  • Differences in Health-Related Quality of Life After Gastric Bypass Surgery : a Cross-Sectional Study

    • Background: Gastric bypass (GBP) is a surgical method with good evidence of sustainable weight loss, reduced obesity-related comorbidities, and improved health-related quality of life (HRQoL). However, long-term data post-GBP is scarce on HRQoL related to other factors than weight loss, such as impact of socio-economic, age, and gender. Aim: To investigate long-term HRQoL in GBP patients. Methods: The study was conducted as a cross-sectional study covering 3 to 9 years post-GBP measuring HRQoL using RAND-36. Association to weight loss, time since surgery, gender, educational level, occupation, and age was analyzed. The participants were included on the basis that they had received a GBP that was performed by Region Skåne, the southernmost administrative healthcare region in Sweden. Recruitment to the study was by mail invitation for an online survey. Results: Of the total population of 5310 persons receiving the questionnaire, 1339 of the 1372 responders fulfilled the inclusion criteria. Those with low educational level, unemployed, persons on sick leave or disability support, and those with less weight loss reported the lowest HRQoL. The longer time since surgery, the lower the HRQoL. Conclusion: Less weight loss, longer time since GBP, lower educational level, and lower degree of employment all affect HRQoL negatively after GBP surgery.   • Femoral fracture classification in the Swedish Fracture Register - A validity study

    • © 2019 The Author(s). Background: A total of more than 270,000 fractures are registered in the Swedish Fracture Register (SFR), a national quality register. Fractures are classified following the AO/OTA classification, commonly by a junior doctor. As a step in the process of validating the data in the SFR, several studies of the accuracy of the fracture classification have already been published. The aim of this study was to evaluate the accuracy of femoral fracture classification in the SFR. Methods: One hundred and eighteen femur fractures were randomly selected from the SFR. Three experienced orthopaedic surgeons individually classified these fractures on two separate occasions and a gold standard classification was established. This classification was compared with the original classification in the SFR. Inter- and intraobserver agreement was calculated. Results: The agreement between the classification in the SFR and the gold standard classification was kappa = 0.65 for the AO/OTA group and kappa = 0.83 for the AO/OTA type. This corresponds to substantial and almost perfect agreement, according to Landis and Koch. The kappa values for interobserver agreement ranged from 0.64-0.76 for the AO/OTA group and 0.76-0.85 for the AO/OTA type. The kappa values for intraobserver agreement ranged from 0.79-0.81 for the AO/OTA group and 0.91-0.93 for the AO/OTA type. Conclusions: The classification of femoral fractures in the Swedish Fracture Register is substantial (AO/OTA group) to almost perfect (AO/OTA type) and as accurate as in previous studies. The present study also shows that the agreement between the SFR classification and a gold standard classification is in the same range of agreement as between experienced raters. In contrast to previous studies, the classifications in the SFR are made by an unselected group of mostly inexperienced classifiers. The results indicate that the fracture classification in a national quality register can be accurate enough to permit the evaluation of fracture treatment in specific groups of fractures.

  • Climate labelling and the importance of increased vegetable consumption

    • Nearly a third of the climate change impacts attributed to human consumption are believed to be caused by food and drink. Climate communication and marketing efforts have been the focus of an interdisciplinary research exploring how food retailers, in their unique intermediary role between producers and consumers, could guide consumers’ food choices in a more climate-friendly direction, with examples mainly from Sweden. Consumers are not aware of what actions are the most important from a climate point of view. A guide to more climate friendly protein choices (meat guide) was created within the project, with the aim of developing a way of informing about the environmental impacts of meat and other sources of protein. While food waste was considered an important issue, climate effects of transportation and packaging was of greater concern to the consumers in the study, who found it more important to buy local and seasonal food, and organic products, than to choose vegetarian alternatives. Observational studies in European stores, as well as interviews with representatives of Swedish supermarkets, revealed that direct climate messages were scarce. A recommendation to supermarkets is to promote vegetarian choices in store and develop the fruit and vegetable department. A novel suggestion is to encourage retailers to build on ambiguous categories and indirect messages, in particular local, seasonal and organic, in communicating climate mitigating behaviour and promote an increase in consumption of vegetables.

  • Communicating GHG mitigating food consumption in-store An observational study of food retailers in five European countries

    • Purpose - Current food consumption patterns contribute negatively to greenhouse gas (GHG) emissions and climate change. Positioned at the interface between producers and consumers, retailers have the potential to reduce this problem through informing their customers on the consequences of their actions. The purpose of this paper is to understand the variety of ways European retailers go about informing their customers on the linkage between food choice and climate change as well as which actions they advocate in-store to reduce the problem.Design/methodology/approach - In-store walkthroughs lasting between 40 minutes and 1.5 hours were carried out at 30 grocery stores in five European countries to identify climate mitigating communications. The observations targeted any message produced and transmitted by the retailer where links between food and climate change were drawn.Findings - The diversity in climate mitigating food communication we expected to find across Europe did not materialize. Only four out of the 30 retailers visited transmitted to their customers any information showing a direct link between food consumption and climate change. Indirectly, the authors found some retailers communicating food choices believed to lead to GHG reduction without linking them to climate change. Finally the authors found several retailers communicating what the authors argue are ambiguous messages to their customers where sustainability issues were confounded with climate ones. The dearth of climate mitigating food communications reveals the complexity in informing customers on such issues but also a possible lack of interest on the part of both parties.Originality/value - This research contributes empirically to knowledge of how retailers communicate climate mitigating food consumption to consumers.
