    • Takotsubo syndrome (TTS) is a poorly recognized heart disease that was initially regarded as a benign condition. Recently, it has been shown that TTS may be associated with severe clinical complications including death and that its prevalence is probably underestimated. Since current guidelines on TTS are lacking, it appears timely and important to provide an expert consensus statement on TTS. The clinical expert consensus document part I summarizes the current state of knowledge on clinical presentation and characteristics of TTS and agrees on controversies surrounding TTS such as nomenclature, different TTS types, role of coronary artery disease, and etiology. This consensus also proposes new diagnostic criteria based on current knowledge to improve diagnostic accuracy.

    • The clinical expert consensus statement on takotsubo syndrome (TTS) part II focuses on the diagnostic workup, outcome, and management. The recommendations are based on interpretation of the limited clinical trial data currently available and experience of international TTS experts. It summarizes the diagnostic approach, which may facilitate correct and timely diagnosis. Furthermore, the document covers areas where controversies still exist in risk stratification and management of TTS. Based on available data the document provides recommendations on optimal care of such patients for practising physicians.

  • Lived experiences: a focus group pilot study within the MentALLY project of mental healthcare among European users

    • BackgroundMental healthcare is an important component in societies' response to mental health problems. Although the World Health Organization highlights availability, accessibility, acceptability and quality of healthcare as important cornerstones, many Europeans lack access to mental healthcare of high quality. Qualitative studies exploring mental healthcare from the perspective of people with lived experiences would add to previous research and knowledge by enabling in-depth understanding of mental healthcare users, which may be of significance for the development of mental healthcare. Therefore, the aim of the current study was to describe experiences of mental healthcare among adult Europeans with mental health problems.MethodIn total, 50 participants with experiences of various mental health problems were recruited for separate focus group interviews in each country. They had experiences from both the private and public sectors, and with in- and outpatient mental healthcare. The focus group interviews (N=7) were audio recorded, transcribed verbatim and analysed through thematic analysis. The analysis yielded five themes and 13 subthemes.ResultsThe theme Seeking and trying to find help contained three subthemes describing personal thresholds for seeking professional help, not knowing where to get help, and the importance of receiving help promptly. The theme Awaiting assessment and treatment contained two subthemes including feelings of being prioritized or not and feelings of being abandoned during the often-lengthy referral process. The theme Treatment: a plan with individual parts contained three subthemes consisting of demands for tailored treatment plans in combination with medications and human resources and agreement on treatment. The theme Continuous and respectful care relationship contained two subthemes describing the importance of continuous care relationships characterised by empathy and respect. The theme Suggestions for improvements contained three subthemes highlighting an urge to facilitate care contacts and to increase awareness of mental health problems and a wish to be seen as an individual with potential.ConclusionFacilitating contacts with mental healthcare, a steady contact during the referral process, tailored treatment and empathy and respect are important aspects in efforts to improve mental healthcare. Recommendations included development of collaborative practices between stakeholders in order to increase general societal awareness of mental health problems.

  • Characterization of ECRH plasmas in TOMAS

    • To improve the plasma performance and control the density and plasma quality during the flat top phase, wall conditioning techniques are used in large fusion devices like W7-X and in JT60-SA. To study the performance of electron cyclotron wall conditioning, numerous experiments were performed on the TOroidally MAgnetized System, which is operated by LPP-ERM/KMS at the FZ-Jülich. It is a facility designed to study plasma production, wall conditioning, and plasma-surface interactions. The produced electron cyclotron resonance heating plasmas are characterized in various conditions by density and temperature measurements using a movable triple Langmuir probe in the horizontal and the vertical direction, complemented by video and spectroscopic data, to obtain a 2D extrapolation of the plasma parameters in the machine. A way to calibrate the triple Langmuir probe measurements is also investigated. These data can be used to determine the direction of the plasma drift in the vessel and identify the power absorption mechanisms. This will give more insight in the plasma behavior and improve the efficiency of wall conditioning and sample exposure experiments.

  • Genetic structure and colonization processes in European populations of the common vole, Microtus arvalis

    • The level of genetic differentiation within and between evolutionary lineages of the common vole (Microtusarvalis) in Europe was examined by analyzing mitochondrial sequences from the control region (mtDNA) and 12nuclear microsatellite loci (nucDNA) for 338 voles from 18 populations. The distribution of evolutionary lineagesand the affinity of populations to lineages were determined with additional sequence data from the mitochondrialcytochrome b gene. Our analyses demonstrated very high levels of differentiation between populations (overall FST:mtDNA 70%; nucDNA 17%). The affinity of populations to evolutionary lineages was strongly reflected in mtDNAbut not in nucDNA variation. Patterns of genetic structure for both markers visualized in synthetic genetic mapssuggest a postglacial range expansion of the species into the Alps, as well as a potentially more ancient colonizationfrom the northeast to the southwest of Europe. This expansion is supported by estimates for the divergence timesbetween evolutionary lineages and within the western European lineage, which predate the last glacial maximum(LGM). Furthermore, all measures of genetic diversity within populations increased significantly with longitude andshowed a trend toward increase with latitude. We conclude that the detected patterns are difficult to explain only byrange expansions from separate LGM refugia close to the Mediterranean. This suggests that someM. arvalis populationspersisted during the LGM in suitable habitat further north and that the gradients in genetic diversity may representtraces of a more ancient colonization of Europe by the species.

  • Job stress and major coronary events: results from the Job Stress, Absenteeism and Coronary Heart Disease in Europe study

    • AIMS: The intention of this study is to investigate the relationship of the demands/control/strain model with hard coronary events in an epidemiological, prospective, multicenter, European study. METHODS AND RESULTS: Six cohorts (Brussels, Ghent, Lille, Barcelona, Goteborg and Malmo) from four European countries (Belgium, France, Spain and Sweden) consisting of 21 111 middle-aged male subjects participated between 1993 and 1996 in the baseline survey of the Job Stress, Absenteeism and Coronary Heart Disease in Europe (JACE) study. The Karasek strain model of psychological demands (five items)/control (nine items) was used. During a mean follow-up of 40 months 185 acute coronary events or coronary deaths were observed. Age-adjusted hazard ratios (HRs) for developing an acute coronary event were 1.46 [CI 95% confidence interval (1.08-1.97)] for high against low psychological demands and 1.53 (95% CI 1.0-2.35) for strained (high demands plus low control) against relaxed (low demands plus high control) groups. After adjustment for standard cardiovascular risk factors the HR for developing a coronary event for those above or equal to the median against those below the median of psychological demands was 1.46 (95% CI 1.08-1.97) whereas the HR for strained against relaxed groups is 1.46 (95% CI 0.96-2.25). Sensitivity analyses confirmed the robustness of the results. CONCLUSION: In this European, multicenter, prospective, epidemiological study the Karasek job strain model was an independent predictor of acute coronary events, with the psychological demands scale emerging as the important component.

    • Local vibration absorbers, such as tuned vibration absorbers or local active control systems, can be used to reduce the response of structural components. The optimisation of the position, the number and the properties of these absorbers, is however a problem of many unknowns. This paper studies the use of an efficient simulation technique, namely the wave based method (WBM), in order to obtain these optimal parameters. In contrast to the finite element method (FEM), where the problem is divided into a large number of small elements in which the field variable is described using simple polynomial shape functions, the WBM divides the problem into a small number of domains in which exact solutions of the governing equation are used as basis functions. This approach not only leads to a numerical efficient technique for the simulation of a system response, it also has inherent advantages over the FEM in an optimisation context. This paper studies the advantages of the WBM in a gradient based optimisation scheme and applies the proposed method for the optimisation of local resonators in order to reduce the vibrational response of a simple plate.
