  • TECLA-an innovative technical approach for prostate cancer registries.

    • Objective: To present a code-driven, electronic database for patients TrEated with robotic-assisted radiCaL prostAtectomy (TECLA), developed at Innlandet Hospital (IH), Trust, Norway, for research, local quality control and to deliver data to the National Cancer Registry of Norway (CRN). Clinical data are directly extracted from the structured documentation in the electronic medical record (EMR). Materials and methods: The urological department at IH treats about 200 patients with robotic-assisted radical prostatectomy (RARP) annually. All consenting patients registered with the procedure code for RARP are included in TECLA. Clinical data are obtained automatically from the EMR, by structured forms. Patient-reported outcome and experience measures (PROMs and PREMs) are filled in by the patients on an iPad or a smartphone. Results: The basic construct of TECLA is presented. From August 2017 to June 2018, 200 men were treated with RARP, of which 182 (91%) provided consent for inclusion in the register. Of these, 97% completed the PROM survey before treatment and 91% at 3 months follow-up. PREMs were completed by 78%. All clinical variables for the hospital stay and for the 6-week follow-up were more than 95% complete. Conclusion: This entirely electronic surgical quality register is easy to use, both for patients and clinicians, and has a high capture rate. The data collection is linked to the clinicians' workflow, without double data entry, so entering data does not add any extra work. The register design can be used by other hospitals for various surgical procedures.

    • Dalton is a powerful general-purpose program system for the study of molecular electronic structure at the Hartree-Fock, Kohn-Sham, multiconfigurational self-consistent-field, MOller-Plesset, configuration-interaction, and coupled-cluster levels of theory. Apart from the total energy, a wide variety of molecular properties may be calculated using these electronic-structure models. Molecular gradients and Hessians are available for geometry optimizations, molecular dynamics, and vibrational studies, whereas magnetic resonance and optical activity can be studied in a gauge-origin-invariant manner. Frequency-dependent molecular properties can be calculated using linear, quadratic, and cubic response theory. A large number of singlet and triplet perturbation operators are available for the study of one-, two-, and three-photon processes. Environmental effects may be included using various dielectric-medium and quantum-mechanics/molecular-mechanics models. Large molecules may be studied using linear-scaling and massively parallel algorithms. Dalton is distributed at no cost from for a number of UNIX platforms.

  • Experience Measures after Radical Prostatectomy: A Register-Based Study Evaluating the Association between Patient-Reported Symptoms and Quality of Information

    • Patient-reported data are important for quality assurance and improvement. Our main aim was to investigate the association between patient-reported symptoms among patients undergoing radical prostatectomy and their perceived quality of information before treatment. In this single-centre study, 235 men treated with robotic-assisted radical prostatectomy (RARP) between August 2017 and June 2019, responded to a follow-up questionnaire 20-42 months after surgery. A logistic regression analysis was performed to assess the association between patient-reported symptoms, measured with Expanded Prostate Cancer Index Composite for Clinical Practice (EPIC-CP), and the perceived quality of information. Adverse effects were defined as a higher EPIC score at follow-up than at baseline. The majority (77%) rated the general information as good. Higher EPIC-CP at follow-up was significantly associated with lower perceived quality of information, also after adjustment for age and level of education (bivariate model OR 1.12, 95% CI 1.07; 1.16, p < 0.001 and multiple model OR 1.12 95% CI 1.08; 1.17, p < 0.001). The share who rated information as good was almost identical among those who reported more symptoms after treatment and those who reported less symptoms (78.3% and 79.2%). Consequently, adverse effects could not explain the results. Our findings suggest a need for improvement of preoperative communication.

  • Predictors of upgrading from low-grade cancer at prostatectomy in men with biparametric magnetic resonance imaging

    • Introduction Prostate-specific antigen (PSA) density has previously been identified as a predictor of histological upgrading at radical prostatectomy, but how information from pre-treatment biparametric magnetic resonance imaging (bpMRI) contributes needs further clarification. The objective of this register-based study was to identify predictors of upgrading at prostatectomy in men with Grade group (GG) 1 and pre-treatment bpMRI. Material and methods This single-center study included men with GG 1 cancer on prediagnostic biopsy, who underwent bpMRI and robotic-assisted radical prostatectomy (RARP) between March 2014 and September 2019. We estimated logistic regression models to explore predictors for upgrading. The explored potential predictors were age, PSA density, tumor stage and Prostate Imaging Reporting and Data System (PI-RADS) score (dichotomised 1-3 versus 4-5). Results Upgrading was observed in 56% (73/130) of the men. PSA density was the only significant predictor for upgrading (unadjusted OR = 1.7, 95% CI 1.2; 2.4 adjusted OR = 1.7, 95% CI 1.2; 2.5). The probability of upgrading was lower for men with a PIRADS 1-3 than for PIRADS 4-5, but the difference was not statistically significant (adjusted OR 0.4, 95% CI 0.2; 1.1, p = 0.082). Among men with PI-RADS 1-3, the probability increased with increasing PSA density (p = 0.036). With PI-RADS 4-5 the probability of upgrading was high over the entire PSA density range. Conclusions PSA density is a clinically important factor to predict upgrading from GG1 when bpMRI shows PI-RADS 1-3. In men with PI-RADS 4-5 on bpMRI, the probability of an undetected GG 2-5 cancer is high regardless of the PSA density.

    • BACKGROUND: Previous studies of video-assisted techniques for parathyroidectomy in patients with primary hyperparathyroidism have found similar or better results compared with bilateral neck exploration. The aim of the present study was to compare open minimally invasive parathyroidectomy with the video-assisted technique for primary hyperparathyroidism in a multicentre randomized trial. METHODS: Some 143 patients were randomized to open (n = 75) or video-assisted (n = 68) parathyroidectomy after positive sestamibi scintigraphy. There were no differences in preoperative data. The open operation was performed through a 15-mm incision. The video-assisted techniques used were minimally invasive video-assisted parathyroidectomy (MIVAP) or video-assisted parathyroidectomy using the lateral approach (VAPLA). Data were collected prospectively including postoperative pain scoring. RESULTS: The procedure was significantly quicker for the open compared to the video assisted operations: mean(s.d.) 60(35) versus 84(47) min (P = 0.001). Both groups of patients had similar conversion rates and the same outcome, with comparable incision lengths, low scores for postoperative neck discomfort, high cosmetic satisfaction and low complication rates. CONCLUSION: Open minimally invasive parathyroidectomy for primary hyperparathyroidism was quicker than either video-assisted technique.

    • The possibility of non-volatile particle agglomeration in engine exhaust was experimentally examined in a Euro VI heavy duty engine using a variable cross section agglomeration pipe, insulated and double walled for minimal thermophoresis. The agglomeration pipe was located between the turbocharger and the exhaust treatment devices. Sampling was made across the pipe and along the centre-line of the agglomeration pipe. The performance of the agglomeration pipe was compared with an equivalent insulated straight pipe. The non-volatile total particle number and size distribution were investigated. Particle number measurements were conducted according to the guidelines from the Particle Measurement Programme. The Engine was fuelled with commercially available low sulphur S10 diesel. Experiments conducted in heavy duty engine relevant operating points were done to sweep the effect of (i) Mass flow rate in the exhaust (ii) Temperature in the exhaust and (iii) Engine speed and thus exhaust pressure pulsation frequencies in the exhaust. The test matrix included eleven operating points at steady-state. The results show that, using the agglomeration pipe, neither significant non-volatile particle reduction nor noticeable change in particle size distribution could be proven. In the current study, nucleation of non-volatile particles could not be observed along the straight pipe. Furthermore, it was found that the variable cross-section agglomeration pipe and straight pipe showed similar results in the total particle number and particle size distribution with respect to non-volatile particles.

    • Brief introduction: What are microclimates and why are they important?Microclimate science has developed into a global discipline. Microclimate science is increasingly used to understand and mitigate climate and biodiversity shifts. Here, we provide an overview of the current status of microclimate ecology and biogeography in terrestrial ecosystems, and where this field is heading next.Microclimate investigations in ecology and biogeographyWe highlight the latest research on interactions between microclimates and organisms, including how microclimates influence individuals, and through them populations, communities and entire ecosystems and their processes. We also briefly discuss recent research on how organisms shape microclimates from the tropics to the poles.Microclimate applications in ecosystem managementMicroclimates are also important in ecosystem management under climate change. We showcase new research in microclimate management with examples from biodiversity conservation, forestry and urban ecology. We discuss the importance of microrefugia in conservation and how to promote microclimate heterogeneity.Methods for microclimate scienceWe showcase the recent advances in data acquisition, such as novel field sensors and remote sensing methods. We discuss microclimate modelling, mapping and data processing, including accessibility of modelling tools, advantages of mechanistic and statistical modelling and solutions for computational challenges that have pushed the state-of-the-art of the field.What's next?We identify major knowledge gaps that need to be filled for further advancing microclimate investigations, applications and methods. These gaps include spatiotemporal scaling of microclimate data, mismatches between macroclimate and microclimate in predicting responses of organisms to climate change, and the need for more evidence on the outcomes of microclimate management.

  • Future Fuels for DISI Engines: A Review on Oxygenated, Liquid Biofuels

    • Global warming and climate change have led to a greater interest in the implementation of biofuels in internal combustion engines. In spark ignited engines, biofuels have been shown to improve efficiency and knock resistance while decreasing emissions of unburned hydrocarbons, carbon monoxide and particles.This study investigates the effect of biofuels on SI engine combustion through a graphical compilation of previously reported results. Experimental data from 88 articles were used to evaluate the trends of the addition of different biofuels in gasoline. Graphs illustrating engine performance, combustion phasing and emissions are presented in conjunction with data on the physiochemical properties of each biofuel component to understand the observed trends.Internal combustion engines have the ability to handle a wide variety of fuels resulting in a broad range of biofuel candidates. Three groups of oxygenated liquid biofuels were investigated in this review: alcohols, ethers and furans. While the investigated alcohols showcase properties associated with increased engine efficiencies (such as higher chemical knock resistance, greater charge cooling and faster laminar flame speeds). They also pose the challenge of greater fuel consumption due to lower energy densities than gasoline. Ethers and furans, on the other hand are favored by current engine designs as they exhibit properties (such as the energy density) closer to gasoline alongside increased chemical knock resistance.The compiled data summarizes the possibilities to improve efficiency and fuel economy for biofuel and binary blends in SI engines. However, the results also, show that some of the trends are more complex than anticipated. The effect of biofuels on combustion speed, regulated emissions and exhaust temperatures are not proven to be as self-evident as reported in previous studies. Results on multiple blends with focus on the effect of blending on properties would help improve the picture of the effect of future fuels on SI combustion.

  • Characterizing Prospective Elementary Teachers' Mathematical Knowledge for Teaching Fractions

    • Although research has delved into the challenges faced by mathematics teachers and students across various educational levels when working with fractions, these challenges persist. Fractions are an important but difficult content area in elementary school mathematics, and many future elementary teachers enter teacher education with limited knowledge of fractions. This lack of prior knowledge, combined with diverse experiences and attitudes toward the teaching and learning of mathematics, presents a challenge for teacher education programs aiming at enhancing prospective teachers’ knowledge to enable them to teach fractions effectively. Teacher education is a crucial time to attain a comprehensive pedagogical knowledge of mathematics, and this thesis characterizes prospective elementary teachers’ (n = 61) knowledge for teaching fractions as an outcome of Swedish teacher education. Through four empirical studies, the thesis also addresses the concept of mathematical identity and investigates the topic in light of the Mathematical Knowledge for Teaching framework.The findings highlight substantial differences between the participants in the subject matter and pedagogical content knowledge domain of fractions, revealing difficulties in both their procedural and conceptual knowledge. Challenges also arise in analyzing different fraction solutions and combining knowledge of fraction content with knowledge about elementary students, teaching, and the national curriculum document. This difficulty can be a hindrance for the quality teaching of fractions. Moreover, incoherent fraction knowledge occurs across the different knowledge categories included in the Mathematical Knowledge for Teaching framework. The findings also reveal uncertainty in relation to work with fractions and prospective teachers’ mathematical identities. A new framework for the analysis of different aspects of fractions is also presented in the thesis, and the findings are discussed considering the context of Swedish teacher education and the Mathematical Knowledge for Teaching framework.
