  • Safe Efficient Vehicle Solutions -On Driving Forces for Future Road Transportations

    • The primary objective of this paper is to present the most relevant factors and driving forces that influence future sustainable road transportations and exemplify how they may influence the development. The research methodology used is explorative scenarios where data collected from workshops, expert panels and surveys lay the foundations for the explanatory models [1]. Several driving forces are identified. However, two of them are found to be more important for the study as they have a strong influence on the development of the road transport system; yet it is uncertain how these driving forces will develop. The first of these driving forces is the ability of the authorities to take an active role when developing a sustainable transport system and the second how actively people will demand and support changes in the vehicles and the transport system. Four different future road transportation scenarios have been created to explore how changes in these two driving forces will influence the development of vehicles and road transport system; these scenarios are explained together with characteristics of future road transportation solutions. It is concluded that plans for technology development need to consider the uncertainties of these driving forces in order to enable creation of robust development roadmaps.

  • Is a hyperosmolar pump prime for cardiopulmonary bypass a risk factor for postoperative delirium? : A double blinded randomised controlled trial

    • Objective: Postoperative delirium (POD) is common after cardiac surgery. We have previously identified plasma sodium concentration and the volume of infused fluids during surgery as possible risk factors. Both are linked to the selection and composition of the pump prime used for cardiopulmonary bypass (CPB). Present study aims to examine whether hyperosmolality increases the risk for POD.Design: Patients ≥65 years (n = 195) scheduled for cardiac surgery were prospectively enrolled into this double blinded randomised clinical trial. Study group received a pump prime containing mannitol and ringer-acetate (966 mOsmol) (n = 98) vs. ringer-acetate (388 mOsmol) (n = 97) in the control group. Postoperative delirium was defined according to DSM-5 criteria based on a test-battery pre- and postoperatively (days 1–3). Plasma osmolality was measured on five occasions and coordinated with the POD assessments. The primary outcome was the POD incidence related to hyperosmolality as the secondary outcome.Results: The incidence of POD was 36% in the study group and 34% in the control group, without intergroup difference (p=.59). The plasma osmolality was significantly higher in the study group, both on days 1 and 3 and after CPB (p<.001). Post hoc analysis indicated that high osmolality levels increased the risk for delirium on day 1 by 9% (odds ratio (OR) 1.09, 95% CI 1.03–1.15) and by 10% on day 3 (OR 1.10, 95% CI 1.04–1.16).Conclusions: Use of a prime solution with high osmolality did not increase the incidence of POD. However, the influence of hyperosmolality as a risk factor for POD warrants further investigation.

  • Internet-based vestibular rehabilitation versus standard care after acute onset vertigo: a study protocol for a randomized controlled trial

    • Background: Dizziness and vertigo affect around 15% of adults annually and represent common reasons for contacting health services, accounting for around 3% of all emergency department visits worldwide. Vertigo is also associated with excessive use of diagnostic imaging and emergency care and decreased productivity, primarily because of work absenteeism. Vestibular rehabilitation is an evidence-based treatment for chronic dizziness and supervised group exercise therapy has recently been shown to be effective after vestibular neuritis, a common cause of acute onset vertigo. However, such interventions are not readily available and there is a need for more easily accessible tools. The purpose of this study is to investigate the effects on vestibular symptoms of a 6-week online vestibular rehabilitation tool after acute onset vertigo, with the aim of aiding vestibular rehabilitation by presenting a more accessible tool that can help to reduce recovery time. Methods: Three hundred twenty individuals diagnosed with acute vestibular syndrome (AVS) will be recruited from multiple hospitals in Sweden and the effects of an online vestibular rehabilitation tool, YrselTraning, on vestibular symptoms after acute onset vertigo will be compared to standard care (written instructions leaflet) in a two-armed, evaluator-blinded, multicenter randomized controlled trial. The primary outcome will be the Vertigo Symptom Scale Short Form (VSS-SF) score at 6 weeks after symptom onset. Secondary outcomes include effects of the intervention on activities of daily living, mood and anxiety, vestibular function recovery, mobility measures, health economic effects, and the reliability of the Swedish VSS-SF translation. Discussion: Participants using the online vestibular rehabilitation tool are expected to recover earlier and to a greater extent from their symptoms as compared to standard care. Since up to 50% of people with AVS without treatment develop persistent symptoms, effective treatment of AVS will likely lead to a higher quality of life and help reduce the societal costs associated with dizziness and vertigo.

    • [Acyl CoA]monoacylglycerol acyltransferase 2 (MGAT2) is of interest as a target for therapeutic treatment of diabetes, obesity and other diseases which together constitute the metabolic syndrome. In this Letter we report our discovery and optimisation of a novel series of MGAT2 inhibitors. The development of the SAR of the series and a detailed discussion around some key parameters monitored and addressed during the lead generation phase will be given. The in vivo results from an oral lipid tolerance test (OLTT) using the MGAT2 inhibitor (S)-10, shows a significant reduction (68% inhibition relative to naive, p < 0.01) in plasma triacylglycerol (TAG) concentration.

Insects as food are considered to leave smaller ecological footprints than conventional livestock (beef, pigs, and poultry) regarding feed, land and water needs, as well as greenhouse gas and ammonia emissions. As an example regarding water, taken from a recent TV documentary, if a family of four people got their protein need from insects one day a week instead of from conventional livestock, over a year they would save the planet about a million Liters of water. This is in agreement with figures above. One of the main reasons that insects for food in general have such positive environmental properties is that they are cold-blooded and thus use no energy to maintain their body temperature, in contrast to mammals (beef, pig) and birds (poultry), so their feed conversion is considerably more effective than that of conventional livestock. However, differences in feed conversion may also have other reasons, and are complicated to explain, as discussed in a recent review of Life Cycle Assessments of edible insects [9]. In addition, insects can be reared on organic side streams. For example, mealworms can recycle organic residues into high-quality potential feed rich in energy, protein and fat. Insects as food are generally considered as healthy, nutritious alternatives to conventional meats such as chicken, pork, and beef. They are rich in protein, 40-60% of dry matter, with content of essential amino acids similar to beef and soy. Unsaturated fat content is 10-30% of dry matter, and they are high in calcium, iron and zinc. However, there are significant differences depending on insect species, rearing method and feed, etc. .

    • This paper gives an overview of the Swedish trunk road system and present objectives, guidelines concerning capacity and level of service procedures to assess these issues in the feasibility and design procedure are also described.An important goal in Sweden is investments and speed limit changes over a 10 year time scale to decrease the C02-exposure. The long term speed limit overview with the objective to require median barriers at speed limits over 80 kph with results so far is presented. By now over 50 % of the former traffic load over 80 kph is decreased to 80 kph. Some 2,700 km have been retrofitted to 2+1 median barrier roads with speed limit mainly 100 kph.An overview is also given of the updated Swedish Highway Capacity Manual with new chapters especially on jam densities, entry lanes, weaving areas and traffic signals. Some interesting research projects are also briefly covered. These are 2+1 median barrier roads, capacities at motorway work zones, speed harmonisation with variable speed limits on motorways to increase capacity, ramp metering and Drive Me (autonomous driving full scale tests).

    • There is still a lack of pharmacological treatment options for traumatic brain injury (TBI), the dominant cause of death and disability in persons under the age of 40 in the developed part of the world. Clinical TBI is a markedly complex disease, categorized into different subtypes that differ in their pathophysiology, treatment requirements, and long-term consequences. For successful development of novel treatment options, refined preclinical evaluation in rodent TBI models is mandatory. Since persisting cognitive deficits, impaired motor function, depression, and personality changes are common sequelae in TBI patients, preclinical models must produce clinically relevant behavioral deficits. Additionally, clinical TBI is a markedly heterogeneous disease with a severity span from immediately fatal to mild injuries with minor and passing symptoms. Ideally, a rodent TBI model should thus be adjustable in terms of injury severity. One of the most widely used rodent TBI model is the fluid percussion injury (FPI), which meets many of the criteria for a clinically relevant experimental model. The FPI technique relies on a fluid pressure pulse being transmitted into the skull cavity of the animal, allowing for a degree of brain displacement. By placing the craniectomy and the injury site either over the midline of the skull (the central FPI; cFPI) or over one hemisphere (the lateral FPI; lFPI) the injury shows either more diffuse (cFPI) or more focal (lFPI) characteristics. Although FPI has many advantages over other TBI models, including the possibility to vary important injury characteristics, the outcome after TBI may be influenced by other features such as gender, age, species, and even strain which should be considered in the design of the rodent models. In this chapter, we discuss the limitations and advantages, as well as the special considerations necessary when using the FPI model in rodents.
