  • Outcomes after perioperative SARS-CoV-2 infection in patients with proximal femoral fractures: an international cohort study

    • Studies have demonstrated high rates of mortality in people with proximal femoral fracture and SARS-CoV-2, but there is limited published data on the factors that influence mortality for clinicians to make informed treatment decisions. This study aims to report the 30-day mortality associated with perioperative infection of patients undergoing surgery for proximal femoral fractures and to examine the factors that influence mortality in a multivariate analysis.SettingProspective, international, multicentre, observational cohort study.ParticipantsPatients undergoing any operation for a proximal femoral fracture from 1 February to 30 April 2020 and with perioperative SARS-CoV-2 infection (either 7 days prior or 30-day postoperative).Primary outcome30-day mortality. Multivariate modelling was performed to identify factors associated with 30-day mortality.ResultsThis study reports included 1063 patients from 174 hospitals in 19 countries. Overall 30-day mortality was 29.4% (313/1063). In an adjusted model, 30-day mortality was associated with male gender (OR 2.29, 95% CI 1.68 to 3.13, p<0.001), age >80 years (OR 1.60, 95% CI 1.1 to 2.31, p=0.013), preoperative diagnosis of dementia (OR 1.57, 95% CI 1.15 to 2.16, p=0.005), kidney disease (OR 1.73, 95% CI 1.18 to 2.55, p=0.005) and congestive heart failure (OR 1.62, 95% CI 1.06 to 2.48, p=0.025). Mortality at 30 days was lower in patients with a preoperative diagnosis of SARS-CoV-2 (OR 0.6, 95% CI 0.6 (0.42 to 0.85), p=0.004). There was no difference in mortality in patients with an increase to delay in surgery (p=0.220) or type of anaesthetic given (p=0.787).ConclusionsPatients undergoing surgery for a proximal femoral fracture with a perioperative infection of SARS-CoV-2 have a high rate of mortality. This study would support the need for providing these patients with individualised medical and anaesthetic care, including medical optimisation before theatre. Careful preoperative counselling is needed for those with a proximal femoral fracture and SARS-CoV-2, especially those in the highest risk groups.Trial registration numberNCT04323644

    • Observations of low-mass stars have frequently shown a disagreement between observed stellar radii and radii predicted by theoretical stellar structure models. This ‘radius inflation’ problem could have an impact on both stellar and exoplanetary science. We present the final results of our observation programme with the CHaracterising ExOPlanet Satellite (CHEOPS) to obtain high-precision light curves of eclipsing binaries with low-mass stellar companions (EBLMs). Combined with the spectroscopic orbits of the solar-type companions, we can derive the masses, radii, and effective temperatures of 23 M-dwarf stars. We use the PYCHEOPS data analysis software to analyse their primary and secondary occultations. For all but one target, we also perform analyses with Transiting Exoplanet Survey Satellite (TESS) light curves for comparison. We have assessed the impact of starspot-induced variation on our derived parameters and account for this in our radius and effective temperature uncertainties using simulated light curves. We observe trends in inflation with both metallicity and orbital separation. We also observe a strong trend in the difference between theoretical and observational effective temperatures with metallicity. There is no such trend with orbital separation. These results are not consistent with the idea that the observed inflation in stellar radius combines with lower effective temperature to preserve the luminosity predicted by low-mass stellar models. Our EBLM systems provide high-quality and homogeneous measurements that can be used in further studies of radius inflation.

    • This paper presents an overview of the current status of the Virtual Atomic and Molecular Data Centre (VAMDC) e-infrastructure, including the current status of the VAMDC-connected (or to be connected) databases, updates on the latest technological development within the infrastructure and a presentation of some application tools that make use of the VAMDC e-infrastructure. We analyse the past 10 years of VAMDC development and operation, and assess their impact both on the field of atomic and molecular (A&M) physics itself and on heterogeneous data management in international cooperation. The highly sophisticated VAMDC infrastructure and the related databases developed over this long term make them a perfect resource of sustainable data for future applications in many fields of research. However, we also discuss the current limitations that prevent VAMDC from becoming the main publishing platform and the main source of A&M data for user communities, and present possible solutions under investigation by the consortium. Several user application examples are presented, illustrating the benefits of VAMDC in current research applications, which often need the A&M data from more than one database. Finally, we present our vision for the future of VAMDC.

    • Palaeobiogeographical data on Cambrian trilobites obtained during the twentieth century are combined in this paper to evaluate palaeoceanographic links through c. 30 myr, once these arthropods biomineralized. Worldwide major tectonostratigraphic units are characterized at series intervals of Cambrian time and datasets of trilobite genera (629 for Cambrian Series 2, 965 for Cambrian Series 3, and 866 for the Furongian Series) are analysed using parsimony analysis of endemicity. Special attention is given to the biogeographical observations made in microcontinents and exotic terranes. The same is done for platform-basinal transects of well-known continental margins. The parsimony analysis of endemicity analysis resulted in distinct palaeogeographical area groupings among the tectonostratigraphic units. With these groupings, several palaeobiogeographical units are distinguished, which do not necessarily fit the previously proposed biogeographical realms and provinces. Their development and spatial distributions are broadly controlled by Cambrian palaeoclimates, palaeogeographical conditions (e.g. carbonate productivity and anoxic conditions) and ocean current circulation.

    • We present a high-resolution angle-resolved photoemission spectroscopy study in conjunction with first-principles calculations to investigate how the interaction of electrons with phonons in graphene is modified by the presence of Yb. We find that the charges transferred from Yb to the graphene layer hybridize with the graphene pi bands, leading to a strong enhancement of the electron-phonon interaction. Specifically, the electron-phonon coupling constant is increased by as much as a factor of 10 upon the introduction of Yb with respect to as-grown graphene (<= 0.05). The observed coupling constant constitutes the highest value ever measured for graphene and suggests that the hybridization between graphene and the adatoms might be a critical parameter in realizing superconducting graphene.

    • INTRODUCTION: A study of patients with low back pain (LBP) had revealed altered central pain processing. At an equal pain level LBP patients had considerably more neuronal activation in the somatosensory cortices than controls. In a new analysis of this dataset, we further investigated the differences in central pain processing between LBP patients and controls, looking for possible pathogenic mechanisms.METHODS: Central pain processing was studied by functional magnetic resonance imaging (fMRI), using equally painful pressure stimuli in a block paradigm. In this study, we reanalyzed the fMRI data to statistically compare pain-elicited neuronal activation of both groups.RESULTS: Equally painful pressure stimulation resulted in a significantly lower increase of regional cerebral blood flow (rCBF) in the periaqueductal gray (PAG) of the LBP patients. The analysis further revealed a significantly higher increase of rCBF in LBP than in HC in the primary and secondary somatosensory cortex and the lateral orbitofrontal cortex (LOFK), elicited by these same stimuli.CONCLUSIONS: These findings support a dysfunction of the inhibitory systems controlled by the PAG as a possible pathogenic mechanism in chronic low back pain.

  • Magnetizing lead-free halide double perovskites

    • Spintronics holds great potential for next-generation high-speed and low-power consumption information technology. Recently, lead halide perovskites (LHPs), which have gained great success in optoelectronics, also show interesting magnetic properties. However, the spin-related properties in LHPs originate from the spin-orbit coupling of Pb, limiting further development of these materials in spintronics. Here, we demonstrate a new generation of halide perovskites, by alloying magnetic elements into optoelectronic double perovskites, which provide rich chemical and structural diversities to host different magnetic elements. In our iron-alloyed double perovskite, Cs2Ag(Bi:Fe)Br-6, Fe3+ replaces Bi3+ and forms FeBr6 clusters that homogenously distribute throughout the double perovskite crystals. We observe a strong temperature-dependent magnetic response at temperatures below 30 K, which is tentatively attributed to a weak ferromagnetic or antiferromagnetic response from localized regions. We anticipate that this work will stimulate future efforts in exploring this simple yet efficient approach to develop new spintronic materials based on lead-free double perovskites.
