  • Risk Factors for Dental Erosion After Bariatric Surgery : A Patient Survey

    • INTRODUCTION: Treatment of obesity by bariatric surgery has increased in recent years. Reported side effects that may predispose to dental erosion include reflux, vomiting, and an increased frequency of intake of food and drink.OBJECTIVE: The aim was to investigate long-term dietary behaviour and experiences related to symptoms of dental erosion at least 5 years after bariatric surgery.METHODS: An online questionnaire study was conducted amongst 250 patients who had undergone bariatric surgery at King Saud Medical City in Saudi Arabia 5 years ago or more. It comprised 36 questions on demographic data, dietary habits, general health, dental health, and oral symptoms. The data were analysed using Chi-square and sign tests (significance level P < .05).RESULTS: A significant increase in acidic reflux and vomiting was found after bariatric surgery and appeared to increase with time after surgery. Also, a significant association between presence of acidic reflux and symptoms of dental erosion was found. However, 68.5% reported improved overall well-being after surgery. The response rate was 21.6% (most were female, aged 30-59 years). Respondents were generally not advised to visit a dentist in connection with bariatric surgery.CONCLUSIONS: This long-term cross-sectional study suggests a time-dependent, increasing occurrence of vomiting and acidic reflux after bariatric surgery. Vomiting and reflux became even more common after 5 to 10 years. A significant relationship emerged between a high frequency of acidic reflux and a high frequency of oral symptoms related to dental erosion. Daily occurrence of general symptoms related to dumping syndrome were reported by the majority. However, in a 5- to 10-year perspective, general symptoms related to dumping syndrome and symptoms from dental erosion did not seem to detract from the respondents' overall satisfaction with daily living. Oral health problems might be reduced if patients who had bariatric surgery were referred to a dentist for prevention and monitoring.

    • Allergic contact dermatitis is the most prevalent form of human immunotoxicity. It is caused by reactive low molecular weight chemicals, that is, haptens, coming in contact with the skin where hapten-peptide complexes are formed, activating the immune system. By using sensitizing fluorescent thiol-reactive haptens, that is, bromobimanes, we show how keratinocytes respond to hapten exposure in vitro and reveal, for the first time in a living system, an exact site of haptenation. Rapid internalization and reaction of haptens with keratin filaments were visualized. Subsequently, keratinocytes respond in vitro to hapten exposure by release of membrane blebs, which contain haptenated keratins 5 and 14. Particularly, cysteine 54 of K5 was found to be a specific target. A mechanism is proposed where neoepitopes, otherwise hidden from the immune system, are released after hapten exposure via keratinocyte blebbing. The observed expulsion of modified keratins by keratinocytes in vitro might play a role during hapten sensitization in vivo and should be subject to further investigations.

  • Treatment of deep caries lesions in adults: randomized clinical trials comparing stepwise vs. direct complete excavation, and direct pulp capping vs. partial pulpotomy

    • Less invasive excavation methods have been suggested for deep caries lesions. We tested the effects of stepwise vs. direct complete excavation, 1 yr after the procedure had been carried out, in 314 adults (from six centres) who had received treatment of a tooth with deep caries. The teeth had caries lesions involving 75% or more of the dentin and were centrally randomized to stepwise or direct complete excavation. Stepwise excavation resulted in fewer pulp exposures compared with direct complete excavation [difference: 11.4%, 95% confidence interval (CI) (1.2; 21.3)]. At 1 yr of follow-up, there was a statistically significantly higher success rate with stepwise excavation, with success being defined as an unexposed pulp with sustained pulp vitality without apical radiolucency [difference: 11.7%, 95% CI (0.5; 22.5)]. In a subsequent nested trial, 58 patients with exposed pulps were randomized to direct capping or partial pulpotomy. We found no significant difference in pulp vitality without apical radiolucency between the two capping procedures after more than 1 yr [31.8% and 34.5%; difference: 2.7%, 95% CI (−22.7; 26.6)]. In conclusion, stepwise excavation decreases the risk of pulp exposure compared with direct complete excavation. In view of the poor prognosis of vital pulp treatment, a stepwise excavation approach for managing deep caries lesions is recommended.

  • Pre-service teachers’ experiences of the transition from analogue to digital learning during the Covid-19 pandemic

    • The transition from analogue campus-based learning to digital distance learning during the Covid-19 pandemic affected our society in many ways. This study set out to explore the experiences of 30 pre-service teachers when transitioning to digital distance learning. The pre-service teachers (PSTs) participated in a series of focus-group interviews that were subsequently analysed using qualitative content analysis. The results indicate that the transition to digital distance learning worked well and that the technology that was used performed satisfactorily. However, the PSTs experienced both opportunities and obstacles with respect to distance learning. The opportunities associated with digital distance learning were reported to be instantiated by teacher-led ‘flipped classroom’ teaching sessions and the additional freedom that was enjoyed by the PSTs (in both space and time). An obstacle associated with digital learning was the feeling that important aspects of interpersonal interaction disappeared. The PSTs remained sedentary and isolated at home, and the learning experience became somewhat dysfunctional when the teaching was not clearly structured and teacher-lead. The study concludes that it is not possible to replace IRL (‘in real life’) teaching entirely with digital distance learning. However, the results suggest that a hybrid form of teaching or elements of digital teaching can work well as a complement to future campus-based courses.

  • A-tents in the Central Sierra Nevada, California: A geomorphological indicator of tectonic stress

    • A-tents and blisters are minor landforms associated with the release of compressive stress in combination with erosional off-loading in hard crystalline bedrock. The uplifted and westward tilted granitic batholith of the Sierra Nevada, California, comprises hundreds of Cretaceous intrusions, where sheeted bornhardts stand juxtaposed, and have been exposed to compressional stress during several time periods. Field mapping north of Yosemite Valley identified 79 A-tents with a preferred orientation ENE-WSW suggesting the application of directionally applied stress. A-tent, joint and bornhardt orientation imply NE-SW tension and NW-SE compression. The stress has probably been released after the rapid Tertiary uplift and erosional unloading in the late Quaternary. We suggest that the A-tents in this part of the Sierra Nevada are related to intrinsic compressional stress imposed on the bedrock during emplacement but before mid-Cretaceous transpression.

  • Fractures and drainage in the granite mountainous area - A study from Sierra Nevada, USA

    • This paper provides an analysis of relationships between drainage patterns and fractures in the part of Sierra Nevada, California, north of the Yosemite Valley. Bedrock is Cretaceous granite and cut by numerous lineaments of various orientation, length and geomorphic expression. We have mapped fractures and drainage lines from aerial photographs, 1:40000 scale, in four test areas ranging in size from 32.5 to 266km(2). Azimuths are shown on rose diagrams for fractures and drainage lines and then visually and statistically compared. The coincidence of drainage and fracture patterns is strong, which implies causal relationships. In plan, the majority of valleys follow fractures even if this locally means a different orientation in respect to the regional slope arising from tectonic tilt of the range. Main streams occupy deeply incised troughs coincident with 'master fractures' of regional extent. Among two principal fracture directions, SSW-NNE to SW-NE and WSW-ENE, the former exerts more control on the drainage lines. The presence of a central zone of structural weakness within the major valleys provided significant constraints for the course of glacial erosion and may explain why multiple Pleistocene glaciers did not succeed in transforming valley cross-sections into expected U-shapes.     • Granite landforms in Sierra Nevada, California, and the southern part of Sweden were used to test whether a Schmidt hammer can be used to distinguish surfaces of different age and origin. Based on 3260 readings, statistically significant differences were obtained from surfaces formed and/or affected by different geomorphological processes. Aeolian, fluvial or glacially polished surfaces yield higher values than surfaces exposed to surface weathering, which are distinguishable from surfaces at a weathering front caused by deep weathering.   • Dietary intake of heterocyclic amines in relation to socioeconomic, lifestyle and other dietary factors: estimates in a Swedish population

    • Objectives: To estimate the dietary intakes of heterocyclic amines (HCAs), to examine the intakes in relation to socio-economics, lifestyle and other dietary factors and to compare the classification of subjects by intake of HCA versus intake of meat and fish. Design: Cross-sectional analysis within the Malmo Diet and Cancer (MDC) cohort. Data were obtained from a modified diet history, a structured questionnaire on socioeconomics and lifestyle, anthropometric measurements and chemical analysis of HCAs. HCA intake was cross-classified against meat and fish intake. The likelihood of being a high consumer of HCAs was estimated by logistic regression analysis. Dietary intakes were examined across quintiles of HCA intake using analysis of variance. Setting: Baseline examinations conducted in 1991-1994 in Malmo, Sweden. Subjects: A sub-sample of 8599 women and 6575 men of the MDC cohort. Results: The mean daily HCA intake was 583 ng for women and 821 ng for men. Subjects were ranked differently with respect to HCA intake compared with intake of fried and baked meat and fish (kappa = 0.13). High HCA intake was significantly associated with lower age, overweight, sedentary lifestyle and smoking. intakes of dietary fibre, fruits and fermented milk products were negatively associated with HCA intake, while intakes of selenium, vegetables, potatoes, alcohol (among men) and non-milk-based margarines (among women) were positively associated with HCA intake. Conclusions: The estimated daily HCA intake of 690 ng is similar to values obtained elsewhere. The present study suggests that lifestyle factors (e.g. smoking, physical activity, fruit and vegetable intakes, and types of milk products and margarines) may confound associations between HCA intake and disease. The poor correlation between HCA intake and intakes of fried meat and fish facilitates an isolation of the health effects of HCAs.
