  • Effects of long-term exposure to air pollution on natural-cause mortality : an analysis of 22 European cohorts within the multicentre ESCAPE project

    • Background Few studies on long-term exposure to air pollution and mortality have been reported from Europe. Within the multicentre European Study of Cohorts for Air Pollution Effects (ESCAPE), we aimed to investigate the association between natural-cause mortality and long-term exposure to several air pollutants. Methods We used data from 22 European cohort studies, which created a total study population of 367 251 participants. All cohorts were general population samples, although some were restricted to one sex only. With a strictly standardised protocol, we assessed residential exposure to air pollutants as annual average concentrations of particulate matter (PM) with diameters of less than 2.5 mu m (PM2.5), less than 10 mu m (PM10), and between 10 mu m and 2.5 mu m (PMcoarse), PM2.5 absorbance, and annual average concentrations of nitrogen oxides (NO2 and NOx), with land use regression models. We also investigated two traffic intensity variables-traffic intensity on the nearest road (vehicles per day) and total traffic load on all major roads within a 100 m buff er. We did cohort-specific statistical analyses using confounder models with increasing adjustment for confounder variables, and Cox proportional hazards models with a common protocol. We obtained pooled effect estimates through a random-effects meta-analysis. Findings The total study population consisted of 367 251 participants who contributed 5 118 039 person-years at risk (average follow-up 13.9 years), of whom 29 076 died from a natural cause during follow-up. A significantly increased hazard ratio (HR) for PM2.5 of 1.07 (95% CI 1.02-1.13) per 5 mu g/m(3) was recorded. No heterogeneity was noted between individual cohort effect estimates (I-2 p value=0.95). HRs for PM2.5 remained significantly raised even when we included only participants exposed to pollutant concentrations lower than the European annual mean limit value of 25 mu g/m(3) (HR 1.06, 95% CI 1.00-1.12) or below 20 mu g/m(3) (1.07, 1.01-1.13). Interpretation Long-term exposure to fine particulate air pollution was associated with natural-cause mortality, even within concentration ranges well below the present European annual mean limit value.

    • The EMEP filter pack for measuring gaseous and particulate reduced and oxidised nitrogen compounds in air has been modified with a PM10 inlet to meet the EU air quality directive for particle measurements. The filter pack also collects sulphur compounds as well as base cations and sodium chloride. The EMEP filter pack has been used for 20 years now and it is important that the PM10 inlet doesn't affect the results. The cut-off for the filter pack recommended by EMEP is unknown and may be affected by wind. Here it was found that the cut-off for the EMEP filter pack is larger than it is for PM10. This implies that higher calcium concentrations will be obtained with the original EMEP filter pack without a PM10 inlet. All comparisons of the two filter packs show excellent agreement for sulphur dioxide and man made sulphate particles. Some losses of total nitrate have been found with the PM10 inlet especially of particulate nitrate. Some losses of particulate ammonium also take place but do not affect the total concentration.

  • Assessing atmospheric nitrogen deposition to natural and semi-natural ecosystems - Experience from Danish studies using the DAMOS

    • Local agricultural emissions contribute significantly to the atmospheric reactive nitrogen loads of Danish terrestrial ecosystems. In the vicinity of the sources this may be up to 6-8 kg N ha(-1) yr(-1) depending on location and ecosystem type. This contribution arises from dry deposition of gas phase ammonia derived from local livestock production. Long-range transport, however, often constitutes the largest contribution to the overall atmospheric terrestrial reactive nitrogen loadings in Denmark. This is often in the range 10-15 kg N ha(-1) yr(-1) and consists mainly of aerosol phase nitrate and ammonium (reaction products of nitrogen oxides and ammonia), but also dry deposition of other reactive nitrogen compounds (mainly nitrogen oxides in the form of gas phase nitric acid and nitrogen dioxide). In Denmark's environmental management of the sensitive terrestrial ecosystems modelling tools are required that account for both the local and the long-range transported contributions. This motivated development of the Danish Ammonia MOdelling System (DAMOS) that has been successfully applied to the assessment of atmospheric nitrogen loadings to sensitive Danish ecosystems. We present here three different examples of such assessments. Our results show that ecosystems located in Western Denmark (Case 1) receive the highest loads of atmospheric nitrogen depositions which generally exceed the critical load. This part of the country has the highest livestock density. In the Eastern part of the country, the atmospheric loadings are often below or close to the lower end of the interval for critical load values. These lower loads in Eastern Denmark (Case 2) are due to lower density of agricultural activities, as well as, lower precipitation rates, which leads to less wet deposition of reactive nitrogen. In general there is a gradient in atmospheric deposition over the country, with the highest depositions in the South-Western part of Denmark (Case 3) due to long-range transport contributions from North-Western Europe, but also due to local ammonia deposition associated with the high local emission from the high density livestock farming in this area.     • Road transport is the main anthropogenic source of NOx in Europe, affecting human health and ecosystems. Thus, mitigation policies have been implemented to reduce on-road vehicle emissions, particularly through the Euro standard limits. To evaluate the effectiveness of these policies, we calculated NO2 and NOx concentration trends using air quality and meteorological measurements conducted in three European cities over 26 years. These data were also employed to estimate the trends in NOx emission factors (EFNOx, based on inverse dispersion modeling) and NO2:NOx emission ratios for the vehicle fleets under real-world driving conditions. In the period 1998–2017, Copenhagen and Stockholm showed large reductions in both the urban background NOx concentrations (−2.1 and −2.6% yr−1, respectively) and EFNOx at curbside sites (68 and 43%, respectively), proving the success of the Euro standards in diminishing NOx emissions. London presented a modest decrease in urban background NOx concentrations (−1.3% yr−1), while EFNOx remained rather constant at the curbside site (Marylebone Road) due to the increase in public bus traffic. NO2 primary emissions —that are not regulated— increased until 2008–2010, which also reflected in the ambient concentrations. This increase was associated with a strong dieselization process and the introduction of new after-treatment technologies that targeted the emission reduction of other species (e.g., greenhouse gases or particulate matter). Thus, while regulations on ambient concentrations of specific species have positive effects on human health, the overall outcomes should be considered before widely adopting them. Emission inventories for the on-road transportation sector should include EFNOx derived from real-world measurements, particularly in urban settings.

  • Air pollution and lung cancer incidence in 17 European cohorts : prospective analyses from the European Study of Cohorts for Air Pollution Effects (ESCAPE)

    • BACKGROUND: Ambient air pollution is suspected to cause lung cancer. We aimed to assess the association between long-term exposure to ambient air pollution and lung cancer incidence in European populations.METHODS: This prospective analysis of data obtained by the European Study of Cohorts for Air Pollution Effects used data from 17 cohort studies based in nine European countries. Baseline addresses were geocoded and we assessed air pollution by land-use regression models for particulate matter (PM) with diameter of less than 10 μm (PM10), less than 2·5 μm (PM2·5), and between 2·5 and 10 μm (PMcoarse), soot (PM2·5absorbance), nitrogen oxides, and two traffic indicators. We used Cox regression models with adjustment for potential confounders for cohort-specific analyses and random effects models for meta-analyses.FINDINGS: The 312 944 cohort members contributed 4 013 131 person-years at risk. During follow-up (mean 12·8 years), 2095 incident lung cancer cases were diagnosed. The meta-analyses showed a statistically significant association between risk for lung cancer and PM10 (hazard ratio [HR] 1·22 [95% CI 1·03-1·45] per 10 μg/m(3)). For PM2·5 the HR was 1·18 (0·96-1·46) per 5 μg/m(3). The same increments of PM10 and PM2·5 were associated with HRs for adenocarcinomas of the lung of 1·51 (1·10-2·08) and 1·55 (1·05-2·29), respectively. An increase in road traffic of 4000 vehicle-km per day within 100 m of the residence was associated with an HR for lung cancer of 1·09 (0·99-1·21). The results showed no association between lung cancer and nitrogen oxides concentration (HR 1·01 [0·95-1·07] per 20 μg/m(3)) or traffic intensity on the nearest street (HR 1·00 [0·97-1·04] per 5000 vehicles per day).INTERPRETATION: Particulate matter air pollution contributes to lung cancer incidence in Europe.FUNDING: European Community's Seventh Framework Programme.
