    • The design of 6th Generation (6G) wireless networks points towards flexible connect-and-compute technologies capable to support innovative services and use cases. Targeting the 2030 horizon, 6G networks are poised to pave the way for sustainable human-centered smart societies and vertical industries, such that wireless networks will be transformed into a distributed smart connectivity infrastructure, where new terminal types are embedded in the daily environment. In this context, the RISE-6G project aims at investigating innovative solutions that capitalize on the latest advances in the emerging technology of Reconfigurable Intelligent Surfaces (RISs), which offers dynamic and goal-oriented radio wave propagation control, enabling the concept of the wireless environment as a service. The project will focus on: i) the realistic modeling of RIS-assisted signal propagation, ii) the investigation of the fundamental limits of RIS-empowered wireless communications and sensing, and iii) the design of efficient algorithms for orchestrating networking RISs, in order to implement intelligent, sustainable, and dynamically programmable wireless environments enabling diverse services that go well beyond the 5G capabilities. RISE-6G will offer two unprecedented proof-of-concepts for realizing controlled wireless environments in near-future use cases.

  • De novo DHDDS variants cause a neurodevelopmental and neurodegenerative disorder with myoclonus

    • Subcellular membrane systems are highly enriched in dolichol, whose role in organelle homeostasis and endosomal-lysosomal pathway remains largely unclear besides being involved in protein glycosylation. DHDDS encodes for the catalytic subunit (DHDDS) of the enzyme cis-prenyltransferase (cis-PTase), involved in dolichol biosynthesis and dolichol-dependent protein glycosylation in the endoplasmic reticulum. An autosomal recessive form of retinitis pigmentosa (retinitis pigmentosa 59) has been associated with a recurrent DHDDS variant. Moreover, two recurring de novo substitutions were detected in a few cases presenting with neurodevelopmental disorder, epilepsy, and movement disorder. We evaluated a large cohort of patients (n=25) with de novo pathogenic variants in DHDDS and provided the first systematic description of the clinical features and long-term outcome of this new neurodevelopmental and neurodegenerative disorder. The functional impact of the identified variants was explored by yeast complementation system and enzymatic assay. Patients presented during infancy or childhood with a variable association of neurodevelopmental disorder, generalized epilepsy, action myoclonus/cortical tremor, and ataxia. Later in the disease course they experienced a slow neurological decline with the emergence of hyperkinetic and/or hypokinetic movement disorder, cognitive deterioration, and psychiatric disturbances. Storage of lipidic material and altered lysosomes were detected in myelinated fibers and fibroblasts, suggesting a dysfunction of the lysosomal enzymatic scavenger machinery. Serum glycoprotein hypoglycosylation was not detected and, in contrast to retinitis pigmentosa and other congenital disorders of glycosylation involving dolichol metabolism, the urinary dolichol D18/D19 ratio was normal. Mapping the disease-causing variants into the protein structure revealed that most of them clustered around the active site of the DHDDS subunit. Functional studies using yeast complementation assay and in vitro activity measurements confirmed that these changes affected the catalytic activity of the cis-PTase and showed growth defect in yeast complementation system as compared with the wild-type enzyme and retinitis pigmentosa-associated protein. In conclusion, we characterized a distinctive neurodegenerative disorder due to de novo DHDDS variants, which clinically belongs to the spectrum of genetic progressive encephalopathies with myoclonus. Clinical and biochemical data from this cohort depicted a condition at the intersection of congenital disorders of glycosylation and inherited storage diseases with several features akin to of progressive myoclonus epilepsy such as neuronal ceroid lipofuscinosis and other lysosomal disorders.

  • Prognostic Factors and Risk Groups in T1G3 Non-Muscle-invasive Bladder Cancer Patients Initially Treated with Bacillus Calmette-Guerin : Results of a Retrospective Multicenter Study of 2451 Patients

    • Background: The impact of prognostic factors in T1G3 non-muscle-invasive bladder cancer (BCa) patients is critical for proper treatment decision making. Objective: To assess prognostic factors in patients who received bacillus Calmette Guerin (BCG) as initial intravesical treatment of T1G3 tumors and to identify a subgroup of high-risk patients who should be considered for more aggressive treatment. Design, setting, and participants: Individual patient data were collected for 2451 T1G3 patients from 23 centers who received BCG between 1990 and 2011. Outcome measurements and statistical analysis: Using Cox multivariable regression, the prognostic importance of several clinical variables was assessed for time to recurrence, progression, BCa-specific survival, and overall survival (OS). Results and limitations: With a median follow-up of 5.2 yr, 465 patients (19%) progressed, 509 (21%) underwent cystectomy, and 221 (9%) died because of BCa. In multivariable analyses, the most important prognostic factors for progression were age, tumor size, and concomitant carcinoma in situ (CIS); the most important prognostic factors for BCa-specific survival and OS were age and tumor size. Patients were divided into four risk groups for progression according to the number of adverse factors among age >= 70 yr, size >= 3 cm, and presence of CIS. Progression rates at 10 yr ranged from 17% to 52%. BCa-specific death rates at 10 yr were 32% in patients >= 70 yr with tumor size >= 3 cm and 13% otherwise. Conclusions: T1G3 patients >= 70 yr with tumors >= 3 cm and concomitant CIS should be treated more aggressively because of the high risk of progression. Patient summary: Although the majority of T1G3 patients can be safely treated with intravesical bacillus Calmette-Guerin, there is a subgroup of T1G3 patients with age >= 70 yr, tumor size >= 3 cm, and concomitant CIS who have a high risk of progression and thus require aggressive treatment.

    • OBJECTIVES: To determine if a re-transurethral resection (TUR), in the presence or absence of muscle at the first TUR in patients with T1-high grade (HG)/Grade 3 (G3) bladder cancer, makes a difference in recurrence, progression, cancer specific (CSS) and overall survival (OS).PATIENTS AND METHODS: In a large retrospective multicentre cohort of 2451 patients with T1-HG/G3 initially treated with bacille Calmette-Guérin, 935 (38%) had a re-TUR. According to the presence or absence of muscle in the specimen of the primary TUR, patients were divided in four groups: group 1 (no muscle, no re-TUR), group 2 (no muscle, re-TUR), group 3 (muscle, no re-TUR) and group 4 (muscle, re-TUR). Clinical outcomes were compared across the four groups.RESULTS: Re-TUR had a positive impact on recurrence, progression, CSS and OS only if muscle was not present in the primary TUR specimen. Adjusting for the most important prognostic factors, re-TUR in the absence of muscle had a borderline significant effect on time to recurrence [hazard ratio (HR) 0.67, P = 0.08], progression (HR 0.46, P = 0.06), CSS (HR 0.31, P = 0.07) and OS (HR 0.48, P = 0.05). Re-TUR in the presence of muscle in the primary TUR specimen did not improve the outcome for any of the endpoints.CONCLUSIONS: Our retrospective analysis suggests that re-TUR may not be necessary in patients with T1-HG/G3, if muscle is present in the specimen of the primary TUR.

  • Learning to Estimate RIS-Aided mmWave Channels

    • Inspired by the remarkable learning and prediction performance of deep neural networks (DNNs), we apply one special type of DNN framework, known as model-driven deep unfolding neural network, to reconfigurable intelligent surface (RIS)-aided millimeter wave (mmWave) single-input multiple-output (SIMO) systems. We focus on uplink cascaded channel estimation, where known and fixed base station combining and RIS phase control matrices are considered for collecting observations. To boost the estimation performance and reduce the training overhead, the inherent channel sparsity of mmWave channels is leveraged in the deep unfolding method. It is verified that the proposed deep unfolding network architecture can outperform the least squares (LS) method with a relatively smaller training overhead and online computational complexity.

To control the RIS, channel state information (CSI) is needed, which entails significant pilot overhead. To reduce this overhead and the need for frequent RIS reconfiguration, we propose a novel framework for integrated localization and communications, where RIS configurations are fixed during location coherence intervals, while BS precoders are optimized every channel coherence interval. This framework leverages accurate location information obtained with the aid of several RISs as well as novel RIS optimization and channel estimation methods. Performance in terms of localization accuracy, channel estimation error, and achievable rate demonstrates the effectiveness of the proposed approach. © 2023 IEEE.

    • 6G networks have been envisioned as key enablers for numerous emerging applications, including smart cities and homes, intelligently connected vechiles, smart manufactoring, and industrial Internet-of-Things (IoT). These applications require both extreme wireless connectivity as well as highly accurate and reliable sensing capability. Indeed, among many 6G visions, a common theme is that sensing will play a more significant role than ever before. By equipping wireless systems with the sensing functionality, 6G networks will go beyond classical communication and provide ubiquitous sensing services to measure, or to image, surrounding environments. This sensing functionality and the corresponding ability of the network to collect sensory data from the environment are seen as the foundation for building intelligence in the future smart world. Toward that end, there is a strong need to jointly design sensing and communication operations in 6G networks, which motivates the recent research of Integrated Sensing and Communications (ISAC).
