violates query privacy since the attribute name of a query is secret.

  • Cognitive antecedents and formation pathways of confined feeding mode adoption by herders in China

    • Along with the implementation of the Rangeland Ecological Subsidy and Award Scheme, a modal shift of livestock production from open grazing to confined feeding is now encouraged in China's pastoral regions, in the hope of relieving grazing pressure meanwhile sustaining herders' livelihood. What cognitive factors and how they affect the adoption of confined-feeding mode by herders are yet unanswered. Drawing on the theory of planned behaviour and using a field survey dataset of 250 herder households in Qinghai Province, this study explores the critical cognitive antecedents and formation mechanisms of herders' behaviour towards the new mode. The results of structural equation modelling suggest that the confined-feeding adoption is a reasoned action, whereby both perceived behavioural control and subjective attitude exert significantly positive impacts. For the former, economic rationality is the main driving force, whereas for the latter, the acquisition of financial and technical support is the key. Two pathways, namely ‘attitude → intention → behaviour’ and ‘perceived control → intention → behaviour’, stand out in forming the adoption behaviour. Although subjective norms show no significant impact on intension, they serve as contextual factors that help shape perceived control and attitude. The findings contribute to improve understanding on the behavioural responses of herders to novel technologies, practices and policy interventions, and have important implications for informing the design of intervention initiatives in pastoral regions of China and beyond.

  • Prostate Specific Antigen and Prostate Cancer in Chinese Men Undergoing Initial Prostate Biopsies Compared with Western Cohorts

    • Purpose We determined the characteristics of Chinese men undergoing initial prostate biopsy and evaluated the relationship between prostate specific antigen levels and prostate cancer/high grade prostate cancer detection in a large Chinese multicenter cohort. Materials and Methods This retrospective study included 13,904 urology outpatients who had undergone biopsy for the indications of prostate specific antigen greater than 4.0 ng/ml or prostate specific antigen less than 4.0 ng/ml but with abnormal digital rectal examination results. The prostate specific antigen measurements were performed in accordance with the standard procedures at the respective institutions. The type of assay used was documented and recalibrated to the WHO standard. Results The incidence of prostate cancer and high grade prostate cancer was lower in the Chinese cohort than the Western cohorts at any given prostate specific antigen level. Around 25% of patients with a prostate specific antigen of 4.0 to 10.0 ng/ml were found to have prostate cancer compared to approximately 40% in U.S. clinical practice. Moreover, the risk curves were generally flatter than those of the Western cohorts, that is risk did not increase as rapidly with higher prostate specific antigen. Conclusions The relationship between prostate specific antigen and prostate cancer risk differs importantly between Chinese and Western populations, with an overall lower risk in the Chinese cohort. Further research should explore whether environmental or genetic differences explain these findings or whether they result from unmeasured differences in screening or benign prostate disease. Caution is required for the implementation of prostate cancer clinical decision rules or prediction models for men in China or other Asian countries with similar genetic and environmental backgrounds.

    • 【Abstract】Objective To investigate and analyze the Chinese medicine constitution types of pregnant metaphase women in Fuzhou of China. Methods Cross-sectional study and stratified sampling were used. A scale, , was as a tool for investigation. 1000 scale copies were handed out. 989 scale copies were got after excluding the scale copies with logic error. Constitution types were described by constituent ratio. Results In Fuzhou, the Chinese medicine constitution types of pregnant metaphase women were as following: Yang-deficiency type was 28.5%, damp-heat type was 25.5%, Yin-deficiency type was 25.2%, Qi-depression type and Qi-deficiency type were 23.1% respectively, gentleness type was 20.2%, stasis type was 19.1%, phlegm type was 10.9%, and special intrinsic type was 7.0%. The front three constitution types in different age groups: 20 years old~group: Qi-deficiency type was 29.4%, gentleness type was 24.8%, Yin-deficiency type and yang-deficiency type were 24.2% respectively; 25 years old~group:Yang-deficiency type was 27.6%, Yin-deficiency type and damp-heat type were 23.3% respectively; 30 years old~group: damp-heat type was 34.4%, Yang-deficiency type was 33.9%, Yin-deficiency type was 30.8%. The distribution of constitution types in different education background groups was similar as that of total constitution types of pregnant metaphase women. Conclusions The constitution type’s characteristics of pregnant metaphase women in Fuzhou were inclined to deficiency, heat and damp, and Qi-depression. Guided by the theory of “Preventive Treatment of Disease”, the staff working on antepartum care may provide targeted care according to different body constitution types of pregnant women.

    • Location based services (LBS) extensively utilize proximity testing to help people discover nearby friends, devices, and services. Current practices rely on full trust to the service providers: users share their locations with the providers who perform proximity testing on behalf of the users. Unfortunately, location data has been often breached by LBS providers, raising privacy concerns over the current practices. To address these concerns previous research has suggested cryptographic protocols for privacy-preserving location proximity testing. Yet general and precise location proximity testing has been out of reach for the current research. A major roadblock has been the requirement by much of the previous work that for proximity testing between Alice and Bob both must be present online. This requirement is not problematic for one-to-one proximity testing but it does not generalize to one-to-many testing. Indeed, in settings like ridesharing, it is desirable to match against ride preferences of all users, not necessarily ones that are currently online. This paper proposes a novel privacy-preserving proximity testing protocol where, after providing some data about its location, one party can go offline (nap) during the proximity testing execution, without undermining user privacy. We thus break away from the limitation of much of the previous work where the parties must be online and interact directly to each other to retain user privacy. Our basic protocol achieves privacy against semi-honest parties and can be upgraded to full security (against malicious parties) in a straight forward way using advanced cryptographic tools. Finally, we reduce the responding client overhead from quadratic (in the proximity radius parameter) to constant, compared to the previous research. Analysis and performance experiments with an implementation confirm our findings.

  • Heterogenic Distribution of Aromatic L-Amino Acid Decarboxylase Neurons in the Rat Spinal Cord

    • Aromatic L-amino acid decarboxylase (AADC) is an essential enzyme in the synthesis of serotonin, dopamine, and certain trace amines and is present in a variety of organs including the brain and spinal cord. It is previously reported that in mammalian spinal cord AADC cells (called D-cells) were largely confined to a region around the central canal and that they do not produce monoamines. To date, there has not been a detailed description of their distribution and morphology in mammals. In the present study this issue is systematically investigated using immunohistochemistry. We have found that AADC cells in the rat spinal cord are both more numerous and more widely distributed than previously reported. In the gray matter, AADC neurons immunolabeled for NeuN were not only found in the region around the central canal but also in the dorsal horn, intermediate zone, and ventral horn. In the white matter a large number of glial cells were AADC-immunopositive in different spinal segments and the vast majority of these cells expressed oligodendrocyte and radial glial phenotypes. Additionally, a small number of AADC neurons labeled for NeuN were found in the white matter along the ventral median fissure. The shapes and sizes of AADC neurons varied according to their location. For example, throughout cervical and lumbar segments AADC neurons in the intermediate zone and ventral horn tended to be rather large and weakly immunolabeled, whereas those in comparable regions of sacrocaudal segments were smaller and more densely immunolabeled. The diverse morphological characteristics of the AADC cells suggests that they could be further divided into several subtypes. These results indicate that AADC cells are heterogeneously distributed in the rat spinal cord and they may exert different functions in different physiological and pathological situations.
